[Pseudomonas aeruginosa bacteremia associated with hematologic disorders [I]. Predisposing factors and clinical manifestations].

We experienced 57 episodes of Pseudomonas aeruginosa bacteremia in 55 patients with hematologic disorders in a 16-year period. Ninety-five percent of the patients had hematologic malignancies such as acute leukemia. All but one patient received cytotoxic or immunosuppressive therapy at or prior to the onset of bacteremia. Seventy-seven percent of the episodes occurred during profound granulocytopenia of below 100/mm3. All the patients acquired their infection in the hospital, and 96% had received antibiotic therapy during the preceding two weeks. Periodontal, anorectal, lower respiratory tract, and urogenital infections were the sources of bacteremia in about three-quarters of the episodes. Periodontal infection tended to progress to cellulitis of the face or the floor of the mouth, often resulting in bacteremia of the unimicrobial type, while anorectal infection predisposed to abscess formation, frequently leading to bacteremia of the polymicrobial type. Cellulitis at onset was seen in 35% of the episodes. Most sites of infection did not become apparent until one to three days after the onset of fever, probably because of depressed inflammatory response associated with severe granulocytopenia. The majority of patients complained of gastrointestinal symptoms such as nausea and vomiting, abdominal pain, diarrhea, and abdominal fullness at the onset of bacteremia. Major complications included bacteremic shock (63%), impaired consciousness (25%), ecthyma gangrenosum or hemorrhagic gangrenous cellulitis (18%), and jaundice (12%). Furthermore, there were one case each of endocarditis and disseminated intravascular coagulation. It was thus suggested that the clinical picture of P. aeruginosa bacteremia complicating hematologic disorders is influenced by the predisposing conditions associated with the underlying diseases and their treatment.

[Pseudomonas aeruginosa bacteremia associated with hematologic disorders [II]. Blood culture isolates and surveillance cultures].

We experienced 57 episodes of Pseudomonas aeruginosa bacteremia in 55 patients with hematologic disorders such as acute leukemia over a 16-year period. All the patients were treated on the same hospital ward. A total of 57 blood culture isolates of P. aeruginosa were divided into nine serogroups. Seventy-four percent of the isolates belonged to four serogroups, which became preponderant one after the other. Surveillance throat and/or stool cultures grew the organisms identical to the isolates from the blood at or prior to the onset of bacteremia in 75% of the episodes. Only 11% of the patients had had P. aeruginosa cultured at admission. The acquisition of the organism was closely associated with antibiotic therapy for other presumed or proved infection. On the other hand, 60% of the episodes occurred during the administration of at least one in vitro effective antibiotic. In five episodes, the patients had received an antipseudomonal penicillin and an aminoglycoside in combination, both of which proved effective in vitro against the infecting organism, when bacteremia occurred. In managing P. aeruginosa bacteremia complicating hematologic disorders, it was thus suggested that surveillance cultures should be regularly carried out, and that attention should be drawn to the occurrence of "breakthrough" bacteremia.

[Pseudomonas aeruginosa bacteremia associated with hematologic disorders [III]. Prognostic factors].

We reviewed 57 episodes of Pseudomonas aeruginosa bacteremia in 55 patients with hematologic disorders such as acute leukemia during a 16-year period, focusing especially on the prognosis. Survival at one week after onset was observed in only 39% of the episodes. Prognosis was significantly better in patients with unimicrobial bacteremia than in those with polymicrobial bacteremia (21/42 vs 1/15, p less than 0.01), in patients without shock than in those with shock (13/21 vs 9/36, p less than 0.02), in patients with granulocyte count at onset of at least 100/mm3 than in those with more marked granulocytopenia (10/13 vs 12/44, p less than 0.01), in patients with an increase in granulocyte count by at least 100/mm3 during their infection than in those without any subsequent increase (18/18 vs 4/13, p less than 0.001), and in patients with total serum protein level at onset of at least 6.0 g/dl than in those with hypoproteinemia (18/32 vs 4/25, p less than 0.01). Patients with bacteremia secondary to urogenital infection tended to have a higher one-week survival rate than those with pneumonia followed by bacteremia (4/8, 50% vs 2/9, 22%). With regard to the antibiotic treatment of unimicrobial bacteremia, 14 (70%) of 20 patients receiving therapy with one or two anti-pseudomonal beta-lactam antibiotics and an aminoglycoside in combination that were effective in vitro against the infecting organism survived, and so did only seven (32%) of 22 patients receiving therapy with either one in vitro effective beta-lactam or aminoglycoside or inadequate drugs (p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

Development and aging of brain midline structures: assessment with MR imaging.

The development and aging of four brain midline structures--the pituitary gland, pons, cerebellar vermis, and corpus callosum--were studied. The dimensions and area of these structures were measured by means of midsagittal magnetic resonance imaging. The study group consisted of 94 patients newborn to 15 years old and 49 patients and seven volunteers 16-60 years old. Except for growth spurts in the 1st year and in the 10-15-year age range, the pituitary gland showed linear growth. The pons, cerebellar vermis, and corpus callosum all showed exponential growth. The cerebellar vermis showed the sharpest 1st-year growth spurt, followed by the corpus callosum and the pons. The pituitary gland showed a decrease in size in the 51-60-year age range. The corpus callosum also showed a tendency to diminish in size but to a lesser degree. There were no statistically significant declines in the size of either the pons or the cerebellar vermis in the 51-60-year age range.

A multifunctional cytokine (IL-6/BSF-2) and its receptor.

Interleukin 6 (IL-6)/B cell stimulatory factor 2 is a multifunctional cytokine produced by both lymphoid and nonlymphoid cells. IL-6 regulates immune response, acute phase reaction, and hematopoiesis. It was found that IL-6 production by T cells is dependent on macrophages, and IL-6 is one of essential factors for pokeweed mitogen induced immunoglobulin production. Both high- and low-affinity IL-6 receptors were identified. The molecular cloning of IL-6 receptors demonstrated that this receptor is a member of the immunoglobulin superfamily. The deregulated production of IL-6 is suggested to be involved in the pathogenesis of autoimmune diseases and in the development of multiple myeloma.

Azoospermic male with a balanced Y-autosome translocation.

Structural chromosomal aberrations, such as translocations or inversions, may cause spermatogenic failure. We report a rare chromosomal anomaly 46,X,t(Y;3)(q12;p21) in an azoospermic male with normal phenotype. Twenty-two cases of a balanced Y-autosome translocation have been reported in the literature and the majority of them have been detected among azoospermic males at male infertility clinics. This structural chromosomal anomaly brings spermatogenic arrest at the primary or secondary spermatocyte.

Cytogenetic survey of subfertile males in Japan.

Chromosomal analysis of 554 consecutive Japanese male patients attending a subfertility clinic revealed 18 (3.2%) major chromosomal aberrations. The incidence of sex chromosomal anomalies and autosomal anomalies is 1.3 and 2.0%, respectively, which is significantly higher than that for the general population. All 7 sex chromosomal anomalies were found among the azoo- or oligozoospermic patients, while the incidence of autosomal anomalies for the normozoospermic patients was statistically similar to that for the azoo- or oligozoospermic patients. Therefore, cytogenetic analysis is recommended not only for patients with reduced sperm density but also for subfertile patients with normozoospermia.

Effect of vasopressin antagonist on antidiuresis by oxotremorine microinjected into the hypothalamic supraoptic and paraventricular nuclei in a water-loaded and ethanol-anesthetized rat.

Microinjection of the muscarinic agonist oxotremorine into the hypothalamic supraoptic (SON) and paraventricular (PVN) nuclei which contain cell bodies of vasopressinergic neurons induced potent antidiuretic effects in water-loaded and ethanol-anesthetized rats. The effects included both decreases in urine outflow and increases in urine osmotic pressure. However, no significant changes in various visceral functions other than antidiuresis such as mean blood pressure, heart rate, respiration rate and rectal temperature were observed when oxotremorine was microinjected into the SON. Only a slight change in mean blood pressure (approx. 10 mmHg decrease) was observed by the microinjection into the PVN. Intravenous preinjection of a vasopressin (AVP) V1 V2 antagonist that has one of the most potent V2 (antidiuretic)-antagonist activities, d(CH2)5-D-Tyr(Et)VAVP, inhibited nearly completely the antidiuretic effects induced by the microinjection of oxotremorine. The results demonstrated that oxotremorine stimulated muscarinic receptors in the hypothalamic SON and PVN, released AVP and induced an antidiuretic effect through AVP-receptors in the kidney.

Evaluation of microencapsulated islets in agarose gel as bioartificial pancreas by studies of hormone secretion in culture and by xenotransplantation.

Islets were microencapsulated in agarose gel for examination of the possible use of microencapsulated islets as a bioartificial pancreas. Microencapsulated islets secreted insulin into the culture medium (RPMI-1640) and could rapidly increase their insulin release in response to a glucose challenge even after greater than 100 days. Hamster islets in groups of 400-1000 encapsulated in microbeads containing 11-14% (wt/wt) agarose were xenogenically transplanted into the peritoneal cavity of five diabetic mice. The longest normoglycemic period in these mice was 53 days, which was markedly longer than the normoglycemic period obtained by nonencapsulated islets. Agarose seems to be a suitable basic material for encapsulating islets, because the islets can easily be microencapsulated without any adverse effect on the islet function.

On the presence and function of closed lymphatic stomata in the diaphragm of the golden hamster.

The peritoneal surface of the golden hamster diaphragm was examined for closed lymphatic stomata by scanning electron microscopy (SEM) and serial sectioning. Closed stomata were absent on SEM but present on serial sectioning. Some closed stomata in the serial sections were reconstructed using a computerized image analysis system. They were less than 10 microns in diameter and consisted of an outer mesothelial margin and an inner lymphatic wall. The lymphatic wall was formed by several endothelial cells adjoined with various junctional abutments. The discrepancy of results between SEM and serial sectioning, and the functional aspects of lymphatic stomata are discussed.

Bronchoconstrictive properties and potentiating effect on bronchial responsiveness of inhaled thromboxane A2 analogue (STA2) in guinea pigs.

Effect of subthreshold concentration of inhaled STA2, a thromboxane A2 (TXA2) analogue, on bronchial responsiveness to histamine was investigated in anesthetized and artificially ventilated guinea pigs. Percent increase in pressure of the airway opening (Pao) by aerosol histamine (50, 100 micrograms/ml) was significantly potentiated by subthreshold dose of aerosol STA2 (0.10 micrograms/ml) which was determined by dose-response curve of % increase in Pao by inhaled STA2 (0.033, 0.10, 0.33, 1.0 micrograms/ml). These results demonstrated that thromboxane A2 could contribute to bronchial hyperresponsiveness which is one of the major clinical features of bronchial asthma.

Current concepts of B cell modulation.

Three interleukins with distinct functions, IL-4, IL-5 and IL-6, are involved in the regulation of B cell response into antibody producing cells. The studies with recombinant interleukins, however, demonstrated that the activities of these interleukins were not restricted to B lineage cells but showed a wide variety of biological functions on various tissues and cells. One of the most typical example of multifunctional interleukins is IL-6. It acts not only on B cells as B cell differentiation factor but also on T cells, hematopoietic stem cells, hepatocytes, nerve cells and myeloma cells. Deregulation of the expression of these interleukins was shown to be responsible for various diseases, such as i) IL-4 vs. immediate type hypersensitivity and ii) IL-6 vs. autoimmunity and multiple myelomas.

Plasma glycosyltransferase activity after ABO-incompatible bone marrow transplantation and development of an inhibitor for glycosyltransferase activity.

Plasma glycosyltransferase activities were studied in eight patients after ABO-incompatible bone marrow transplantation. The ABO red blood cell type completely changed from the recipient type to the donor type; however, preexistent plasma glycosyltransferase activities of the recipient type did not change in seven of eight patients after marrow transplantation. Weak transferase activities of the donor type were observed in all of the patients after marrow grafting. One patient with acute and chronic graft-versus-host disease produced a very potent inhibitor that was active on both A- and B-transferase activities. Because this inhibitory activity was absorbed by a protein A-coupled Sepharose column, it was strongly suggested that this inhibitory activity was mediated by an IgG antibody for a transferase.

Osteoma cutis associated with disordered ossification of the clavicle. A case report.

Congenital multiple osteomas of the skin associated with disordered ossification of the clavicle occurred in a four-year-old boy. There were no clinical and laboratory findings of pseudohypoparathyroidism, pseudopseudohypoparathyroidism, or fibrodysplasia ossificans progressiva in this case. The clavicular bony mass was excised in segments. Samples of the cutaneous osteomas were removed for histologic study. On microscopic examination, the specimen from the clavicle consisted of trabecular bone in hyaline cartilage, resembling enchondral ossification, but the specimen from the osteoma cutis revealed a mature membraneous bone without cartilage.

[Study of long-survival cases with liver metastasis of colorectal cancer by hepatic arterial infusion chemotherapy].

We performed arterial infusion chemotherapy through hepatic artery in 33 patients with liver metastasis of colorectal cancer. Regarding the survival rate, the 1-year survival rate was 55.6% and the 5-year rate was 27.8% in cases of H2, and 1-year as 28.9% in H3. According to criteria of Saito-Koyama, the response rate was 24.2%, including 2 CR and 6 PR cases. There were more patients with H2 factor and without peritoneal dissemination who survived for one year and more. The dose of 5-FU was 9.6 +/- 3.6 g in case of longer survivals and 5.1 +/- 3.0 g in non-responders. There were significant differences between them. The pretreatment value of CEA was lower than 100 ng/ml in 8 of 11 longer survivals, and the CEA values were decreased after this treatment in 9 of 11 responders. On the other hand, it was noted that the non-responders had a higher level of CEA such as more than 100 ng/ml, and in these cases CEA was not decreased after treatment. There are two favourable responders who are still living 9 and 5 years after successful treatment. Their pretreatment value of CEA was lower at 13.6 and 23.8 ng/ml, respectively, and these levels were decreased to 1.0 ng/ml with administration of 5-FU with ADM and 5-FU with MMC. It was concluded that arterial infusion chemotherapy was good for treatment of metastatic liver tumors from colorectal cancers.
