[Incidence of otosclerosis in a subpopulation of Galicia, Spain].

We have made a prospective study of the patients who were analysed for the first time at the Outpatients Department in the Otolaryngology Section of the University Complex Hospital of Santiago de Compostela all along 1997. A whole of 1,050 patients have been included in this study. The most frequent diagnosis were neurosensorial hypoacusis (29.1%), chronic otitis media (27.8%) and in the third place, otosclerosis. This appeared in 83 cases, which represents a 8.7% of the fulfilled diagnosis. The incidence for our health area reached 19 new cases of otosclerosis per 100,000 inhabitants a year. When only the number of stapedectomies were included, it reached a 7.8%. The prevalence of the female population was clear, reaching a 77%. Diagnosed patients' age was from 30 to 50 years, although a considerable percentage (37%) were older. The incidence reached by our studies is higher to other values previously published. We think this responds to the fact that the health system covers all the population and to the importance a good audition is given nowadays.

[Unifocal eosinophilic granuloma of the temporal bone (Langerhans cell histiocytosis].

Eosinophilic granuloma is one of the diseases of unknown etiology traditionally known as histiocytosis X and now called histiocytosis of the Langerhans cells since it has been found to originate by proliferation of this cell line. We report a clinical case of unifocal eosinophilic granuloma of the temporal bone in an adult. We reviewed the literature on the diagnosis, prognosis, treatment, and assessment of these patients. The importance of making a thorough analysis of all the anatomical structures in imaging is emphasized as a way of making a precise early diagnosis. The treatment of choice is surgical removal of the tumor. Irradiation is used for inaccessible or incompletely resected lesions. The prognosis of unifocal eosinophilic granuloma is very good, with successful disease control being achieved in 95% of cases.

[Epistaxis: prospective study on emergency care at the hospital level].

We prospectively studied 279 patients with epistaxis referred to ENT specialist from emergency room in our hospital in one year. Masculine sex (62%), medium or old ages (median 56 year), and associated diseases (HBP 22.9%, anticoagulants or antiplatelet drugs 11.1%), were the most common. Predominant local etiologic factors were trauma (12.9%) and inflammation (14%). Epistaxis had been essential in 36.9%. The most frequent location in all ages was the anterior one, especially located in the Kiesselbach's area, although the incidence of the posterior epistaxis increases in characteristic way from fourth decade, affecting more to men. Most of epistaxis (> 99%) were treated successfully by conservative approach and only two patients required surgical or interventional therapy. Patients between 30-59 year and the posterior epistaxis was the most refractory to the treatment. Hospitalized patients were older than ambulatory ones and had more associated diseases. Their average hospital stay was 9.2 days.

[Laryngeal tuberculosis: report of 10 cases].

The aim of this paper is to present a retrospective study on laryngeal tuberculosis cases presented at our Department from 1982 to 1993. In addition to usual techniques for tuberculosis diagnosis (cultures, Mantoux test and chest X-rays), biopsies for histological sections were carried out. Epidemiological, clinical diagnosis and treatment aspects are presented; the higher male incidence and the common suspicion of laryngeal carcinoma are emphasized. When we compare our results with those of several reports, we confirm the increase in the number of cases diagnosed in the last three years of the study. We emphasize the importance of histopathological diagnosis to rule out a neoplasm.

[Papillomas in otorhinolaryngology: clinics, diagnostics and treatment].

The clinical, diagnostic, and therapeutic features of 257 papillomas of the ear, nose and throat were studied. These tumors were found in a population of about 500,000 inhabitants over a 20-year period (1973-1992). The clinical manifestations of papillomas were varied and more dependent on location than on the size or number of lesions. The most frequent symptom was a foreign body sensation. Twenty-four percent of cases were asymptomatic and the diagnosis was made during a general physical examination; such diagnoses were most frequent in the pharynx. The mean duration of symptoms before visiting a physician was 10 months (longer in papilloma of the oral cavity and shorter in the pharynx). The initial clinical diagnosis was correct in 63% of cases. Lesions of the oral cavity presented most diagnostic problems and those of the pharynx the least. A clinical diagnosis of cancer was made most often in the larynx. Treatment was surgical in every case. Twenty percent required more than one operation to remove the papilloma.

[Carcinoma of small neuroendocrine cells in major salivary glands. Presentation of two clinical cases and literature review].

To date, only 45 cases of oat cell carcinoma of the major salivary glands have been reported in the literature, which represents less than 1% of primary malignant tumors of the parotid glands. Two subtypes exist: ductal and neuroendocrine. We report two new cases of neuroendocrine oat cell carcinoma of the parotid gland which were treated by surgery associated with radiation therapy and chemotherapy. The literature review showed a predominance of males (61.7%), age 50-70 years, and location in parotid gland (83%). Treatment consists of radical surgery of the tumor associated with radiation therapy and chemotherapy. The prognosis is poor, with survivals at one, three and five years of 77.7%, 40.7%, and 29.6%, respectively, although this is better than the prognosis for other sites.
