Sulfur and hydrogen isotope anomalies in meteorite sulfonic acids.

Intramolecular carbon, hydrogen, and sulfur isotope ratios were measured on a homologous series of organic sulfonic acids discovered in the Murchison meteorite. Mass-independent sulfur isotope fractionations were observed along with high deuterium/hydrogen ratios. The deuterium enrichments indicate formation of the hydrocarbon portion of these compounds in a low-temperature environment that is consistent with that of interstellar clouds. Sulfur-33 enrichments observed in methanesulfonic acid could have resulted from gas-phase ultraviolet irradiation of a precursor, carbon disulfide. The source of the sulfonic acid precursors may have been the reactive interstellar molecule carbon monosulfide.

Effect of yogurt on symptoms and kinetics of hydrogen production in lactose-malabsorbing children.

Lactase-deficient adults demonstrate improved lactose absorption and fewer symptoms when consuming yogurt than when consuming milk containing equivalent amounts of lactose. To examine this effect and its mechanisms in children, we compared symptoms and hydrogen production as an index of lactose malabsorption after typical servings of milk, pasteurized yogurt, and yogurt containing active live culture in 14 lactose-malabsorbing children (mean age 9.5 y). Symptoms and interval breath-hydrogen concentrations were recorded for 8 h after ingestion of 12 g lactose served as milk and yogurts. Lactose-malabsorbing children experienced significantly fewer symptoms after consuming yogurt containing active cultures than after consuming milk (P < 0.005). Pasteurized yogurt showed an intermediate effect. Lactose from yogurt was not better absorbed than was lactose from milk, as indicated by similar areas under the hydrogen curve; however, yogurt was associated with a delayed time to rise and lower rate of rise of the hydrogen curve. The rate of hydrogen rise correlated with the degree of symptoms. In children, mechanisms other than enhanced lactose absorption from yogurt may lead to changes in the kinetics of hydrogen production, which in turn are associated with improved tolerance.

Minimally invasive abdominoplasty: surgical technique development and report of three cases.

Endoscopically assisted rectus abdominis plication is a safe and effective method of repair in patients with mild to moderate central abdominal wall flaccidity. A procedure was developed in animals and in a human cadaver using a single umbilical incision, gasless dissection of the anterior rectus abdominis fascia, and suture plication under endoscopic guidance. This procedure was performed successfully in three patients. Early postoperative results were comparable to those obtained by standard rectus abdominis plication. Moreover, adjunctive suction lipectomy or a limited abdominal skin resection can be used to expand the contouring capability of this technique.

Use of intravenous alfentanil-midazolam anesthesia for sedation during brief endourologic procedures.

Traditionally, general or regional anesthesia has been employed during endourologic procedures. The success of sedative-analgesic sedation during SWL prompted an investigation into its use in minor endourologic procedures. We compared intravenous alfentanil-midazolam anesthesia (n = 30) with spinal anesthesia (n = 35) during procedures such as ureteroscopy and stent placement using physiologic measures and patient and anesthetist ratings. The former series was prospective; the latter was retrospective. Physiologic measures remained stable and oxygen saturation was above 90% in all patients in both groups during the procedures. The average anesthesia and recovery room times were significantly shorter for patients given alfentanilmidazolam: 23.3 v 36.1 minutes and 85.5 v 179.6 minutes, respectively (P < 0.001; t-test), and this difference reduced the anesthesia cost an average of 46%. The anesthetists rated the intravenous regimen satisfactory in 92% of the patients, and 94% of the patients rated the procedure as comfortable. All 18 of the patients who had previously had spinal anesthesia considered alfentanil-midazolam superior. Intravenous alfentanil-midazolam sedation anesthesia can provide adequate analgesia for many minor endourologic procedures while shortening the anesthesia recovery time.

Use of alfentanil sedation anesthesia with the Dornier HM3 lithotripter.

Our results with the combination anesthetic technique of midazolam-alfentanil during elective outpatient extracorporeal shock wave lithotripsy on the Dornier HM3 (N = 79) were compared with those of epidural anesthesia in the same setting (N = 81). The mean anesthesia time and recovery room time were significantly shorter (72.85 v 113.58 minutes and 115.0 v 159.20 minutes, respectively) with the combination technique. No procedures in the alfentanil group had to be discontinued because of patient discomfort. Side effects with alfentanil were minimal, and oxygen saturation remained above 90% for all patients. Combination midazolam-alfentanil anesthesia is safe and allows the urologist to treat renal and ureteral calculi effectively and efficiently without using general or regional anesthesia.

Immunohistochemical localization of basic fibroblast growth factor within the mouse uterus.

Uterine samples were either rapidly frozen in liquid nitrogen or placed in Bouin's fixative. A commercial primary polyclonal antibody made in rabbits against human recombinant basic fibroblast growth factor (bFGF) was used. Western blot analysis indicated that the antibody was specific for bFGF and did not react with acidic FGF. The primary antibody was followed by either goat anti-rabbit immunoglobulin G (IgG) conjugated to the fluorescent phycobiliprotein tracer phycoerythrin or biotinylated goat anti-rabbit IgG and a biotin-avidin-peroxidase complex. Specificity controls using adjacent sections were carried out by (i) substituting normal rabbit sera for the primary antisera, (ii) omitting the primary antisera or (iii) extracting sections with NaCl (2 mol l-1) prior to the immunochemical procedures. No binding of the antibody was observed with any of the specificity control sections. The connective tissue stroma and the basal lamina associated with uterine glandular and surface epithelial layers were positive for bFGF. Localization was not observed within surface or glandular epithelial cells. The basal lamina and endothelial cells associated with blood vessels within the uterus and the smooth muscle cells of the myometrium were positive for bFGF. There were no differences in uterine localization patterns or intensity during the oestrous cycle or after ovariectomy and steroid hormone supplementation. These studies demonstrate the specific localization of bFGF within the mouse uterus.

Assessment and management of donor pain following marrow harvest for allogeneic bone marrow transplantation.

We studied the time course and intensity of pain of multiple bone marrow aspirations in 30 healthy adult marrow donors receiving acetaminophen with codeine for analgesia immediately after marrow harvesting for allogeneic bone marrow transplantation. Upon discharge, donors were supplied with acetaminophen (315 mg) plus codeine (30 mg) tablets and instructed to use one or two tablets up to every 4 h as needed for pain control. Donors used analgesic medication for a mean (+/- SE) of 3.3 +/- 0.5 days (range = 1-13 days) and reported less than complete pain relief. Subjects reported more pain at time of medication than between doses, indicating that the analgesic was at least partially effective. Male donors tended to report more pain and use more analgesic than did females. We conclude that donors self-regulate their analgesic usage to achieve maximal relief and that incomplete relief with acetaminophen plus codeine may be due to limited efficacy of this analgesic preparation. Our findings suggest that donor pain management may be improved by use of more powerful analgesics.

Attribution of blame in incest.

This study was designed to identify the empirical structure of attitudes relating to attribution of blame in incest. A second purpose was to determine how variables such as gender, physically abused status, and sexually abused status influence the attribution of blame in incest. A sample of 201 male and 211 female college students was administered the Jackson Incest Blame Scale. Four factors emerged from the factor analysis of the total sample supporting the hypothesis that attribution of blame in incest is a multidimensional construct including victim, offender, situational, and societal factors. A difference in the level of victim blame was found between male and female samples. Results are discussed with regard to their implications for further research and training. Other potential uses of the Jackson Incest Blame Scale are suggested.

Language lateralization as measured by dichotic VRT and recognition tasks.

A dichotic color-naming VRT task and a dichotic digits recognition task were administered. The first required the naming of five colors paired with their backward pronunciation: the second of five nonsimilar digits. Forty-eight right-handed male and female subjects were used, controlling for the factors of sex, handedness, and familial sinistrality. A highly significant left-ear advantage was obtained, suggesting that previous results of dichotic studies may only pertain to a limited class of phenomic stimuli. Results implicate different cerebral processes in the analysis of familiar and novel auditory and phenomic stimuli.

Attitudes toward rape among mental health professionals.

This study examined the attribution of blame in rape among mental health professionals from one state. Thirty-eight professionals (76% of the total population) completed the Attribution of Rape Blame Scale, a 20-item questionnaire reflecting different ways people account for rape. The responses were factor analyzed yielding four clear factors: Societal Blame, Victim Blame, Assailant Blame, and Sociological Status Blame. Analyses of variance were then performed on the participants' four factor scores to determine if there were differences in blame attribution across age or gender of respondents. The only significant finding was a gender difference on the societal blame factor. Results of the factor analysis, ANOVAs as well as implications for training of professionals are discussed.
