[Noninvasive intraoperative measurement of intracapillary hemoglobin oxygenation and relative hemoglobin concentration in surgical skin flaps].

We describe the Erlangen microconducting spectrometer (EMPHO I), a device that measures the oxygen saturation and relative haemogloblin content in the capillaries. These factors determine the oxygen supply and survival of cutaneous flaps. The EMPHO I was used for intraoperative measurements in pectoralis major myocutaneous flaps and showed reproducible results, so that it is valid for intraoperative monitoring in plastic surgery.

[Epidemiology of cardiovascular risk factors in 1505 females working in hospitals].

One thousand and five women working in 26 wards of Paris hospitals were included in the study during their mandatory annual examination. The prevalence of arterial hypertension, calculated on this casual measurement (two readings) was 15.1 per cent; 6 per cent of the women were under medication for high blood pressure. In one fifth of the treated hypertensive women the blood pressure levels were not controlled by treatment. Two-thirds of the hypertensive women were untreated. Concerning the working conditions, psychic stress was associated with high blood pressure in women aged 30 to 39. The percentage of smokers was high (45 per cent below 30 years of age, 27 per cent between 30 and 40). The weight of 15 per cent of the women was at least 20 per cent over the "ideal" weight. The prevalence of cardiovascular risk factors is high among these female hospital workers. Prevention therefore must be developed in order to improve their health and to give the patients an opportunity to benefit from advice in this field during their stay in hospital.

[Cardiovascular mortality as a function of left electric ventricular hypertrophy. In a male population].

The prevalence of electrocardiographic left ventricular hypertrophy was studied in a population of 19,622 men born and living in France, aged from 40 to 69 years, who volunteered to have a medical check-up at the health centre of preclinical investigations, Paris, between 1972 and 1977. Enquiries were made among local authorities and physicians to determine the number of deaths and their causes. During the follow-up (median 9.5 years), 1,285 deaths were recorded. The overall cardiovascular mortality was 384 deaths: 178 patients died of ischaemic heart disease, 66 of sudden death, 93 of cerebral vascular accident and 47 of various cardiovascular diseases. The presence of an electrocardiographic left ventricular hypertrophy increased the incidence of overall mortality and all causes of cardiovascular mortality in all age-groups. An analysis of survival after adjustment for age, systolic blood pressure, cholesterol level and smoking habits indicated that left ventricular hypertrophy is indeed a predictor of total cardiovascular mortality and of mortality due to ischaemic heart disease.

Formation of 4-oestrene-3,17-dione (19-norandrostenedione) by porcine granulosa cells in vitro is inhibited by the aromatase inhibitor 4-hydroxyandrostenedione and the cytochrome P-450 inhibitors aminoglutethimide phosphate and ketoconazole.

The origin and biosynthesis of 4-oestrene-3,17-dione (19-norandrostenedione), a major steroid in porcine ovarian follicular fluid, was investigated by culturing granulosa cells from 4-6 mm follicles of prepubertal gilts with radiolabelled androstenedione and 19-hydroxyandrostenedione. Steroid metabolites were purified by solvent extraction and lipophilic column chromatography, and analysed by C18 reverse-phase high-performance liquid chromatography. 19-Hydroxyandrostenedione, 19-norandrostenedione and oestradiol-17 beta were obtained as major metabolites from androstenedione, while 19-norandrostenedione and oestradiol-17 beta were the major products from 19-hydroxyandrostenedione. Serum alone or serum plus FSH significantly enhanced formation of 19-norandrostenedione and oestradiol-17 beta from each substrate, compared with controls. Micromolar concentrations (1 mumol/l) of 4-hydroxyandrostenedione, an aromatase inhibitor, significantly reduced formation of 19-norandrostenedione and oestradiol-17 beta by granulosa cells cultured with serum and FSH. Formation of 19-norandrostenedione and oestradiol-17 beta from androstenedione and 19-hydroxyandrostenedione was also significantly inhibited by aminoglutethimide phosphate, a cytochrome P-450 inhibitor known to block the conversion of androstenedione to oestrogens. Ketoconazole, an inhibitor of the cytochrome P-450 dependent 17,20-lysase, blocked formation of 19-norandrostenedione and oestradiol-17 beta only at millimolar concentrations. These results suggest that 19-norsteroid and oestrogen formation from C19 aromatizable androgens may share a common or overlapping pathway, and imply that 19-norsteroid and oestrogen synthesis is mediated by cytochrome P-450 dependent enzymes.

Bacterial antigen processing in macrophages infected with the obligate intracellular bacterium, Mycobacterium avium.

The intracellular pathway followed by Mycobacterium avium antigens was studied by immunofluorescence and immunoelectron microscopy after infection in bone marrow-derived macrophages with live or gamma ray-killed bacteria. Rabbit antiserum raised against whole sonicated bacteria was used to localise antigens. The acidity of the phagosomal compartment was also investigated by immunoelectron microscopy. Preliminary results seem to indicate the phagosomal compartment is less acidified than lysosomes and that M. avium antigens are very slowly processed.

[Methods and results of arterial hypertension control in a working population].

The health benefits of hypertension treatment at the community level depend both of the quality of the blood pressure control obtained for each patient treated individually and of the percentage of the hypertensive patients actually aware, treated and controlled for hypertension in the entire population. The data collected in 1987 in a group of 7,901 subjects working in medium-sized companies of the Paris region were analysed, and compared with those obtained in a very comparable population in 1979-80. Among patients aware of their disease, the percentage of treated patients was higher in 1987 than in 1979, 55 p. 100 and 46 p. 100 respectively in men (p less than 0.001); 70 p. 100 in 87, 59 p. 100 in 79 for women (p less than 0.01). Hypertension was treated and controlled in 27 p. 100 of the men (16 p. 100 in 1979), and 42 p. 100 of women (27 p. 100 in 1979). Awareness was significantly lower among migrant workers than among european personnel (p less than 0.001). The lower the educational level, the lower the compliance with antihypertensive treatment, ranging from 99 p. 100 for the highest educational level, to 82 p. 100 among low educated subjects (p less than 0.001). The hospitalization rate did not differ according to sociocultural factors. Hypertension was detected by the worksite physician in 51 p. 100 of the male hypertensive patients. He was the sole physician who examined 17 p. 100 of the patients in the previous year. These results suggest a moderate improvement in hypertension detection and control. However, development of intervention strategies and their evaluation are clearly needed.

Alcohol consumption and impaired glycoregulation results in a population of 6665 salaried employees.

Alcohol consumption and glycosuria were found to be associated (p less than 0.001) in a population of 6571 salaried employees who underwent a systematic examination. The prevalence of glycosuria was found to range from 1.3% among 2609 non-drinkers to 5% among 816 heavy drinkers (six glasses or more of alcoholic beverage daily). This association was still significant after adjustment for age, sex and body mass index. Similarly, a positive association was observed between fasting glycemia and alcoholic intake in a subgroup of 998 subjects when such a result was available (p less than 0.05).

Phosphohexose isomerase in hypernephroma. Significance as serum tumor marker, comparison to other glycolytic enzymes and isozyme patterns in normal and tumor tissue.

In hypernephroma an overall diagnostic sensitivity of 72% and a specificity of 87% was found for the serum tumor marker phosphohexose isomerase (PHI). Both in early stage disease and in well differentiated tumors a sensitivity of about 60% was reached. In contrast the sensitivity of three other glycolytic enzymes tested was found to be less than 20%. Since the cancer induced elevation of PHI activity in the tumor was found to be comparable to those of the other test enzymes, elevated PHI serum activities cannot be attributed to overproportional PHI synthesis and unspecific cell-lysis. In 6 of 10 cases studied differences in the PHI isozyme pattern between the tumor and the normal tissue were found suggesting the occurrence of cancer associated structural alterations of PHI.

Bacterial antigen immunolabeling in macrophages after phagocytosis and degradation of Bacillus subtilis.

After phagocytosis of Bacillus subtilis 168 by bone marrow-derived macrophages, the intracellular pathway followed by different antigens was studied by immunofluorescence and immunoelectron microscopy. Three different rabbit antisera were used: (i) an antiserum to B. subtilis whole cells mainly recognizing the cell wall constituents, (ii) an antiserum to teichoic acid, and (iii) an antiserum to peptidoglycan recognizing the disaccharide tetrapeptide molecules resulting from peptidoglycan degradation. During the first 3 h after phagocytosis of B. subtilis, the three antisera were confined to the same vacuolar compartments, as follows. They were first found in phagosomes gathered in the perinuclear region. Upon bacterial degradation, the three antisera colocalized in an increasing number of small dense vesicles, located in the perinuclear region, that seemed to result from the fragmentation of phagolysosomes. These vesicles correspond to an acidic compartment since they also stained for 3-(2,4-dinitroanilino)-3'-amino-N-methyldipropylamine, a drug known to accumulate in the acidic compartments of cells. At later time points, the antigens recognized by the three antisera followed different pathways. After 18 h, teichoic acid and peptidoglycan were no longer detectable in macrophages whereas an antigen(s) labeled with antiserum to B. subtilis whole cells remained stocked for several days in small acidic vesicles randomly distributed throughout the macrophage. This compartment appeared to be different from the one labeled during the first 3 h after ingestion of bacteria. These results suggest that the transport rate and the compartments implicated in antigen processing differ according to the antigen.

Activities of phosphohexose isomerase and other glycolytic enzymes in normal and tumor tissue of patients with neoplastic diseases: comparison with serum activities and correlation to tumor staging and grading.

Of five glycolytic enzymes tested, only the serum activity of phosphohexose isomerase (PHI) was elevated in the vast majority of 140 patients with gastrointestinal, kidney or mammary carcinomas. However, in the tumor tissues all enzymes were increased to about the same extent. Thus, the measured increase in the serum activity of PHI is not due to a specific overproduction in the malignant cells. A general rise in the glycolytic enzyme activities in the malignant tissue could be detected already in early stages without metastases and in well-differentiated tumors.

Endocytic membrane traffic with respect to phagosomes in macrophages infected with non-pathogenic bacteria: phagosomal membrane acquires the same composition as lysosomal membrane.

A morphometric analysis was made to study membrane traffic in bone marrow-derived macrophages, containing phagosomes with partially degraded Bacillus subtilis. Cell surface glycoproteins, labeled with radioactive galactose by terminal glycosylation, provided a covalent autoradiographic membrane marker. Membrane compartments were characterized in terms of cytochemical staining for horseradish peroxidase taken up by receptor-mediated endocytosis. The area, composition, and exchange rates of endocytic membrane compartments were measured as in a previous analysis for non-infected macrophages, devoid of phagosomes. In direct comparison with this earlier study, the present data allowed an assessment of the involvement of phagosomes in the interactions between endocytic membrane compartments. The presence of phagosomes led to a 30% reduction of lysosomal membrane area. The rate at which cell surface-derived label flowed into the lysosomal membrane pool was reduced by the same fractional amount. This suggested a linear relationship between flow rate and membrane area. The initial flow rate of label into phagosomes was higher than expected, based on their membrane area being only about 60% that of lysosomes. This rate could only be measured during the early phase of the experiments when phagosomes were younger, therefore displaying a fast exchange rate, reminiscent of the endosome compartment. However, steady-state conditions, at late times, strongly suggested that phagosomes with degraded contents finally acquire membrane of lysosomal origin. First, the composition of phagosome membrane became the same as that of lysosomes, remaining unchanged as compared to non-infected cells. Second, the membrane area of phagosomes amounted to the loss of lysosomal membrane area in infected cells.

[Relationship between blood pressure level and different types of alcoholic beverages].

The relationship between alcohol consumption (AC) has been repeatedly confirmed. However, the respective contribution of the various types of beverages has not been clearly established. The cross-sectional data of the initial examination of the Paris prospective study II of the GREA, concerning 4547 male civil servants were thus analysed. Among subjects without any antihypertensive medication, systolic (SBP) and diastolic (DBP) blood pressure were positively associated with total AC; the differences between the first and the fifth quintile were respectively 6 and 3 mmHg (p less than 0.01 for wine, beer and spirits consumption). Using a linear combination of wine, beer and spirits consumption significantly improved the prediction of BP, as compared to total AC (p less than 0.001). In the multivariate analysis including age and body mass index (BMI), the consumption of 40 ml of alcohol from beer was associated with an increase of 5.7 mmHg for SBP and 2.6 mmHg for DBP (p less than 0.001). The elevation was 2.3 (SBP, p less than 0.001) and 0.8 mmHg (DBP, p less than 0.01) for wine consumption. Spirits consumption was associated with DBP (1.4 mmHg, p less than 0.001), but not with SBP. In conclusion, a positive relationship was observed with each of the three types of alcoholic beverages studied; however, this association was more pronounced for beer than for the two other beverages.

Social, professional conditions and arterial hypertension: an epidemiological study in Dakar, Senegal.

A group of men and women (1315 and 554, respectively) employed by six companies in Dakar, Senegal, underwent a cross-sectional study. The prevalence of arterial hypertension, according to the WHO criteria, was found to be 7.4% among the men and 10.2% among the women. Risk factors for high blood pressure were: age, body mass index, illiteracy and factors related to occupation, such as occupational category and working conditions (e.g. noise and shift work). Therapeutic control was low in this community. Awareness of hypertension was high, but a high percentage of false positive diagnosis was observed among subjects labelled as hypertensives, on the basis of inaccurate blood pressure measurement.

Relationship between alcohol consumption and hypertension prevalence and control in a French population.

Six thousand six hundred thirty two subjects, employed in 420 small and medium-sized companies in the Paris region were examined in a cross-sectional study. Their alcohol consumption, as obtained by interview was found to be higher among males than among females, among workers than among managers, executives, and clerks. Alcohol consumption was positively associated with age, body mass index, coffee and cigarette consumption, occupational exposure to noise and working nights or alternating shifts. A positive, continuous, relationship was observed, for men and women, between alcohol intake and both systolic and diastolic blood pressure. This association was highly significant in the multivariate analysis (multiple linear regression) where alcohol intake, following age and body mass index, was the third predictive factor of blood pressure level in the stepwise regression. The positive association between alcohol consumption and prevalence of arterial hypertension was aggravated by the poor control of hypertension which was found among drinkers. Awareness of hypertension, compliance with an antihypertensive treatment and its efficacy, were negatively associated with alcohol intake. The findings stress the importance of alcohol consumption which was found to be a major risk factor for arterial hypertension and noncompliance with antihypertensive treatment in this population.
