Growing skull fracture of childhood with reference to the importance of the brain injury and its pathogenetic consideration.

It is recognized that the presence of a dural tear is essential in the development of a growing skull fracture. We present nine cases of growing skull fracture, stressing the importance of parenchymal injury beneath the skull and dural defects as pathogenic factors, and two cases which showed pathological conditions similar to those of growing skull fracture. According to our clinicla analysis, all of these cases followed head trauma in the first year of life. The area of cranial defect varied in size among the individual cases but was progressive in all instances. Convulsions were the most frequent symptom. With both radiologicla examinations and operative findings, it is recognized that dural tears and local cerebral injury are usually severe, and various localized ventricular enlargements are always found beneath the fracture. In many cases fibrous granulation was found filling in the bone defect and adhering to the contused cortex. The mechanism of the enlarged bone defect can be explained by classifying the lesions into three groups according to the type of tissue present: (1) granulation type; (2) cyst type, and (3) mixed type. Each type prevents the bone edge from spreading out, offering a suitable means to transmit mechanical pulsations of brain and cerebrospinla fluid. The formation of granulation tissue and dense scarring is an important factor in the growing skull fracture.

Cell-mediated immunity in pregnant women.

Phytohemagglutinin-induced lymphocyte transformation (PHA-T) was depressed in pregnant women, as compared with that in nonpregnant women. Pregnancy serum had a suppressive action on PHA-T which was enhanced with the advance in pregnancy. Hydrocortisone, progesterone, alpha-fetoprotein and trophoblast-specific antigen, were demonstrated as immune suppressive factors. From these results, it was concluded that cell-mediated immunity might be reduced in pregnant women and that this reduction might be one of the causes for the maintenance of pregnancy.

[Two cases showing subarachnoid hemorrhage from angioblastic meningioma and vascular hamartoma (author's transl)].

As a causative factor in spontaneous subarachnoid hemorrhage, vascular anomalies, especially aneurysm or arteriovenous malformation, have been generally recognized. On the other hand, subarachnoid hemorrhage from brain tumor and cryptic vascular malformation are rare. We experienced two cases showing subarachnoid hemorrhage from angioblastic meningioma and vascular hamartoma as an initial symptom. Case 1: A 48-year-old woman, who complained of severe headache and vomiting on Feb. 10th, 1972, gradually became lethargic. Lumbar puncture revealed moderately hemorrhagic C.S.F.. On the fifth day after the onset, she was admitted to our hospital. On admission she showed disorientation and disturbance of resent memory. Aphasia and agnosia were slightly observed. On ophthalmologic examination right homonymous lower quadrant hemianopsia was observed. The carotid angiogram showed slight square shift of the anterior cerebral artery to the right side, elevation of the middle serebral artery and a homogeneous tumor stain in the occipital region in capillary phase. A walnut sized tumor invading the middle portion of the left lateral sinus and showing firm adhesion to the tentrium was found. There was an intracerebral hematoma behined the tumor. The tumor, the tentrium and the lateral sinus were extirpated en bloc and the intracerebral hematoma was aspirated. Histologically, the tumor was angioblastic meningioma. Case 2: A 7-year-old boy, who complained of severe abrupt headache, nuchal pain and vomiting on Sept. 17th, 1972, became gradually lethargic. Lumbar puncture revealed hemorrhagic C.S.F., On the tenth day after the onset, he was admitted to our hospital. He showed confusion and agitation. The carotid angiogram showed an unrolling of the pericallosal artery, but no findings of space taking lesions. An air study indicated a globular filling defect protruding into the anterior horn of the right lateral ventricle. The tumor located in the laterobasal wall of the anterior horn was removed picemiel by transventricular approach. Histologically, the tumor was vascular hamartoma. Furthermore, we discussed various brain tumors showing subarachnoid hemorrhage as an initial symptom, its frequency and bleeding mechanism on the literature.

Radioligand receptor assay for urinary and serum hCG.

A radioligand receptor assay system for urinary hCG and serum hCG was developed, using 1100-2000 times g fractions of homogenates of pig testis. This method is specific for hCG and LH, and the limit of detection for hCG was 20 mIU, which could be significantly discriminated from zero at the 95% confidence level. A nonspecific inhibition reaction which exerts its influence on the assay system has been found in the urine and serum, and in performing assays for urinary and serum hCG, it was necessary to remove the inhibition reaction by diluting the urine more than eight fold or subject the serum to an extraction procedure. Since the hCG of normal pregnancy and trophoblastic neoplasia showed a dose response curve similar to that of the International Standard hCG, it became clear that these hCG could be assayed with this method. The potency ratio R/I measured by RRA and RIA of urinary hCG and serum hCG was different from those of normal pregnancy and trophoblastic neoplasia.

Clostridia in soil of the Antarctica.

From the soil in the area around the Syowa Station, the East Ongul Island, the Antarctica, a total of 193 strains of clostridia were isolated and identified. It was surprising that the soil samples taken from the places which were considered to be scarcely contaminated by human beings and animals contained many clostridia. One hundred and fifty-five strains were assigned to 11 species, including C. perfringens, C. bifermentans, C. sordellii, C. sporogenes, C. plagarum, C. paraperfringens, C. septicum, C. tertium, C. cadaveris, C. butyricum and C. felsineum, but 38 strains remained unidentified. C. perfringens, C. bifermentans and C. sordellii were isolated very frequently and C. sporogenes less frequently. All the strains of C. sordellii were nonpathogenic and had almost the same characteristics as those of C. bifermentans except for the attitude in the urease test. The peculiar distribution and characteristics of the clostridia in the Antarctic soil were discussed in comparison with those found in the soil in Japan.

[Clinical experiences of lividomycin on respiratory tract infections (author's transl)].

Clinical effects of a newly developed aminoglycoside antibiotic lividomycin, were investigated in 13 patients suffering from respiratory infections. 1. A total of 13 patients with the following infectious diseases was treated with 1 g/day of lividomycin for 3 to 11 days (average 6.7 days): Acute pneumonia 3 cases, bronchiectasis 3 cases, acute bronchitis 5 cases, suppurative diseases of the lung 1 case, and pyothorax 1 case. As the results, the antibiotic was effective in 5 patients, moderately effective in 4 patients and ineffective in 4 patients, and thus the global effective rate was 62.9% (9/13). 2. As in the case of kanamycin, lividomycin indicated fairly good sensitivity against a total of 17 strains comprising Staphylococcus aureus (7 strains), alpha-Streptococcus (2 strains), beta-Streptococcus (3 strains), gamma-Streptococcus (1 strain), Neisseria (2 strains) and Haemophilus (2 strains) which were isolated from sputum. 3. No side effects attributable to lividomycin were observed.

[Some observations on prognosis in congenital hydrocephalus (second report) -with reference to the physical handicap in survival cases (author's transl)].

The prognosis of surgically treated congenital hydrocephalus has lately proven to give a remarkably increased expectation of life for patient. However, numerous reports hve shown that the prognosis of congenital hydrocephalus have not always shown favourable turns from the viewpoints of physical and mental disability. This problem has been considered many times over the past years. But, an important problem has been left unsolved. The children of social and physical disability are most serious problem, so we attempt to obain a new interpretation about the influence factors upon the prognosis. Present report consists of follow-up studies of long surviving patients who have studied the fatty acid metabolism in the cerebrospinal fluid during postoperatively, the influeence on prognosis of complicated central nervous system malformation and discussed motor and mental disability in survival cases. Since lipids are essential factors in composing the brain tissues, quantitative composition of fatty acid inC.S.F. was determined by the method of gaschromatography using the arachidic acid as an indicator...
