[Fluorometric determination of selenium in breast milk: studies on wet ashing].

A fluorometric method for the determination of selenium content in breast milk was studied. After the wet ashing of breast milk with nitric acid and perchloric acid at 160 degrees C for about 2.5-3 hours and the reduction with 6 mol/l hydrochloric acid, selenium was determined by 2,3-diamino-naphthalene fluorescence reaction. The method described here is very simple and all procedures were performed in a single-test-tube, and fifty samples could be assayed within 5 hours. With this method, analytical recoveries of 20 ng/ml and 50 ng/ml selenium added to breast milk were 95.3% (n = 10, cv = 5.96%) and 100.8% (n = 10, cv = 5.72%). Selenium contents of breast milk obtained from 37 Japanese women delivered healthy were 22.0-71.6 ng/ml, mean = 40.79 ng/ml (1 week postpartum) and 12.0-40.8 ng/ml, mean = 24.52 ng/ml (1 month postpartum).

Characteristics of hepatocellular carcinoma originating in the caudate lobe.

Nine patients with hepatocellular carcinoma originating in the caudate lobe who underwent hepatic resection were studied. The caudate lobe was divided into three parts, according to the criteria of Kumon, including the Spiegel lobe, the paracaval portion and the caudate process. The tumors were located in the Spiegel lobe in four, the paracaval portion in four and the caudate process in one. Surgical procedures consisted of right hepatic lobectomy in one, central bisegmentectomy in one and caudate lobectomy in seven. The mean surgical time was 379 +/- 129 min; the mean estimated blood loss was 2,994 +/- 2,303 ml. The above-mentioned surgical risks were more clearly recognized in the paracaval portion than in the Spiegel lobe. In addition, most patients experienced significant post-operative complications. Six of eight patients with 6 mo or longer follow-up had recurrences, and two of six patients died. The characteristics of hepatocellular carcinoma in the caudate lobe were as follows: (a) a higher surgical risk, and more definite risk in the paracaval portion; and (b) a higher rate of early recurrence. A left lobectomy for the Spiegel lobe, a right or left trisegmentectomy for the paracaval portion and a right lobectomy for the caudate process would be ideal from the point of view of the portal supply of the caudate lobe. However, in cirrhotic patients either a caudate lobectomy or partial resection might be preferable because longer survival can be expected.

[Cortical atrophy and higher cortical dysfunction in systemic lupus erythematosus].

The relationships between cortical atrophy and higher cortical dysfunction in systemic lupus erythematosus (SLE) were investigated. The subjects were 27 females, right-handed patients with SLE. None of them had a history of cerebro-vascular disorders. The CT index of cortical atrophy included Huckman number, Evans' ratio, Ventricular index, and Cella-media index calculated from the brain CT. As controls, 27 age- and sex- matched, healthy subjects were studied and compared with the SLE group. All underwent the following neuropsychological examinations: intelligence and mental state, linguistic function, and recognition and praxis. As a result, Huckman number and Evans' ratio were significantly higher in the SLE group than in controls, clearly showing evidence of cortical atrophy. With regard to the relationships between cortical atrophy and neuropsychological tests, there were significant relationships between digit span and Huckman number and Evans' ratio, and between word fluency and Evans' ratio. These findings suggested that the atrophy of frontal lobe was characteristic in SLE, being associated with dysfunction of frontal lobe.

Xanthogranulomatous cholecystitis masquerading as gallbladder carcinoma.

We herein present a case of xanthogranulomatous cholecystitis which involved both the liver and transverse colon, clinically mimicking gallbladder carcinoma. Such cases may sometimes be judged inoperable due to extensive extra-gallbladder invasion, and thus it is necessary for physicians to take this lesion into consideration when making a diagnosis. An intraoperative biopsy is necessary, therefore, even when the features seem to clearly indicate inoperable carcinoma.

Inhibition of allograft rejection by anti-T-cell receptor-alpha beta monoclonal antibodies preserving resistance to bacterial infection.

Anti-CD3 monoclonal antibody (mAb) has been administered in clinical organ transplantation to reverse acute allograft rejection; however, severe immunodeficiency can result from such mAb treatment and cause an increased incidence of opportunistic infections. Therefore, new model systems are required in order to establish better methods for suppressing allograft rejection while preserving resistance to opportunistic infections. In this study, we compared the effects of the in vivo administration of anti-T-cell receptor-alpha beta (TcR alpha beta) mAb, H57-597, with those of anti-CD3 mAb, 145-2C11. Much to our surprise, the in vivo administration of anti-TcR alpha beta mAb prior to skin grafting led to a longer allograft survival than that of anti-CD3 mAb at any of the comparable dosages examined. In the lymphoid organs of mice treated with anti-TcR alpha beta mAb, TcR alpha beta-bearing cells were almost completely depleted, while TcR gamma delta-bearing cells remained at a relatively increased level on day 14 after anti-TcR alpha beta mAb treatment. The in vitro stimulation by anti-TcR gamma delta mAb clearly showed that such TcR gamma delta-bearing cells were functionally intact. Furthermore, the mice treated with anti-TcR alpha beta mAb, but not anti-CD3 mAb, were observed to be resistant to infection with Listeria monocytogenes. Finally, treatment with H57-597, but not with 145-2C11, led to a marked prolongation of skin allograft survival in the thymectomized mice. These results strongly suggest that anti-TcR alpha beta mAb, which partially preserved anti-bacterial resistance, may be more effective in preventing graft rejection than anti-CD3 mAb in the periphery, and indicate that anti-TcR alpha beta mAb may thus be potentially applicable for human transplantation. In addition, these results also indicate that the TcR gamma delta-bearing cells alone, at least in the absence of TcR alpha beta-bearing cells, do not contribute to allograft rejection in vivo.

[A case of chronic necrotizing pulmonary aspergillosis effectively treated with miconazole inhalation].

A 63-year-old male was hospitalized due to persistent cough on August 19, 1991. He was suspected to have pulmonary tuberculosis based on the findings of chest roentgenograms. He was therefore given anti-tuberculous drugs and antibiotics as of the first hospital day. Approximately two months later, he developed high fever, and the infiltrative shadows on his chest roentgenograms worsened. Therefore, he was readmitted to our hospital on October 31, 1991. Despite the use of several antibiotics, his high fever persisted and the infiltrative shadows on chest roentgenography continued to worsen after admission. Although no pathogenic microorganisms were isolated, in spite of frequent sputum examinations, we strongly suspected pulmonary aspergillosis because serum precipitin antibody against aspergillus antigens was positive. Intravenous administration of miconazole was thus initiated and lead to reduction in the fever. The fever recurred but, after the start of miconazole inhalation, gradually diminished. Serum precipitin antibody subsequently became negative. We diagnosed this case as chronic necrotizing pulmonary aspergillosis based on the clinical course. It is often difficult to confirm the presence of aspergillus hyphae. Therefore, this disease should be suspected on the basis of serum precipitin antibody. In such cases, a therapeutic diagnosis should be attempted.

Effects of estrogen and progesterone on platelet-activating factor acetylhydrolase activity in ovariectomised rats.

OBJECTIVE: To evaluate the effect of estrogen and progesterone on plasma platelet-activating factor acetylhydrolase (PAF-AH) activity in ovariectomized rats. METHODS: Plasma PAF-AH activity was examined in ovariectomized rats treated with 17 alpha-ethynylestradiol (0.25 mg/kg; 6 days) or medroxyprogesterone (50 mg/kg; 6 days). The intra- and inter-assay coefficients of variation for plasma PAF-AH activity were 2.5% and 4.4%, respectively. Plasma lipoprotein profiles on day 5 were determined with commercially available kits. RESULTS: Ovariectomized rats showed no significant change in plasma PAF-AH activity over a 14 day period. Estrogen decreased the PAF-AH activity up to 85%, and progesterone increased PAF-AH activity up to 44% in ovariectomized rats. There was a positive correlation between plasma PAF-AH activity and high density lipoprotein cholesterol (r = 0.96, p < 0.001). CONCLUSION: These data suggest that estrogen and progesterone exert an effect on plasma PAF-AH activity in ovariectomized rats, possibly via high density lipoprotein cholesterol.

Pilot study of cyclophosphamide-doxorubicin-vincristine-cisplatin-etoposide hybrid chemotherapy in small cell lung cancer.

BACKGROUND: Small cell lung cancer (SCLC) is highly sensitive to chemotherapy. Despite the introduction of intensive combination chemotherapy, long-term disease-free survivors are still rare. The emergence of drug-resistant tumor cells during chemotherapy is presumed to be the major cause of poor outcome. METHODS: A pilot Phase II study of hybrid chemotherapy for patients with SCLC was conducted between October 1986 and March 1988. Dose and schedule for each drug in the regimen were as follows: cyclophosphamide, 700 mg/m2 intravenously (IV), day 1; doxorubicin, 30 mg/m2 IV, day 1; vincristine, 1.4 mg/m2 IV, day 1; cisplatin, 60 mg/m2 IV, day 8; and etoposide, 100 mg/m2 IV, days 8 and 9. Courses were repeated every 4 weeks for up to six cycles. Patients with limited disease (LD) received chest irradiation of 5000 cGy when a maximal response was achieved. Only patients with LD who achieved a complete response (CR) received prophylactic cranial irradiation of 3000 cGy. RESULTS: Thirty-six patients were enrolled and fully evaluated for tumor response and toxicity. All 20 patients with LD responded to the regimen, and 14 (70%) of those achieved a CR. Of 16 patients with extensive disease (ED), 7 CR and 7 partial responses were noted, indicating an overall response rate of 88%. The median survival time was 23.6 months for patients with LD and 12.6 months for those with ED. Myelosuppression was the major toxicity, but it was generally well tolerated. CONCLUSIONS: These results indicate that the hybrid regimen is a highly active one for the treatment of patients with SCLC and warrants additional clinical trials.

Role of basic residues for the binding of omega-conotoxin GVIA to N-type calcium channels.

Each of four basic residues of omega-conotoxin GVIA was replaced with alanine to study the role of basic residues for the binding of this toxin to N-type calcium channels. The activities of these analogs were estimated from the inhibitory action on 125I-omega-conotoxin GVIA binding to chick brain synaptic plasma membranes. The replacement of Arg17, Lys24 and Arg25 resulted in no significant change in the activity and all of the analogs gave the same IC50 value (0.15 nM) as that of native omega-conotoxin GVIA. The inhibitory action of [Ala2]omega-conotoxin GVIA (K2A) was 40-times less potent (IC50 = 5.5 nM); however, full inhibition was achieved at a concentration above 0.1 microM. These results indicate that the Arg residue is not essential for the activity of omega-conotoxin GVIA. The nature of association to ion channels may be different between omega-conotoxin GVIA and mu-conotoxin GIIIA.

Bidirectional modulation of long-term potentiation by carbachol via M1 and M2 muscarinic receptors in guinea pig hippocampal mossy fiber-CA3 synapses.

Participation of muscarinic M1 and M2 receptors in the modulation of long-term potentiation (LTP) was studied in the mossy fiber-CA3 synapse of guinea pig hippocampal slices. The magnitude of tetanus-induced LTP was attenuated in the presence of 0.01-0.1 microM carbachol, at which concentration the pre-tetanus amplitude of field excitatory postsynaptic potential (fEPSP) was not affected. The attenuation of LTP by the low concentration of carbachol was reversed by an M2 muscarinic antagonist, AF-DX 116, but not by an M1 antagonist, pirenzepine. On the contrary, a high concentration (10 microM) of carbachol decreased the pre-tetanic amplitude of fEPSP, however, the magnitude of LTP was significantly larger than that in control slices in which pre-tetanic amplitude of fEPSP was reduced to the level of carbachol-treated slices by reducing the intensity of stimulation or extracellular Ca2+ concentration. The augmentation of LTP by 10 microM carbachol was blocked by pirenzepine but not by AF-DX 116. These results suggest that the synaptic plasticity in the guinea pig hippocampal mossy fiber-CA3 synapse is inhibited and facilitated by muscarinic agonist through muscarinic M2 and M1 receptors to inhibit and facilitate the LTP, respectively.

Regeneration across 'stepping-stone' nerve grafts.

The ability of small nerve segments interposed between synthetic conduits to increase the total nerve gap distance across which successful nerve regeneration would occur was studied. Fifty adult male Lewis rats were randomized into five groups. In Group I a segment of resected sciatic nerve was repaired by a nerve graft. Group II had alternating silicone tubing/nerve graft/silicone tubing replacement of the resected nerve segment (single stepping stone group). Group III had silicone tubing/nerve graft/silicone tubing/nerve graft/silicone tubing repair of the nerve deficit (double stepping stone). Group IV had a single long silicone conduit repair. Group V control underwent a sham operation. Nerve regeneration was evaluated using walking track pattern analysis, electrophysiologic assessment and histomorphological evaluation. 'Stepping stone nerve grafts' enhanced regeneration across nerve gaps in comparison to a single long conduit, but were inferior to a single long nerve graft. In the repair of long nerve gaps, the use of multiple short conduits with interposed short nerve segments could provide a source of trophic factors to enhance regeneration.

V gene analysis of anticardiolipin antibodies from MRL-lpr/lpr mice.

We analyzed the VH and V kappa genes of 14 hybridomas producing anticardiolipin antibodies (aCL) from MRL-lpr/lpr mice, in an attempt to address the question of whether aCL are generated by Ag-driven stimulation or by polyclonal B cell activation. Five of six aCL from one mouse and both aCL from the other mouse carried 98 to 100% homologous VH and V kappa genes, respectively, thereby indicating that aCL were derived from the oligoclonal B cell precursor in that individual mouse. Of nine clones, six (67%) used the VH gene of the J558 family, and three (43%) of seven clones used the V kappa gene of the V kappa 23 group. Three to five somatic mutations were detected in all VH and V kappa genes of the examined aCL. These results suggest the possibility that usage of VH/V kappa genes in aCL is not random and that aCL consist of somatically mutated Ig genes.

[Transcatheter hepatic segmental chemo-Lipiodol-embolization against hepatocellular carcinoma accompanied by tumor thrombosis in the portal vein].

We performed transcatheter hepatic segmental chemo-Lipiodol-embolization against hepatocellular carcinoma accompanied by tumor thrombosis in the portal vein. CT scan after embolization demonstrated good Lipiodol accumulation in the tumor thrombosis in some cases. In one resected case, remarkable necrosis of tumor thrombosis was proved histologically. In five non-resected cases, 6-month survival rate was 100%. We hope this therapeutic method can be useful for hepatocellular carcinoma with tumor thrombosis in the portal vein.

Appearance of calretinin-immunoreactive neurons in the upper layers of the rat superior colliculus after eye enucleation.

The effects of retinal deafferentation on a calcium-binding protein, calretinin, in the upper layers (superficial gray layer and optic nerve layer) of the rat superior colliculus were examined. In intact rats and on the ipsilateral side of unilaterally eye-enucleated rats, the superficial gray layer and optic nerve layer contained a few dispersed calretinin-immunoreactive cells. On the contralateral side to the enucleation, the number of immunostained cells in the superficial gray layer and optic nerve layer was increased. These findings suggest that retinal deafferentation results in an increase in contents of calretinin in some cell bodies within the upper layers of the superior colliculus.

[Three-dimensional MR angiography of the celiac artery with single breath holding].

We performed a three-dimensional MR angiography of the celiac artery with single breath holding. The measuring time was just twenty-three seconds. In four healthy volunteers, celiac arteries were visualized clearly, moved cephalad with expiration and caudad with inspiration. In two patients with the median arcuate ligament compression of the celiac artery, the degree of stenosis increased with deep expiration and decreased with deep inspiration. Using this technique, we obtained different vascular images with each respiratory phase. This technique is considered to be a useful three-dimensional MR angiography in the abdominal region without respiratory artifacts.

Structural and functional analyses of the Arg-Gly-Asp sequence introduced into human lysozyme.

To determine the functional conformation of the Arg-Gly-Asp (RGD) sequence, we have constructed mutant proteins by inserting 4-12 amino acid residues from the RGD region of human fibronectin between Val74 and Asn75 of human lysozyme. RGDS-, GRGDSP-, TGRGDSPA-, VTGRGDSPAS-, and AVTGRGDS-PASS-introduced mutant lysozymes were expressed in yeast, purified, and designated as RGD4, -6, -8, -10, and -12, respectively. Using baby hamster kidney cells, RGD8, RGD10, and RGD12 were shown to possess high cell adhesion activity nearly equal to 10% of human vitronectin activity. RGD4 and RGD6 exhibited somewhat lower cell adhesion activity. The activities of these mutant proteins were inhibited by the addition of either GRGDSP peptide or polyclonal antibody against vitronectin receptor, as was the case for the vitronectin activity. The results suggest that the cell adhesion signals are transduced to cells through the interaction with the vitronectin receptor. The three-dimensional structures of RGD4 and RGD8 were determined at 1.8-A resolution by x-ray crystallography. A model of the inserted region in RGD4 could be built in the electron density map, but the positions of the preceding residues, Ala73-Val74, were uncertain. The inserted region in RGD8 did not demonstrate continuous electron densities. The results suggest that these RGD sequence-containing regions are highly flexible and that such flexibility could allow the conformation of the RGD regions to be induced to fit into the binding pocket of the integrin receptor.

[Clinical significance of CT in evaluation of the gastrocolic trunk and its tributaries].

The gastrocolic trunk (GT), consists of the veins from stomach, duodenum, pancreas and colon, passes transversely in front of the head of the pancreas and drains to the superior mesenteric vein at the lebel of the uncus. Pancreatic and/or portal venous pathology may cause dilatation or occlusion of the GT. The purpose of this study is to clarify the usefulness of X-ray computed tomography (CT) for evaluation of the GT and its tributaries in the diagnosis of pancreatic disease. The CT examinations of 176 patients (97 with pancreatic disease, 79 without pancreatic or liver disease for comparison) were reviewed retrospectively. In the comparison group, a normal GT was identified in 81% of CT scans with 5mm thick sections. Abnormal findings of the GT and its tributaries were seen in 30 cases with the involvement(occlusion or stenosis) of the portal venous system in the group of pancreatic diseases. The GTs were occluded in 17 cases with involvement of GT or SMV and dilated in 5 cases with involvement of the portal vein, SMV, or splenic vein by pancreatic diseases. In 3 cases, GTs were minimally dilated without abnormality of the portal venous system. The right gastroepiploic veins were dilated in 15 cases with splenic vein obstruction (5 patients), GT obstruction (6), SMV stenosis (3), stenosis of main portal vein (1). Assessment of the GT and its tributaries on CT would be useful for evaluation of the extension of the pancreatic disease.
