Bone mineral loss, bone histomorphometry and vitamin D metabolism in patients with rheumatoid arthritis on long-term glucocorticoid treatment.

Long-term glucocorticoid treatment might interfere with the vitamin D metabolism. The serum concentrations of 25-OHD were significantly reduced whereas the circulating levels of 1,25-(OH)2D were normal in 50 patients with rheumatoid arthritis on long-term treatment with small doses of prednisone. The bone mineral content of the forearm was significantly reduced, but the degree of bone loss did not correlate with duration of treatment or dose of prednisone given. Quantitative bone histomorphometry was performed in 18 patients. Apart from a significant correlation between serum 25-OHD and the fractional trabecular bone volume, no relationships were observed between bone histomorphometry and vitamin D metabolites or serum iPTH. The results indicate that the bone loss was due to a decreased osteoblastic activity rather than to an impaired vitamin D metabolism.

Correction of respiratory acidosis and transient hypomagnesemia.

Four elderly patients with established chronic obstructive airways disease were admitted with a days' to weeks' history of increasing dyspnea. Acute respiratory acidosis was diagnosed and mechanical ventilation instituted. A few hours after initiation of treatment, arterial pH was normal in all patients, but serum magnesium concentrations decreased over the next 1-3 days. Thereafter, a spontaneous normalization was seen. It is suggested that the transient hypomagnesemia is due to correction of the acidosis per se.

Raised CK-MB isoenzyme after exercise in a patient with alcoholic myopathy.

The relation between exercise, total serum creatine kinase activity, and serum creatine kinase MB isoenzyme in a patient with alcoholic myopathy was investigated. After a short-term exercise the serum values of creatine kinase MB isoenzyme rose to high levels within hours. This finding is important for obvious differential diagnostic reasons.

Severe hypophosphataemia during recovery from acute respiratory acidosis.

Three elderly patients with established chronic obstructive airways disease were admitted with a short history of increasing dyspnoea and tiredness and (in two cases) a deterioration in mental state. Acute respiratory acidosis was diagnosed and mechanical ventilation instituted. Two hours after beginning mechanical ventilation the mean arterial pH had risen to 7.40, but all patients showed a dramatic fall in the serum phosphate concentration (lowest value 0.3 mmol/l (0.9 mg/100 ml] accompanied by a low urinary excretion of phosphate. No patient could tolerate withdrawal of mechanical ventilation until the serum and urinary concentrations of phosphate had returned to normal. Recovery from acute respiratory acidosis should be added to the list of conditions associated with severe hypophosphataemia.

Lingual infarction and sudden blindness due to giant cell arteritis.

A 72-year-old woman suffered from giant cell arteritis (GCA) which developed into lingual infarction and monocular blindness. Temporary obscuration of vision and lingual symptoms such as increasing malaise, pain and intermittent claudication may precede the catastrophic results of arteritis. Emphasis is laid on early recognition and treatment of GCA.

Vitamin D metabolism in insulin-dependent diabetes mellitus.

A significant loss of bone has been observed in diabetes mellitus. The pathogenesis is unknown, but an impairment of vitamin D metabolism might be involved. Consequently, we have studied vitamin D metabolism in five groups of insulin-dependent diabetic patients. Significantly reduced levels of serum 25(OH)D were seen only in patients with diabetic nephropathy. The serum levels of 1,25 (OH)2D were reduced only in diabetic ketoacidosis but normalized during recovery. It is concluded that vitamin D metabolism is largely normal in adult insulin-dependent diabetes, and it seems unlikely that a disturbance of the vitamin D metabolism can explain the bone loss in the ordinarily controlled diabetics.
