Acute myelomonocytic leukaemia with 11q23 abnormality during multiple myeloma: is this related to anthracycline?

The clinical course of a 66-year-old man diagnosed with multiple myeloma is described. Chemotherapy including alkylating agents had no effect, and so he was treated with vincristine, doxorubicin, and dexamethasone. His bone tumor was treated with localized radiation after two courses of chemotherapy. After these treatments, monocytosis was found and dysplastic changes were noted in the bone marrow. A cytogenetic study revealed t(9;11)(p22;q23), an abnormality which had previously been absent. A diagnosis of myelodysplastic syndrome (MDS) was newly established, and transformation to acute non-lymphocytic leukaemia (ANLL) was observed 6 months later. The cumulative doses of melphalan, cyclophosphamide, doxorubicin, and radiation therapy were 432 mg, 4,200 mg, 120 mg, and 4,000 cGy, respectively. The cytogenetic abnormality suggested that this patient's MDS/ANLL was related to doxorubicin and not talk to alkylating agents, although the dose of doxorubicin administered was quite low.

Laparoscopic treatment of a retrocaval ureter.

A retrocaval ureter was treated laparoscopically with good success. The retrocaval segment of the ureter was easily separated and the ureter was reanastomosed using a laparoscopic suturing technique. Laparoscopic surgery is useful for the treatment of a retrocaval ureter with minimal postoperative pain and a short convalescence.

Respiratory effects of CO2 pneumoperitoneum during transperitoneal laparoscopic urological surgery.

OBJECTIVE: To evaluate the effects of CO2 pneumoperitoneum on respiratory function during urological laparoscopic surgery with exposure of the retroperitoneal space in the lateral decubitus position. METHODS: Arterial blood gas analysis and measurements of the partial pressure of CO2 in exhaled gas (PetCO2) were performed during CO2 pneumoperitoneum in 12 patients who underwent transperitoneal laparoscopic surgery for the retroperitoneal organs (the retroperitoneal group): the results were compared to those of 11 patients who underwent laparoscopic varicocelectomy (the varicocele group) with the same anesthesia and intra-abdominal pressure. RESULTS: The increase in PaCO2 and PetCO2 and the decrease in arterial pH during CO2 insufflation were significantly greater in the retroperitoneal group than in the varicocele group. PaCO2, PetCO2 and arterial pH continued to change in the retroperitoneal group, whereas these values reached a plateau at 30 min after the beginning of CO2 insufflation in the varicocele group. CONCLUSIONS: The respiratory effects of CO2 pneumoperitoneum are more pronounced during laparoscopic urological surgery with opening of the retroperitoneum. It is possible that exposure of the retroperitoneal space increased CO2 absorption during insufflation.

Mathematical simulation of unidirectional tissue formation: in vitro transanastomotic endothelialization model.

In vitro transanastomotic endothelialization was studied using a mathematical model with the Fisher equation. The Fisher equation is a nonlinear parabolic equation which has a cell migration term and a population growth term. The mathematical model was used to simulate recent experiments performed to investigate quantitatively the unidirectional formation of a bovine endothelial cell monolayer in vitro. The two parameters included in the equation were estimated using a trial and error method in which the calculated solutions of the various values of the two parameters are compared with the experimental data and the best fit pair is adopted: one parameter, D, which represents the unidirectional cell migration rate and the other, k, which represents the population growth rate. The calculated solutions fit the experimental data well. We also simulated the healing of a mechanically disrupted endothelial monolayer sheet. The significance of cellular biomechanics in tissue formation and design of tissue-engineered devices is discussed.

Female gender as a determinant of cough threshold to inhaled capsaicin.

Chronic, nonproductive cough and cough associated with the use of angiotensin converting enzyme inhibitors, are more frequently observed in females as compared to males. To examine the influence of sex, age, height, weight and pulmonary function on airway cough sensitivity, cough threshold to inhaled capsaicin, an index of the airway cough sensitivity, was measured in 160 nonsmoking, nonatopic healthy subjects. Forty young males (aged 24 +/- 2 yrs) 40 young females (aged 22 +/- 2 yrs) 40 middle-aged males (aged 48 +/- 5 yrs) and 40 middle-aged females (aged 50 +/- 7 yrs) were studied. The cough threshold was defined as the lowest concentration of inhaled capsaicin causing five or more coughs. The cough threshold was 3-5 fold lower in females than in males both in young (p<0.001) and middle-aged (p<0.005) subjects. Cough threshold was weakly but significantly correlated to height, weight, forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) when all subjects were considered together but not when each group was considered separately. Multiple regression analysis revealed that sex difference was the significant predictive factor for the cough threshold in either age group. These results confirm that cough sensitivity is heightened in females and suggest that influence of height and pulmonary function on the cough threshold may have resulted from sex difference.

Involvement of PAF in postallergic propranolol-induced bronchoconstriction in guinea-pigs.

Administration of propranolol can provoke bronchoconstriction in asthmatic patients. Recently, we successfully developed a guinea-pig model for propranolol-induced bronchoconstriction (PIB). We hypothesized that such bronchoconstriction may result from the inflammatory mediators released by an allergic reaction. The purpose of this study was to examine the role of platelet-activating factor (PAF) in the development of PIB after allergic reaction. Propranolol, at a concentration of 10 mg.mL-1 was inhaled 20 min after antigen challenge in passively sensitized, anaesthetized and artificially-ventilated guinea-pigs. The animals were treated intravenously with PAF antagonists, E6123 (1 and 10 micrograms.kg-1) or Y-24180 (1 and 10 mg.kg-1), 10 min before or 15 min after antigen challenge. Propranolol inhaled 20 min after antigen challenge caused bronchoconstriction. E6123 and Y-24180 administered 15 min after antigen challenge as well as 10 min before antigen challenge reduced the PIB in a dose-dependent manner. We conclude that platelet-activating factor may contribute to the development of propranolol-induced bronchoconstriction after allergic reaction in our guinea-pig model.

Peptidase inhibitor-induced antidiuresis mediated through angiotensin and opioid receptors in the rat hypothalamus.

We examined the effects on urine outflow rate after microinjections of thiorphan (a carboxypeptidase inhibitor) and bestatin (an aminopeptidase inhibitor) into the hypothalamic paraventricular (PVN) and supraoptic (SON) nuclei of anesthetized hydrated rats to determine the possible role of neuropeptides in the regulation of urine production. After individual microinjection of the peptidase inhibitors into the nuclei, only thiorphan at 100 nmol administered into the PVN significantly decreased the urine outflow rate. Two consecutive microinjections of the peptidase inhibitors at 100 nmol each into the nuclei induced potent antidiuresis. These effects after microinjections of the peptidase inhibitors into the PVN and SON were diminished by pretreatment with [Sar1,Ile8]angiotensin (ANG) II (an ANG II receptor antagonist) and naloxone (an opioid receptor antagonist) in the PVN and with [Sar1,Ile8]ANG II in the SON, respectively. A vasopressin (AVP) receptor antagonist, d(CH2)5-D-Tyr(Et)VAVP (i.v.), completely blocked the antidiuresis by microinjections of the peptidase inhibitors into both the nuclei. Urinary osmotic pressure was significantly increased by consecutive microinjections of the peptidase inhibitors into the PVN and SON. These results suggest that endogenously-released ANG II and opioid peptides in the PVN and ANG II in the SON regulate urine production mediated through increased AVP release.

Adrenergic neural connections between the bilateral supraoptic nuclei of the rat hypothalamus.

Our previous study has demonstrated that unilateral microinjection of norepinephrine (NE) into the right supraoptic nucleus (SON) of anesthetized hydrated rats elicited dose-dependent decreases in the urine outflow rate. This was antagonized by pretreatment with phenoxybenzamine (an alpha-antagonist) and timolol (a beta-antagonist) in the same SON. In the present study, we examined the effects of NE, microinjected into the right, left and bilateral SON, on the urine outflow rate in order to investigate neural connections between the bilateral SON. NE administered by those three routes dose-dependently decreased the urine outflow rate. The order for the antidiuretic potency was as follows: the effect elicited by the intrabilateral-SON microinjection > the intra-left-SON microinjection = the intra-right-SON microinjection. The antidiuresis of NE microinjected into the right SON was inhibited by an electrolytic left-SON lesion and by pretreatment with phenoxybenzamine (20 nmol) and timolol (100 nmol), but not by atropine (300 nmol) in the left SON. These findings suggest adrenergic neural connections from the right to left SON, contributing to the regulation of urine production. Furthermore, there is a possibility that stimulation of endogenously-released NE in the bilateral SON is amplified through these neurons and elicits more potent effects than those produced in either the right or left nucleus.

Ouabain-induced cell proliferation in cultured rat astrocytes.

Ouabain markedly stimulated not only [3H]thymidine incorporation but also [3H]uridine incorporation into astrocytes. The effects were observed at 36-48 hr and 12-72 hr after addition of ouabain, respectively. The dose-response curves were both bell-shaped types with a peak at 10(-3) M for thymidine incorporation and 2 x 10(-3) M for uridine incorporation. Ouabain increased cell number as determined by an assay using 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide and by a method using a hemocytometer. Low concentration of external K+ mimicked the effect of ouabain in stimulating [3H]-thymidine incorporation, and high concentration of external K+ blocked the effect of ouabain. In contrast to astrocytes, ouabain did not stimulate [3H]thymidine incorporation into C6 glioma and fibroblast cells. The effect of ouabain on [3H]thymidine incorporation in astrocytes was dependent on external Ca2+, and it was blocked by cycloheximide. These findings indicate that prolonged Na+, K(+)-ATPase inhibition causes cell proliferation in cultured astrocytes in cell-specific and Ca(2+)-dependent manners.

Ca2+ depletion facilitates taurine release in cultured rat astrocytes.

Removal of external Ca2+ facilitated endogenous taurine release in cultured rat astrocytes. The stimulated release was not affected by furosemide, sucrose, tetrodotoxin and 3,4-dichlorobenzamil, but partially inhibited by nifedipine. Omission of external Na+ increased basal taurine release, and the effects of Na+ removal and Ca2+ depletion on the release were additive. The Na(+)-free condition did not affect Ca2+ paradox-induced cell death in astrocytes. These findings suggest that Ca2+ depletion facilitates taurine release in a mechanism independent of volume and the Na+ gradient and that the release is not involved in Ca2+ paradox-induced delayed cell toxicity in astrocytes.

Immunochemical and biochemical identification of the rice seed protein encoded by cDNA clone A3-12.

Previously isolated cDNA clone A3-12 that was expressed in E. coli as the fusion protein with Trp E showed immunoreactivity with the mouse antibody raised against isolated alpha-globulin from rice seed. The N-terminal amino acid sequences determined for the purified alpha-globulin and its tryptic peptides were identical with the deduced amino acid sequence reported, except for two residues at the protein N terminus. An error in the reported sequence was confirmed by re-sequencing the cDNA, the nucleotide sequence for the two N-terminal residues being shown to be CAGCTG and not CACGTG. Thus, the protein encoded by cDNA clone A3-12 was identified to be the major rice seed globulin, alpha-globulin, with an apparent molecular mass of 26kDa.

Rice allergenic protein and molecular-genetic approach for hypoallergenic rice.

Allergenic proteins with a molecular mass of about 14 to 16 kDa were isolated from a rice salt-soluble fraction based on the reactivity with IgE antibodies from patients allergic to rice. cDNA clones encoding these allergenic proteins were isolated from a cDNA library of maturing rice seeds, and the deduced amino acid sequences showed considerable similarity to wheat and barley alpha-amylase/trypsin inhibitors, which have recently been identified as major allergens associated with baker's asthma. An antisense RNA strategy was applied to repress the allergen gene expression in maturing rice seeds. Immunoblotting and ELISA analyses of the seeds using a monoclonal antibody to a 16-kDa allergen showed that allergen content of seeds from several transgenic rice plants was markedly lower than that of the seeds from parental wild type rice.

Effects of the glycoprotein sugar chains on the amino-carbonyl reaction of chicken and Japanese quail ovomucoids with glucose.

The amino-carbonyl reactions of two glycoproteins, chicken ovomucoid and quail ovomucoid, which have similar amino acid sequences but different sugar chains, were compared. The free guanidino groups, but not amino groups, of chicken ovomucoid decreased more rapidly than those of quail ovomucoid during reaction with glucose. Protein polymerization and formation or brownish or fluorescent compounds also occurred more quickly in the chicken ovomucoid. These results suggested that the sugar chain moieties affect the amino-carbonyl reaction of glycoproteins with deducing sugars, especially at advanced stages of the reaction.

Characterization of the bovine kappa-casein gene promoter.

kappa-Casein gene promoter was localized within a 570-bp fragment (-552/+18) of a 5'-flanking region by the gene transfection assay. Deletion mutation analysis in mammary epithelial cell line, HC11, suggested that there are regulatory element in a region from -439 through -125. Some nuclear proteins from lactating rate mammary gland bind to this region specifically. One of them expressed preferentially during pregnancy bound to a 132-bp fragment (-439/-308) and another expressed preferentially during lactation bound to a 183-bp fragment (-307/-125).

Rhabdomyolysis complicating polymicrobial sepsis in a patient with acute leukemia.

Rhabdomyolysis with myoglobinuria is an uncommon complication of sepsis, whether monomicrobial or polymicrobial, even in its severe form. We describe a middle-aged woman with acute leukemia who developed rhabdomyolysis and myoglobinuria during the fatal course of chemotherapy-induced sepsis due to Bacteroides thetaiotaomicron and Enterococcus faecalis, followed by multiple organ dysfunction syndrome. In addition, autopsy revealed disseminated infections with Aspergillus and Candida. None of these organisms has been reported to be involved in the pathogenesis of rhabdomyolysis. Therefore, the etiology of the rhabdomyolysis in this case was probably multifactorial, with polymicrobial septic processes being possibly important contributing factors.

Pulmonary mucormycosis in a hematology ward.

During a 24-year period, seven patients with leukemia undergoing cytotoxic or immunosuppressive therapy developed pulmonary mucormycosis in our hematology ward. The autopsy incidence in patients with acute leukemia was 2.1%, with no significant rise during the last decade. Most cases occurred in early autumn. Two patients diagnosed premortem were successfully treated with antifungal drugs, whereas five patients diagnosed postmortem died within three weeks following radiographic identification. Persistent fever despite antibiotic coverage was the first clinical presentation, followed mostly by the radiographic appearance of rounded densities, subsequently progressing to hemorrhagic infarction with hemoptysis and/or pleuritic pain. Crescentic cavitation developed when hematologic remission was achieved. Thus, certain radiographic patterns with or without pulmonary infarct syndrome in the proper clinical setting should arouse suspicion of mucormycosis, providing the opportunity for early diagnosis and adequate treatment.

Guillain-Barré syndrome and ethylene diamine tetraacetic acid-dependent pseudothrombocytopenia associated with mumps.

A 35-year-old man with Guillain-Barré syndrome and ethylene diamine tetraacetic acid (EDTA)-dependent pseudothrombocytopenia associated with serologically-confirmed mumps is presented. A polyneuropathy developed 18 days after the onset of mumps, that improved with plasmapheresis. A decreased platelet count was observed 25 days after the onset of mumps attributed to platelet agglutination in blood anticoagulated with EDTA, and the agglutination was prevented by other anticoagulants. Guillain-Barré syndrome associated with mumps is rare and EDTA-dependent pseudothrombocytopenia following mumps has never been documented. This represents the first report of Guillain-Barré syndrome and EDTA-dependent pseudothrombocytopenia following mumps. Furthermore, our findings suggest activation of the humoral immune response as a potential pathogenesis.
