A 66 year old woman was admitted for right hypochondriac pain and an abdominal tumor. A tumor of the pancreatic head with duodenal infiltration and liver metastases was found by laparotomy, and pancreaticoduodenectomy was performed. In addition, there was associated cholelithiasis and a goiter. Histopathologically, a tumor of the islets of Langerhans was suspected. The tumor tissue was positive by the Hellman-Hellerström silver impregnation method, and electron microscopy confirmed the presence of secretory granules similar to D cells. The tumor tissue contained a large quantity of somatostatin (1.3 micrograms/g). Finally, the diagnosis of somatostatin-producing tumor originating in the pancreas was made.
An experimental model of carrageenan-induced pneumonia in rabbits was used to study the proliferation and desquamation of alveolar lining epithelium, light and electron microscopically. Observation at 5 hr after intrapulmonary injection of 0.5% lambda carrageenan solution revealed epithelial cell (predominantly type 1 pneumocyte) injury, exposing the basement membrane of the alveolar wall, and that at 40 hr demonstrated an appearance of many large pneumocytes among the alveolar lining epithelium as a reparative reaction. This proliferative response of immature type 2 pneumocytes reached to the maximum in 60 and 72 hr followed by an intense desquamative response of the excessive alveolar epithelium in 5 and 6 days. The sloughed off epithelial cells were demonstrated as cell sheets or as clusters of epithelial cells. Inflammatory infiltrates in alveolar and interstitial spaces consisted of neutrophils and monocytes (or macrophages) at early stages, and mononuclear cells or macrophages at latter stages. Observation in 2nd and 3rd week showed accumulation of macrophages with many osmiophilic lamellar bodies in the alveolar spaces. The alveolar wall lining epithelium was still cuboidal or elliptical in shape in some alveoli and membranous in the others. Thickened alveolar septa at this stage were also noted. Thus this animal model appears to be a valuable tool for further understanding of the events leading to proliferation and desquamation of the alveolar lining epithelium.

A new, simple, and rapid procedure for determining chlorinated pesticide residues in milk is described. The entire acetonitrile extract from milk is passed through a 0.5 g activated charcoal chromatographic column. Chlorinated pesticides adsorbed on the charcoal are eluted with 100 mL acetone-hexane (1 + 1). The eluate is washed with water and 1% sodium carbonate solution, and chlorinated pesticides are extracted with hexane. The extract is concentrated and measured by electron capture gas chromatography. Recoveries from 50 mL milk samples fortified with 0.04-1.6 micrograms of different BHC isomers, 0.05-2 micrograms DDT and its metabolites, and 0.05-0.5 micrograms dieldrin ranged from 86.9 to 103.2%.

A 24-yr-old male patient that suffered from chronic tetany since school age. At the age of 20 tetanic convulsions occurred due to hypocalcemia. His mother also had chronic tetany due to pseudohypoparathyroidism. At the age of 24, hypocalcemia caused by pseudohypoparathyroidism was noted. Hypopotassemia persisted even when the hypocalcemia improved with the administration of 1 alpha-hydroxycholecalciferol and calcium lactate. Other findings were normal blood pressure, high levels of plasma renin activity and serum aldosterone, a fall in blood pressure after angiotensin II antagonist infusion, blunted pressor response to angiotensin II infusion and hyperplasia of the juxtaglomerular cells. These results were compatible with Bartter's syndrome. Plasma prostaglandins E2 and F2 alpha in standing position were suppressed after indomethacin administration. To our knowledge this is thought to be the first report of a case of pseudohypoparathyroidism associated with probable Bartter's syndrome.
An autopsy case of polymyositis with prominent hematoxylin body formation in various organs and tissues, florid focal and segmental necrotizing glomerulonephritis, and some manifestations of progressive systemic sclerosis was presented. A 34-year-old woman had erythematous edema on the face, joint pain, muscular weakness, cutaneous sclerosis and Raynaud's phenomenon for about 5 years. She had also a long history of pulmonary tuberculosis. Autopsy revealed polymyositis involving skeletal, smooth and heart muscles. This was widespread consisting of focal and extensive primary degeneration of muscle fibers and interstitial inflammatory cell infiltrates near and surrounding small veins and venules accompanied by the derivation of hematoxylin bodies from inflammatory cell nuclei, and in the posterior pharyngeal muscles severely affected, muscle substance was literally erased, leaving interstitial fibrous tissues and blood vessels. This focally restricted, severe necrosis of the skeletal muscle gives a special feature to the polymyositis in the present case. The diagnosis of polymyositis with prominent hematoxylin body formation and some manifestations of progressive systemic sclerosis was applied as a rather descriptive designation, but yet it does not absolutely exclude the possibility of a variant of SLE in an additional effort to search for an underlying disorder.

An autopsy case of a 69-year-old male with acute diffuse interstitial fibrosing pneumonitis complicated by bilateral renal cortical necrosis was presented. Autopsy revealed acute diffuse interstitial fibrosing pneumonitis, bilateral renal cortical necrosis, non-bacterial thrombotic endocarditis, involving the aortic and mitral valves, and some interesting vascular lesions, dissemination of fibrinoid change of arterioles and fibrin thrombus of small vessels in various organs; accumulation of polymorphonuclear leukocytes in the lumen of the smaller interlobular arteries and arterioles of the kidney with cellular infiltration and disintegration of the wall; severe disorganization of the wall with intraluminar and intramural fibrinous exudation in smaller branches of the hepatic artery; diminution and disarrangment of muscle fibers and patchy hyalinization in the media of the renal and interlobar arteries. The inter-relationship between acute diffuse interstitial fibrosing pneumonitis, bilateral renal cortical necrosis which may be regarded as a 'hallmark' of the generalized Shwartzman reaction, and disseminated intravascular coagulation was discussed.
