Pressurization of bioactive bone cement in vitro.

We have developed a bioactive bone cement consisting of MgO-CaO-SiO2-P2O5-CaF2 glass-ceramic powder (AW glass-ceramic powder), silica glass powder as an inorganic filler, and bisphenol-a-glycidyl methacrylate (bis-GMA) based resin as an organic matrix. The efficacy of this bioactive bone cement was investigated by evaluating its pressurization in a 5-mm hole and small pores using a simulated acetabular cavity. Two types of acetabular components were used (flanged and unflanged sockets) and a commercially available polymethylmethacrylate (PMMA) bone cement (CMW 1 Radiopaque Bone Cement) was selected as a comparative control. Bioactive bone cement exerted greater intrusion volume in 5-mm holes than PMMA bone cement in both the flanged and unflanged sockets 10 minutes after pressurization (p < 0.05). In the small pores the bioactive and PMMA bone cements exerted almost identical intrusion volumes in flanged and unflanged sockets 10 min after pressurization. The intrusion volume in the flanged socket 10 minutes after pressurization was greater than that in the unflanged socket in all groups (p < 0.05). These results show that bioactive bone cement intrudes deeper into anchor holes than PMMA bone cement.

Depth and intensity of equivalent current dipoles estimated through an inverse analysis of surface electromyograms using the image method.

The depth and intensity of equivalent current dipoles that can create the surface potentials of active motor units in human skeletal muscles are estimated through an inverse analysis of surface electromyographic (EMG) potentials in an attempt to measure detailed muscular activity non-invasively. The inverse analysis is conducted by repetition of forward analyses. In the study, the image method is used for forward analysis, because it is the simplest potential calculation method for electric currents in a semi-infinite volume conductor. Using this method, surface EMG potentials are calculated for current sources assumed to be located in a muscle. An inverse analysis is then carried out by searching for the depth and intensity of such current sources that would minimise the sum of squares difference between measured and calculated surface EMG potentials. The inverse analysis is applied to surface EMG potentials measured from the biceps brachii of three healthy subjects. As a result, the individual current sources are estimated to be 2.7 +/- 1.6 mm deep and 0.5 +/- 0.9 nAm in intensity, whereas the total current intensity for individual motor units is 2.4 +/- 2.9 nAm.

Transcutaneous PCO2 monitoring in the evaluation of hyperventilation of patients with recurrent nerve paralysis.

Hyperventilation during phonation is one of the causes of fatigue in patients with vocal cord disorders. Transcutaneous (TC) PCO2 and PO2 were analyzed during phonation in patients with known recurrent nerve paralyses (RNP). There was no significant change in TCPCO2 in eight normal subjects. In cases with unilateral RNP and incomplete glottic closure, TCPCO2 decreased during phonation. This decrease in TCPCO2 resulted from hyperventilation during phonation. The TCPCO2 did not decrease in one case with RNP and complete glottic closure. In one case with incomplete glottic closure, the decrease in TCPCO2 and fatigue disappeared after treatment. These results suggest that hyperventilation is one of the causes for easy fatiguability during phonation in patients with RNP, Findings also show that measurements of TCPCO2 during phonation are useful for evaluating the cause of fatigue in patients with vocal disorders.

Quantitative evaluation of the voice range profile in patients with voice disorder.

In 1953, Calvet first displayed the fundamental frequency (pitch) and sound pressure level (intensity) of a voice on a two-dimensional plane and created a voice range profile. This profile has been used to evaluate clinically various vocal disorders, although such evaluations to date have been subjective without quantitative assessment. In the present study, a quantitative system was developed to evaluate the voice range profile utilizing a personal computer. The area of the voice range profile was defined as the voice volume. This volume was analyzed in 137 males and 175 females who were treated for various dysphonias at Kyushu University between 1984 and 1990. Ten normal subjects served as controls. The voice volume in cases with voice disorders significantly decreased irrespective of the disease and sex. Furthermore, cases having better improvement after treatment showed a tendency for the voice volume to increase. These findings illustrated the voice volume as a useful clinical test for evaluating voice control in cases with vocal disorders.

An ACTH-producing pituitary carcinoma developing Cushing's disease.

An autopsy case of an ACTH-producing pituitary carcinoma in a 59-year-old man who developed Cushing's disease is reported. The surgically removed pituitary tumor was diagnosed as chromophobe adenoma, however, pulmonary metastases appeared 2 years after the operation. Autopsy revealed a residual pituitary tumor in the sella turcica with systemic metastases to the lungs, liver, pulmonary lymph nodes, hypothalamus, dura mater, and the subarachnoid space of the midbrain and spinal cord. Immunohistochemistry revealed ACTH positivity in the tumor cells. Further immunohistochemical study showed positive high expression of Ki-67 in the tumor removed at surgery as well as in the autopsied tumor. Ki-67 labeling index provided valuable information about the invasive and proliferative potential compared to noninvasive benign pituitary adenoma.

Changes in surface EMG parameters during static and dynamic fatiguing contractions.

The effect of contraction types on muscle fiber conduction velocity (MFCV), median frequency (MDF) and mean amplitude (AMP) of surface electromyography was examined in the vastus lateralis of 19 healthy male adults. The subjects performed knee extension both statically and dynamically until they were exhausted. The static contraction was a sustained isometric extension of the knee at a joint angle of 90 degrees with 50% of the maximum voluntary contraction (MVC) load. The dynamic contraction was a repetitive isotonic extension of the knee between the angles of 90 degrees and 180 degrees with the same 50% MVC load at a frequency of 10 times per minute. MFVC during the static contraction significantly decreased during the exercise (p < 0.01). On the other hand, MFVC during the dynamic contraction did not significantly change throughout the exercise. MDF decreased and AMP increased during both types of contractions (p < 0.01). Because the blood flow within the muscle is maintained during the dynamic contraction by enhanced venous return from the contracting muscle, these results suggested that MFVC is affected by the metabolic state in the muscle and the changes in MDF cannot be explained only by that of MFVC.

Magnetic fields produced by single motor units in human skeletal muscles.

OBJECTIVE: To develop new non-invasive ways of analyzing human skeletal muscle function, biomagnetic measurement was applied to the vastus lateralis and vastus medialis of 3 healthy adult males using a 64 channel superconducting quantum interference device system. METHODS: Discharges from single motor units were detected by simultaneously recorded surface electromyography. Magnetic signals were averaged 64-158 times at zero-crossing timing in the surface electromyographic signal. RESULTS: Six motor units detected in the 3 subjects produced large magnetic fields with peak-to-peak amplitudes of 1-2 pT. Magnetomyographic isofield maps showed current sources arising from motor endplate regions and propagating in directions opposite to fiber ends. The absolute intensity of current moments in muscle fibers within motor units was estimated based on dipole fitting. The estimated moment was 23.9-114.3 nAm for repolarization dipole. Dividing these moment values by the typical dipole moment of 0.286 nAm in a single muscle fiber, the number of muscle fibers in motor unit was estimated to be 84-400. CONCLUSIONS: Magnetic recording may provide a new non-invasive way of analyzing and diagnosing human muscle function.

Molecular typing of Escherichia coli O157:H7 (H-) isolates from cattle in Japan.

A total of 77 Escherichia coli O157:H7 (H-) isolates from cattle in Japan were investigated by molecular biological methods. Most of these isolates (43 isolates) possessed the stx-2 gene, but not stx1. Fifteen bacteriophage types and 50 pulsed-field gel electrophoresis (PFGE) profiles were observed. One isolate was indistinguishable from the human outbreak strain by these methods. This indicates that cattle must be considered as a possible source of human E. coli O157:H7 infection in Japan.

Chemical studies on antioxidant mechanism of curcuminoid: analysis of radical reaction products from curcumin.

In the course of studies on the antioxidant mechanism of curcumin, its radical reaction was investigated. Curcumin was reacted with radical species, which were generated from the pyrolysis of 2, 2'-azobis(isobutyronitrile) under an oxygen atmosphere, and the reaction products from curcumin were followed by HPLC. The reaction at 70 degrees C gave several products, three of which were structurally identified to be vanillin, ferulic acid, and a dimer of curcumin after their isolation. The dimer was a newly identified compound bearing a dihydrofuran moiety, and its chemical structure was elucidated using spectroscopic analyses, especially 2D NMR techniques. A mechanism for the dimer production is proposed and its relation to curcumin's antioxidant activity discussed. The time course and gel permeation chromatography studies of the reaction were also investigated, and the results indicate that the dimer is a radical-terminated product in the initial stage.

Evaluation of the antioxidant activity of environmental plants: activity of the leaf extracts from seashore plants.

The antioxidant activity of the methanolic extracts of the leaves of 39 plant species was examined. These leaves were collected from the plants growing on subtropical seashores. The activity was evaluated by three kinds of assay methods, which included the DPPH radical scavenging assay, linoleic acid oxidation assay, and oxidative cell death assay. Two extracts from Excoecaria agallocha and Terminalia catappa showed remarkably potent activity in all assay systems. The HPLC analysis of the extracts indicated the presence of the same antioxidant and isolation work for the compound identified ellagic acid. The isolated ellagic acid showed strong antioxidant activity in the assay systems used.

Remodeling of vascular wall in Crohn's disease.

To clarify the changes in vascular resistance in Crohn's disease, we studied changes in the thickness of arterial media histometrically. The resected specimens of mesenterium and intestine from 18 patients with Crohn's disease were used. Each intestinal lesion was classified into three grades: no inflammation, exudative change, and fibrosing change. In the ileum, significant atrophy in the media of the submucosal peripheral arteries was noted during exudative and fibrosing changes, while in the submucosal larger vessels or in the mesenterium, the thickness of the arterial media did not change. In the colon, atrophy in the media of the submucosal arteries was noted in all stages except in the larger vessel during exudative change, while no significant changes were noted in the mesenterium. In conclusion, it is suggested that circulatory disturbance occurs even in the early stage in Crohn's disease, and there seems to be increased vascular resistance in the area between the deep submucosal and the distal mesenteric arteries.

Plate Tectonics at 3.8-3.7 Ga: Field Evidence from the Isua Accretionary Complex, Southern West Greenland.

A 1&rcolon;5000 scale mapping was performed in the Isukasia area of the ca. 3.8-Ga Isua supracrustal belt, southern West Greenland. The mapped area is divided into three units bounded by low-angle thrusts: the Northern, Middle, and Southern Units. The Southern Unit, the best exposed, is composed of 14 subunits (horses) with similar lithostratigraphy, bound by layer-parallel thrusts. Duplex structures are widespread in the Isua belt and vary in scale from a few meters to kilometers. Duplexing proceeded from south to north and is well documented in the relationship between link- and roof-thrusts. The reconstructed lithostratigraphy of each horse reveals a simple pattern, in ascending order, of greenstone with low-K tholeiitic composition with or without pillow lava structures, chert/banded iron-formation, and turbidites. The cherts and underlying low-K tholeiites do not contain continent- or arc-derived material. The lithostratigraphy is quite similar to Phanerozoic "oceanic plate stratigraphy," except for the abundance of mafic material in the turbidites. The evidence of duplex structures and oceanic plate stratigraphy indicates that the Isua supracrustal belt is the oldest accretionary complex in the world. The dominantly mafic turbidite composition suggests that the accretionary complex was formed in an intraoceanic environment comparable to the present-day western Pacific Ocean. The duplex polarity suggests that an older accretionary complex should occur to the south of the Isua complex. Moreover, the presence of seawater (documented by a thick, pillow, lava unit at the bottom of oceanic plate stratigraphy) indicates that the surface temperature was less than ca. 100 degrees C in the Early Archean. The oceanic geotherm for the Early Archean lithosphere as a function of age was calculated based on a model of transient half-space cooling at given parameters of surface and mantle temperatures of 100 degrees and 1450 degrees C, respectively, suggesting that the Archean oceanic lithosphere was rigid. These conclusions-rigidity and lateral plate movement-support the idea that the modern style of plate tectonics was in operation only 0.7-0.8 G.yr. after the formation of the Earth.

Endoscopic correction of pectus excavatum.

Pectus excavatum has been most commonly corrected by either the sternal elevation or turnover methods. Both of these procedures require a long skin incision in the anterior chest wall. Endoscopic techniques have been introduced into the treatment of pectus excavatum since 1994 to minimize the skin incision to approximately 1 inch. Thirty patients with pectus excavatum (25 men and 5 women) underwent surgery employing the centimeter incision method assisted by the endoscope. The mean age at the time of surgery was 11.9 years (range, 4-45 years). The patients were classified as having one of three types of pectus excavatum: Type I (symmetrical and localized) was seen in 18 patients (60%), type II (symmetrical and diffuse) was seen in 5 patients (17%), and type III (localized or diffuse but asymmetrical) was seen in 7 patients (23%). Although the results of the thoracic cage correction achieved using our procedure were excellent overall, the results were best for type I, with clear improvement achieved in the funnel index-0.48 to 0.63. All of the patients recovered well without any severe complications, and both the patients and their families found the results obtained using this method to be quite satisfactory, especially because of the minimal postoperative scar.

Early phase adaptations of muscle use and strength to isokinetic training.

The purpose of this study was to investigate the effect of short periods of isokinetic resistance training on muscle use and strength. Seven men trained the right quadriceps femoris muscles (QF) 9 d for 2 wk using 10 sets of 5 knee extensions each day. Isometric and isokinetic torques of QF were measured at six angular velocities. Cross-sectional areas (CSA) of QF were determined from axial images using magnetic resonance imaging (MRI). Transverse relaxation time (T2) and activated area of QF, which represented the area greater than the mean resting T2 + ISD in MR[pixels, were calculated at rest and immediately after repetitive isokinetic knee extensions based on T2-weighted MR images. Muscle fiber types, fiber area, and phosphofructokinase (PFK) activities were determined from biopsies of the vastus lateralis muscle. No changes were found in CSA of QF, muscle fiber types, fiber area, and PFK activities after the training. Isometric and isokinetic peak torques at 60-240 degrees x s(-1) and relative area of QF activated by knee extensions increased significantly after the training. These results suggest that muscle strength increases after short periods of isokinetic resistance training without muscle hypertrophy would be due to increased muscle contractile activity.

The available period and kind of exercise for increasing osteo sono assessment index in women.

PURPOSE: The purpose of this study is to determine the optimum period and kind of exercise which are more effective for increasing the calcaneus osteo sono assessment index (OSI) in adolescent women. METHODS: The subjects consisted of 600 female students aged from 19 to 21 studying at the Department of Food and Nutrition at Nakamura Gakuen University. The calcaneus OSI was measured by ultrasonic bone absorptiometry. The factors associated with an increased OSI were determined using piecewise linear regression analyses. RESULTS: Experience in performing impact-loading exercise either in junior high school or high school was found to be a significant predictive factor for increased calcaneus OSI after adjusting for age and weight. Regarding other exercise-related factors such as subjective intensity, frequency, hours at a time, and total duration of exercise, a piecewise liner regression analysis showed that a total duration of over 7 yr and a high subjective intensity of the impact-loading exercise during high school and junior high school periods caused a significant increase in the calcaneus OSI. However, the OSI decreased when the subjective intensity of the impact-loading exercise during the high school period was reported to be "very strenuous." CONCLUSIONS: Impact-loading exercise in junior high school and high school is important for increasing calcaneus OSI; however, impact-loading exercise in high school with a "very strenuous" subjective intensity was also found to possibly decrease the calcaneus OSI.

Primary hepatic lymphoma associated with primary biliary cirrhosis.

We report a case of primary hepatic lymphoma in a 55-yr-old female patient with primary biliary cirrhosis and Sjögren's syndrome. On July 1994, a tumor measuring 11 mm in diameter was detected in the right lobe of the liver by abdominal ultrasonography. A needle biopsy specimen showed the lesion to contain small- and medium-sized lymphoid cells without obvious atypia, and a provisional diagnosis of pseudolymphoma was made. About 2 yr later, the tumor increased to 15 mm in diameter, necessitating a second needle biopsy. Histological and genetic examinations confirmed non-Hodgkin's lymphoma of diffuse, mixed small and large cell, B-cell type. However, the size of the tumor remained almost stable (16 mm in diameter) over a period of 7 months after diagnosis, without any treatment for lymphoma, indicating a low grade malignancy. We document hepatic lymphoma as an additional complication of primary biliary cirrhosis.
