Small hepatocellular carcinoma with minute satellite nodules.

BACKGROUND/AIMS: To investigate the clinicopathologic characteristics of small hepatocellular carcinoma with minute satellite nodules. METHODOLOGY: We investigated the clinicopathologic characteristics of 131 solitary small (< or = 2.0 cm in diameter) hepatocellular carcinomas including 105 hepatocellular carcinomas without minute satellite nodules and 17 hepatocellular carcinomas with minute satellite nodules smaller than 5 mm, and also discuss the clinical significance. RESULTS: None of the clinical backgrounds of the patients and pathologic features of the main tumor, except for the average of preoperative serum alpha-fetoprotein, were significantly different between the two groups. Firstly, minute satellite nodules demonstrated that the maximum diameter of all minute satellite nodules was 1.5-4.0 mm, secondly, the moderately to poorly differentiated hepatocellular carcinomas had 4 or more minute satellite nodules within 1 cm from the main tumor, while well differentiated hepatocellular carcinomas may have 1 or 2 minute satellite nodules 6 cm or more away, and thirdly, 4 or more minute satellite nodules may present within 1 cm in intrahepatic metastasis cases, while 1 or 2 minute satellite nodules may be present 6 cm or more away from the main tumor in multicentric occurrence cases. CONCLUSIONS: At least 13% of solitary small hepatocellular carcinomas had preoperatively undetectable minute satellite nodules. In case of moderately to poorly differentiated hepatocellular carcinomas, hepatic resection as well as percutaneous ethanol injection should be performed including the surrounding liver tissue at least 1.0 cm from the main nodule. On the other hand, in well-differentiated hepatocellular carcinomas, which may indicate multicentric occurrence, closer observation and careful follow-up after therapy are recommended.

Electroporation-mediated transfer of cytokine genes into human esophageal tumors produces anti-tumor effects in mice.

Electroporation facilitates transfer of chemicals or plasmid DNA from extracellular milieu into cells by increasing the permeability of the cell membrane. Delivery of electric pulses to established tumors thereby can improve the susceptibility of tumors to an anti-cancer agent administered. We examined whether electroporation-mediated transfer of cytokine genes into solid tumors could produce anti-tumor effects in the tumor-bearing mice. Plasmid DNA containing cytokine genes were injected into human esophageal T.Tn tumors developed in nude mice and electric pulses were then delivered. Administration of murine GM-CSF or human IL-2 gene followed by electroporation significantly suppressed the subsequent growth of T.Tn tumors and prolonged the survival of the inoculated mice. In contrast, electroporation-mediated introduction of a control gene, human GM-CSF gene, whose products do not bind to murine GM-CSF receptors, did not achieve any anti-tumor effects. In vivo transfection of cytokine genes with electroporation could be a possible therapeutic strategy for established solid tumors.

Enhanced therapeutic efficacy on lymph node metastasis by the use of peplomycin adsorbed on small activated carbon particles.

Mice into which 5 x 10(5) MH134 tumor cells had been inoculated subcutaneously opposite the foot-pad of the left hind paw on day 0, received subcutaneous injection of 0.25 mg/mouse of peplomycin in the form of PEP-CH (peplomycin adsorbed on activated carbon particles) or peplomycin aqueous solution into the foot-pad of the left hind paw on day 10. The left popliteal lymph nodes were transferred intraperitoneally to normal mice (the recipients) on day 14. In the PEP-CH group the survival time of the recipients was statistically significantly longer and the transferred tumor cell number estimated by calibration curve was markedly smaller in comparison with those in the other treatment groups.

Intra-arterial injection of adriamycin/mitomycin C lipiodol suspension in liver metastases.

Intra-arterial injection of a suspension of adriamycin and/or mitomycin C in Lipiodol was performed in 17 patients with hepatic metastases, which at angiography were poorly vascularized. Accumulation of Lipiodol in the tumors was demonstrated at computed tomography (CT) in 15 of 17 patients examined within one week. Follow-up with CT showed that Lipiodol remained in the tumor during the first month in 94 per cent, after 2 months in 31 per cent, and after 3 months in 17 per cent. In the non-tumor part of the liver Lipiodol disappeared earlier, and one month after injection it could no longer be traced on CT. In 8/17 cases (47%) CT, after intra-arterial injection of Lipiodol, gave superior information compared with CT after intravenous contrast enhancement. Tumor response was achieved in 9 of 16 cases. Particularly in metastases originating from cancer of the colon and stomach response was observed with a decrease in tumor size in 8 of 10 patients.

Well-differentiated hepatocellular carcinoma: isointense to liver parenchyma on superparamagnetic iron oxide-enhanced MR.

We present a patient with hepatocellular carcinoma that was identified on precontrast MR and became isointense to liver parenchyma after the intravenous administration of superparamagnetic iron oxide. The tumor was well-differentiated hepatocellular carcinoma and contained approximately the same number of Kupffer cells as liver parenchyma on pathological examination. Our case suggests that superparamagnetic iron oxide may give confusing results if the hepatic mass, e.g., hepatocellular carcinoma, contains Kupffer cells.

Strategies for intractable ascites after hepatic resection: analysis of two cases.

Intractable ascites after hepatic resection is one of the gravest complications pertaining to deterioration in a patient's quality of life. Two cases are presented with intractable ascites that did not respond to ordinary conservative treatment after hepatic resection but were eventually successfully treated. The authors propose the following strategies: the conducting of aggressive and extensive examinations including angiography; and a peritoneovenous shunt for patients demonstrating no definite cause, to improve their quality of life.

Effects of MPC-1304, a novel calcium antagonist, on stroke-prone spontaneously hypertensive rats.

The effects of MPC-1304, a newly developed 1,4-dihydropyridine derivative, on blood pressure and hypertensive complications in stroke-prone spontaneously hypertensive rats fed a high-salt diet (0.8% NaCl), were investigated. The antihypertensive effectiveness of nicardipine was used for the purpose of comparison. MPC-1304 and nicardipine were added to the diet, in doses of 0.01% (0.01% MPC-1304 diet), 0.03% (0.03% MPC-1304 diet) and 0.1% (0.1% nicardipine diet), respectively, throughout the experimental period (8 to 30 weeks of age). This chronic ingestion of MPC-1304 and nicardipine inhibited the development of hypertension and reduced the concentration of blood urea nitrogen, creatinine, triglyceride and total cholesterol in serum. Treatment with MPC-1304 inhibited the incidence of cerebral stroke, cardiac fibrosis, proliferative and fibrinoid arteriolitis and malignant nephrosclerosis. There was no significant difference in the antihypertensive effectiveness between 0.01% MPC-1304 and 0.1% nicardipine diets. Thus, MPC-1304 had antihypertensive effects in stroke-prone spontaneously hypertensive rats.

Halothane-induced cytotoxicity in isolated rat hepatocytes: an electron microscopic study.

To determine the direct cytotoxicity of halothane for liver cells, the effects of this agent on isolated rat hepatocytes were investigated by electron microscopy and by examining cell viability. Soon after treatment with lower concentrations of halothane (0.5% and 0.75%), there were marked changes in the plasma membrane. Cell viability was reduced to 70% of the control. At higher concentrations of halothane (1% and 3%), degeneration of mitochondria was noted. Cell viability was reduced to 40% of the control. Halothane, at lower concentrations, appeared to induce early morphological alterations of the plasma membrane, while higher concentrations damaged the organella.

Argyrophilic nucleolar organizer regions (AgNOR) of resected intrahepatic cholangiocellular carcinomas.

BACKGROUND/AIMS: Thxe argyrophilic nucleolar organized resion (AgNOR) is loop DNA encoded for rRNA production on the nucleoli, which controls cell proliferation and protein synthesis (1,2). Based on numerous studies, it has been suggested that the AgNOR number correlates with both cellular kinetics and the malignant grade of the tumor. MATERIALS AND METHODS: The argyrophilic nucleolar organized regions (AgNOR) of 14 operated cases with intrahepatic cholangiocellular carcinoma (IHCCC) were examined. RESULTS: The number of AgNOR per nucleus (the AgNOR score) was found to be related to both the level of carcinoembryonic antigen (CEA) (r = 0.63, P = 0.016) and the size of the tumor (r = 0.62, P = 0.017). Regarding the age, sex, the presence or absence of lymph node metastasis, the grade of TNM classification, and the main location of the tumor, no statistical significance was recognized. On the other hand, both histological difference and differentiation were observed to remarkably reflect the AgNOR score (P = 0.03). The cancer-free survival and survival rate showed no relationship to the AgNOR score, but patients with an AgNOR number lower than 2.0 showed a poorer cancer-free survival rate as well as a poorer survival rate. Two patients with a long survival, who have been free from cancer for more than 5 years, showed a low AgNOR score (1.6, 1.7). CONCLUSIONS: The AgNOR method is simple, low-cost and can easily be applied to paraffin-embedded sections. Therefore, this score is considered to be useful in identifiying malignant potentiality in intrahepatic cholangiocellular carcinoma.

Surgical results of spontaneously ruptured hepatocellular carcinoma.

Seventeen patients with ruptured hepatocellular carcinoma (HCC) underwent a hepatic resection from 1985 to 1992 at either Kyushu University Hospital or Iizuka Hospital in Japan. They all underwent elective surgical procedures except for one patient who experienced a repeated rupture during hospitalization. A comparative study between the patients with ruptured HCC and non-ruptured large HCC measuring more than 6 cm in size (n = 29) revealed that the 3-year disease-free survival rate of the ruptured HCC group was 14.5% which was worse than that of the non-ruptured group at 39.9%. There was no difference in the rates of peritoneal dissemination, distant metastasis or intrahepatic recurrence between the two groups. Regarding the intrahepatic recurrence pattern, widespread multinodular recurrence frequently occured in the ruptured patients, which was thought to be caused by the high incidence of venous invasion in the primary tumor. The intratumor pressure of the ruptured HCC appeared to be elevated and is considered to be one of the main reasons for the high incidence of venous invasion.

Surgically resected primary malignant lymphoma of the liver.

Primary malignant lymphoma of the liver is extremely rare, and its preoperative or even premortem diagnosis is still difficult. The authors herein report a case of primary malignant lymphoma of the liver in a 51-year-old Japanese man. The findings at angiography were non-specific, but a hypoechoic mass with a halo was demonstrated by ultrasound sonography, which revealed rapid progression. The tumor volume doubling time was 9.6 days. An extended right hepatic lobectomy with lymph nodes dissection was done on the basis of a preoperative diagnosis of hepatocellular carcinoma, while the diagnosis based on immunohistological studies was T cell malignant lymphoma of the liver. After the adjuvant chemotherapy, the patient continues to be free of disease for three years and nine months. We herein describe the optimum methodology for making a preoperative diagnosis of primary malignant lymphoma of the liver. We also consider that a curative hepatic resection with adjuvant chemotherapy for primary malignant lymphoma of the liver is an adequate treatment for this disease.

A primary adenosquamous carcinoma of the liver with an elevated level of serum squamous cell carcinoma related antigen.

A case of primary adenosquamous carcinoma of the liver with an elevated level of serum squamous cell carcinoma related antigen is herein reported. Various hypotheses on the pathogenesis of adenosquamous carcinoma of the liver have been set forth previously, however there is still no widely accepted theory because of the absence of any sufficient evidence. The postoperative transition of serum squamous cell carcinoma related antigen and the immunohistochemical findings using anti-involucrin antigen in this case support the hypothesis that the squamous cell carcinoma component arises as a result of the metaplastic transformation of adenocarcinoma cells.

Prognosis of well differentiated small hepatocellular carcinoma--is well differentiated hepatocellular carcinoma clinically early cancer?

BACKGROUND/AIMS: The purpose of this study is to examine whether or not well differentiated (w-d) hepatocellular carcinoma (HCC) is indeed clinically early cancer. MATERIALS AND METHODS: Seventy six patients with solitary small HCCs up to 3 cm in diameter, who underwent hepatectomy, were observed for at least 2 years for possible recurrence. These patients were divided into two groups: 10 patients with w-d HCCs (Edmondson and Steiner's grade I) and 66 patients with less differentiated (l-d) HCCs (Edmondson and Steiner's grade I-II, II-III, and III). RESULTS: The histological analysis revealed that w-d HCCs had lower incidences of fibrous capsule formation (P<0.01), when compared to l-d HCCs. There were no significant differences in the incidence of intrahepatic metastasis, or portal vein invasion. In a resected specimen of w-d HCC, barium sulfate and gelatin were injected into portal vein and a transparent specimen was made. The transparent specimen showed that the portal vein in the tumor seemed to be intact. Microscopically, cancer cell infiltration into the fibrous frame of the portal tract was present. There were no significant differences in the disease free survival between the two groups. An analysis of tumor volume doubling time in recurrent foci suggested that minute cancerous foci had been present at the time of operation. CONCLUSIONS: W-d HCCs were clinically demonstrated not to be early cancer, because there was no significant difference in disease free survival between the patients with w-d and l-d HCCs.
