[A study of genetic epidemiology on child bronchial asthma].

OBJECTIVE: To examine the relationship between child bronchial asthma and genetic fastor. METHODS: A case-control study including 372 pedigree was carried out. RESULTS: Child bronchial asthma had obvious familiar clustering (chi 2 = 24.8, P < 0.01). The prevalence rate of the first and the second degree relatives of proband was significally higher than that of the controls. The segregation ration of asthma was 0.14 (95% CL 0.098-0.18). The h2 of the first and second degree relatives of asthma were 78.18% and 55.02%, of which 83.64% for female and 73.48% for male. CONCLUSION: The genetic model of asthma belonged to polygenetics. Genetic fastor was a main risk fastor for asthma, especially for female patients.

[Repairing the soft tissue defect in the heel with a double-bridge pattern flap of the posterior ankle].

OBJECTIVE: This is to introduce a method for repairing the soft tissue defect in the heel. METHODS: The double-bridge pattern flap in the posterior ankle region was designed with the pedicles of the flap being in each side of the malleoli. According to the defect, the lower bridge-pattern flap with an arc-shaped upper margin was designed first. Then the upper bridge-pattern flap was designed with a "lambda"-shaped margin. The flap was raised with the deep fascia and advanced down to cover the defect. In dissecting the flap, care should be taken not to damage the posterior tibial nerve and blood vessel bundle. The flap was used in 37 patients. RESULTS: All the flaps survived except 2, in whom partial necrosis occurred in the upper bridge-pattern flap; they healed after dress changings. Follow-up of more than one year indicated that no ulcer took place in all the transferred flaps. CONCLUSION: The blood supply and elasticity of the flap are good. Its sensation and resistance to friction and shearing are satisfactory. Distinguishable distance of two points in the flaps was 13 to 14 mm.

[The use of reverse fasciocutaneous pedicle flap in repairing electrical burn wound on extremity].

OBJECTIVE: This is to introduce a method for reconstruction of extremities with electrical injury. METHODS: Thirty-six reverse fasciocutaneous pedicle flaps were designed to be used for the reconstruction of extremities with electrical injury in 20 patients. The size of the flaps ranged from 5 cm x 3 cm to 18 cm x 8 cm. The length and width of the pedicle ranged 3-6 cm and 1.5-4 cm. The ratio of the length to width of the flap ranged from 1.6:1 to 4.5:1. All flaps had been successfully transplanted with 180 degrees rotation. RESULTS: All flaps survived, except individual flaps showing mild swelling and disperse blisters. CONCLUSION: Satisfactory results both in functional and cosmetic aspects had been achieved with this kind of flap after followed-up for over one year.

Holistic approach to health education on AIDS.

OBJECTIVE: To identify a method for health education using the example of an educational seminar on AIDS prevention and control at Hebei Medical University in China, 1994. METHODS: Our goal is the theoretical formulation of a unified community approach for problem solving through the combination of the self regulative approach and the case analytical approach. "Quality improvement", "essential quality" and "human ecosystems" provide the basic ideas for better understanding the integration of culture, science and technology within the "cultural norm". RESULTS: The present seminar was successfully organized for Chinese medical students based on the theme of "living with AIDS". Through this program, we identified that self regulative and case analytical approach were major issues for holistic approach to health education. We also identified that a community approach provides the scientific basis for health education in the community. CONCLUSIONS: The present paper generates a healthcare paradigm for surveillance in health/medical education. Through a better understanding of community approach, we related "community care" (emphasizing self, mutual, professional and institutional care) to the "practical norm"; and "preventive epidemiology" (emphasizing the unification of quality and quantity, as well as systems and population) to the "research norm".

[Experiences of early management of mass burn casualties].

OBJECTIVE: Inquire into the effective method of treatment to mass burn casualties. METHODS: The experiences of 22 batches of mass burns (403 cases) were reviewed. RESULTS: 1. The receiving hospital should take effective measures for initial care, and report to the higher health department. 2. Health department should send additional specialists to the hospital if necessary, and a leader should be appointed. 3. Ambulances with special pass or helicopter should be deployed to ensure dispatch of the helpers. 4. Emphasis on local hospital's initial treatments, including fluid resuscitation, establishment of an adequate airway, early wound management to prevent and premature dissolution of eschar. 5. Proper triage of patients, and severe burn patients should be hospitalized to burn units with adequate facilities. 6. Transportation: Patients can be transferred along where there are good transportation facilities, with fluid resuscitation, on the other hand, when transportation if poorly developed, transfer the patients after fluid resuscitation until the general condition becomes stable. CONCLUSION: 1. Mass burn casualties can be handled with satisfactory results if emergency care and professional treatment are well organized. 2. Mass burn casualties are usually caused by human errors, therefore they can be prevented.

[A pathological study on the correlation of HPV infection with precancerous cervical lesions].

OBJECTIVE: To study the correlation of cervical condyloma and cancerous lesions with HPV infection. METHODS: Cervical biopsies and histopathological examinations were performed on 179 cases which had abnormal cervical cytological smears. PCR was used to study the HPV-DNA of 128 cases and in situ hybridization (ISH) was used to study 10 cases. RESULTS: 1. Morphologic observations. Most cervical condyloma cases were of the morphologically flat type (97.7%). Two koilocyte types were observed, the classical, or so called diagnostic koilocyte type (39.7%) and the atypical type (60.3%). Cervical condyloma often occurred together with cervical intraepithelial neoplasia (CIN, 42.5%). 2. PCR HPV DNA subtype analysis. Of the 58 cervical flat condyloma, 5 were PHV6/11 positive (8.6%) and 28 were HPV16/18 positive (48.3%). 86.1% of those with atypical koilocyte and 9.1% of those with diagnostic koilocyte had HPV16/18 infection. 66.7% of the lesions in which condyloma coexisted with CIN(2-3) had HPV16/18 infection. CONCLUSIONS: Most cervical condyloma lesions were of the flat type. The appearance of atypical koilocyte is correlated to HPV16/18 infection, which in turn is correlated to the degree of CIN malignancy.

[Effects of p21WAF1/CIP1 gene introduction on a human lung cancer cell line with p53 mutation].

OBJECTIVE: To study the effects of p21WAF1/CIP1 gene expression on the biological hehavior of cancer cells with p53 gene mutation. METHODS: Expression of p21 vehicle was constructed and introduced into a human lung cancer cell ling PG with p53 gene mutation, the morphological and growth characteristics of the transfectant were then analysed. The CDK4 and PCNA levels were evaluated and the response to genotoxic agents were observed. RESULTS: The PG cells with mutant p53 were transfected with ectopic p21 gene which resulted in overexpression of p21 (6-fold of the control). The transfectants presented decreased nuclear irregularity and thinner membrane in nuclear atypia, with delayed growth rate and contact growth inhibition. The transfectant also displayed high serum requirement in culture and increased anchorage dependence. Both levels of CDK4 and PCNA were decreased. However, the cisplatin induced cell apoptosis was delayed. CONCLUSIONS: Expression of p21WAF1/CIP1 gene was effective in cancer suppression even in p53 gene mutation. This may have been achieved through lowering of CDK4 or PCNA levels and not involving the apoptosis mechanism. Activation of p21WAF1/CIP1 gene may be considered a bypass for restoration of p53 gene function in cancer cells with p53 gene mutation.

[Determination of three flavonoids and ferulic acid in Pleurospermum hookeri C. B. Clarke var. thomsonii C. B. Clarke by HPLC].

OBJECTIVE: To establish a reversed phase HPLC method for separation and determination of licochalcone A, ferulic acid, isoliquiritigenin and nobiletin in Pleurospermum hookeri var. thomsonii. METHOD: Using ODS column with methanol-water(containing 1% acetic acid)(73:27, 28:72, 59:41) and methanol-water(65:35) as the mobile phase, the flow rate was 1 ml/min, the column temperature was 35 degrees C and UV detection at 345, 320, 345, 332 nm, different extraction methods for the sample were compared. RESULT: The average recoveries were 99.45%, 98.13%, 97.81% and 96.58%, the relative standard deviations were 0.83%, 0.99%, 0.73% and 1.55%, and the contents were 0.728, 0.124, 0.0830 and 0.0237 mg/g. CONCLUSION: This study provides a scientific basis for quality evaluation, quality control and resources development and utilization of P. hookeri var. thomsonii.

[Experimental studies on hyperthermic distilled water combining with saline and dextran for preventing intraperitoneal cancer recurrence due to neoplasm seeding].

In order to know the efficacy of hyperthermic distilled water combining with saline and dextran (DSD) to prevent intraperitoneal cancer from recurrence due to neoplasm seeding, we carried out the experimental studies, including an observation on inhibiting cancer cells from implantation in vivo by abdominal cavity lavation, cytotoxic test and attachment test in vitro. The results of the tests in vitro showed no viable tumor cell exposed to 45 degrees C distilled water for 20 minutes in the tissue culture wells, meanwhile, dextran-40 partially blocked attachment of B16-F10 cells to gelatinized wells. In vivo test, DSD effectively prevented neoplasm seeding of B16-F10 viable melanoma cells in C57BL/6j mouse abdominal cavity and had no obvious toxicity. The efficiency was 70%; when cisplatin was added, the efficiency was enhanced to 88.9% with slight toxicity. These data indicate that the local application of hyperthermic distilled water combined with saline and DSD may prevent intraperitoneal neoplasm from recurring.

[Direct and indirect recognition in pig-to-man xenotransplantation].

OBJECTIVE: To investigate the role of direct and indirect recognition in pig-to-man xenotransplantation. METHODS: Taken the peripheral blood lymphocytes (PBLC) from three Neijiang pigs and two humans as stimulators and respondors, the one-way mixed lymphatic reactions (MLR) of xenograft were carried out, and allo- and self-PBLC as control. RESULTS: Among the three patterns of MLR, syngeneic was MLR the lowest in proliferation, the allogenic MLR was the highest, and the xenogenic MLR was medium. The PBLCs from humans and pigs were matched on HLA-A, B, DR and DQ by means of modified Terasaki assay. The match on pigs was failure because of the pre-existing natural xenogenic antibody in the testing serum. CONCLUSION: The results suggest that the degree of MHC matching still affect the rejection in xenotransplantation, but the present serum assay of MHC matching is not fit for pig.

[Comparative study on livertaxis of lactosaminated human growth hormone and galactosyl-human growth hormone].

This study was conducted to compare the livertaxis of lactosaminated human growth hormone (hGH-L) with that of galactosyl human growth hormone (hGH-Gal), using radioactive tracer technique. hGH-L and hGH-Gal were first prepared. After hGH-L, hGH-Gal and hGH were labelled with 131I or 125I, the data on biodistribution in mice, imaging in rabbits and chicken were obtained. hGH-L and hGH-Gal had remarkable livertaxis and their percentages for uptake in liver were 68.83%-74.65% and 68.18%-74% respectively, which were about two times as high as that of hGH. All of the hepatic bindings were receptor-mediated. But, the synthesizing of hGH-L was more easy-to-do, more mild in reaction conditions, and more economical of raw material as compared with the synthesizing of hGH-Gal. Therefore, hGH-L is a more potential receptor-mediated hepatic targeting secreting somatomedian drug for treating dwarfism.

[Effects of simulated flight hypobaric hypoxia and oxygen inhalation on free radical metabolism in various organs of mice].

Objective. To investigate the effects of two types of simulated flight conditions (hypobaric hypoxia and hypobaric oxygen inhalation) on free radical metabolism in various organs of mice. Method. Sixty male Kunming mice were randomly divided into six groups (n=10 each). The experiment comprises two parts. The first part included three groups: normal controls (A1), 1500 m hypobaric hypoxia for 4 wk (B1) and 8 wk (C1). The second part included another three groups: normal control (A2), 5500 m hypobaric oxygen inhalation for 4 wk (B2) and 8 wk (C2). The exposure time in hypobaric chamber was 2 h/d, 3 d/wk. After experiment, caudal blood was taken for routine examination. The mice were decapitated on the next day and brain, heart, lung, liver and kidney homogenates were prepared for measuring malondialdehyde (MDA) content and superoxide dismutase (SOD) activity. Result. Lipid peroxides in lung was significantly increased in C1 group, and the content of myocardial MDA and myocardial SOD activities in C2 group were markedly higher than those in A2 group. There were no significant differences among body weights, mean corpuscular indices and hemoglobin content in the normal control, hypobaric hypoxia and hypobaric oxygen inhalation groups. It demonstrates that repeated mild hypobaric hypoxia for 8 wk causes free radical damage of lung and repeated exposure to 5500 m hypobaric oxygen inhalation for 8 wk may lead to myocardial peroxidative injury in mice. Conclusion. Simulated flight hypobaric hypoxia and oxygen inhalation may lead to free radicals damage of lung and myocardial peroxidative injury in mice.

[A dynamic study of influences of TMJOA on TMJ disc].

OBJECTIVE: To learn the changes of TMJ disc positions under the influences of TMJ Osteoarthrosis(TMJOA). METHODS: The TMJOA experimental animal model was established by 12 goats. The TMJ disc positions were observed and recorded at different intervals, (1 month, 2 months, 3 months, 4 months and 6 months) then the pathological changes of TMJ specimens were also studied. RESULTS: Significant anteromedial shift of TMJ disc position was observed in some certain TMJOA phases and became more obviously along with the disease progress. CONCLUSION: The shift of TMJ disc may be the secondary change of TMJOA.

[Effects of growth factors on DNA and collagen synthesis of human mandibular condylar cartilage cells].

OBJECTIVE: To investigate the effects of transforming growth factor-beta 1 (TGF-beta 1), insulin-like growth factor-I (IGF-I) and basic fibroblast growth factor(bFGF) on DNA and collagen synthesis of mandibular condylar cartilage(MCC) of human fetus. METHODS: Cell culture, 3H-TdR and 3H-Proline incorporation methods were used. MCC cells were harvested from 4 to 5 months old human fetus. Cells were seeded at 2 x 10(4)/well on 96-well Plate. After synchronization, medium was replaced by DMEM containing 0.4% NCS with various growth factors and concentrations. RESULTS: bFGF stimulated the DNA synthesis significantly, and IGF-I had less effect, while the effect of TGF-beta 1 was insignificant. For collagen synthesis, bFGF caused a dose-dependent increase(60%). A greater effect(98%) was achieved when IGF-1 was added. In contrast, TGF-beta 1 could inhibit collagen synthesis (24%). CONCLUSION: Growth factors play an important part in the proliferation and matrix synthesis of MCC cells, which might be of potential application in treating cartilage destructive lesions.

[Influence of pavulon on facial nerve monitoring during middle ear-mastoid operation].

OBJECTIVE: To evaluate the influence of pavulon (muscle relaxant) on facial nerve monitoring for patient with middle ear-mastoid operation during general anesthesia. METHOD: Intraoperative monitoring of the facial nerve (FNM) was done in 45 ears (44 case). For patient with middle ear-mastoid disease, 38 ears underwent inhaled-intravenous anesthesia with muscle relaxant pavulon (group of general anesthesia), 7 ears underwent local anesthesia (group of local anesthesia). Accelography (ACCG) was used in 10 case for testing the correlation between of FNM and the various degree of recovery from ulnar nerve-musculor blockade. RESULT: FNM could be elicited during different periods of recovery with values of T4/T1 varying from 10%-98%. Pavulon could be used to eliminate muscular vibration from disturbing FNM. Electromyography (EMG) potentials were successfully recorded in 34 ears (89.4%). Anesthesia was uneventful and it may be controled satisfactority. CONCLUSION: Considering the advantage of pavulon. It can be used for general anesthesia during intraoperative monitoring of the facial nerve for children or adults suffering from local complicated diseases.

[Sequencing of cytochrome C oxidase 1 gene of Ancylostoma duodenale and Necator americanus].

AIM: To identify the genetic diversity between Ancylostoma duodenale and Necator americanus. METHODS: Mitochondrial cytochrome C oxidase subunit 1 (CO1) gene was amplified from genomic DNA of human hookworms collected from infected patients in Hejiang County, Sichuan Province, and the purified PCR products were directly sequenced by using Licor auto-sequencer. RESULTS: The PCR products were about 700 bp. Alignment of CO1 gene fragment sequences showed 89.7% similarity between Ancylostoma duodenale and Necator americanus, but still certain nucleotide variations (10.3%) existed. CONCLUSION: CO1 gene sequence can be used as a marker to identify the two species of human hookworms.

[Pharmacological study on asthma-relieving decoction (dingchuan tang) treating asthma].

Asthma-relieving decoction (Dingchuan tang) is a famous prescription of treating asthma, but its pharmacology has been not reported. This experiment showed that Asthma-relieving decoction (Dingchuan tang) can protect experimental asthma that blending of histamine and acetylcholine caused (P < 0.05 [symbol: see text] P < 0.01), can resist external trachea smooth muscle's contracting that histamine caused (P < 0.01), and can promote secretion of phenol red of rat's respiratory tract (P < 0.01). Results indicated that Asthma-relieving decoction (Dingchuan tang) has significant effect of relieving asthma and expelling phlegm.
