[Long-term chemotherapy in patients with stage IV gastric cancer after surgical treatment--a randomized comparative study of FT-207 and UFT].

A prospective randomized and comparative study of FT-207 and UFT was performed for cases of Stage IV gastric cancer involving long-term cancer chemotherapy after surgery. Immediately after the surgery, the subjects were randomly divided into two groups: A group treated by MMC and FT-207: and B group treated by MMC and UFT. From a total of 54 cases, 8 cases were excluded, so that A group consisted of 24 subjects and B group of 22 subjects. There were no differences in background factors between the two groups. In a comparison of all the cases, B group revealed a significantly higher survival rate than A group. These results indicated that the simultaneous use of MMC and UFT was effective as a long-term cancer chemotherapy after surgery for patients with Stage IV gastric cancer.

[Surgical problems encountered in cases of S0 (serosal invasion-free) gastric cancers].

In this paper we have studied 649 resected cases of S0 (macroscopically negative serosal invasion) gastric cancer, this number of S0 gastric cancer resections experienced from a total number of 1692 patients treated at our hospital during the previous 16 years. The five-year survival rate of S0 cancers was 94% for patients given curative resections. Most of S0 cancers were histologically restricted within the pm layer but a few had reached the ss gamma or se layer. Non-curative factors in S0 cancers we recomposed of cancer positive stumps in the IIb-like margin, liver metastasis in the differentiated type carcinomas with ss invasion, and surgically left lymph node metastasis in the advanced carcinomas. As for lymph node dissections, our study reveals that an R2 dissection in the C and M regions, and a R3 or R2 + No.12 dissection for the A region are recommended. Modes of recurrence were characterized by liver or other hematogeneous metastasis in pm cancers or lymph node-positive cancers at the antrum, and by a peritoneal recurrence at the corpus in cases of advanced cancers. Histological ps(-) cancers showed a different survival result than did the S0 cancers and S(+) cancers.

Presence of a tubulin pool in the resting cyst of the hypotrich ciliate, Histriculus muscorum.

Although microtubular organelles have not been observed in the resting cyst of the ciliate, Histriculus muscorum, the cyst was immunocytochemically stained with anti-tubulin antiserum, and one of its polypeptide bands reacted to the same antiserum after the immunoblotting test. Both these findings demonstrate the presence of tubulin in the resting cyst. The reacted band appeared in the 100,000 g supernatant of the cyst but not in the pellet, suggesting a depolymerized state of the tubulin in the resting cyst.

Excitability changes of presumptive ectoderm following mesodermal induction.

Dissociated ectodermal cells of the early newt gastrula which have been treated with CMF (Ca-Mg-free saline) for 5 hr differentiate into muscle cells when cultured in HFCS (heated fetal calf serum) for up to 9-12 days. Similarly dissociated cells placed into FCS (fetal calf serum) culture differentiate into epidermis. Differences in cell-cluster formation have been found between HFCS and FCS in early cell cultures (6 hr), and membrane excitability phenomena associated with the differentiation of these clusters into the muscle cells or epidermal cells have been investigated, respectively. The HFCS cultures consist of cell clusters which have few of microvilli at their surfaces and which form loose contacts by means of lamellipodia. FCS cultures consist of cell clusters which have numerous microvilli at their surfaces and which make tight contacts between cells by means of ridge-structure precursors. The different reaggregation pattern of dissociated ectoderm cells in HFCS reflects changes in the cell membrane surface induced by HFCS. The sequential genesis of action potentials in cells destined to form muscle cells in HFCS is very similar to those produced by somitic muscle cells in vivo and their ionic dependence for generating action potentials is related to epidermal action potentials in vitro (FCS).

Reorganization of contractile elements in the platelet during clot retraction.

Electron microscopic studies have been carried out on human platelets in the clot retraction. In the early stage of clot formation, platelets send out filopodia, in which thin filaments run longitudinally. The thin filaments are often observed to attach to the cell membrane where fibrin strands bind from the extracellular surface. In the later stage of clot formation, thick filaments become observable, mainly in the cell body of the platelets. These thick filaments are arranged to form an ordered array, and thin filaments run parallel to them. The thin filaments often attach to the end of the thick filaments. However, thin filaments are not seen between the arrays of thick filaments. Similar structures are also observed in the cytoskeleton of the contracted platelet. These filaments closely resemble the purified myosin aggregates formed under low ionic strength. Thus, during clot retraction, both actin and myosin in platelets are reorganized into thin and thick filaments, respectively.

Early ultrastructural changes in choroidal arterioles of spontaneously hypertensive rats.

Ultrastructural changes in the vascular smooth muscle cells of spontaneously hypertensive rats (SHR) aged 7-9 months were studied in the long posterior ciliary artery (LPCA) and they were compared with the findings in the same aged control Wistar Kyoto rats (WKY). The systolic blood pressure was higher than 200 mmHg in the SHR at the postnatal fourth month. The muscle cells in the media of the LPCA in the SHR could be divided into three types according to the electron density of their sarcoplasm, i.e., dark, intermediate or light cells. The electron density depended on the content of actin filaments; the number of the filaments per one micron 2 was determined in the specimens where cross sections of the filaments prevailed, and it averaged 48.7 in the dark cells, 34.1 in the light cells. In WKY, all the muscle cells showed the intermediate density and the number of actin filaments averaged 37.9 per micron 2. It was also observed that the muscle cells migrated into the intima and they were called the intimal cells. These cells in SHR appeared as an aggregated mass underneath the endothelium, but in WKY the individual intimal cells were scattered. These findings were thought to be early ultrastructural changes in response to a rapid elevation of the arterial pressure.

Growth patterns and prognosis in early gastric carcinoma. Superficially spreading and penetrating growth types.

One hundred and sixty-seven cases of early gastric cancer were analyzed in terms of growth patterns and were then classified into the Small mucosal type, the superficially spreading (Super) type and the penetrating growth (Pen) type, the latter further subdivided into the Pen A type growing expansively and the Pen B type deepening infiltratively. The Small mucosal, Super, and Pen B types are characterized by a tendency toward dominant depressed lesions, a low incidence of vessel invasion and lymph node metastasis, and a good prognosis after surgery (about 90% of the patients survived for ten years). In contrast, the Pen A type is characterized by dominant elevated lesions usually comprised of well differentiated carcinoma, a relatively high incidence of vessel invasion and lymph node metastasis and a poor prognosis after surgery (64.8% five-year survival), due to early recurrence in the form of liver metastasis. Some attention is given to the related surgical approaches.

A freeze-fracture evidence for lateral expansion of the plasma membrane of rat retinal pigment epithelial cells.

Distribution of the membrane particles of the retinal pigment epithelial cells of Wistar rats was studied by using freeze-fracture replicas. The overall view of the basal surfaces shows a mosaic pattern owing to membrane infoldings and interdigitated cell margins, where the P-face particles are as dense as 4.4 x 10(3)/micron2 and many particle-free patches can be seen on the basal membranes infolded into the cytoplasm. The particles of the lateral membrane are packed most closely as 6.7 x 10(3)/micron2 through the plasma membrane of the cell, where elaborate junctional complexes are seen surrounding the cell. In particular, on the side-junctional zone of the lateral membrane, they are counted as 7.7 x 10(3)/micron2, while the number of particles on the basolateral membrane is 5.5 x 10(3)/micron2. On the apical membrane the density of particles is 4.9 x 10(3)/micron2 and it is as many as on the basal membrane. Such a decreasing gradient of the distribution of the particles from the lateral to the basal or apical membrane is discussed with respect to their function.

An evidence for adrenergic involvement in the choroidal circulation.

The ciliary nerve endings of adult albino rats were observed by transmission electron microscopy after the injection of 5,6-dihydroxytryptamine. The content of both large and small granular vesicles in these endings markedly increase in density by administration of the drug. The result suggests that the endings derive from the adrenergic nerve fiber.

Arteriovenous malformation of the ileum with chronic intestinal bleeding.

A 47 year old man had a ten year history of intestinal bleeding due to arteriovenous malformation of the distal ileum. Selective angiography is the most reliable technique for the diagnosis of the lesion and a correct diagnosis was not made until th selective superior mesenteric arteriography was done after repetitive laparotomy and intestinal resection. Transcatheter embolization was transiently successful. This case seemed to belong to type 2 according to Moore's classification and was probably of congenital origin.
