Concentrations of Tegafur, 5-FU, and Uracil were measured in the tissues and serum of 32 patients with malignant head and neck tumors after administration of UFT or UFT and OK-432. Of the 32 patients, 15 patients were treated with UFT alone and 17 patients with a combination of UFT and a clinical dose of OK-432. The level of 5-FU was higher in the tissues and serum in the patients who received the combined administration of UFT and OK-432 than in those the received UFT alone. In particular, levels of 5-FU in tumor tissue and metastatic lymph node tissue were higher than 0.05 micrograms/g even 14.6 hours after the last administration of UFT. No significant undesirable interactions between UFT and OK-432 were exhibited during combination therapy of these agents in clinically administered doses. From our satisfactory results, clinical cures would be expected by the combined immuno-chemotherapeutic action of these agents.
Using the IL-6 dependent hybridoma, MH60.BSF2, we measured urinary levels of interleukin 6 (IL-6) in 29 patients with active lupus nephritis. We detected IL-6 activity in the urine of 24 (83%) of 29 patients before the initiation of therapy. The median value of urinary IL-6 levels in patients with a histologic diagnosis of WHO class IV on renal biopsy was significantly higher than that in patients with other classes (p < 0.01). After treatment, urinary levels of IL-6 decreased significantly (p < 0.001). These data suggest that urinary levels of IL-6 may be a valuable tool for monitoring the progression of lupus nephritis.
We treated two patients with thrombotic thrombocytopenic purpura (TTP) with multiple plasma exchanges for a short period. Following 6 plasma exchanges during 11 days in patient 1 and the 8 exchanges during 13 days in patient 2, consciousness disturbance and thrombocytopenia improved with a decrease in serum lactate dehydrogenase (LDH) levels in both patients. However, thrombocytopenia rapidly progressed with a rise of serum LDH levels after a few days from cessation of the treatment. They received a few rounds of plasma exchange without beneficial effects and eventually died of massive hemorrhage. The clinical course of these patients indicates that some patients with TTP are refractory to repeated plasma exchanges for a short period. It seems important to determine the target point in blood chemistry values such as the LDH level for plasma exchange at which the treatment could be discontinued without causing relapse of TTP.
We report a rare case of pleural involvement followed by idiopathic retroperitoneal fibrosis in a 55-year-old man. He complained of right chest pain and his chest radiogram showed pleural thickening and calcification. Chest X-ray computed tomogram (CT) also showed a mass-like pleural thickening and a soft tissue density lesion surrounding the thoracic aorta, suggesting fibrosis. By open pleural biopsy, the mass-like pleural lesion was diagnosed as chronic pleuritis. Three years later, the patient experienced difficulty in urination, and bilateral hydronephrosis which was probably due to a retroperitoneal lesion was detected. Abdominal X-ray CT revealed a retroperitoneal lesion of soft tissue density comprised of a smooth and clearly delineated mass which encased the large vessels and ureters, compatible with retroperitoneal fibrosis. Although it was not possible to biopsy the retroperitoneal lesion, the lesion was clinically and radiologically thought to be retroperitoneal fibrosis. We could not determine the etiology of these findings, but such a case of pleural involvement followed by idiopathic retroperitoneal fibrosis was considered to be very rare.

A case of müellerian duct cyst with prostate cancer is reported. The patient was a 64-year-old male, complaining of hemospermia. Prostate cancer was suspected by rectal examination and a cystic mass was detected between the ejaculatory ducts by computed tomography and magnetic resonance imaging. He underwent a transperineal needle aspiration of the cyst. The fluid did not contain spermatozoa. A adenocarcinoma was detected from the prostate by needle biopsy. Therefore, a radical prostatectomy with the resection of the cyst was performed. This is the first report about the resection of müellerian duct cyst with the organs around it in Japan.
The main cause of DIC (disseminated intravascular syndrome) is contact of tissue factor with circulating blood. The main symptoms of DIC are bleeding diathesis caused by consumption coagulopathy and organ dysfunction related to circulatory disturbances due to multiple thrombi. However, the symptoms of DIC differ according to degree of fibrinolysis which is characterized by causal disease of DIC. Usually, enhanced fibrinolysis does not cause organ failure but hemorrhagic diathesis, while impaired fibrinolysis does not cause severe bleeding but organ dysfunction. In almost all cases of acute leukemia and in some cases of solid cancer with DIC, hyperfibrinolysis is common. In cases of severe infection with DIC, impaired fibrinolysis due to abnormal elevation of plasminogen activator inhibitor-1 is frequently seen.

A case of retrocaval ureter in a 10-year-old boy is reported. His chief complaint was asymptomatic total macrohematuria. Physical examination and all laboratory findings were within normal limits, but drip infusion pyelography revealed a right hydronephrosis with "S shaped" ureter. The diagnosis was confirmed by enhanced CT scan. After follow-up for 1 year and a half, right uretero -ureterostomy was performed because the severity of the hydronephrosis had increased. We review 25 cases of retrocaval ureter in childhood reported in the Japanese literature and compare the clinical characteristics between infant cases and adult cases.
A case of cholesterol granuloma with hematoma of the tunica albuginea is reported. A 52-year-old man complained of a painless mass in the left scrotum. The mass was 50 x 25 x 30 mm in size. An operation was performed. Macroscopically the mass originated from the tunica albuginea and was a cystic lesion with a thick fibrous capsule. The cystic lesion was filled with an old hematoma. An extirpation was performed. Microscopically, the sections showed fibrogranulomatous tissue containing innumerable cholesterol clefts and numerous foreign body giant cells. The histological diagnosis was cholesterol granuloma with hematoma. This is the sixth case of cholesterol granuloma of the external genitalia, and is the first case of cholesterol granuloma with hematoma of the tunica albuginea in the literature.
We report a case of chylothorax in a 51-year-old male with non-Hodgkin's lymphoma. Combination chemotherapy reduced the size of mediastinal lymph nodes dramatically, but retroperitoneal lymph nodes remained almost the same, and chylothorax subsequently developed. Lymphangiogram showed reticular spread of contrast material from the thoracic duct downwards but not above the diaphragm. These findings suggest that an obstruction of the infra-diaphragmatic but not the supra-diaphragmatic thoracic duct caused development of collateral lymphatic channels penetrating the diaphragm into the thoracic cavity, with subsequent formation of chylothorax.
A nationwide survey was conducted by the Japan Society of Hyperthermic Oncology (JSHO) to investigate the present status of hyperthermia in Japan. The results of survey and characteristic clinical research conducted in Japan are described here. 1) In Japan, 185 heating equipment units are in operation at 156 institutes. RF capacitive heating equipment accounts for 60%. 2) Records and registration are complete for 9,000 cases treated by hyperthermia. It is noteworthy that deep-seated tumors account for 60% of the total cases. 3) The percentages of different modalities are as follows: hyperthermia alone, 5.3%; thermoradiotherapy, 57.5%; thermochemotherapy, 22.6%; thermochemoradiotherapy, 14.5%. The present status of hyperthermia is well represented by these percentages. 4) The treatment schedule, indications, and treatment results are almost completely established for a combination of radiotherapy with hyperthermia. 5) Thermochemotherapy is performed at 70% of the surveyed institutes, who expect good treatment results. The JSHO organized a joint research team for thermochemotherapy, and present results are satisfactory. 6) Japan contributes much to the progress of the International Congress of Hyperthermic Oncology. The JSHO presents an ideal environment for research on hyperthermic oncology.
Laparoscopic surgery is often associated with complications which never occur in open conventional surgery. We investigated operative and postoperative complications in urological laparoscopic surgery. From February 1990 to November 1992, 91 laparoscopic operations were performed, including 62 varicocelectomies, 5 pelvic lymphadenectomies, 6 adrenalectomies, 5 simple nephrectomies, 2 radical nephrectomies, 1 staged Fowler-Stephens operation and 10 laparoscopies for cryptorchidism or other disorders. The patients ranged from 3 months to 77 years old. Nine (9.9%) of the 91 patients had complications associated with laparoscopic procedures and 4 (4.4%) of them had major complications. Pneumoextraperitoneum occurred in 3 patients. Two patients, who suffered from preperitoneal insufflation or pneumothorax associated with pneumomediastinum, failed to undergo laparoscopy. A small amount of scrotal emphysema developed in one patient, which subsided within 12 hours. Another patient, who underwent radical nephrectomy and para-aortic lymphadenectomy for left renal cell carcinoma, complained of symptoms due to persistent pneumoperitoneum and subcutaneous emphysema, which subsided 2 weeks after the operation. Respiratory acidosis caused by carbon dioxide absorption, which was difficult to be controlled, occurred in the other two patients; one was an adult male with reduced respiratory function and the other was a 1-year-and-8-month-old child. Bleeding during or after the procedure was another serious complication. Vena caval laceration occurred during right simple nephrectomy for pyonephrosis with renal calculi and was managed with laparotomy. Two patients, one after simple nephrectomy and the other after right adrenalectomy, suffered from postoperative bleeding about 300 ml through the penrose drain, which subsided following a bed rest.(ABSTRACT TRUNCATED AT 250 WORDS)
A 24-year-old man underwent right radical orchiectomy for testicular teratocarcinoma in May 1982, which was followed by retroperitoneal lymph node dissection. The patient showed no evidence of recurrent disease at followup examinations. The patient later reported a complete loss of ejaculation postoperatively. Erection and orgasm remained normal. Prior use of a sympathomimetic agent, imipramine hydrochloride, was unsuccessful in producing an antegrade ejaculation. Pregnancy was achieved following the fifth attempt at artificial insemination using semen retrieved from the bladder by the modified Hotchkiss method. A healthy male infant, weighing 3,150 g, was delivered. The treatment modality for male infertility due to retrograde ejaculation after retroperitoneal lymph node dissection for testicular cancer is discussed.
A congenitally dysfunctional form of prothrombin, Prothrombin Himi, shows reduced fibrinogen clotting activity, although it retains full hydrolytic activity toward synthetic substrates. To elucidate the structural abnormality of the variant prothrombin, we first performed genetic analysis of dysprothrombin. Polymerase chain reaction amplification of the exons 8 through 14 of the proband and her family members' prothrombin genes, which code the thrombin moiety, followed by single-strand conformation polymorphism analysis, identified two variant conformers in exon 10 specific to this family. One variant allele detected in the father was inherited by the proband and one of her sisters, and the other detected in the mother was also inherited by them. This result indicates that the proband has two different base pair changes in the gene. Sequencing showed two novel point mutations in the proband's gene. One is a T to C transition at position 8751, resulting in the substitution of threonine for methionine at codon 337 (Thrombin Himi I). The other is a G to A transition at 8904, resulting in the substitution of histidine for arginine at codon 388 (Thrombin Himi II). By sequencing analysis of her parents, it was determined that Thrombin Himi I was inherited from the father and Thrombin Himi II from the mother. These results confirm that Prothrombin Himi is compound heterozygous for two dysfunctional prothrombin molecules.

Repeated treatment with 8-hydroxy-2-(di-n-propylamino)tetralin (8-OH-DPAT) resulted in significant attenuation of 8-OH-DPAT-induced hypothermia and adrenocorticol effect in mice of both sexes, while it did not affect the 8-OH-DPAT-induced decrease in 5-hydroxyindoleacetic acid in the hypothalamus in either sex. The attenuated responses developed more rapidly in female than in male mice, indicating sex differences in the adaptive regulation of the 5-HT1A receptor-mediated responses.
