The somatic mutation frequency of the transforming growth factor beta receptor type II gene varies widely among different cancers with microsatellite instability.

Disruption of the DNA mismatch repair system, characterized by microsatellite instability (MSI), plays an important role in the course of human carcinogenesis. Frequent somatic mutations in a polyadenine (poly(A)) tract and two GT repeats within the coding region of the transforming growth factor beta (TGFbeta) receptor II (RII) gene were reported in colorectal cancers with MSI. We examined mutations of RII in cancers of various organs with MSI and found deletions at the poly(A) tract in eight of nine (89%) gastric cancers and four of five (80%) colorectal cancers. In contrast, no mutations were found in cancers of the pancreas, endometrium, or lungs. These results suggest that TGFbeta-mediated growth control plays a very important role in the stomach and colorectum.

[A case of AIDS with intractable cryptococcal meningitis].

A 38-year-old hemophiliac, who had been infected with HIV by the administration of blood products and had been diagnosed as AIDS by the onset of Pneumocystis carinii pneumonia, was admitted to our hospital with the complaints of headache and vomiting. After he was diagnosed as cryptococcal meningitis using the microscopy, cryptococcal antigen detection and culture of cerebrospinal fluid, treatment with amphotericin-B and fluconazole was started. As there was no clinical improvement, spinal drainage was performed and acetazolamide administered in order to reduce the intracranial pressure. Treatment was changed from AMPH-B and FLCZ to a combined therapy of AMPH-B and itraconazole. As his clinical features showed improvement, he was discharged home on a maintenance dose of ITCZ and acetazolamide after having been hospitalized for three months. This case-report may be of use in the management of cryptococcal meningitis in patients with AIDS.

[A case of inflammatory breast cancer treated with preoperative intra-arterial infusion chemotherapy].

A 35-year-old female with inflammatory breast cancer was treated with preoperative intra-arterial infusion chemotherapy. One cycle consisted of cyclophosphamide 700 mg, epirubicin 20 mg and 5-FU 1,750 mg. After 4-cycles of intra-arterial infusion chemotherapy, the size of tumor and regional lymph nodes were remarkably decreased. Histological examinations revealed that almost all the cancer cells had disappeared in the resected specimen. In the patient, preoperative intra-arterial infusion chemotherapy was very effective in local control, which enabled us to perform a histologically curative operation. In conclusion, multimodal treatment including intra-arterial infusion chemotherapy is a promising treatment for inflammatory breast cancer.

Ring chromosome 21 transmitted from mother to daughter: its stability in a lymphoblastoid cell line.

A female infant with a high-pitched cry and hypertelorism but an otherwise normal facies was found to have the karyotype 46,XX,r(21)(p11.2q22.3). The r(21) was transmitted from the phenotypically normal mother. In both cases, the structure and behavior of the r(21) were rather stable in peripheral lymphocyte cultures. This stable nature of the r(21) was also confirmed in a lymphoblastoid cell line derived from the proband, where the normal-sized r(21) was persistent in most cells in prolonged culture for at least 5 months.

Mucocutaneous lymph node syndrome with various arterial lesions.

The authors present a four-month-old boy with acute febrile mucocutaneous lymph node syndrome who had hemiparesis of the left side of face and body, electrocardiographic findings suggesting myocardial infarction, angiographic evidence of obstruction of the right middle cerebral artery, and aneurysms of the coronary arteries and the brachial arteries. The authors suggest that the disease of this boy is a syndrome of systemic vasculitis.
