A histochemical study of fibronectin (FN) of the large intestine by the immunoperoxidase technique.

Histochemical studies of fibronectin in cancer and adenoma of the large intestine were conducted by the immunoperoxidase technique. The fibronectin immunohistochemical reaction was present in frozen sections, whereas it was obviously suppressed in the formalin-fixed sections. In the normal mucosa of the large intestine, the reaction was strong in the basement membrane of epithelial cells and weaker around the interstitial cells of the lamina propria mucosa. The reaction in cancer tissues was negative in the basal part of the gland, while the interstitial connective tissue was strongly positive for fibronectin. In adenoma, there was a correlation between the intensity of the fibronectin reaction and the degree of epithelial cell atypism; that is, in a gland with severe atypism the reaction was very weak or absent, while in a gland with mild atypism a weak reaction still persisted. It is concluded that this technique will serve as a useful supplementary technique for the purpose of studying the biological role of fibronectin, and that the fibronectin reaction has a close relationship with malignancy and with cellular atypism of the epithelium of the large intestine.

The importance of burn wound size in determining the optimal calorie:nitrogen ratio.

Calorie requirements in hypermetabolic burned patients have been established based on burn size and patient weight. However, the requirement for nitrogen (N), especially with variable open wound sizes, is not clear. Thirty-six adult burned patients were fed with diets containing two different calorie to N (kcal:N) ratios--150:1 (group 1) and 100:1 (group 2)--and N balance was studied with wounds of different sizes. Calorie requirements were calculated with the Curreri formula (25 cal X weight (kg) + 40 Cal X % body surface area [BSA] burned). Daily N balance was calculated from total N intake less total N output (estimated by adding 4 gm to the measured 24-hour urea N production). The data were selected on 233 patient-days when actual calorie intakes were 100% to 150% of the calculated requirement. For a wound size of 1% to 10% BSA, group 1 patients had an N balance of +2.33 gm/day and group 2, +5.54 gm/day. For a wound size of 11% to 30% BSA, N balance was +0.13 and +3.49 gm/day, respectively. For a wound size of 31% to 60% BSA, N balance was -3.38 and +3.88 gm/day, respectively. The traditional 150:1 kcal:N ratio may be adequate for patients with open wounds of less than 10% BSA. Patients with greater than 10% BSA open wounds require a dietary regimen containing a nonprotein kcal: N ratio of 100:1 in order to achieve positive N balance.

[Prognostic analysis of malignant peritoneal effusions from gastric cancer].

The prognosis of 135 cases with malignant peritoneal effusion from gastric cancer was analyzed clinically and cytologically. The histologic type of the primary gastric cancer or therapeutic differences had no significant influence on the prognosis. Previous gastrectomy or absence of distant metastases had no marked effect on survival. The formation of large clusters of cancer cells on smear preparations of peritoneal effusion was interpreted as a sign of good prognosis. The cancer cell number and the presence of lymphocytes on smear preparations bore no relationship to prognosis.

[Using computer for radiotherapy planning].

Our Computer system linked with CT has been found to be very useful for radiotherapy treatment planning. We apply this system to treatment planning of about one half of the total cases of external beam radiotherapy and of all cases of brachytherapy. In the field of computer aided automation of radiotherapy treatment, we estimate automatic verification of treatment parameters and automatic recording of them are practically useful in routine radiotherapy. Conformation Radiotherapy can be realized to be practical when digital computer and CT are employed. On the basis of 300 cases of our clinical experiences of computer controlled conformation radiotherapy, the indication of conformation radiotherapy and clinical significance are described in this paper.

[Computer controlled conformation radiotherapy].

We have developed two systematic methods of Computer Controlled Conformation Radiotherapy in advance of any other institutes in the world. From our clinical experiences of five years, it is clear that we can employ the computer controlled radiotherapy with significant accuracy. Two hundreds and ninety patients have been treated by this computer controlled conformation radiotherapy. Based upon these clinical experiences, we established the indication for the conformation radiotherapy and methods of treatments methods for each lesion. The advantage of the conformation radiotherapy is capability of delivering higher dose to the tumor while protecting surrounding tissue. This advantage results in minimizing complication in whole body and maximizing local control rate. This paper discusses the advantage in clinical use for brain tumor, prostatic carcinoma and pancreatic carcinoma.

Immunochemical studies on thermal denaturation of ovomucoid.

The thermal denaturation of ovomucoid was investigated by immunochemical methods, namely immunoprecipitation analyses and antibody-Sepharose 4B column chromatography. In the immunoprecipitation analyses, heated ovomucoid (90 degrees C, 90 min, pH 7.2) required about twice the antigen addition of the native protein to approach maximal precipitation with specific antibody, and the maximal immunoprecipitation was decreased to 80% of that by native ovomucoid. However, heated protein inhibited the binding of antibody with native ovomucoid, and 100% inhibition was attained at about 4-times the antigen addition necessary for the native protein. Heated ovomucoid (100 degrees C, 120 min) showed little immunoprecipitation and inhibition. To ovomucoid antigenicity was diminished more slowly than the trypsin inhibitory activity by heating, e.g., heated ovomucoid (90 degrees C, 120 min) retained more than 30% of the antigenicity but little trypsin inhibitory activity. By passing through the immunoaffinity column, heated ovomucoid (90 degrees C, 90 min) was separated into two fractions, either with (fraction II) or without (fraction I) antigenicity. Fraction II contained smaller fractions of ordered secondary structure than native ovomucoid, and trypsin inhibitory activity of fraction II was only 24% of the native one. These results indicated that thermally denatured ovomucoid was heterogeneous regarding the conformational damage caused by heating, and the structure around some antigenic sites in an ovomucoid molecule was retained even after the backbone conformation was partially destroyed and trypsin inhibitory activity was lost.

On fragmentation and elimination of ovarian oocytes.

Examination of numerous fragmented oocytes in the ovary revealed an asynchronous relationship between nuclear and cell divisions. The asynchrony was observed in non-fragmented oocytes as well and was considered to be one of the common processes leading towards oocyte elimination of the ovary. The present study additionally demonstrates the morphology of follicle cell degeneration observed on the surface of fragmented oocytes.

Mutagenic effects of combinations of chemical carcinogens and environmental pollutants in mice as shown by the micronucleus test.

The effects of combinations of the chemical carcinogens, dimethylnitrosoamine (DMN) or N-methyl-N'-nitro-N'-nitrosoguanidine (MNNG), with the environmental pollutants, cadmium (Cd) or polychlorinated biphenyls (PCB) were examined by Schmidt's micronucleus method with ddY strain mice. DMN had a positive mutagenic effect by oral administration, with a dose response between 25 and 100 mg/kg body weight. MNNG gave a slight positive, though not consistent result, and seemed to be more sensitive at an earlier stage after administration of the drug. Cd was given for up to 7 days in drinking water as a solution of CdCl2 at concentrations of 3, 30 or 300 ppm as Cd. PCB was administered orally at 100 mg/kg daily for 6 days as a solution in corn oil, or subcutaneously injected in ethanol. Neither Cd nor PCB gave positive results in the micronucleus test. However, a synergistic positive effect of Cd with DMN was seen, but PCB with either of these nitrosamines was not positive.

Surgical anatomy of the nasal cavity and paranasal sinuses.

The knowledge acquired by the following method can be useful not only comparison with computerized tomographs which are frequently used in otorhinolaryngology, but also will make a highly effective contribution to surgical anatomy and the prevention of complications in surgery of the nose and paranasal sinuses. The sphenoid sinus and the adjacent tissues were studied on specimens removed en bloc from 12 cadavers. Horizontal sections were made and were macroscopically and microscopically observed for surgical and anatomical studies. The optic nerve was frequently in contact with the lateral wall of the paranasal sinus which was paper-thin in sinuses with marked pneumatization. In some sinuses with marked pneumatization, the course of the internal carotid artery was in contact with the posterior wall of the sphenoid sinus. The clinical anatomy should be kept in mind to avoid complications during operative procedures. Knowledge of clinical anatomy is also useful in understanding the images of CT scanning of the paranasal sinuses.

Clinical effects of endonasal sinusectomy with reconstruction of the nasal cavity (Takahashi's method).

A follow-up investigation of the results of endonasal sinusectomy with reconstruction of the nasal cavity (Takahashi's method) was carried out in 535 of 1,338 patients who underwent endonasal sinusectomies during the 6-year period from 1967 to 1972. The results of subjective symptoms were as follows: cured 196 (36.6%); improved 234 (47.5%); unchanged 69 (12.9%); aggravated 12 (2.2%). In other words, 450 (84.1%) of the patients were satisfied with the results of the operations. Good results have similarly been obtained in cases of nasal allergy which shows increasing tendency of late, and concomitant reconstruction of the nasal cavity and desensitization therapy have resulted in improved responses.
