Relationship between the cytologic characteristics of intraperitoneal free cancer cells and the prognosis in patients with gastric cancer.

The presence of free cancer cells in the peritoneal cavity of 387 patients with gastric cancer was analyzed using specimens obtained by intraoperative lavage of the pouch of Douglas. If cancer cells were found in the specimen, the case was classified with respect to the number and arrangement (clustered or isolated) of the cancer cells found. Study of the relationship between the histologic type of cancer and the presence of free cancer cells showed that poorly differentiated adenocarcinomas and signet ring-cell carcinomas were associated with a higher incidence of free cancer cells than were other types of cancer. The relationships between the characteristics of the free cancer cells and the postoperative survival rate were also studied. Patients whose free cancer cells were present in clusters were associated with a more favorable prognosis than were those with no such clusters. Patients with small numbers of free cancer cells survived longer than did those with large numbers of free cancer cells.

Effect of cervical spinal cord stimulation (cSCS) on cerebral glucose metabolism and blood flow in a vegetative patient assessed by positron emission tomography (PET) and single photon emission computed tomography (SPECT).

This paper presents, for the first time, assessments of cerebral glucose metabolism and cerebral blood flow before and after cervical spinal cord stimulation (cSCS) in a vegetative patient. Regional cerebral metabolic rates for glucose (rCMRglc) were measured with positron emission tomography and 18F-fluoro-2-deoxy-D-glucose. Regional cerebral blood flow (rCBF) was determined with single photon emission computed tomography and N-isopropyl p-123I-iodoamphetamine. Global CMRglc in the patient was less than one-third of that in normal control subjects. An increase in rCMRglc after one week of cSCS was observed in the cerebral cortex, predominantly on the right side, where the baseline level of rCMRglc before cSCS was more severely depressed. The pattern of increase in rCBF was almost the same as that in rCMRglc. The results suggest that cSCS activates glucose metabolism and that this is followed by an increase in cerebral blood flow.

CD44 participates in tumor cell adhesion to endothelial cells in the experimental metastatic process in B16BL6 melanoma cells.

There are several pieces of evidence suggesting a relationship between CD44 expression and tumor metastasis, but the role of CD44 in the metastatic processes is unclear. We analysed the role of CD44 in the experimental metastatic processes of B16BL6 melanoma cells using anti-CD44 monoclonal antibody (clone IM7). B16BL6 melanoma cells expressed CD44 and these cells treated with anti-CD44 monoclonal antibody increased in the experimental metastatic ability, indicating that CD44 participates in metastatic processes in B16BL6 cells. Furthermore, the adhesiveness of B16BL6 cells to endothelial cells and the retention of these cells in the lung increased by treatment with anti-CD44 monoclonal antibody. These results suggested that the CD44-mediated adhesion of tumor cells to endothelial cells was involved in the experimental metastatic process of B 16BL6 cells.

Effect on sperm-immobilizing antibodies on the spontaneous and calcium-ionophore (A23187)-induced acrosome reaction.

OBJECTIVE: To study the effect of sperm-immobilizing antibodies on spontaneous and A23187-induced acrosome reactions. MATERIAL AND METHODS: Swim-up spermatozoa obtained from a fertile donor were incubated with 16 sera containing sperm-immobilizing antibodies obtained from infertile women and 3 control sera obtained from healthy females. The acrosome loss, either occurring spontaneously or induced by 10-microM A23181, were examined by staining sperm with Pisum sativum agglutinin labeled with fluorescein isothiocyanate. RESULTS: The incidence of the spontaneous acrosome reaction of spermatozoa incubated with anti-sperm antibodies (6.25 +/- 1.4%) was significantly (P < .001) lower than that of the spermatozoa incubated with control sera (12 +/- 0.8%). The incidence of A23187-induced and inducible (incidence of induced minus spontaneous) acrosome reactions of spermatozoa incubated with sera-positive for sperm antibodies (10.4 +/- 1.3% and 4.2 +/- 1.5%) was also significantly lower (P < .001) than that of spermatozoa incubated with control sera (30.7 +/- 0.5% and 18.7 +/- 1.2%). CONCLUSION: Sperm immobilizing antibodies interfere with fertilization by inhibiting the acrosome reaction.

The importance of the real-time fluoroscopic intraoperative direct cholangiogram in the laparoscopic cholecystectomy using a new instrument.

BACKGROUND/AIMS: Laparoscopic cholecystectomy (LC) has become an accepted standard operative technique for gallstone treatment worldwide. On the other hand, complications, such as bile duct injuries, have been reported recently with the expansion of indication for LC. Intraoperative cholangiogram (IOC), to minimize the risk of bile duct injury, is now considered to be essential for safe LC. There are disadvantages to IOC such as increased operating time, the possibility of bile duct injury and the difficulties of manipulation. MATERIAL AND METHODS: We have developed a method for real-time fluoroscopic cholangiograms using a new instrument designed by our group for safe LC. First, a round-tip stylet is inserted through a sheath to coax it gently through the spiral valves of the cystic duct. Secondly, the stylet is removed and the cholangiogram catheter is inserted smoothly. Digital C-arm fluoroscopy provides "real-time" imaging of biliary tree. As a result, we became able to obtain a clear cholangiogram easily in a very short time. RESULTS: In the first 136 patients, direct cholangiograms were attempted in 106 cases and successfully completed in 102 cases (96.2%). CONCLUSION: With the development of real-time fluoroscopic intraoperative direct cholangiogram, we are able to cope with bile duct injuries and anomalies, and unsuspected bile duct stones.

Benzyl-alpha-GalNAc inhibits sialylation of O-glycosidic sugar chains on CD44 and enhances experimental metastatic capacity in B16BL6 melanoma cells.

We examined the effect of benzyl-alpha-N-acetylgalactosamine (benzyl-alpha-GalNAc), which specifically inhibits the synthesis of O-linked oligosaccharides, on the expression of the peanut agglutinin (PNA)-binding sugar chains (Gal beta 1-3GalNAc) of CD44 and on its experimental metastatic ability in B16BL6 melanoma. Benzyl-alpha-GalNAc increased the PNA-binding, but not Concanavalin A (Con A), wheat germ agglutinin (WGA), and leukoagglutinating phytohemaagglutinin (L-PHA)-binding to the cell surface. Benzyl-alpha-GalNAc markedly increased the PNA-binding to CD44 by decreasing the sialylation of these sugar chains. The experimental metastatic ability of B16BL6 cells was enhanced in association with this modulation of the sugar chains. These findings suggested that unsialylated Gal beta 1-3GalNAc sugar chains on the cell surface and/or CD44 are involved in the metastatic process of B16BL6 melanoma cells. On the other hand, the known functions of CD44, such as adhesion to hyaluronate, fibronectin, collagen, and endothelial cells, as well as homotypic aggregation, were not affected by benzyl-alpha-GalNAc regardless of the enhancement of metastatic ability induced by this glycosylation inhibitor. CD44 might have other metastasis related functions that remain to be elucidated.

Characteristic endoscopic features of intestinal lymphangiectasia: correlation with histological findings.

Six patients with histologically proven intestinal lymphangiectasia were evaluated for the endoscopic features. White villi and/or spots, previously reported as endoscopic findings, were seen. Furthermore, white nodules and submucosal elevations with or without white mucosa were observed. All patients demonstrated the appearance of submucosal elevations. The four characteristic endoscopic features were correlated with histological findings. It is considered that these endoscopic features are of value for the diagnosis of intestinal lymphangiectasia in patients with protein-losing enteropathy.

A case of small cell carcinoma of the stomach.

A case of small cell carcinoma of the stomach is reported. A 53-year-old male was referred to our hospital for elective surgery for gastric cancer. Pre-operative examinations revealed no metastases. Gastrectomy was performed curatively, and there were no gross findings of metastases. Histologically, the tumor was composed of intermediate-sized cells with hyper-chromatic nuclei and scanty cytoplasm. These cells were argyrophilic and positive for chromogranin A. A small portion of the tumor consisted of conventional adenocarcinoma (signet ring cell carcinoma and tubular adenocarcinoma). No lymph node metastasis was observed microscopically. However, 7 months after the operation, splenic and hepatic metastases were detected, and the patient died very soon thereafter. Small cell carcinoma of the stomach is a very rare disease. In literature, only 15 cases have been cured surgically. Among them, only one case had been diagnosed as small cell carcinoma before the operation, which suggests the difficulty of pre-operative diagnosis. The prognosis of this disease is very poor compared with the common type of gastric carcinoma. Considering the poor prognosis of this particular disease, adjuvant chemotherapy might be mandatory in all cases even if surgically curative resection is performed.

Regulatory effects of 1alpha,25-dihydroxyvitamin D3 on inflammatory responses in psoriasis.

Topical application of 1alpha,25-dihydroxyvitamin D3 (VD3) is thought to be beneficial in psoriasis because of its action in regulating keratinocyte proliferation and inflammation mediated by various cytokines. We assessed the effect of VD3 on the production levels of interferon (IFN)-alpha, interleukin (IL)-6, IL-8 and tumor necrosis factor (TNF)-alpha in mitogen-stimulated peripheral blood mononuclear cells (PBMC) of psoriatic patients. The results demonstrated that VD3 significantly inhibited IFN- , IL-6, and IL-8 levels produced by concanavalin A (Con A)-stimulated PBMC of psoriatic patients in a dose-dependent manner, and reduced mRNA expression for IFN- and IL-8. These findings suggest that in addition to the direct anti-proliferation effect on keratinocytes, VD3 may down-regulate the inflammatory cytokine production by infiltrating cells in psoriatic lesions.
