An epidemiological survey on ossification of ligaments in the cervical and thoracic spine in individuals over 50 years of age.

An epidemiological survey on ossification of the spinal ligaments was performed on a total of 1,058 subjects over the age of 50 years by means of roentgenography of the cervical and thoracic spine. Ossification of the posterior longitudinal ligament (OPLL) of the cervical spine was detected in 34 subjects (3.2%) with a predilection for men, whereas OPLL in the thoracic spine was found in 8 (0.8%). There were 325 cases (30.7%) of ossification of the anterior longitudinal ligament (OALL) of stage II or above by Forestier's classification in the region from the cervical to thoracic vertebrae, and these cases included a significantly greater number of men. Ossification of the ligamenta flava (OLF) was observed in 48 cases (4.5%). As for the coexistence of ossification of these ligaments, 364 individuals (34.4%) had at least one instance of OPLL and OALL (stage II or above) in the region from the cervical to thoracic spine, and OLF in the thoracic spine.

[Clinical evaluation of astromicin administered by intravenous drip infusion. Report III. Bacterial infections in the field of urology].

Intravenous drip infusion of a new aminoglycoside agent, astromicin (ASTM), was used for various urological infections, and its clinical efficacy and safety were studied. Each of almost all the patients tested was given 200 mg of ASTM twice a day by intravenous drip infusion. Among 114 cases with complicated urinary tract infections (UTI) evaluated according to the criteria for evaluation of drug efficacy in UTI, clinical response was excellent in 16, moderate in 54 and poor in 44. The efficacy rate was 61.4%. Among 137 cases who were evaluated for side effects, headache was observed in 1 case (0.7%), which recovered spontaneously the day after the withdrawal of the drug. Slight elevation in S-GOT, S-GPT, Al-P and others was observed in 12 cases (8.8%) as abnormal laboratory test values.

[Cooperative phase II study of epirubicin (EPI) in bladder cancer, renal pelvic and ureteral tumors--Urological Cooperative Study Group of EPI].

A phase II study of a new anthracycline anti-cancer antibiotics, epirubicin (EPI), was undertaken in 71 patients with urothelial malignancies; 40 with advanced urothelial malignancies and 31 with superficial bladder cancer. Out of them 32 patients with advanced stage of urothelial cancer were evaluated for the systemic use of EPI, while 30 patients with superficial bladder cancer for intravesical use. Intravenous administration of this new anticancer antibiotic, at a dosage of 60 mg/m2 every three weeks, showed the response rate of 20.0% for advanced bladder cancer and 14.3% for renal pelvic and ureteral tumors. In cases of superficial bladder cancer, at a dosage of 60 mg/30 ml X 3 day every week in principal, the response rate was 66.7%. Eight out of 30 patients showed complete disappearance of the tumor. Twelve patients also showed more than 50% tumor regression. As for adverse effects no serious cardiotoxicity was demonstrated. Anorexia and other gastrointestinal side effects, such as nausea and vomiting, were also seen. Alopecia and myelosuppression were the major adverse effects among patients with systemic EPI administration. With intravesical use of EPI, cystitis syndrome was the major toxicity. However, no systemic side effects were noted in these cases. In conclusion, EPI was assumed to be effective for the treatment of advanced urothelial tumors and superficial bladder cancer.

[Study of coagulum pyelolithotomy].

Coagulum pyelolithotomy was carried out on 20 patients from January, 1980 to April 1984. Nephrolithotomy was jointly carried out on 4 of them, but residual stones were observed in 4 cases (20%). No side effect such as hepatitis was observed in any cases. While inferring the cause of residue, experimental study was made on tension, coagulation time and mixing method in respect to coagulum formation. In the experiment on tension, no significant difference was observed when thrombin concentration was changed, but when calcium was added, tension increased 4 fold. As thrombin concentration rose, so shortened the coagulation time. It was inferred that a better coagulum is made available if fibrinogen and thrombin are mixed outside the kidney.

[Diagnosis of renal trauma].

We experienced 25 cases of renal trauma in our hospitals during the past 5 years, 20 cases were male and 5 cases were female, ranging in age 10 to 51 years old with average 26 years old, and the chief complaints were gross-hematuria and pain of renal region. We studied the IVP and CT patterns of these cases. IVP revealed 13 cases of renal contusion, 11 cases of renal lacerated wound and 1 case of renal fragmentation. The CT scan of 14 cases showed that it was useful for the diagnosis of the degree of renal trauma, especially, for hematoma surroundings the kidneys. The hematoma surrounding the kidneys was clearly noted in 7 cases by CT scan, but the unclearness of renal contours and iliopsoas muscle was considered to suggest the existence of hematoma surrounding the kidneys by IVP. All of the hematoma surrounding the kidneys was absorbed by conservative therapy, but 5 of the 7 cases were treated within 3 months.

[The clinical effects of Tsumura No. 111 Seishin-rensi-in on irregular complaints of the urinary tract].

We studied the effects of Tsumura No. 111. Seisin-rensi-in on irregular complaints accompanying urological and renal diseases. Of 105 patients who visited our medical facilities, 99 served as the subject of this study. In principle, Tsumura Seisin-rensi-in was administered at a dose of 2.5 g x 3 times/day before meals for 4 or more weeks. Of 99 cases, 8 showed excellent results (8.2%), and 42 showed positive effects (41.2%). Adverse effects were observed in 7 cases, and in 4 cases drug administration was discontinued. This drug was recognized to be effective against irregular complaints accompanying chronic cystitis and acute or chronic pyelonephritis.

[Relation between fibrin/fibrinogen degradation products in malignant fluids and the fibrinolytic activity of the peritoneum].

The mechanism of production of fibrin/fibrinogen degradation products (FDP) in malignant fluid was investigated. Levels of FDP in three malignant fluids were high (1200-3000 mcg/ml), but they declined to about 500 mcg/ml after anti cancer treatment. The average levels of FDP in 12 malignant fluids were almost the same as those in nine benign fluids. In a vitro experiment, the fibrinolytic activity of the peritoneum was found to be much higher than that of the tumor. In conclusion, production of FDP in fluid is more closely related to the fibrinolytic activity of the peritoneum than that of the tumor. Intracavitary administration of antifibrinolytic agents is thought to be effect for malignant effusion.

[Phase II collaborative study of (2''R)-4'-0-tetrahydropyranyladriamycin (THP) for urological malignancies--Urological Co-operative THP Study Group].

A Phase II clinical trial of a new anthracycline, (2''R)-4'-0-tetrahydropyranyladriamycin (THP), was performed in 137 patients with urological malignancies. Out of them, 111 patients were evaluated for tumor responses and 125 patients were evaluated for adverse effects. In cases of intravenous administration, overall response rate was 18.5% (22.2% for bladder cancer, 30.0% for tumors of the renal pelvis and ureter, and 6.7% for prostatic cancer). In the case of intra-arterial administration, overall response rate was 42.9% (50.0% for bladder cancer). For 50 patients with superficial bladder cancer intravesical chemotherapy with THP was performed. Sixteen patients showed complete disappearance of the tumor, 2 patients showed more than 90% tumor regression and 12 patients showed more than 50% tumor regression, respectively. Overall response rate was 60%. Cardiotoxicity was minimal. Alopecia was noted in a total of 16 patients, but this was minimal. Leukocytopenia was the major adverse effect among patients undergoing systemic THP administration. In conclusion, THP was most effective against transitional cell carcinoma of the urinary tract.

[Diagnosis and treatment of renal angiomyolipoma].

We have hitherto reported 6 cases of renal angiomyolipoma. Recently, we encountered two more such cases. Case 1 is a 34-year-old woman with fever as the chief complaint. DIP revealed a tumor mass in the right upper pelvic pole. This mass showed a strong echo level on ECHO and adipose tissue of low density on CT scan. Therefore, the patient was diagnosed as having renal angiomyolipoma. Since liposarcoma was not ruled out by the examination of frozen sections during operation, nephrectomy was performed. Case 2 is a 40-year-old woman. Diagnosed as having bilateral renal angiomyolipoma, she underwent right nephrectomy 14 years ago. Two years ago, she had heavy hematuria, and had embolization of the left renal artery. She has had no bleeding since the embolization. We are of the view that ECHO and CT can are very useful for diagnosis of renal angiomyolipoma, and embolization for heavy hematuria, a complication, should be performed first of all.

[5-FU concentration in blood and tissue of patients with renal cell carcinoma after administration of UFT].

UFT (3 capsules; 300mg FT) was administered to five of 10 patients with renal cell carcinoma, and concentrations of FT, 5-FU and uracil in the serum and tissues (normal renal tissues, renal tumor tissues and liver) were determined 5.2 hours on average, after administration. The levels were also compared with these in the five patients administered 300 mg of FT. There was no difference in FT concentration between the serum and the tissues in the group administered UFT, but the concentration of 5-FU in tumor tissues was significantly higher (25.6 times) than that in the serum. The level was also higher (3.2 times) than that in normal renal tissues. There was a positive correlation between the concentration of 5-FU in the tissues and the concentration of uracil in the tissues. Although there was no difference in the concentration of FT between serum and tissues in patients administered UFT or FT, the concentration of 5-FU in patients administered UFT was definitely higher than that in patients administered FT; the concentration of 5-FU in the tumor tissues of patients given UFT was 3.9 times higher than in those given FT. Thus, UFT induced a concentration of 5-FU in tumor tissues that was maintained at a high level, suggesting that an excellent antitumor effect on renal cell carcinoma can be expected with UFT.

[Clinical study of UFT in renal cell carcinoma].

A clinical study of UFT, a mixture of FT and uracil in a molar ratio of 1 : 4, was conducted on 12 patients with renal cell carcinoma. UFT was continuously administered at doses of 300, 400 or 600 mg per day. CR was recorded in 2 patients, PR in 2, NC in 7 and PD in 2, respectively. The overall response rate was 33.3%. Concerning side effects, 2 of the 12 patients suffered from anorexia and stomatitis, but no hepatic disorders or bone marrow suppression was observed. We concluded that UFT therapy was effective for renal cell carcinoma.
