Odor sensitivity is impaired in HIV-positive cognitively impaired patients.

An estimated 7-28% of patients infected by the human immunodeficiency virus (HIV) develop dementia and at least 50% develop mild neurocognitive impairment. Past studies have shown odor identification impairments in HIV + neurocognitively impaired patients. It is difficult, however, based on an odor identification test to state with certainty that individuals with cognitive impairment have sensory olfactory deficits, because odor identification tests are known to draw upon cognitive skills. In the present study odor detection sensitivity was evaluated using an ascending, forced-choice, two-alternative, odor threshold test for butanol. Subjects were divided into three groups, HIV seropositive (HIV+) neurocognitively impaired, HIV+ neurocognitively unimpaired, and HIV negative, based on neurological and psychological testing. An analysis of variance revealed significantly poorer odor sensitivity for the HIV+ neurocognitively impaired group than for the two control groups. A significant negative correlation between degree of cognitive impairment and olfactory sensitivity was also found. We suspect that the olfactory deficits found in the HIV+ neurocognitively impaired subjects are primarily due to damage to the central nervous system; however, nasal infection may be a contributing etiology.

Liposuction to debulk the pectoralis major myocutaneous flap.

Pectoralis major myocutaneous flaps are very useful for head and neck reconstruction. However, excess bulk in these flaps due to subcutaneous fat can be a disadvantage in oral cavity, oropharyngeal, and cervical esophageal reconstruction. A variety of surgical modifications have been tried in an effort to overcome this problem. Liposuction was proposed by a member of this research group as a means to debulk such flaps. The purpose of this fresh cadaver study was to determine what effect this procedure might have on blood supply to the skin overlying the pectoralis major muscle. Gross and histologic observations suggest that liposuction would not significantly affect the vascular perfusion of these flaps. This report represents a preliminary study previous to a clinical trial using a potentially useful technique. It is hoped by the authors that this report will stimulate others to evaluate it's possible clinical utility.

Epidermoid carcinoma arising from within an epidermoid inclusion cyst.

The unusual occurrence of epidermoid carcinoma within an inclusion cyst arising on the forehead of a patient is documented. The importance of any irregular change or clinically abnormal behavior in such a lesion is underscored and the necessity for routine histological examination of all tissue excised is reaffirmed.

Nasofrontal angle changes in rhinoplasty.

In emphasizing changes in the nasal tip, the rhinoplastic surgeon often neglects modifications that should be made in other aspects of the profile. Techniques that advance, retrodisplace, and inferiorly or superiorly displace the nasofrontal angle are discussed, calling attention to this part of the profile. Nasofrontal angle modifications can (1) eliminate certain hazards of tip changing along, (2) make possible effects that cannot be achieved with tip positioning alone, and (3) make rhinoplasty more predictable in certain cases.

Ethical dilemmas in head and neck cancer.

Ethical dilemmas in head and neck cancer daily confront physicians, patients, families, and society at large. For every situation there are potential benefits and real burdens, as well as risks. There are no "right" answers for any given situation, but there are processes for assisting the primary decision makers involved in reaching meaningful and appropriate decisions. This paper asks some pertinent ethical questions and suggests some ways of responding to the issues.
