Deletions of p15 and/or p16 genes as a poor-prognosis factor in adult T-cell leukemia.

PURPOSE: To determine the frequency of the deletions of p15/p16 genes in adult T-cell leukemia (ATL) cells and to evaluate their value in the diagnosis of clinical subtypes of ATL patients and the prediction of their clinical outcome. MATERIALS AND METHODS: Peripheral-blood samples from 114 patients with ATL were examined by Southern blot analysis. In five chronic-type patients who showed disease progression to acute type, serial samples also were examined. RESULTS: Among 114 patients, 28 (24.6%) showed the deletions of p15 and/or p16 genes. The results were well correlated with the clinical subtypes. Patients with deleted p15 and/or p16 genes had significantly shorter survival times than the patients in whom both genes were preserved (P < .0001). A similar decline in survival time was observed in the analyses within the same subtypes. In multivariate analysis using the Cox proportional hazard model, the deletions of p15 and/or p16 genes emerged as an independent prognostic indicator. Moreover, three of the five chronic-type patients who progressed to acute type lost the p16 gene alone or both the p15 and p16 genes at their exacerbation phase. CONCLUSION: The results suggest the following: (1) that the deletions of p15 and/or p16 genes play a key role in the progression of ATL; and (2) that these deletions are reliable prognostic factors that predict shortened survival times.

Synthesis and antimicrobial characteristics of 4,4'-(alpha,omega-polymethylenedithio)bis(1-alkylpyridinium iodide)s.

Bis-quaternary ammonium compounds (bis-QACs), 4,4'-(alpha,omega-polymethylenedithio)bis(1-alkylpyridinium iodide)s (4DTBP-m,n), which have 3 to 10 carbon atoms in the connecting methylene chain (m) and 8 to 18 carbon atoms of the N-alkyl chain (n), were synthesized. 4DTBP-6,12 exhibited a wide antimicrobial spectrum against gram-positive and gram-negative bacteria and fungi. The activity was stronger than those of N-dodecylpyridinium iodide (P-12), benzyldodecyldimethylammonium chloride and 2-(4-thiazolyl)benzimidazole. The bactericidal activities of 4DTBP-m,n were scarcely affected by the lengths of the alkyl chain and methylene chain. The bis-QAC that showed the highest activity was 4DTBP-6,8 (minimum inhibitory concentration (MIC) = 1.6 microM, minimum bactericidal concentration (MBC) = 2.6 microM), and its activity was about 10 times that of N-hexadecylpyridinium iodide (P-16), which was the most active in the P-n series. In addition, 4DTBP-6,12 showed a high bactericidal activity in the ranges of pH 5 to 8.5 and 10 to 40 degrees C, in contrast to mono-QACs. The bis-QACs synthesized in this study have excellent bactericidal properties.

Ether extract of fetal calf serum protects cultured rat cortical neurons against glutamate cytotoxicity.

The effects of an ether extract of fetal calf serum (EE-FCS) on glutamate-induced cytotoxicity were examined using primary cultures of rat cortical neurons. The simultaneous addition of EE-FCS and glutamate reduced glutamate cytotoxicity in a concentration-dependent manner. EE-FCS also reduced the cytotoxicity induced by S-nitrosocysteine, a nitric oxide (NO) donor. The findings indicate that EE-FCS contains lipid-soluble, non-peptide substances with neuroprotective actions against NO-mediated glutamate neurotoxicity.

Efferent targets of osseous CGRP-immunoreactive nerve fiber before and after bone destruction in adjuvant arthritic rat: an ultramorphological study on their terminal-target relations.

We report the ultramorphological characterization of the terminal-target relation of sensory peptidergic nerve fibers in healthy and diseased osseous tissues. Bone tissue sections were immunoelectronmicroscopically investigated for calcitonin gene-related peptide (CGRP), a neuropeptide widely distributed in sensory peptidergic fibers. Ultramorphological relation of the osseous CGRP-immunoreactive (ir) nerve terminals and their target cells was comparatively analyzed using healthy, arthritic, and postarthritic bone specimens from control and adjuvant-induced arthritic rats. Terminal-like profiles of the osseous CGRP-ir axons were evidenced in direct contact with the metaphyseal osteoblasts and osteoclasts of the control animals. Terminal-like profiles were also noted in the vicinity of the periosteal lining cells. Nonterminal-like profiles did not make intimate spatial relation to the cells/structures surrounding the nerve. Osseous CGRP-ir terminals and axons, which are either uncovered or thinly ensheathed by the supportive tissues, were extensively degenerated in adjuvant-induced infiltration, whereas larger fibers were relatively resistant. Numerous CGRP-ir axons with distinctive features reinnervated the postarthritic, ossifying periosteum. CGRP-ir axons appeared to reinnervate the eroded surface of metaphyseal bone and cartilage as early as the recruited osteoblasts resume osteogenesis in the postarthritic metaphysis. The observed terminal-target relations in the healthy and diseased bone tissues give an ultramorphological basis for the putative trophic, modulatory actions of CGRP innervation of the bone cells.

A histological investigation on tissue responses to titanium implants in cortical bone of the rat femur.

Implant materials are placed under various sites-including cortical bone, spongy bone, and bone marrow-at the same time according to the depth at implantation. Although cortical bone is an important site for the prognosis of implantation, detailed reports on tissue responses to implantation have been meager. The present study aims to reveal tissue responses to pure titanium implantation in rat femoris cortical bone. The rats received titanium bars surgically in their femurs and were sacrificed 1 day to 40 weeks post-implantation. The prepared tissue specimens were processed for light and transmission electron microscopy (TEM). Further histochemical detections were performed. One day post-implantation, empty osteocytic lacunae indicating degeneration of osteocytes were found in pre-existing cortical bone around the implant. Such pre-existing bone was replaced by new bone, but remained in part even 40 weeks post-implantation. Light microscopy showed that direct contact between the implant and new bone was identified 12 weeks post-implantation. Chronological and ultrastructural observation showed that new bone deposition appeared to proceed toward the implant, and that the intervening layer at the interface was derived from the degenerated debris of multinucleated giant cells and/or osteoblasts. Furthermore, it seemed that the width of intervening layer varied in relation to the distance from the blood vessels. The cells showing tartrate resistant acid phosphatase activity possessed cytological features of osteoclasts under TEM; they were frequently observed in perivascular sites near the implants even after osseointegration, suggesting that bone remodeling took place steadily around the implant.

Blood lactate disappearance during breathing hyperoxic gas after exercise in two different physical fitness groups--on the work load fixed at 70% VO2max.

The purpose of this study is to evaluate effects of breathing hyperoxic gas on blood lactate disappearance after submaximal exercise in two different physical fitness groups and to clarify the most effective oxygen concentration in each group. Fourteen healthy male students participated as subjects in this study. They were divided in two groups by difference in their anaerobic threshold (AT). Seven males were treated as Higher AT group and the others as Lower AT group. Subjects wore a T-shirt, short pants and sports shoes and performed three sessions; each consisting of five minutes of exercise and six minutes of rest, at a workload of 70%VO2max on a bicycle ergometer. Hyperoxic gas was breathed only during recovery periods. Oxygen rates of 21, 30, 40, 60, 80 and 100% in inspired gas were employed. According to the results of blood lactate (BLA), the most effective oxygen condition on BLA disappearance was obtained over 60% in Higher AT group and at 30% oxygen in Lower AT group. Thus, it was especially noteworthy that the effects of hyperoxic gas in Higher AT group were different from those of Lower AT group. It is thought that the effects of breathing hyperoxic gas were dependent on physical fitness, which have caused many reports to be in conflict hitherto.

Serum levels of soluble Fas/APO-1 receptor in human retroviral infection and associated diseases.

Fas/APO-1 mediates apoptosis via Fas and Fas ligand transduction. Recently, a soluble form of Fas (sFas) was described which seems to be functionally implicated in the Fas signal system, suggesting a relationship between some disorders and sFas function. We measured sFas-levels in sera from normal controls and patients with disorders linked to human retroviral infection of human immunodeficiency virus (HIV) and human T-cell leukemia virus type-1 (HTLV-1). The sFas level of normal controls. HTLV-1 carriers seronegative for HIV, and patients with HTLV-1 associated myelopathy/tropical paraparesis (HAM/TSP), adult T-cell leukemia (ATL), and AIDS was 1.62 +/- 0.49, 1.90 +/- 0.49, 2.00 +/- 0.59, 3.32 +/- 2.05, and 3.06 +/- 0.92 ng/ml, respectively. Although the level of sFas in patient groups with HAM/TSP, ATL, and AIDS was significantly high in comparison to that of normal controls (p < 0.01), the individual values were highly variable within the groups. The sFas level was statistically correlated to the soluble interleukin-2 receptor (sIL-2R) level, as well as to cells expressing membrane Fas (mFas), indicating the same cellular origin. In some ATL cases, however, serum sFas levels and mFas expression density on leukemic T-cells were discrepant, with especially high levels of the soluble form and a lack of expression of the membrane form observed in 2 cases, sFas detection could serve as a putative marker for active diseases in patients with ATL and AIDS.

[A case of antiphospholipid antibody syndrome successfully treated with prednisolone and low-dose aspirin, with subsequent normal pregnancy].

We reported a 26-year-old female who had experienced 2 miscarriages associated with a possible cause of antiphospholipid antibody. She consulted to Kainan Municipal Hospital with major complaints of miscarriages and positive antiphospholipid antibody. No evidence of obstetric disorder was apparent and routine haematological parameters were normal. Serological studies and clinical evidence did not show SLE. While biologically false positive (reaction of serological test for syphilis) and lupus anticoagulant was negative, IgG anticardiolipin antibody was positive. She was diagnosed as possible antiphospholipid antibody syndrome based on positive anticardiolipin antibody and the history of miscarriages. In 1993, she was started on prednisolone and aspirin therapy; antiphospholipid antibody returned to negative. Treatment with prednisolone (7.5 mg/day) and aspirin (40 mg/day) was continued. She was able to be pregnant. She gave birth successfully at our hospital in August 1995. She and her newborn showed no abnormalities.

Characterization of cytotoxic T lymphocytes recognizing autologous tumor cells with plural HLA-class I alleles in a patient with esophageal cancer.

T cell sublines and clones were established from the parental HLA-A2601-restricted cytotoxic T lymphocyte line (KE-4 CTL, HLA-A2601/2402, -B5101/40012, -Cw0102/0302) to investigate their capability to recognize tumor antigens on HLA-class I alleles other than HLA-A2601. Thirty-nine of 52 (75%) sublines or 30 of 83 (36%) clones produced interferon (IFN)-gamma by recognition of the autologous KE-4 tumor cells. Among these 39 CTL sublines, 22, 22, and 2 sublines produced IFN-gamma by recognition of HLA-A2402+, -B5101+, and Cw0102+ allogenic tumor cells, respectively. Four, 8, and 4 of these 30 CTL clones also produced IFN-gamma by recognition of HLA-A2402+, -B5101+, and Cw0102+ allogenic tumor cells, respectively. The other 13 sublines or 53 clones failed to produce IFN-gamma by recognition of the autologous tumor cells, respectively. These results suggest the presence of CTL recognizing shared tumor antigens on autologous tumor cells expressed by plural HLA-class I alleles in a patient with esophageal SCC.

Frequent hepatic involvement in adult T cell leukemia: comparison with non-Hodgkin's lymphoma.

We examined 111 patients with acute type- or lymphoma type-adult T-cell leukemia (ATL) and compared them with 106 patients with non-Hodgkin's lymphoma (NHL). In addition to skin involvement and hypercalcemia which are already known to be frequent in ATL, ATL patients showed an higher incidence of hepatic involvement. There was more frequent palpable hepatomegaly, higher total bilirubin, GOT, GPT, lactate dehydrogenase (LDH), and alkaline phosphatase values in ATL than in NHL patients (p < 0.0001). Among 36 autopsied liver samples, invasion of ATL cells was confirmed in 22 cases. ATL patients with impaired hepatic function showed shorter survival times than patients without hepatic dysfunction. Moreover, ATL patients showed a worse performance status (PS), a higher incidence of lytic bone lesions, lower total protein (TP) and serum albumin levels than NHL patients. This invasive characters of ATL cells and consequent impaired general condition seemed to be factors affecting the poor prognosis recorded in ATL.

[Clinico-pathological findings of gastrointestinal tract in the leprosy patients].

Endoscopic biopsies either or both for the stomach and colon looking for chronic gastritis and carcinomata followed by the histopathologic examinations of the mucosa were applied to long term leprosy inpatients and controls. 1) In chronic gastritis, glandular atrophy with cellular infiltration and submucosal fibrosis likewise intestinal metaplasia were predominated in the leprosy patients, in the comparison of the cases consisted of 30 leprosy patients and 16 controls. No characteristic changes for leprosy cases were encountered accordingly, however, as the sequences of the medications. 2) The observations of Helicobacter pylori (HP) by means of immunohistologic method were also applied to gastritic cases. However, the positive rates in the above groups were not much different one another. 3) The cases of gastric carcinoma were 14 during 14 years period, among which 9 showed protruded lesions indicating metaplastic changes as the precancerous lesion. 4) The carcinoma of the colon tended to be left sided in the leprosy patients.

[A calculation of an index of the coronary flow reserve using exercise and rest myocardial SPECT with 99mTc-tetrofosmin].

We designed a new method to evaluate the rate of change of myocardial 99mTc-tetrofosmin (TF) uptake between exercise and rest images with a 1-day protocol. Fifteen normal cases were studied. A 370 MBq of TF was injected at peak exercise. Initial (TF1) and delayed (TF2) exercise SPECT images were acquired 30 min and 3 hr after the injection. A 740 MBq of TF was reinjected soon after TF2 acquisition, and rest SPECT images (TF3) were obtained 30 min after the reinjection. Myocardial counts of TF1, TF2 and TF3 were defined C1, C2 and C3 respectively. Then, the rate of change of myocardial TF uptake between exercise and rest (delta MTU: delta myocardial TF uptake) was determined by the following formula. delta MTU = [(C1/(C3-C2)] x R-1] x 100 (%) (R: dose ratio). delta MTU was 57.8 +/- 9.9% in normal cases and roughly correlated with maximal heart rates and double product. If the extraction fraction of TF is not changed under the exercise and resting condition, delta MTU may have some relation with the coronary flow/cardiac output ratio at exercise. In conclusion, delta MTU is a useful index to evaluate coronary flow reserve non-invasively.

[Evaluation of expected AUC of carboplatin (CBDCA) predicted by Chatelut's formula in comparison with Calvert's].

Several studies have shown that carboplatin AUC seems to correlate with therapeutic outcome and hematologic toxicity. Calvert's formula predicts carboplatin AUC using GFR estimated by the 51Cr-EDTA method. It is very complicated to manage radioactive agents, so we use 24-hour creatinine clearance before carboplatin administration as GFR in Calvert's. Recently, Chatelut reported a formula to predict carboplatin AUC using serum creatinine as a variable parameter of renal function as well as age, sex and body weight. We measured the AUC of free Pt by serial sampling in 14 ovarian cancer patients who received carboplatin by one-hour infusion in the initial treatment course. We evaluated the two predictive formulas with measured AUC. Calvert's predicted AUC showed a linear correlation with actual AUC (r = 0.768, p = 0.0013). But using Chatelut's formula, the predicted AUC had a better correlation with actual AUC (r = 0.833, p = 0.0002). Actual AUC correlated inversely with nadir PLT counts (r = -0.707, p = 0.0047). In conclusion, Chatelut's prediction had a better correlation with the actual AUC of carboplatin and nadir of thrombocyte count than Calvert's. Chatelut's formula was thus more useful to predict the carboplatin AUC than Calvert's when 24-hour creatinine clearance was used as GFR.

[Image-guided surgery for epilepsy].

Availability of a neuronavigation system for epilepsy surgery was reported, and its practical use was discussed. Four of nine patients with intractable epilepsy underwent surgical procedures using a neuronavigation system, Viewing wand, from November 1995 to August 1996, in our hospital. The ages of patients were between 9 to 46 years old. Three of them had temporal lobe epilepsy and one had generalized tonic seizures. One of the temporal lobe epilepsy cases had focal cortical dysplasia in the left posterior temporal lobe, and the other one showed that left hippocampal atrophy on MR images. The remaining two patient had no abnormality on MR images. All patients underwent video-EEG monitoring and habitual seizures were recorded at least three times. Ictal and/or interictal SPECT and neuropsychological testing were also performed. Electrocorticograms were recorded intraoperatively in all patients. Surgical procedures using the neuronavigation system were anterior temporal lobectomy, corpus callosotomy and lesionectomy of focal cortical dysplasia. A patient with temporal lobe epilepsy underwent implantation of depth electrodes under the neuronavigation. In temporal lobectomy, image-guided surgery helped to make a decision concerning the safely-resectable size of the lateral temporal cortex and hippocampus. The hippocampus was resected with minimum surgical damage and it made possible a complete histopathological examination. In corpus callosotomy, although it was not easy to confirm the length of the callosal section, the neuronavigation system enabled this to be done quickly. The real-time navigation showed the accurate operating position on three-dimensional images. The location of focal cortical dysplasia was often difficult to identify macroscopically. However, the location of the lesion can be projected to the skin surface under the neuronavigation system. The width of skin incision and craniotomy was able to be made smaller, and the surgery was able to be performed less invasively. The Viewing Wand system was accurate, reliable and easy to operate in these procedures. The navigating error was 2-5 mm. Using CT image data of 5 mm thickness the error was greater, although use of MR image data of 2 mm thickness resulted in relatively small error up to 2-3 mm. The first major factor of the error was the fiducial registration of the patient's head. While the registration was made more strictly with multiple fiducial points, the error was smaller. The second factor was movement of patient's head and/or the navigation arm. The arm and the head should be fixed tightly to the operating table, and it is better if they are fixed together with a supporting arm. The third factor was intraoperative brain shift caused by flow out of the cerebrospinal fluid or removal of mass lesions. This type of error is common in all navigation systems. However, it may be avoided making some real-time feedback system. With the Viewing Wand system, repetition of the intraoperative registration using intracranial anatomical structures reduces this type of error. On the other hand, there were some difficulties on stereotaxic procedures, such as implantation of depth electrodes, using the Viewing Wand. The error was larger than that recorded in other frame-based stereotaxic apparatus. This problem may be improved by a supporting system to fix the probe position. As a neuronavigation system can be widely applied to neurosurgical procedures, we consider that epilepsy and skull-base surgery are the best targets for it because of the minimum possible brain shift. We hope that accurate and less-invasive surgery using a neuronavigation system will contribute to a better outcome for epilepsy patients.

[The relation between pyrimidine nucleoside phosphorylase (PyNPase) activity in colorectal cancer tissues and pathological effects].

Pyrimidine nucleoside phosphorylase (PyNPase) and IL-1 alpha concentration were measured in colorectal cancer tissues from 15 patients who preoperatively administered 5'-DFUR orally. The relation between changes of the PyNPase activity and pathological effects of resected specimens was studied. PyNPase activity changes showed that the increase cases were 9 patients and the decrease cases were 6 patients in pre- and post-administration of 5'-DFUR. The IL-1 alpha concentration in cancer tissues was 2933.65 +/- 2038.65 in the decrease cases and 1207.51 +/- 1378.83 in the increase cases. The level was significantly higher in the decrease case (p < 0.05). And there tended to be more pathologically effective in the decrease cases than in increase cases. The effects of preoperative oral chemotherapy in colorectal cancer was strongly influenced by changes of PyNPase activity and IL-1 alpha concentration in tumor tissues.

[Trial of radiation and cisplatin, carboplatin combination chemotherapy for advanced cancer (W-platinum chemoradiotherapy].

Cisplatin was reported to be an effective radiation sensitizing agent. The effect was also reported to depend on dose intensity. But the incidence of complication was demonstrated in the relationship with the dose escalation curve of anti-cancer agent. The major side effect of CDDP was occasionally serious, as in renal toxicity or bone marrow suppression. W-Platinum is the trial of concurrent chemoradiotherapy. Cisplatin and its derivative, Carboplatin, were selected as effective radiation sensitizing agents and to obtain high-dose intensity and additive cytotoxicities by interaction between the two drugs. Concomitant administration of two platinum anti-cancer agents has the advantage of reduction of side effects compared with administration of single anti-cancer agents to the same degree. The first case was a recurrence of epipharyngeal cancer after 3 courses of chemotherapy, including CDDP or CBDCA. This case was suspected to be cancer-resistant to CDDP. The second case was post-operative residual lung cancer. The pre-operative diagnosis was stage III A. A poor prognosis was expected. This case was disease-free and alive for 1 year after W-Platinum administration. The most frequent complication was bone marrow suppression. Patients were rescued from bone marrow suppression with administration of G-CSF. Renal toxicity could be suppressed with sufficient hydration.

[TCR repertoire of autoreactive T cells in autoimmune disorders].

In human autoimmune diseases such as rheumatoid arthritis, Sjögren's syndrome, and multiple sclerosis, it has been clarified that autoreactive T cells play a crucial role in the generation of autoimmune disorders. Immunohistochemical studies have shown that most infiltrating lymphocytes are CD4 positive alpha beta T cells. Recent studies with polymerase chain reaction (PCR) provides evidence about the T cell receptor (TCR) V beta and V alpha genes on their T cells. Sequence analysis of the complementarity determining region 3 (CDR3), which is a central portion for recognition of antigens by T cells, indicates some conserved amino acid motifs, supporting the notion that infiltrating T cells recognize relatively few epitopes on autoantigen.

[Experimental study of the effectiveness of immunosuppression therapy using FK506 and 15-deoxyspergualin in concordant xenotransplantation].

The effect of combination therapy with FK506 (FK) and 15-deoxyspergualin (DSG) was investigated in a concordant xenotransplantation model in which hamster hearts were implanted into LEW rats. Forty xenograft procedures were carried out in this study. Recipients were divided into eight groups. Control Group received no therapy. Group A, B, and C were administered different dosages of FK; 0.75, 1.0 and 1.25 (mg/kg/day), respectively. In Group D and E, DSG at 10 and 20 mg/kg/day was administered after the transplantation. In Group F and G received combined administration of FK and DSG (Group F; FK 0.5 + DSG 10, Group G; FK 0.5 + DSG 5). Recipients were injected with immunosuppressive agent intraperitoneally from day 0 until cessation of heart beat. At the time of grafting, splenectomy was performed in all groups. FK treatment of the recipients did not lead to significant improvement in graft survival, 7.2 +/- 0.8 and 6.3 +/- 0.7 days, compared with 3.5 +/- 1.23 days in Control. DSG groups showed xenograft survival of 11.5 +/- 0.9 and 8.1 +/- 1.6 days, respectively. The combined therapy prolonged graft survival to 33.67 +/- 11.37 days in Group F, and to 47.3 +/- 11.02 days in Group G. Combined therapy produced complete suppression of antibody formation, but in FK 0.5 + DSG 10 mg/kg/ day treated group, there was abrupt elevation of anti-hamster antibody titer at the time of rejection. Only in combined therapy groups did recipients obtain long-term graft survival, but recipients receiving treatment with FK 0.5 + DSG 10 (mg/kg/day) suffered from severe emaciation, losing nearly 31% of their body weight. However, animals that received a low dose of DSG in combined therapy groups did not lose body weight over 8 weeks. Combination therapy with FK and DSG was shown to prolong graft survival time effectively, and FK 0.5 + DSG 5 (mg/kg/day) was an appropriate dose in xenotransplantation with minimal side effects.
