Degradation of atrial natriuretic peptide in dogs.

To clarify the degradation of atrial natriuretic peptide (ANP) in dogs, plasma ANP concentrations in blood samples drawn from the coronary sinus, and in simultaneously collected blood samples of the arteries and veins of the lung, liver, kidney, spleen and cervix, and of the supramesenteric artery and the portal vein, were measured, both when infused with exogenous ANP (280 ng.kg-1.min-1, N = 7) and when not infused (N = 6). Venous ANP concentrations in most organs showed lower values than arterial levels by 11.6 to 33.6%, suggesting degradation during circulation. Exogenous ANP infusion resulted in a 26.0 to 41.9% arterio-venous decrease in ANP levels. These findings suggest that each organ contributes to ANP degradation, and that no specific organ plays a key role. The plasma ANP concentration was not reduced when blood samples were incubated for 60 min at 37 degrees C. The plasma from the coronary sinus and forearm vein contained essentially identical ANP molecular forms, as shown by gel permeation chromatography. Thus, the blood circulation through various organs does not cause high molecular weight ANP to be changed to other forms.

Relationship of the waist to hip ratio with serum lipids in women.

Recent reports have shown that a high ratio of waist to hip circumference (WHR) may be associated with a high proportion of intra-abdominal fat. The metabolic complications of obesity, which are associated with a high WHR, may therefore relate specifically to the amount of intra-abdominal fat. The purpose of this study was to determine if WHR is associated with the concentration of serum lipids that may predispose individuals to a higher risk for metabolic complications. Serum lipids concentrations were determined in 45 female subjects, ranging from 31 to 40 years in age, and WHR ranging from 0.71 to 0.98. Positive correlations of WHR with the ratio of cholesterol to HDL-cholesterol (r = 0.641, p less than 0.001) and with triglycerides (r = 0.479, p less than 0.001), were found, while a negative correlation existed between WHR and HDL-cholesterol (r = -0.600, p less than 0.001). In order to investigate the relationship between the fat distribution in the body and the serum lipids, the subjects were divided into two groups according to their % Body fat; one group with a % Body fat more than 30 and the other with a % Body fat less than 30. The two groups differed significantly in WHR. In the group of obese women with high WHR (greater than 0.87), the ratio of cholesterol to HDL-cholesterol and triglycerides were higher and HDL-cholesterol lower than in the normal group with low WHR (less than 0.80).(ABSTRACT TRUNCATED AT 250 WORDS)

Long-term observations of the masseter muscle following single or repeated injections of lidocaine hydrochloride into developing mice.

Effects of lidocaine-HCl on extrafusal muscle fibers in the masseter muscle of developing mice were studied histologically and morphometrically. In the affected region, many extrafusal muscle fibers were circular and smaller in diameter than unaffected extrafusal muscle fibers, and the SDH activity of the affected fibers was very irregular. In lidocaine-HCl injected groups, the ratios of extrafusal muscle fibers with central nuclei to total extrafusal muscle fibers were higher than those in the saline solution-injected groups. The diameters of extrafusal muscle fibers in the masseter muscle treated with lidocaine-HCl were smaller than those in the saline solution-injected groups. These changes in the muscle with lidocaine-HCl continued for 45 days after a single injection. Five injections of lidocaine-HCl into developing mice caused long-term degeneration of the masseter muscle. Thus, the present study suggests that a local anesthetic agent caused degeneration of immature muscle fibers of the masseter muscle in developing mice and may result in long-term decrease of masticatory capacity. Therefore, lidocaine-HCl may inhibit the synchronized development of masticatory organs in developing animals.

[Histological investigation of improvement of jaw-bone structure for application of hydroxyapatite dental implant].

UNLABELLED: The purpose of this study was to investigate the possibility of applying the hydroxyapatite dental implant to the jaw-bone with atrophic structure. The lower P3, P4 and the mesial root of M1 of 6 mongrel dogs were extracted. At 4 months, the trabeculae were taken out and porous hydroxyapatite granules were filled in the vacant spongiosa. At 8 months, hydroxyapatite dental implants were placed into this area. After 1 week and after 2 months, specimens were taken out and fixed by 10% formalin alcohol. They were embedded in polyester resin and undecalcified thin sections were prepared. The sections were stained with toluidine blue and observed under light microscopy. RESULTS: 1) A larger amount of bone compared with that of the peripheral spongiosa was observed among the granules. 2) One week after implantation, differentiation around the implant was rather slow in the area of the granules. But in the area of the granules next to the spongiosa, differentiation was normal. 3) Two months after implantation, a new bone was observed, which filled the implantation space and united the implant to the bone among the granules. But on some implant surface, a fibrous connective tissue was seen. These results suggest that this method is able to improve the jaw-bone structure sufficiently to apply the hydroxyapatite dental implant.

[Clinical experience with Ureteromat in transurethral ureterorenoscopy].

By September 1988, we performed 58 ureterorenoscopies (52 for primary ureteral calculi, 3 for ureteral biopsy, 2 for ureteral stricture and 1 for ureteral foreign body) using a Ureteromat (Uromat Storz, West Germany). Continuous saline irrigation with the Ureteromat has obviated mechanical dilation of the ureteral orifice in 93.1% of the cases, which made this endoscopy as easy as the conventional cystourethroscopy. In 4 cases (6.9%), however, balloon dilation of the ureteral orifice was necessary for the passage of a rigid ureterorenoscope. Of 52 ureteral calculi, 37 were assigned for transurethral ureterolithotripsy (TUL), whereas transurethral ureterorenoscopy (TUURS) was indicated to flush the remaining 15 calculi up into renal pelvis for later extracorporeal shock wave lithotripsy. TUL was successful in 81.1% (30/37). The causes of failure were stone migration in 6 cases and ureteral avulsion by electrohydraulic lithotripsy necessitating open surgery in 1 case. The irrigation with the Ureteromat facilitated the manipulation of intraluminal ureteral lesions always under clear vision; by-passing the impacted stone with a guide wire and flushing-up of the calculi were successfully performed in 100%, in spite of the co-existing edematous and inflammatory mucosal changes. In 7 cases where ureteroscope could not be advanced well up to the lesion because of spasm in the iliac ureter, the irrigation pressure was transiently increased up to 200 mmHg to allow safe and easy dislodgement of impacted calculi under fluoroscopic guidance. This procedure was also effective to introduced a ureteral stent over a guide wire in 2 cases of ureteral stricture, which could not be achieved by conventional cystoscopic procedure. Immediate postoperative complications were fever in 8.6% and gross hematuria lasting for more than 4 days in 8.6%.(ABSTRACT TRUNCATED AT 250 WORDS)

[Two cases of ectopic prostatic tissue in the prostatic urethra].

Two cases of ectopic prostatic tissue or adenomatous polyps with prostatic type epithelium in the prostatic urethra were reported. They were 30-and 33-year-old males suffering from gross-hematuria and from urethral pain on urination with terminal gross-hematuria. Endoscopic examination revealed papillary lesions from the dilated orifice of the prostatic utricle. Biopsied pathology demonstrated a tissue similar to that of the prostatic gland. PSA (prostatic specific antigen) stain using immunohistochemical methods was positive. Histogenesis and the importance of this lesion particularly as a cause of hematuria in the young male adult were discussed.

[Regional transarterial infusion of autologous lymphokine-activated killer (LAK) cells for advanced renal cell carcinoma and its preliminary clinical result].

We herein report the preliminary but appreciable results of regional transarterial infusion of 2 gravity subtypes of autologous lymphokine-activated killer (LAK) cells into the metastatic sites in combination with systemic recombinant interleukin-2 (rIL-2) administration in 3 patients with advanced renal cell carcinoma. Leukapheresis was performed once a week and peripheral blood lymphocytes were separated into 2 different subtypes by Percoll gradient centrifugation. These lymphocytes were incubated with rIL-2 for a few days to induce LAK cells. LAK cells were transferred to the metastatic lesions through cannula twice a week. A large iliac bone metastasis disappeared 3 months after the initial LAK cell therapy via a superior gluteal artery. A case of complete disappearance of psoas muscle and para-aortic lymphnode metastasis as well as partial regression of a lumbar bone metastasis was seen after lumbar arterial infusion treatment. Another case with brain metastasis showed a rapid exacerbation of brain edema after one week's LAK therapy. Our treatment modality seems to be worthwhile and promising for treatment of the advanced renal cell carcinoma.

Biological activities of IC201 ((3S,8E)-1,3-dihydroxy-8-decen-5-one), a low molecular weight immunomodulator produced by Streptomyces.

IC201 was found in cultured broth of Streptomyces cirratus as an antitumor antibiotic which was effective in retarding growth of the established solid tumor of Ehrlich carcinoma by treatment starting 8 days after tumor inoculation. It retarded growth of the established solid tumor of IMC carcinoma but had no cytotoxicity at 100 micrograms/ml. IC201 treatment kept NK cell activity of tumor-bearing mice at normal level and stimulated cytostatic activity of peritoneal macrophages. It stimulated phagocytosis of yeast and phorbol myristate acetate-elicited superoxide production by peritoneal macrophages. The addition of IC201 to P388D1 cell cultures enhanced release of interleukin 1 (IL-1) into cultured supernatant but it affected lipopolysaccharide-induced IL-1 production. Although the addition to macrophage-depleted cultures did not show any stimulatory effect, mixed lymphocyte culture reaction was augmented in cultures using spleen cells as stimulator cells taken from mice given IC201. Results indicate that IC201 primarily activates macrophages and the activation may cause modulation of immune responses.

[Priapism with malignant lymphoma: a case report].

A case with priapism in a 66-year-old male due to malignant lymphoma is reported. Physical examination revealed an erected penis and enlarged lymph nodes in left neck and bilateral inguinal areas. Rectal digital examination of prostate revealed an irregular surface and swollen hard left lobe. Open biopsy of the bladder and vesicostomy were carried out. Several nodular lesions in the trigone and in the left wall were excised. Pathologically the diagnosis was malignant lymphoma, diffuse, large cell type and clinical stage was stage IV. CHOP therapy with bleomycin and methotrexate was performed. Priapism disappeared 7 days after the initiation of the chemotherapy, but impotence has been noticed after the chemotherapy for more than one year. This will be the first case with priapism due to malignant lymphoma reported in Japan so far.

Processing of myoelectric signals for estimating the location of innervation zones in the skeletal muscles.

An electrical activity associated with the excitement of muscle fibers arises from the myoneural junctions located at the middle length of the fibers and propagates to the both ends. This propagation can be detected with a multi-electrode array placed on the skin surface. Therefore, the position of the myoneural junctions, or of the innervation zones as a collection of myoneural junctions, can be estimated from the source of the propagation. The authors propose a method for determining the position of the propagation sources automatically with a computer. The method was based on the line-source model of muscle fibers. The positions of the propagation sources were treated as parameters in the model. When some values are assigned to the parameters, the surface potentials are calculated from the currents assumed to propagate on the muscle fibers. The computer searched for the optimal parameters which gave the best fit between the calculated and the measured potentials on the skin surface. Using this method some separate sources of propagation were detected at the middle length of the biceps brachii. The activity of the individual innervation zones was quantitatively described by the average amplitude of the potentials arising from the corresponding sources. This description clarified the changes in the activity of innervation zones when the contraction forces were varied.

[Correlations between first molar eruption, caries incidence and caries experience in primary school children].

A follow-up study was made on the results of oral health examinations which had been conducted for six years on a total of 871 primary school children in the 1st grade (age, 6 years) till they became sixth graders (460 boys and 411 girls). The findings are as follows: 1. When they were first graders, the oral health examination revealed their 56% had erupted the upper first molar, and 73% the lower first molar. 2. The incidence of caries in the upper first molar had occurred mostly between one and two years after eruption. After three to four years, more than half of the pupils had this particular tooth decayed. In the case of the lower first molar, caries had started to develop within one year after eruption. After two years, caries was evident in more than half of the examinees. 3. In the sixth graders, the number of DMF teeth per child came to 3.9 and the DMF teeth rate stood at 17%. Both the number of DMF teeth and the DMF teeth rate were larger in girls than in boys. The rate for an intact first molar teeth were 32% in the upper jaw and 15% in the lower jaw. These rates were lower in girls than in boys. 4. The shorter the period between the eruption of the first molars in both jaws and the caries incidence, the larger the number of the DMF teeth and the DMF teeth rate at sixth graders. For the prevention of caries in permanent teeth, special care should be taken during the short period of time following eruption.

[Statistical analyses of clinical observations made on outpatients visiting the Department of Oral Diagnosis, Meikai University School of Dentistry Hospital, for the past 16 years].

Statistical analyses of clinical observations made on outpatients visiting the Department of Oral Diagnosis, Meikai University School of Dentistry Hospital from June, 1970, to August 1986 were carried out. The total number of new outpatients during this period was 73,708. The results were as follows: 1) The largest number of patients was found in 1978. 2) There was a tendency for the number of new patients to increase in March, June, and August of every year. The smallest number of patients came in December. 3) The greatest number of new patients visiting the hospital was in the 0-9 age category, followed by those in the 20-29 and 30-39 age categories. 4) The greatest chief complaint of patients visiting the hospital was pain, followed by masticatory disturbance and swelling. 5) The number of outpatients coming to the hospital for cosmetic and temporomandibular joint problems has been increasing on a yearly basis over the past 16 years.

[A case of benign fibrous histiocytoma of the lung in a child].

A case of benign fibrous histiocytoma of the lung in a 8 years old boy was presented. He was first admitted in May, 1983, with recurrent pneumonia. Chest X-ray showed a ill-defined mass in the right lower lobe. Bronchoscopy revealed a round tumor, 1 cm. in maximum diameter, with complete obstruction of the Truncus Intermedius. Endoscopic resection was performed and partial obstruction of the Truncus Intermedius remained. He was re-admitted with pneumonia of the right middle lobe in March, 1986. Bronchoscopy showed severe stenosis of the Truncus Intermedius. Right middle and lower lobectomy was performed. The 4 X 4 X 3 cm tumor was located in the median of the Truncus Intermedius. Microscopically, the lesion composed of fibroblast-like cells and histiocyte-like cells. The patient's post-operative course was uneventful. He has no signs of local recurrence or metastasis. We believe, this is the first reported primary benign fibrous histiocytoma of the lung in a child in Japan.

Comparison of ventilatory and heart rate responses to hypoxia at rest and during light exercise in high school judo athletes.

Ventilatory and heart rate responses to hypoxia at rest and during light exercise (30W) were compared in 21 high school judo athletes. The results of ventilatory and heart rate responses to hypoxia were analyzed by the hyperbolic equations, VE = VO + AVE/(PETO2-CVE) and HR = HRO + AHR/(PETO2-CHR), respectively, where VE and HR are the observed ventilation and heart rate, VO and HRO the horizontal asymptote in ventilation and heart rate for infinite endtidal PO2 (PETO2), AVE and AHR the slope constants indicating the magnitude of hypoxic sensitivity, and CVE and CHR the vertical asymptote in PETO2 for infinite ventilation and heart rate. AVE was further recalculated after VE was normalized for a 70 kg body mass, using an allometric coefficient, and was defined as AVEN. The absolute magnitudes of hypoxic response in ventilation and heart rate at PETO2 40 mmHg were also determined as delta V40 = AVE/(40-CVE) and delta HR40 = AHR/(40-CHR), respectively. (1) The enhanced hypoxic ventilatory chemosensitivity previously found in heavy-weight judo athletes at rest was confirmed. However this characteristic disappeared with exercise. (2) It was also confirmed that there was no significant correlation between hypoxic heart rate sensitivity and body weight at rest. With exercise, this was found also to be the case. (3) On the other hand, when the hypoxic responses at rest were compared to those of the exercise studies, significant positive correlations were illustrated in both ventilatory and heart rate responsiveness. These results demonstrated that although ventilatory and heart rate responses did not quantitatively exhibit parallel change, the two activities did demonstrate a certain synergism between rest and exercise.

[Investigation of people's attitudes and reactions towards oral malodour. A preliminary survey conducted on dental hygienics students].

As part of the study on the prevention of oral malodour, people's attitudes and reactions towards oral malodour were investigated in this study. As a preliminary survey, investigation was made by questionnaire on a total of 179 students at three dental hygienics schools in November 1988. The findings are as follows: 1. Of the total number of the students questioned, 47% sometimes feel uneasy about their own oral malodour, while 35% have never thought about it. Only 6% have been told they had oral malodour. 2. Although 98% of the respondents felt sick when they became aware of another person's oral malodour, 53% of them told the other person of their discomfort when the person with oral malodour was a parent, brother, sister, or a very close friend. 3. About half (52%) of the students said, they practice the prevention of oral malodour by brushing their teeth, and 50% like to use mouthwashes. Of the total, 88% wished to know whether they had oral malodour or not. 4. Overwhelming majority knew of mouthwashes through TV commercials and advertisements in magazines, 80% of those could name any one of the tradenames of the mouthwashes. 5. These findings indicated that dental health education on oral malodour and mouthwashes would be important in future.

[Coronary angiography in patients with U wave inversion during coronary artery spasm].

During ergonovine-induced vasospastic angina, U wave inversion without significant ST segment deviation on the precordial electrocardiograms was documented in four patients. Coronary angiography revealed incomplete spastic obstruction of the left anterior descending artery without delayed filling and runoff in three patients. In the remaining patient, the proximal left anterior descending artery was totally occluded and there were well-developed collaterals from the non-spastic artery. Thus, ergonovine-induced U wave inversion was related to the presence of coronary vasospasm, and angiography demonstrated less severe myocardial ischemia in such patients than in cases with ST segment elevation or depression, which is usually associated with subtotal or total obstruction of a major coronary artery without adequate collaterals. In their clinical courses, two patients had episodes of angina with ST segment elevations or depressions. It was suggested that vasospastic angina with U wave inversion alone is one aspect of a continuous spectrum of vasospastic myocardial ischemia.
