[Mandibular position sense during open-close movement].

The purpose of this study was to investigate mandibular position sense with four different jaw openings (5, 10, 15 and 20 mm from intercuspal position) during open-close movements. Eight subjects memorized the reference positions with monitoring their spatial vertical jaw positions using a Mandibular Kinesiograph (M.K.G., K-5). Immediately after memorizing one of those positions, they were required twenty times open-close movements; that is, approaching the memorized position from superior (opening approach) or from inferior position (closing approach) and receding were repeated. Vertical distances from intercuspal position at lower incisal teeth were recorded by MKG and statistically analyzed. 1. In the closing approach, perceived mandibular positions were become lower, but in the opening approach, significantly higher than reference positions (5 mm : p less than 0.00001, 10 mm : p less than 0.05, 15 mm : p less than 0.025, 20 mm : p less than 0.005). 2. In the closing approach, the inclination of regression lines shows significantly the estimated decrement to come near closed position throughout twenty experimental sequences (5 mm : p less than 0.001, 15 mm : p less than 0.025). 3. On 5 mm reference position, variance of data was significantly lower than the others. On 10 mm with opening approach, it was the highest of all. 4. It was suggested that mandibular position sense was affected by mandibular spatial vertical position and direction of approaching during jaw movements, and that neuronal mechanism of controlling mandibular positions and movements were more excellent near tooth contact.

Acute thrombosis of the inferior vena cava and hepatic veins in patients with Budd-Chiari syndrome: CT demonstration.

We reviewed the CT findings in 17 patients with angiographically proved Budd-Chiari syndrome to determine the ability of CT to show acute thrombosis of the inferior vena cava (IVC) and hepatic veins. In eight patients with membranes (web or band) in the IVC, no thrombus was detected with CT or angiography. In the other nine patients, thrombi in the IVC and/or hepatic veins were seen as intraluminal filling defects that did not change in appearance on precontrast and postcontrast CT scans. Attenuation values of intraluminal filling defects of the IVC ranged from 38 to 42 H in four patients. High-attenuation intraluminal filling defects (60-70 H) of the IVC (five patients) and hepatic veins (one of five patients) were detected. Of these five patients, four had acute symptoms and one had chronic vague symptoms. The underlying disease was a web or band in the IVC and hepatic veins in three patients, invasive hepatocellular carcinoma in one, and injury to the IVC wall during hepatectomy in one. Inferior venacavography showed occlusion of the hepatic segment of the IVC in all five patients. Additional angiograms obtained by injection of contrast medium after a catheter tip was placed in the occluded hepatic IVC showed numerous filling defects suggestive of thrombi of recent onset, which correlated with the high-attenuation thrombi seen on CT scans in two patients. In the remaining three patients, high-attenuation areas in the IVC and hepatic veins also were considered to represent thrombi of recent onset because the attenuation values later decreased to 33-42 H. Spontaneous reduction in diameter of the thrombosed segment of the IVC was observed in four of the five patients. Knowledge of the CT features of acute thrombosis of the IVC and hepatic veins is useful in the early diagnosis of Budd-Chiari syndrome.

Disposition of acetone, methyl ethyl ketone and cyclohexanone in acute poisoning.

A case of coma due to the drinking of a liquid cement for polyvinyl chloride resin, containing acetone, methyl ethyl ketone, cyclohexanone and polyvinyl chloride is described. The patient also simultaneously ingested the alcoholic beverage, sake. After gastric lavage, plasma exchanges and direct hemoperfusions, the patient recovered. The concentrations of these chemicals in plasma and urine were analyzed at various time intervals to estimate the clearance. The elimination half lives for acetone and methyl ethyl ketone were 18 hours and 10 hours, respectively. Although cyclohexanone made up the largest component in the solvents, the blood level was extremely low and a large amount of cyclohexanol, a metabolite of cyclohexanone was detected in the blood and urine. The glucuronide metabolite of cyclohexanol was also estimated after the hydrolysis with beta-glucuronidase. Since the conversion of cyclohexanone to cyclohexanol is known to be catalyzed by alcohol dehydrogenase, possible interactions between sake ingestion and cyclohexanone metabolism is proposed.

[Morphologic findings and biologic behavior in the high grade glioma--a postmortem study of 22 cases].

Morphologic features of the autopsied specimen of 22 cases with supratentorial gliomas treated by surgery, radiation and/or chemotherapy were analysed, and the characteristics of recurrence of gliomas were searched for. The cases consisted of anaplastic 12 astrocytoma and 10 glioblastoma. The results were as follows: 1) Characteristic CT findings before death were regrowth of the tumor mass or the occurrence of a new enhanced lesion in 21 out of 22 cases. The enhanced lesion showing regrowth of the tumor located in the same site as the previous tumor mass in 21 cases. The new enhanced lesion resulting from a trans-or subependymal tumor spread, was seen in the ventricular wall, and these findings were a characteristic feature of the recurrence of gliomas. 2) Modes of extension of the tumor were subdivided into 3 types. One was the expansive or infiltrative type caused by regrowth of the residual tumor. In the second pattern, a spread of tumor cells occurred along the myelinated fiber tracts to the brain stem (60%), or to the contralateral cerebral hemisphere through the corpus callosum (50%). The third mode of tumor propagation was cerebrospinal fluid seeding with intraventricular or subarachnoid tumor regrowth (45%). 3) Characteristic histological findings shown in the original tumor bed were those of increased cellularity with endothelial proliferation, widespread necrosis with occlusion of the blood vessels, occurrence of the gemistocytic astrocytes and large bizarre cells. Thickening of wall of the blood vessels due to effect by radiation was followed by occlusion of the blood vessels. Large necrosis in the tumor tissue was caused by those process and others. Necrotic area was mainly circumscribed and corresponded to the territory of the vessels. One of the specific findings in the morphological changes of the tumor cells was giant cell formation which were monstrous cell, giant cell (12 cases out of 22), and gemistocytic cell (in all cases). These specific cells were supposed to the degenerative changes of the tumor cells exposed while withstanding such adverse conditions as hypoxia, radiation and chemotherapy. 4) Infiltration distant from the primary lesion which were defined only by microscopical examination was demonstrated as both through myelinated fiber tracts in 8 cases and through perivascular spaces in 2 cases. Reinvasion of the tumor cells from the subarachnoid spaces to the brain parenchyma was along the Virchow-Robins spaces of the penetrating blood vessels in the latter cases.(ABSTRACT TRUNCATED AT 400 WORDS)

Scatchard analysis of EGF receptor and effects of EGF on growth and TA-4 production of newly established uterine cervical cancer cell line (OMC-1).

Effects of epidermal growth factor (EGF) on growth and tumor antigen-4 (TA-4) production of newly established uterine cervical cancer cell line (OMC-1) are reported. OMC -1 was established from a metastatic lesion of Virchow's lymph node of a large cell non-keratinizing squamous cell carcinoma of the uterine cervix and successively subcultured for about 4 years. Scatchard analysis of EGF binding to OMC-1 cells indicated a single class of binding sites with a dissociation constant (Kd) of 360 pM. The theoretical maximum number of binding sites was 2.4 x 10(4) sites/cell. The growth of OMC-1 cells was stimulated by EGF at 0.01-0.1 nM and inhibited at higher concentrations. The TA-4 production of OMC-1 cells was slightly stimulated by EGF at 0.01-1 nM and significantly stimulated at 10 nM. OMC-1 may serve as one of the available model systems for studies of regulation of proliferation and tumor marker production by EGF, particularly in cervical squamous cell carcinomas.

[Orthognathic surgery treatment of adult Class III malocclusion].

A 19-year-old man was first seen with a chief complaint of mandibular protrusion and TMJ pain. His records showed a class III malocclusion with excessive mandibular body length and poor facial profile. Treatment was done with a multibracket orthodontie appliance combined with a surgical mandibular back using a Obwegeser method. In this report, we appraised the changing rate of MKG and EMG in pre-treatment, post-surgery, and during the retention phase.

[Study on the feeding function and feeding behavior of children. 1. Biting pressure and masticatory efficiency for 3, 4 and 5 years old children].

The purpose of this study was to investigate the biting pressure and masticatory efficiency of 3, 4 and 5 year old children. Some of them had trouble regarding their feeding behavior, either saying that they were not able to bite hard food or that they didn't want to bite hard food. A new method using A.T.P. (Adenosine Triphosphate) granules introduced with rubber finger protectors was used for measurement of the masticatory efficiency. 69 healthy 3, 4 and 5 year old children who had normal occlusion and were caries free were the subject. The results obtained were as follows. 1) The mean of the biting pressure for the 2nd primary molars for 69 children was 21.7 Kg. As for the data of the biting pressure for each age group, it was shown to increase with age (3 to 5 year old). 2) The mean of the masticatory efficiency for 69 children was 0.201 abs. As for the data of the masticatory efficiency for each age, it were shown to increase with age (3 to 5 year old). 3) The coefficient of the correlation of the biting pressure and the masticatory efficiency for 69 children was 0.410 which was considered statistically significant. (P less than 0.05) The coefficient of the correlation of the biting pressure and the masticatory efficiency for 3 year old children was 0.461, the coefficient of the correlation for the same items for 4 year old children was 0.395 and the coefficient of the correlation for 5 year old children was 0.353. All of these were considered statistically significant. (P less than 0.05)

Two patients with hypergonadotropic ovarian failure due to pituitary hyperplasia.

Two patients with hypergonadotropic ovarian failure were examined. Plasma levels of LH and FSH were markedly elevated to over 113 mIU/mL (Second International Reference Preparation--human menopausal gonadotropin standard), but the FSH/LH ratio was 1 or less in both women. The plasma estradiol (E2) level was 65 pg/mL in one woman before medication, and in the other increased from below 25 to 130 pg/mL after large doses of clomiphene citrate treatment. The pituitary tissues removed by transsphenoidal surgery showed hyperplasia with numerous cells immuno-histochemically positive for LH and FSH. After surgery, the elevated plasma gonadotropins decreased and plasma E2 levels increased. Human menopausal gonadotropin after clomiphene citrate treatment induced ovulation in one patient, and cyclic administration of estrogen and progesterone induced ovulation in the other. These results suggest that elevated gonadotropins may suppress ovarian function in some cases.

[In vitro studies of 5-FU sensitivity on uterine cervical cancer cell lines--comparison between squamous cell carcinoma and adenocarcinoma].

In order to improve the postoperative survival rate of patients with cervical cancer, we have treated them with adjuvant chemotherapy (oral Tegafur) and proved this treatment to be useful. However, the prognosis of cervical adenocarcinoma cases has not been improved yet. In this study, 5-FU sensitivity, morphological changes and DNA metabolism of cultured cervical cancer cells were examined using OMC-1 and OMC-4 cell line originating from cervical squamous cell carcinoma and adenocarcinoma, respectively. The EC 50 (Effective Concentration for 50% Cell Kill) of 5-FU on OMC-1 and OMC-4 cells after 96 hours of incubation with 5-FU was 0.13 micrograms/ml and 9.1 micrograms/ml, respectively. The morphological changes were more prominent in OMC-1 cells than in OMC-4 cells after 192 hours of incubation with 0.1 micrograms/ml of 5-FU. The incorporation of 3H-deoxyuridine into the DNA was inhibited more significantly in OMC-1 cells than in OMC-4 cells even at a low concentration of 5-FU. The intracellular FdUMP and thymidylate synthetase (TS) inhibition rate of OMC-1 and OMC-4 after 96 hours of incubation with 0.1 microgram/ml of 5-FU was 4.7 pmol/g and less than 2.6 pmol/g, 58.7% and 60.0%, respectively. These results suggest that 5-FU sensitivity of cervical adenocarcinoma cell line (OMC-4) is lower than that of cervical squamous carcinoma cell line (OMC-1) and it may owe much not to the TS inhibition rate but to the intracellular FdUMP.

Effects of famotidine, a new histamine H2-receptor antagonist, on renal function.

The effects of famotidine on renal function were investigated. Eight healthy men (N) and 8 renal patients with varying degrees of renal failure (R.F.) participated in the trial. They were given either famotidine (40 mg) or cimetidine (800 mg) for 7 days. Cimetidine produced a significant decrease in creatinine clearance (from 131.6 +/- 12.9 to 107.7 +/- 3.4 ml/min (N), and 22.2 +/- 3.9 to 18.1 +/- 3.5 ml/min (R.F.], and increase in serum creatinine (from 0.96 +/- 0.05 to 1.09 +/- 0.04 mg/dl (N), and 3.46 +/- 0.62 to 3.86 +/- 0.51 mg/dl (R.F.], and a decrease in creatinine excretion (from 24.0 +/- 1.1 to 22.6 +/- 0.8 mg/kg/24 hr (N], respectively, although the reduction in creatinine excretion in the renal failure group was small. On the other hand, the famotidine treatment produced no significant changes in renal function in both subjects (creatinine clearance, 136.5 +/- 5.7 to 133.6 +/- 6.0 ml/min (N), and 21.9 +/- 3.6 to 20.9 +/- 4.1 ml/min (R.F.); serum creatinine, 0.98 +/- 0.02 to 0.99 +/- 0.02 mg/dl (N), and 3.24 +/- 0.43 to 3.46 +/- 0.55 mg/dl (R.F.); and urinary creatine, 25.1 +/- 1.0 to 25.3 +/- 1.0 mg/kg/24 hr (N), and 17.2 +/- 1.4 to 16.6 +/- 1.5 mg/kg/24 hr (R.F.]. There were no changes in the percent sodium excretion, or the serum and urinary beta 2-microglobulin in both groups.

[Cytogenetic studies in couples with repeated spontaneous abortions].

Chromosome studies were carried out on both partners of 509 couples with a history of two or more spontaneous abortions. 1) Twenty-six individuals (2.6%) were carriers of a major chromosome abnormality. This incidence is at least six to seven times higher than that in the general adult population. 2) Of these, 10 were reciprocal translocations, 10 robertsonian translocations and 6 numerical aberrations of gonosomes. None of the carriers showed abnormal phenotypes. 3) Chromosome aberrations were more frequent in the women than in their husbands. There were 19 abnormalities in females and 7 in males. 4) The use of banding techniques in chromosome analysis improves the detection of balanced reciprocal translocations. 5) Prenatal diagnosis was performed in 5 subsequent pregnancies of 4 balanced translocation carriers. The fetal karyotypes were 2 normal and 3 balanced translocations. It would seem reasonable to recommend chromosome analysis for couples with repeated spontaneous abortions.

[A case of Ehlers-Danlos syndrome].

Ehlers-Danlos syndrome, an inherited connective tissue disorder, is characterized by skin hyperextensibility, joint hypermobility and skin fragility. The disease is at least classified into 10 types, I to X, based on clinical features, biochemical abnormalities of the connective tissue and the mode of inheritance. A case is reported of a 6-year, 8-month-old girl who showed the features of the Ehlers-Danlos syndrome type VII. The findings were as follows: 1) Soft and velvety touch skin, characteristic joint hypermobility, talipes varus, and ventricular septal defects were present. 2) Widely set eyes, slight stabismus, epicanthal folds of the eyes, antimonogoloid slanted eyes, a broad bridge to the nose and low set ears were observed. 3) Gingival bleeding following toothbrushing, Enamel hypoplasia, high arched palate and laxity of the temporomandibular joint were present. 4) In the findings of the maxillary and mandibular dental arches, there was no tendency towards conical teeth in the deciduous dentition, and the width and length of the dental arches were remarkably narrow and short. 5) The roentogenographic findings revealed that there were no minor root deformities, pulpstones and calcification of the pulp. 6) The measurement of the width, length, and depth of the palate through moiré topography revealed that the palate was narrow and deep compared with the control. 7) As a result of cephalometric diagnosis, the nasion appeared to be protruded, therefore maxillary and mandibular seemed to be relatively retracted, and the gonial angle was obtuse. The dental management of patients with Ehlers-Danlos syndrome was discussed and reviewed.

Double tracer autoradiographic method for sequential evaluation of regional cerebral perfusion.

A new double tracer autoradiographic method for the sequential evaluation of altered regional cerebral perfusion in the same animal is presented. This method is based on the sequential injection of two tracers, 99mTc-hexamethylpropyleneamine oxime and N-isopropyl-(125I)p-iodoamphetamine. This method is validated in the assessment of brovincamine effects on regional cerebral perfusion in an experimental model of chronic brain ischemia in the rat. The drug enhanced perfusion recovery in low-flow areas, selectively in surrounding areas of infarction. The results suggest that this technique is of potential use in the study of neuropharmacological effects applied during the experiment.

[Acute myocardial infarction probably induced by the oral administration of bromocriptine: a case report].

This is a report of a case of acute myocardial infarction which was probably induced by the oral administration of bromocriptine. A 55-year-old man was admitted because of suspected hypothyroidism and dwarfism. The endocrinological examination revealed primary hypothyroidism, and levothyroxine sodium replacement therapy was started. Nine days later, the bromocriptine test was performed for dwarfism. Two hours after the oral administration of 2.5 mg bromocriptine, he had severe chest pain associated with loss of consciousness and hypotension. His electrocardiogram showed marked ST depression and T wave inversion, but no abnormal Q waves. Serial myocardial enzymes and electrocardiograms confirmed acute non-transmural myocardial infarction. During coronary angiography performed about three months later, coronary arterial spasm was induced by the intravenous administration of ergonovine maleate. This experience suggests that bromocriptine, a derivative of ergot alkaloids, can cause coronary arterial spasm, and subsequent myocardial infarction. To our knowledge, bromocriptine-induced myocardial infarction has not been previously reported in Japan.

[Mandibular movement and electromyogram investigation of normal occlusion and reversed occlusion before and after correction of over jet].

The purpose of this investigation was to understand how the mandibular movement and myofunction in reversed occlusion, approach to normal occlusion after the correction of over jet. The experiment was carried out in 30 normal and 26 reversed occlusion school children patients who were classified into 3 groups by morphological analysis. A K6 diagnostic system was used. It recorded muscle activity of maximum clenching, free way space, path of closure, and the maximum velocity of opening and closing mandibular. And recorded a condylar test at the same time. Reversed occlusion classified 3 groups: upper and lower incisor had improper inclinations (D group, 19%), construction bite possible but recognized back and forth discrepancy between maxilla and mandibular (FS group, 50%), discrepancy larger than FS group and construction bite not possible (S group, 31%). Toward upper forth group indicated 77% and toward upper back group indicated 23% in normal occlusion. As for reversed occlusion the former indicated 92% and the latter indicated 8%. Differences in connection with orthodontic appliance and period of treatment were not so large. Back and forth mandibular movement, as measured by a condylar test, was greater for reversed occlusion than for normal occlusion, but this value tended to decrease after orthodontic treatment. In reversed occlusion, maximum opening, free way space, maximum velocity of opening and closing and muscle activity of rest position didn't show significant difference (p less than 0.05) among each group before and after the correction of over jet. Temporalis and masseter activity of maximum clenching, except temporalis among the D group, approached the values for normal occlusion. In the S group, muscle activity of maximum clenching with a cotton-roll, was lower before treatment, but approached to data in normal occlusion. These results prove that improvement of over jet in reversed occlusion, by ordinary orthodontal diagnosis and treatment plan, quasi-normalizes mandibular movement and muscle activity in reversed occlusion.

[A case of focal nodular hyperplasia in the liver].

A case of focal nodular hyperplasia of the liver was reported, in which various diagnostic imaging modalities were performed. Hepatic colloid scan (SPECT) in this case shows typical focal accumulation in the lesion. Ultrasonic image shows a sharply demarcated hypoechoic lesion in the left lobe. But no significant informations of this tumor were obtained in the plain CT, CE-CT and angiography. Echo-guided biopsy was performed and focal cirrhotic finding in the lesion without liver cirrhosis was obtained.

[A case of focal fatty liver difficult to distinguish from the liver metastasis of rectal cancer on CT].

The CT findings of focal fatty liver were previously described as follows, 1) nonspherical shape, 2) indistinct border, 3) absence of mass effect and 4) subsegmental appearance. We presented a case of focal fatty liver with spherical shape and distinct border, resembling the findings of liver tumor. In this case we could not rule out the liver metastasis from rectal cancer to the end. CT value and follow up examination are very useful to establish the proper diagnosis.

Vascular "sphincter" and microangioarchitecture in the central nervous system: constriction of intraparenchymal blood vessels following a treatment of vasoconstrictive neurotransmitter.

The site of action of neuropeptide Y (NPY), a potent vasoconstrictive neurotransmitter, on the intraparenchymal blood vessels in the rat parietal cortex was demonstrated using a corrosion cast technique with scanning electron microscopy. Our observations were confined to the cortical area where the regional cerebral blood flow (rCBF) had been reduced significantly by in situ application of NPY. A striking finding in that area was the diffuse narrowing of the perforating arteries in the upper cortical layers. Ring-like compressions on the corrosion casts, presumably formed by active vascular sphincters along the arteries, capillaries and venules within the brain parenchyma, seemed to be more prominent in the perforating arteries of the NPY-treated cortex as compared with those of control cortex. We conclude that NPY-containing nerve fibers along the parenchymal blood vessels may take part in regulating the rCBF primarily by reducing the caliber of the proximal perforating arteries.
