Stimulation of intestinal cytokinetics and mucin turnover in rats fed wheat bran or cellulose.

Rats were fed defined diets containing no fiber, 10% wheat bran or 10% cellulose, and intestinal morphology and cytokinetics were assessed by light microscopy and autoradiography, respectively. In bran-fed animals, there were no differences in morphological appearance of the jejunum, in the number of cells/villus column or in numbers of goblet cells compared to controls. Autoradiographic analysis, at one and 24 h after [3H]thymidine, however, suggested an increased turnover and villus transit of intestinal cells. There was also a 2.5 fold increase in incorporation of labeled sulfate, and a 2-fold increase in [3H]glucose incorporation into total intestinal glycoproteins and mucins. Similar, albeit less dramatic results were obtained in rats fed diets containing cellulose. These studies provide evidence that diets containing certain fiber derivatives can alter aspects of intestinal cell turnover, and support the earlier morphological observations suggesting increased goblet cell secretory activity in response to feeding these fiber derivatives.

Neoplastic proliferation in a case of epidermotropic high-grade malignant lymphoma associated with hypercalcemia.

We report a case of epidermotropic high-grade malignant lymphoma associated with hypercalcemia occurring in a European woman aged 30 years. The clinical, histological, and biological presentation of the patient could enter the spectrum of HOTS (hypercalcemia, osteolysis, T cell lymphoma syndrome). The proliferation of neoplastic lymphocytes was very high in patch, plaques, and nodules of the skin.

[Diagnosis and therapy of cutaneous T-cell lymphomas].

We report the observation of 16 cases of cutaneous T cell lymphoma consisting of 11 cases with mycosis fungoides and 5 cases with high-grade malignant lymphomas. A standardized clinical evaluation is proposed for patients with low-grade malignant lymphomas for whom we favor the use of non-aggressive therapeutic regimens. High-grade lymphomas need a more complete systemic exploration. Treatment of cutaneous T cell lymphomas should be started with the least aggressive antineoplastic therapy.

A comparative study between interdermal tests with phytohemagglutinin and delayed hypersensitivity reactions elicited by tuberculin.

Intradermal (i.d.) tests with phytohemagglutinin (PHA) and tuberculin are used in clinical practice to evaluate the cell-mediated immunity. The biologic significance of the skin response to PHA, clinically evaluated by the extent of erythema and induration, was studied histologically after incorporation of tritiated thymidine. It was compared to tuberculin tests. The rate of recruitment and the respective amount of inflammatory cells varied between patients and in time. In PHA tests the inflammatory reaction occurred in two phases, the first one consisting in the predominance of polymorphonuclears, the second one being characterized by chemotactism of lymphoblasts many of which synthesized DNA. The "in vivo" reaction to PHA is therefore complex; the same clinical evaluation corresponds to markedly different stages and events in the biological reaction. The histological grading of the PHA test is therefore desirable in the evaluation of the cell-mediated immunity because only the blastic proliferation is of importance in its rating and not the edema and the accumulation of polymorphonuclears.

Granulomatous vasculitis of Wegener.

We report a case of granulomatous vasculitis of Wegener. Systemic involvement occurred in spite of chemotherapy and concomitantly with the development of immunological disturbances. The differential diagnosis with other angiocentric granulomatoses and the estimation of the risk of extension to internal organs are primordial because of their prognostic importance.

Flexible programs for the prediction of average amphipathicity of multiply aligned homologous proteins: application to integral membrane transport proteins.

Simple flexible programs (TREEMOMENT and PILEUPMOMENT) are described for depicting the average amphipathicity (hydrophobic moment) along multiply aligned sequences of a family of evolutionarily related proteins. The programs are applicable to any number of aligned sequences and can be set for any desired angle corresponding to a residue repeat unit in a protein secondary structural element such as 100 degrees per residue for an alpha-helix or 180 degrees per residue for a beta-strand. These programs can be used to identify amphipathic regions common to the members of a protein family. The use of these programs is exemplified by showing that some families of integral membrane transport proteins (i.e. permeases of the bacterial phosphotransferase system (PTS) and the anion exchangers of animals) exhibit strikingly amphipathic alpha-helical structures immediately preceding the first hydrophobic transmembrane segment of their membrane-embedded domain(s). Other families, such as the major facilitator superfamily of uniporters, symporters and antiporters, do not exhibit this structural feature. The amphipathic structures in PTS permeases have been implicated in membrane insertion during biogenesis.

Pseudomyxoma peritonei: a case series and review of the literature.

The purpose of this study was to use descriptive methodology to study the management of patients with pseudomyxoma peritonei (PMP) at the Princess Margaret Hospital. This is a case series of patients with a diagnosis of PMP treated between January 1, 1995 and December 31, 2001. A health record search using the morphology code for PMP was done and identified 70 patients. Additionally, an unrestricted Medline search was conducted between 1990 and 2002 for PMP. Eight cases of PMP were treated by the Gynecologic Oncology service at Princess Margaret Hospital during the study period. The patients ranged in age from 43 to 84 and presented with a pelvic mass and/or increasing abdominal girth. All but two patients underwent appendectomy as part of their surgery. Postoperative management was by gynecologic oncology for seven of eight patients. An appendiceal origin was confirmed for six patients and highly suspicious for one patient. The remaining patient underwent appendectomy, but it was not sufficiently sectioned for diagnosis. Chemotherapy was given to patients in a non-uniform fashion. Removal of mucinous tumors of the ovary should include appendectomy. Consultation from a general surgical oncologist should be sought where appendiceal origin is confirmed. Coding errors may occur in medical records at a frequency greater than is anticipated.

Primary chemotherapy and adjuvant tumor debulking in the management of advanced-stage epithelial ovarian cancer.

The aim of this article was to review the experience with neoadjuvant chemotherapy and interval surgical debulking in patients with metastatic epithelial ovarian cancer. A retrospective chart review was carried out to identify patients treated with neoadjuvant platinum/Taxol chemotherapy and interval debulking. Cox regression modeling was used to identify significant predictors of progression-free interval. The Kaplan-Meier method was used to estimate the survival statistic for the study group. Sixty-one patients were identified after being treated with neoadjuvant chemotherapy and interval debulking surgeries. All surgeries were performed after three cycles of platinum/Taxol combination chemotherapy. Eighty percent of patients had a residual disease status of 2 cm or less after surgery. Suboptimal debulking was statistically associated with tumor involvement of the upper abdominal organs (P < 0.001) and non-normalization of CA125 before surgery (P= 0.03). The perioperative complication rate was 7%. At a mean follow-up time of 19 months, 77% of patients were still alive. Cox regression modeling identified the microscopic tumor residual status as the only significant predictor of progression-free interval. The estimated median survival for the group was 41.70 months (95% confidence interval = 13.84-69.56 months). Neoadjuvant chemotherapy with interval debulking surgery appeared to be safe and feasible in patients with metastatic epithelial ovarian carcinoma.

Quality of life assessments in epithelial ovarian cancer patients during and after chemotherapy.

To study the immediate and long-term effects of chemotherapy on quality of life (QoL) of advanced ovarian cancer patients. All consecutive patients undergoing chemotherapy for metastatic ovarian cancer and those presenting for follow-up post chemotherapy were recruited. Participants were asked to fill out a short QoL questionnaire (Functional Assessment Cancer Therapy-Ovarian) during each clinic visit. Two-factor analysis of variance analyses were used to examine the effects of chemotherapy treatment and current disease status on QoL scores. Ninety-four patients on chemotherapy and 159 follow-up patients participated. Patients on chemotherapy for recurrent disease had a significantly worsened overall, emotional, and ovarian cancer-specific concerns QoL scores compared to those receiving first-line chemotherapy. There were favorable significant differences between those on follow-up compared to those on chemotherapy in the mean overall QoL scores and in the means of physical, functional, and concern scores. There were significant differences favoring patients with complete response compared to those with partial response or progressive disease in the mean overall QoL scores as well as the physical, emotional, functional, and concern domains mean scores. There were improvements in most QoL measures after completion of chemotherapy. Complete disease remission remained important in maintaining improved QoL.

Prognostic significance of tumor necrosis in ovarian cancer patients treated with neoadjuvant chemotherapy and interval surgical debulking.

The objective of this study was to study the significance of tumor necrosis documented at the time of interval surgical debulking after neoadjuvant chemotherapy. Retrospective chart reviews were carried out from 1997 to 2005 to identify ovarian cancer patients treated with neoadjuvant chemotherapy. Patients' demographics together with disease characteristics, treatment-related variables, and outcomes were recorded. Cox proportional hazard models were built to model time to progression using predictor variables such as age, cancer stage, tumor grade, residual disease, percentage change in CA125 level from baseline, and degree of necrosis in resected tumor specimens. One hundred one patients were included in the study. Optimal debulking was achieved in 74% of the patients. Cox regressions revealed three significant predictive variables of time to first progression: younger age (hazard ratio [HR] = 0.95, 95% CI 0.92-0.98, P= 0.004), residual disease (P= 0.048), and the absence/minimal tumor necrosis after three cycles of neoadjuvant chemotherapy (HR = 1.97, 95% CI 1.01-3.87, P= 0.048). The estimated median survival was 50.66 months (95% CI 46.12-55.20). The lack of or minimal tumor necrosis after neoadjuvant chemotherapy is an independent risk factor for recurrent disease.

Psychologic morbidities prior to loop electrosurgical excision procedure in the treatment of cervical intraepithelial neoplasia.

The objective of this study was to assess and compare anxiety and distress in patients undergoing colposcopic examinations and loop electrosurgical excision procedure (LEEP). Patients seen for evaluation of cervical intraepithelial neoplasia (CIN) and LEEP were recruited. All patients received further teaching with respect to their abnormality right after the colposcopic evaluation by nursing staff. The Hospital anxiety and Depression Scale (HADS) and the Psychosocial Effects of Abnormal Pap Smears (PEAPS) questionnaires were used to measure and compare distress between the two groups. Linear regression models were built to identify significant predictive variables for psychologic morbidities. Twenty-one colposcopy and 20 LEEP patients participated in this study. No significant demographic differences were noted. Eighty-one percent of patients having colposcopy and 65% of those undergoing LEEP can be classified as having significant anxiety and depression based on the HADS questionnaire. Patients undergoing LEEP scored significantly better than colposcopy patients on the mean total PEAPS score and on the self-belief/cancer concern and effects on sexual relationship dimension scores. Significant psychologic morbidities exist in patients diagnosed with CIN. Face-to-face individualized education and support after colposcopy can decrease patients' distress at subsequent treatment visits.

Further studies on substrates inducing metacyclogenesis in Trypanosoma cruzi.

Epimastigotes of Trypanosoma cruzi, Peru strain, incubated in Contreras' artificial triatomine urine transformed into metacyclic trypomastigotes when 10 mM L-glutamine, L-asparagine or D-fructose was added to the medium. Metacyclogenesis with these substrates was comparable to the percent metacyclic morphotype formation induced by L-proline and significantly greater than that stimulated by 10 mM D-glucose. Sodium acetate (10 mM) increased transformation induced by L-proline, and L-hydroxyproline (10 mM) increased transformation induced by D-fructose. Phosphoenolpyruvate (10 mM) inhibited L-proline-induced metacyclic trypomastigote stage formation. Three antimetabolites, azetidine 2-carboxylate (5 mM), malonic acid (1 mM), and desthiobiotin (5 mM), completely inhibited D-fructose-induced but not L-proline-induced transformation. The Costa Rica, Y, and CL strains of T. cruzi showed different patterns of percent metacyclogenesis with substrates that induce transformation in the Peru strain.

Phylogenetic characterization of the epithelial Na+ channel (ENaC) family.

Twenty-one sequenced protein members of the epithelial Na+ channel (ENaC) family have been identified and characterized in terms of their sizes, hydropathy profiles, sequence similarities and phylogenies. These proteins derive from mammals, the frog Xenopus laevis and the worm Caenorhabditis elegans. The eleven sequenced vertebrate proteins fall into four subfamilies designated alpha, beta, gamma and delta. The 10 C. elegans proteins do not cluster with the vertebrate proteins, and they all proved to be distantly related to each other. Nonetheless, the 21 ENaC proteins exhibit the same apparent topology, each with two transmembrane spanning segments separated by a large extracellular loop. All but two ENaC proteins possess highly conserved extracellular domains containing numerous conserved cysteine residues as well as adjacent C-terminal amphipathic transmembrane spanning segments, postulated to contribute to the formation of the hydrophillic pores of these oligomeric channel protein complexes. It is proposed that the well-conserved extracellular domains serve as receptors to control the activities of the channels. A topological model for the ENaC family proteins is presented.

Novel insights into the mechanism of t(14;18)(q32;q21) translocation in follicular lymphoma.

The t(14:18)(q32:q21) chromosomal translocation and the ensuing overexpression of the BCL-2 proto-oncogene are strongly associated with the pathogenesis of follicular lymphoma. At the molecular level, the translocation process arises from the illegitimate rearrangement between the BCL-2 proto-oncogene and the immunoglobulin heavy chain (IgH) locus. Due to the presence of the D(H) and J(H) gene segments from the IgH locus as well as de novo nucleotide additions at the breakpoints, the translocation process has been assumed to result from a mistake occurring during V(D)J recombination in early B-cells in the bone marrow. However, recent detailed molecular analyses of both the direct and reciprocal breakpoints have revealed that the t(14;18) translocation is a more complex process than previously thought, and have challenged this traditional view. Here we review these observations, and discuss the intriguing possibility that t(14;18) translocation could preferentially occur in the germinal centers during receptor revision, and involves both V(D)J recombination and somatic hypermutation mechanisms.

Action of exogenous potassium and calcium ions on in vitro metacyclogenesis in Trypanosoma cruzi.

The cations Ca2+ and K+ and the anions Cl-, HCO3-, and PO4- were studied for their contribution to metacyclic trypomastigote formation of Trypanosoma cruzi in starvation media consisting of phosphate-buffered saline (PBS) + 10 mM proline + 10 mM sodium acetate as well as one of the following salts: 0.035% NaHCO3 (PBSNPA), 0.035% K2CO3 (PBSKPA) or 0.035% K2HPO4 (PBSPPA). Isolates CL and DM28c were activated to transform with 5% CO2 and the percent metacyclogenesis determined after incubation for 96 h in PBS starvation media. Maximal metacyclogenesis was found with CaCl2 and KCl. In the presence of K+, the percent transformation was highest with the phosphate salt, followed by the carbonate and the chloride salts. Cells incubated in PBSNPA and the cationic ionophores A23187 (5 x 10(-6) M), lasalocid (5 x 10(-6) M), and valinomycin (10(-8) M) do not survive; addition of 2 mM CaCl2 or 17 mM KCl to DM28c cells, reversed the lethal action of the ionophores permitting differentiation into metacyclic forms. The addition of CaCl2 to CL cells incubated in ionophores abrogated the lethal effect of the ionophores but transformation was significantly different than in control preparations. Adding KCl to ionophore incubated cells resulted in normal levels of transformation except in the case of valinomycin. DM28c and CL cells incubated in PBSKPA show significantly greater metacyclogenesis in the presence of 5 mM EGTA. These results indicate that exogenous concentrations of several cations and anions significantly influence T. cruzi metacyclogenesis and that the degree of response by the parasite to free ion levels may be strain dependent.

Anterior ankle arthroplasty: a retrospective study.

A retrospective analysis of surgical resection of symptomatic tibiotalar impingement exostosis was performed at the Podiatry Hospital of Pittsburgh. Thirty-four patients with a postoperative follow-up ranging from 17 to 97 months, with an average of 36 months were studied. The study has demonstrated an 88% satisfaction rate with patients who sustained anterior ankle arthroplasty at The Podiatry Hospital of Pittsburgh.
