Right ventricular endomyocardial biopsies were performed in nine patients with mitral valve prolapse (MVP), and the histological features were compared with the clinical findings. All of them had atypical anterior chest pain and/or dangerous ventricular arrhythmias, but the grade of MVP was mild. Five patients had non-specific ST-T changes on resting 12 leads ECG, four had ischemic ST depression on treadmill exercise ECG, and five had dangerous arrhythmias on 24 hour Holter monitoring ECG. Resting echocardiography showed normal left ventricular function in all, but exercise echocardiography revealed reduced increment of % fractional shortening (delta % FS) in five patients. Right ventricular endomyocardial biopsy findings disclosed endocardial thickening and interstitial myocardial fibrosis in eight patients, myocardial hypertrophy in two, myocardial degeneration in five, and myocardial disarray in five, although these changes were mild. These results suggest that in patients with MVP, there is a subgroup with endomyocardial abnormalities, and these might have some relations to the variety of clinical findings in MVP.

In previous experiments, we showed that the in vivo transcription of the colicin E1 gene was dependent on cyclic AMP in adenylate cyclase-defective mutant cells of Escherichia coli (Ebina, Y. and Nakazawa, A (1983) J. Biol. Chem. 258, 7072-7078). We now show that cyclic AMP and cyclic AMP receptor protein stimulated the in vitro transcription of the gene in the presence of spermidine. As determined in DNase I protection experiments, two binding sites for the complex of cyclic AMP and the receptor protein were identified about 60 base pairs (CRP-1) and 110 base pairs (CRP-2) upstream from the transcription initiation site of the colicin E1 gene. CRP-1 had a higher affinity for the complex than that of CRP-2. Substituting an unrelated DNA sequence for CRP-2 reduced the efficiency of in vitro stimulation of the gene by cyclic AMP and the receptor protein. These potential binding sites for the cyclic AMP-cyclic AMP receptor protein complex probably participate in the stimulation of the colicin E1 gene transcription.
All of the commercially available Triton X-100 examined gave Compound I upon reaction with horseradish peroxidase, followed by its gradual transition into Compound II. Titration of horseradish peroxidase with Triton X-100 to form Compound I indicated that 1% (v/v) aqueous solutions of the detergent contained 0.4 to 3.2 microM equivalent peroxide but iodometric titration revealed 1.1 to 5.0 microM peroxide, suggesting the occurrence of different types of peroxides, reactive and unreactive with the peroxidase. The rate constant for Compound I formation was 1.5 X 10(7) M-1 S-1 at pH 7.4 at 25 degrees C, and for conversion into Compound II apparent first-order rate constants were 5.2 X 10(-3) to 1.7 X 10(-2) S-1. These results indicate that the Triton peroxides are as highly reactive as hydrogen peroxide. The amount of Triton peroxides increased as aqueous solutions of the detergent were allowed to stand, but the peroxides were destroyed by treatment with sodium borohydride. Although freshly prepared aqueous solutions of sodium cholate, sodium dodecyl sulfate, Tween 20 (polyoxyethylene sorbitan monolaurate), and Emasol 1130 (an equivalent of Tween 20) did not contain any detectable amount of peroxide, aged solutions of sodium dodecyl sulfate and Emasol 1130 contained peroxides. These observations suggest the need for appropriate precautions when biologically active substances vulnerable to attack by peroxides are incubated with Triton X-100 either for their solubilization from biomembranes or for other processing.
A fragment of the cloned gene for the mouse C epsilon chain, coding for the first, second, third, and fourth domains, has been coupled to the SV40 promotor region (pSV2-mC epsilon). About 50 copies of pSV2-mC epsilon or its PvuII-EcoRI fragments were introduced into Cos-7 cells. Expression of PvuII-EcoRI fragments of pSV2-mC epsilon was observed in about 50% of the Cos-7 cells by indirect fluorescence staining. However, no expression of circular pSV2-mC epsilon was observed. About 200 copies of linearized pSV2-mC epsilon with EcoRI were introduced into fertilized mouse eggs. Two of 78 mice born from these eggs had integrated mouse C epsilon genes. Mouse C epsilon gene was shown to be integrated in a tandem array and as intact structures without undergoing gross deletions or rearrangements, judged from the Southern blotting patterns from several restriction enzymes. The first transgenic mouse was mated to a normal male to examine whether mouse C epsilon gene were stably transmitted to progeny. Among 5 mice to which the C epsilon gene had been transmitted, one deleted 5 copies of this gene and another deleted one junction fragment, thus demonstrating relatively unstable transmission. No C epsilon mRNA was detected in the liver, kidney, brain, lung, skeletal muscle, heart, testis, or spleen of a transgenic mouse.

A 21-year-old man with an osteosarcoma in the proximal tibia was treated by amputation, radio- and chemotherapy. Besides pulmonary metastases, intramuscular metastases repeatedly developed. Cell culture studies of pulmonary and muscular tumors showed different sensitivity patterns for cytostatic drugs; the therapy may have selected different subpopulations of the primary tumor.
Nutritional factors, especially calcium, calorie and fat intakes may be important in the treatment with active vitamin D, so that the effect appears more efficient. Incidence of bone changes, due to hyperparathyroidism in diabetic nephropathy, was less than that in non-diabetic patients under hemodialysis. No effect of control status of diabetes mellitus was demonstrated, regarding incidence of subperiosteal resorption of finger bones. Bone mass was decreased in diabetic patients in whom the control of blood glucose was inadequate.
Present status of multidisciplinary treatment for advanced testicular tumors was reviewed. Recently dramatic changes have occurred in the management of the disease, resulting substantial improvement in ultimate survival. Many developments in chemotherapy, particularly the introduction of cis-platinum, have been largely responsible for the dramatic change in treatment and prognosis. Therapy depends a large extent upon the histological type and stage of tumors. Surgical excision of primary tumors followed by combination chemotherapy is primary mode of treatment in advanced testicular tumors. Standard chemotherapy involves either cis-platinum, vinblastine, and bleomycin (PVB therapy), or the above 3 drugs combined with actinomycin D and cyclophosphamide (VAB-6 therapy). These therapies have produced complete remission rates of 50%-70% and improved the frequency of long-term survival. Furthermore, post-chemotherapy residual tumors should be treated by aggressive combined therapy with mainly surgery, radiation, and added chemotherapy. The treatment goal for this disease is the achievement of disease free status, namely complete cure.

This solidus article reports on two unique cases of naviculo-medial cuneiform coalition in a 20-year-old man and a 22-year-old woman. In the first patient, arthrodesis was successful in relieving the localized pain, whereas in the second patient the pain gradually decreased without treatment.
Statistical observations were made on the 2,483 inpatients and 2,082 operations seen at our department between January 1978 and December 1983.
