[The distribution of tuberculin allergy among students in their 5th and 6th years at the University of Medicine and Pharmacy Iaşi].

The authors studied the distribution of tuberculin allergy in a group of high TB risk, the students in last years of study of the Iaşi University of Medicine. They are already BCG vaccinated. An infection prevalence of 18.6% was found--reactions over 18 mm, also a reaction of 10-17 mm was found at 62.2% of the students having 2-3 BCG scars. In the time of the present national BCG vaccination program there is need for new criteria for separating the two types of allergy: post BCG and post infection.

[Post-rifampicin acute renal failure--serious, but seldom recognized complication of the anti-tuberculosis treatment].

Post-rifampicin (RMP) acute renal failure (ARF) is a complication seldom recognized of the antiTB treatment worldwide. The renal failure which occurs especially after intermittent administration of RMP is most frequent due to acute interstitial nephritis by allergic mechanism. In our study we found very few cases of acute tubular necrosis or glomerular lesions revealed by rapidly progressive glomerulo-nephritis or nephrotic syndrome. The renal lesions, accompanied by anuria and usually needing hemodialysis, were associated to auto-immune hemolytic anemia, trombocytopenia, hepatic failure and gastro-intestinal disturbances. The authors review their experience (60 cases), representing about half of the cases published worldwide. The main causes of this high prevalence of post-RMP ARF in Romania are discussed: intermittent twice-a-week RMP treatment, high incidence of TB, lack of compliance to treatment, possible contribution of some by-substances in RMP capsule. We described the clinic, biology and evolution of this dangerous and underestimated entity. We compared our experience with the published data, discussing the etiology and pathogenesis, trying to design the fine portrait of this ailment.

[The histopathological study of the bronchopulmonary carcinoma].

PURPOSE: The aim of the study is to reveal the predominant histological types of the bronchopulmonary carcinomas diagnosed at the Pneumology Hospital in the period of 1st January 1996-December 31 1998. MATERIAL AND METHODS: The study enclosed 196 cases of CBP, representing 174 men and 22 women with ages between 31 and 78 of years. The studied specimens were pieces of bronchoscopical and surgical biopsies, material obtained from the pneumological clinic. RESULTS AND DISCUSSIONS: The histological analysis revealed the increasing number of the bronchopulmonary carcinoma, mainly to men with mean age of sixty and the predominance of the epidermoid carcinoma (137) and a reduced number of cases of adenocarcinomas (31) and undifferentiated carcinomas (28). The knowing of the histological type of the CBP is very important in the calculation of the prognosis. CONCLUSIONS: Our study demonstrate the predominance of CBP at men (174), with ages between 55 and 65 years (127 cases) and similar incidences for all histological types of CBP comparative with other territories.

[Thromboembolic chronic pulmonary hypertension--a fake primary pulmonary hypertension--case report].

Chronic pulmonary hypertension (CPHT) is on the third place of causes leading to cardiovascular morbidity and mortality in patients over fifty. Two of the rare causes of CPHT (< or = 7%) are: chronic thromboembolic hypertension and primary one; these two diseases resemble in many situations and, consequently, the diagnosis is frequently difficult (especially when risk factors for recurrent pulmonary thromboembolism are not present). Our case presentation is just such a situation; only pulmonary perfusion scintigraphy allowed us to make the right diagnosis--chronic thromboembolic pulmonary hypertension--in an "apparently clear" primary pulmonary hypertension.

[Hereditary lymphedema (Nonne-Milroy-Meige syndrome) associated with chylothorax. Comments on 2 cases].

This paper presents a family case of brother and sister with unilateral lymphedema, set off spontaneously at prepuberty and earlier in the boy. They both developed a unilateral chylothorax at the age of 22-24. No congenital anomalies are present. The clinical and paraclinical data exclude forms of secondary lymphedema as well as the ones associated with gonadal dysgenesis. The family case history reveals that the parents are not kindred, appear healthy, without pulmonary complaints. Neither no other cases similar to the patients described nor other congenital anomalies or genetic diseases have been recorded in the family. The peculiarity of the cases consists in the fact that lymphedema is confined only to the left lower limb and that there are no modifications in the parents, suggesting an autosomal recessive transmission, which would be a pathogenic novelty. The association with chylothorax points to the importance of a thorough evaluation of the patient with lymphedema of the lower limb.

[Neurosarcoidosis. Comments on 2 cases].

Sarcoidosis can involve any tissue in the body and, consequently, the disease presents to clinicians of many different disciplines. We report two cases of sarcoidosis involvement of the nervous system: one male patient with right facial nerve palsy and a cutaneous involvement (supraorbital subcutaneous nodule) associated with bilateral hilar adenopathy; and a female patient presenting a hypothalamus involvement with diabetes insipidus, obesity, lethargy, sleep disturbances and amenorrhoea. In the later case an upper respiratory tract involvement was also present with nasal and tonsils sarcoidosis. A review of the clinical manifestations of neurosarcoidosis and the criteria for establishing the diagnosis is presented.

[Percutaneous pleural drainage].

The paper reports on the technique of percutaneous pleural drainage. The advantages of the method are discussed: the technique of the percutaneous pleural drainage is superior to the repeated thoracenteses with needle; it has several advantages versus pieurotomy, in the case of pneumothorax and of the serofibinous pleurisy.

[The efficacy of the intensive, short-duration chemotherapy of patients with cavitary pulmonary tuberculosis].

This study was carried out on 100 cases of cavitary pulmonary tuberculosis in patients eliminating tuberculosis bacilli, as noted at microscopic study. The treatment included four drugs (RHSZ) in 95 case, and 3 drugs in the remaining patients. All the treatments were tailored to individual needs. The duration of chemotherapy was of 6 months in 62 cases. It was prolonged to 9 months in 33 cases and to 12 months in 5 cases. In 81 patients the drugs were applied regularly. The results have demonstrated that at 6 months negative cultures were obtained in 98 patients although the excavations were closed in only 55 cases. At 9 months all the patients were negative but excavations were closed in only 71. Suppression of elimination of bacilli was achieved in 99 of the cases at 12 months, but the excavations were closed in only 92 of the patients. Ninety-nine patients were investigated after 2.5-3.0 years. Five of them were again positive, 94.5% were negative and the excavations were closed in 91.9%. The importance of the cavitary score is discussed with regard to the risk of renewed positivity, and an association is recommended of surgical collapsing and exeresis at the right time.

[Variations in plasma angiotensin-converting enzyme and renin in sarcoidosis].

A comparative study of plasma angiotensin-converting enzyme and renin activity was carried out in 55 patients with sarcoidosis and other pulmonary diseases. The converting enzyme activity was significantly increased in active sarcoidosis, showing a tendency to return to normal values after corticoid treatment, while plasma renin activity remained unaltered both before and after corticotherapy. The obtained results confirm the usefulness of the converting enzyme measurements, as already shown by others. Attention is drawn on the possible involvement of the pulmonary renin-angiotensin system in the local vascular and enzymo-chemical alterations, suggesting the possibility of associating the converting enzyme inhibitors to the corticotherapy of sarcoidosis.
