Distal ulnar physeal injury.

We reviewed 18 patients and two traumatic amputation specimens with injuries involving the distal ulnar physis. Type 1 growth mechanism injuries were the most common fracture pattern with premature physeal closure and ulnar shortening occurring in 55% of the patients. Other consequences included radial bowing, ulnar angulation of the distal radius, and ulnar translocation of the carpus. Most of the patients, however, were asymptomatic. Initial radiographic diagnosis of this physeal injury may be difficult because of the relatively late ossification of the distal ulnar physis. Concomitant ulnar physeal injuries must be suspected in any injury to the distal radius, especially when an ulnar metaphyseal or styloid fracture is not readily evident.

Acute Disease↗

Simultaneous dislocation of the five carpometacarpal joints.

Two new cases are added to the literature on four previously reported cases of simultaneous dislocation of the five carpometacarpal joints. The priorities of management of other more extensive injuries often delays definitive treatment and in the two cases reported here open reduction and internal fixation was necessary to restore the functional structure of the hand. Restoration of normal grip and pinch strength was obtained in both patients with stable, pain-free carpometacarpal joints.

Six generations of a family with multiple limb deficiencies.

Ectrodactyly, with or without other limb reduction defects, is a rare congenital anomaly. The pattern of inheritance is usually autosomal dominant, but with variable morphology. We report six generations of a family with this anomaly, with a progressive increase in severity and extent of involvement. Implications of genetic counseling are discussed.
