The time course of the change of cellular ATP level was studied in L5178Y cells exposed continuously to various temperatures and compared with that of cell death determined by means of a dye-exclusion test. The ATP level first increased at all temperatures tested and then decreased at temperatures higher than 42 degrees. The onset of this decrease always preceded the beginning of cell death. The results support the idea that the cellular lethality of heat treatment is well correlated with cellular ATP content in L5178Y cells.

Eighty patients with ureteral stones underwent transurethral ureterolithotripsy using a rigid ureteroscope. In 68 of the patients (85%), the stones were removed successfully. Most of the 12 failures were removed by a percutaneous approach. The main complications of transurethral ureterolithotripsy were high fever (35%), ureteral injury (10%) and ureteral stricture (1.6%). Ureteroscopic procedures are thought to be useful in stone removal and diagnostic evaluation of abnormality of upper urinary tract.

Sixty-eight cases of cancer were treated by combined use of Thermotron RF-8 hyperthermia and radiotherapy, and 16 cases by combined use of hyperthermia and anti-cancer drug. The results of analysis of treatment are summarized as follows. Thermo-radiotherapy of shallow-seated tumor: Shallow-seated tumor at a depth of 7 cm or less from the skin surface was present in 43 cases. Most of these were metastatic tumors of lymph node, chest wall and abdominal wall. Pathological findings and primary effect; The effect of treatment was evaluable in 40 cases, the breakdown being as follows: CR 21 cases 53%, PR 13 cases 40%, and NR 3 cases 7%. The effectiveness ratios (CR/PRa) were as follows: adenocarcinoma 13 cases out of 18, 72%; squamous cell carcinoma 12 cases out of 15, 80%. The effectiveness ratios were thus almost the same. Tumor size and primary effect: Out of 19 cases of tumor 5.9 cm or smaller, 17 cases obtained CR. Out of 17 cases of tumor 6 cm or larger, there were 5 cases of CR. Cases of larger-sized tumor thus showed a poorer effect of treatment. Recurrence after radiotherapy: Nine cases of recurrence after radiotherapy were treated by combined use of hyperthermia and radiotherapy. Irradiation of 20 to 40 Gy was combined with 5 to 10 rounds of hyperthermia. The effectiveness ratio was 78%, i.e., 7 cases out of 9. Cases of recurrence of radio-resistant tumor after radiotherapy responded well to low-dose irradiation. Thermo-radiotherapy of deep-seated tumor: Twenty-two cases of deep-seated tumor were treated with the combined use of radiation therapy and hyperthermia. They comprised 10 cases of rectal cancer, 3 cases each of gastric cancer and uterine cancer, 2 cases each of lung cancer and sarcoma, and 2 cases involving other regions. The treatment results were: 3 cases each of CR, 15 cases of PR, and 4 cases of NR. The effectiveness ratio (CR + PRa) was 8 cases out of 22, i.e., 36%. Thermo-chemotherapy: Systemic administration of anti-cancer drug was combined with hyperthermia, and 16 cases were treated. They comprised 11 cases of gastric cancer, 3 cases of colo-rectal cancer, and one case of bile duct cancer. The administered drug was 5-FU in 8 cases, MMC in 7 cases, and CDDP in one case. The treatment results were; one case of CR, 4 cases of PR, 6 cases of MR, 3 cases of NC, and 2 cases of PD. More cases of lymph node tumor showed excellent results.
Effect of high dose of N4-behenoyl-1-beta-D-arabinofuranosylcytosine (BH-AC) was studied clinically and pharmacologically in non-Hodgkin's lymphoma and acute leukemia. The schedule of drug administration consisted of daily i.v. infusion for 5 consecutive days. The daily close was initiated with 500 mg/m2 which was escalated up to 1300 mg/m2. Eruption was found to be dose-limiting toxicity and there was no myelosuppression. The plasma concentration curve of BH-AC showed a biphasic curve. The half-lives of the initial phase (t1/2 alpha) and the second phase (t1/2 beta) by the administration (1 hr iv infusion) of 700 mg/m2 were 1.08 and 3.65 h respectively. The plasma 1-beta-D-arabinofuranosylcytosine was detected for 12-24 hr after infusion (Cmax: 0.26 microgram/ml).

In conjunction with the clinical development of intraoperative radiation therapy (IORT), the ability to demonstrate an improvement in therapeutic ratio is critical and an evaluation of treatment complications is essential. The tolerance dose of the normal tissue in IORT is discussed, and if small field is applied, the greater part of tissues or organs tolerate intraoperative irradiations up to 25 Gy. The optimal dose has been estimated on each organs with malignant tumors. Intraoperative "boost" dose electron therapy given in combination with external beam irradiation has been demonstrated as good treatment modalities concerning with therapeutic ratio.

Administration of 2-Br- alpha-ergocryptine (bromocriptine = CB-154) in combination with an estrogen-receptor blocking agent tamoxifen were performed in two patients with prolactinoma and non-functioning adenoma, respectively. Case 1 was a 50-year-old male with hyperprolactinemia, impaired pituitary function and visual disorders, in whom a large invasive sellar mass lesion was disclosed by CT scans, which extended supra- and parasellarly and extracranially into paranasal sinuses and pterygopalatine fossa on the left side. As the effect of bromocriptine therapy was partial in tumor size reduction and decrease of serum PRL level and it could not be gained further improvement except for the well recovered visual acuity, tamoxifen was used together with bromocriptine resulting further reduction of tumor size and normalization of serum PRL level. Unexpectedly the medication was ceased during and after a couple of bypass surgery for angina pectoris, and it was followed by elevation of serum PRL level and regrowth of the sellar tumor as well as impairment of vision. By tamoxifen therapy the visual acuity showed some improvement, but the serum PRL level and the tumor size remained as before. Then the combination therapy with bromocriptine and tamoxifen was started again. Case 2 was a 38-year-old female with three children, who had secondary amenorrhea, galactorrhea, borderline level of serum PRL with impaired pituitary function and visual disorders. Under the diagnosis of a non-functioning pituitary adenoma with supra-sellar extension, a craniotomy was done and intracapsular partial removal of the tumor was made, revealing a chromophobe adenoma in light microscopy and undifferentiated cell adenoma in electron-micrographs.(ABSTRACT TRUNCATED AT 250 WORDS)
From August, 1976, through May, 1986, 111 patients with brain metastasis from lung cancer received various treatments according to their systemic conditions. Forty-two out of 55 patients without systemic metastasis were selected for surgical removal followed by radiation therapy, and the remaining 13 inoperable patients received radiation therapy alone. The median survival of the former group was 14.5 months with a 1-year survival rate of 57.2%, and was 11.0 months with a 1-year survival rate of 38.1% for the latter group. Twenty patients among the former group received intraoperative radiation therapy, and the result obtained was the same as that of the remaining 22 patients treated by surgery and conventional external radiation therapy. As to the conservative therapy applied for the 47 cases with systemic metastasis, radiation therapy, surgical decompression or steroid therapy were planned. The overall median survival of patients of this group was 3.5 months, with a 6-month survival rate of 27.7%. Radiation therapy combined with cisplatin was evaluated to be effective, showing more than 50% reduction of tumor volume in 73.9% of patients. The surgical removal of metastatic tumors should be considered for patients without systemic metastasis, and should also be followed by radiation therapy. Such cases would be expected to survive for more than 1 year, whereas there is no suitable treatment for patients with systemic metastasis.
