Velocardiofacial syndrome is the most common microdeletion syndrome in humans. It is secondary to a chromosome 22q11 rearrangement and is characterized by craniofacial abnormalities, heart defects and learning disability. We report a case of a 10-year-old girl with a chromosome 22q11 deletion who, in addition to learning difficulties, hypernasal speech and mild dysmorphic features, had weakness and wasting of the shoulder girdle muscles but no cardiac involvement. Brain magnetic resonance imaging revealed narrowing of the cervicomedullary junction. The clinical features of this patient with velocardiofacial syndrome further expand the spectrum of abnormalities associated with this condition.
This report concerns the cases of three European caucasian brothers who developed keloid scars as teenagers. The eldest brother's keloid followed surgery, the second brother's followed ear piercing and the youngest brother's followed chicken pox scarring. No other member of any generation of their family is known to have suffered from the condition. Such a family as this, with an inherited tendency to form keloids, provides an interesting source of material for genetic research. An understanding of the genetics of keloids, particularly the isolation of a specific gene or group of genes could prove a major advance in the understanding and subsequently in the treatment of the condition.

An audit of patients with velopharyngeal incompetence treated by the Orticochea pharyngoplasty was undertaken. 54 patients who had been treated by a single surgeon over a 10-year period were identified. All patients had pre- and postoperative speech therapy and were followed up for an average of 6 months. 49/54 patients had lessening of their nasal escape; in 40/54 it was eliminated completely. Nasal resonance was assessed as normal in 46/54 of patients postoperatively. The results are compared with those of other similar studies.

The Nicolle prosthesis was used to replace 101 metacarpophalangeal joints in patients with rheumatoid arthritis over a ten-year period. Mean active flexion achieved was 30 degrees. Mean ulnar deviation was 27 degrees. 4% of joints had to be removed after a mean interval of 42 months, for infection. None of the implants had fractured.
A method of continuously assessing peroperative blood loss is presented. A calibrated trap which collects all the blood loss is placed in the suction tubing. Bipolar diathermy and suction maintain a bloodless wound. A test using 20 ml of fresh venous blood showed that approximately 5 ml remained clotted in the tubing and 15 ml entered the trap. The method is particularly applicable to cleft lip and palate surgery.
Since 1981 the Leicestershire Support Service has offered a combination of professional and lay advice to parents of new-born cleft babies. This paper evaluates this service by interviewing retrospectively the parents of two groups of children, those born before the service was available and those born since. As a control group, a prospective study of parents with unclefted children was carried out. The strengths and weaknesses of the current Support Service are identified and recommendations for improvements are proposed.

This paper presents the results of 80 out of 98 severed flexor tendons repaired between 1976 and January 1981 using the Kleinert technique with immediate dynamic splintage. In this series 13 repairs were in the thumb and 85 in the fingers (19 in Zone 1; 66 in Zone 2). Only 10% of the injuries were industrial. A total of 80 tendon repairs were available for assessment using the methods of White and Kleinert; 60% of these were graded excellent to good, with 55% excellent to good for Zone 2 repairs. An "injury score" has been devised to indicate severity of injury and this correlated well with poor results in Zone 2 injuries. Better results were obtained in the young and in well motivated patients. Delayed repairs did badly, especially in Zone 2.
Twelve patients with lichen sclerosus et atrophicus of the vulva were treated by a nitrous oxide cryoprobe. All had had severe itching which was relieved in the post-operative period in 75% and for 3 years in 50%. Eight had histologically proven leukoplakia which cleared post-operatively in five (62.5%) and remained clear at three years in four (50%). In one patient pruritus, leukoplakia and ultimately squamous carcinoma appeared.
The problems of extensive scarring involving the median and ulnar nerves at the wrist are discussed. Possible methods of separating the dermal cicatrix from the nerves are skin flaps or muscle flaps. A case is presented in which an abductor digiti minimi muscle flap was used. The reasons for this choice are discussed and the technique described.
