Williams syndrome and deficiency in visuospatial recognition.

This study aimed to assess the visuospatial abilities of five children with Williams syndrome (four males aged 9 years 3 months, 7 years 11 months, 8 years 1 month, and 10 years 8 months respectively, and one female aged 6 years 3 months). First, the children's visuospatial abilities were examined by asking them to copy a figure. Second, their cognitive processing abilities were assessed using the Japanese Kaufman Assessment Battery for Children. This test was used because it is an objective one, standardized in Japan, and is a measure of fluid ability including spatial localization. Participants scored significantly low on the spatial memory subtest indicating that there was a deficit in spatial localization. Children's performance in line copying tasks improved when the dots were in colour. Results suggest a deficit in the dorsal stream of visual cognition, with a relatively preserved ventral stream.

Immune response to a 26-kDa protein, alkyl hydroperoxide reductase, in Helicobacter pylori-infected Mongolian gerbil model.

BACKGROUND: The host immune response is thought to play an important role in the outcome of Helicobacter pylori infection. The successful development of the H. pylori-infected Mongolian gerbil model that mimics human disease has enabled study of the antibody response against H. pylori antigens. MATERIALS AND METHODS: Serum samples from ulcer and carcinogenesis models of H. pylori-infected gerbils were used to screen for H. pylori antigens that cause a humoral immune response in the infected hosts. H. pylori alkyl hydroperoxide reductase (AhpC) is one such antigen on which we report here. The tsaA gene encoding AhpC was amplified by PCR from H. pylori ATCC 43504 strain, cloned into pMAL(TM)-c2 expression vector and expressed in Escherichia coli. Maltose-binding protein fusion protein (MBP-AhpC) was purified by a MBP affinity column. Using purified recombinant AhpC protein as an antigen, the antibody response and changes of antibody levels against AhpC in the gerbil models were studied by Western blotting and ELISA. RESULTS: Antibody against AhpC was negative in the early stages of infection, and became positive in the gerbils with the emergence of gastric diseases such as chronic active gastritis, gastric ulcer and gastric cancer. The antibody levels (ELISA) increased gradually over time and were higher in gerbils with gastric ulcer than that in gerbils without ulcers. CONCLUSIONS: Use of the gerbil model that mimics human H. pylori infection is likely to provide insights into the role of H. pylori-specific antigens possibly related to the subsequent development of gastric diseases.

Glomerulocystic kidney associated with subacute necrotizing-encephalomyelopathy.

A 22-year-old man with subacute necrotizing-encephalomyelopathy (SNE; Leigh's disease) was diagnosed as having progressive renal dysfunction. The clinical diagnosis of Leigh's disease was obtained by the typical central nervous lesions, abnormalities in other organs, and increased lactate concentrations in blood and cerebrospinal fluid. We performed an open biopsy of the right kidney. Light microscopic studies of the renal specimen showed diffuse glomerulocystic kidney (GCK) with tubulointerstitial damage. Electron microscopic examination showed marked swelling and increase in the number of mitochondria of the renal tubular epithelial cells. Therefore, it is suggested that mitochondrial disease seems to play an important role in developing GCK.

Anticarcinogenic antioxidants as inhibitors against intracellular oxidative stress.

Oxidative stress has been implicated in the pathogenesis of numerous diseases, including cancer. In the present study, the protective effect of natural antioxidants, such as quercetin and tea polyphenols, on intracellular oxidative stress was studied. Here we report a novel function of quercetin and tea polyphenols, as potential inhibitors of 4-hydroxy-2-nonenal (HNE)-induced intracellular oxidative stress and cytotoxicity. In rat liver epithelial RL34 cells, a potent electrophile HNE dramatically induced the productions of reactive oxygen species (ROS), which correlated well with the reduction in cell viability. We found that quercetin and tea polyphenols, such as epigallocatechin gallate and theaflavins and their gallate esters, significantly inhibited the HNE-induced ROS production and cytotoxicity. In addition, HNE induced a transient decrease in the mitochondrial membrane potential (delta psi), which was also retarded by the antioxidants. These data suggest that the antioxidants, such as quercetin and tea polyphenols, are inhibitors against mitochondrial ROS production.

Use of bleomycin- and heat shock-induced calreticulin promoter for construction of a mammalian expression vector.

Addition of bleomycin (Bm) to an NIH/3T3 cell culture induced the overproduction of four cellular proteins [Kumagai and Sugiyama (1998) J. Biochem. 124, 835-841]. The two proteins were identified on N-terminal amino acid sequence analysis as calreticulin and mitochondrial matrix protein P1, which are known as heat shock proteins, respectively. In this study, we cloned the calreticulin promoter region from the genomic DNA of NIH/3T3 cells and observed that heat shock treatment at 42 degrees C or the addition of Bm to the cell culture caused overexpression of the luciferase gene controlled by the cloned calreticulin promoter. This suggests that Bm induces the transcriptional activation of stress-heat shock genes. We constructed an expression vector for mammalian cells, which is controlled by the calreticulin promoter.

Vaccination with calpain induces a Th1-biased protective immune response against Schistosoma japonicum.

A large subunit of calpain, a calcium-activated neutral proteinase, from Schistosoma japonicum was cloned and expressed in Escherichia coli. When BALB/c mice were immunized with purified recombinant calpain (r-calpain) emulsified in complete Freund's adjuvant, a significant reduction in the number of recovered worms and also in egg production per female worm was observed (P<0.01). Spleen cells of the immunized mice showed enhanced production of gamma interferon (IFN-gamma) by activated CD4(+) T cells. Considering our observation of elevated expression of inducible nitric oxide synthase mRNA in immunized mice, r-calpain-induced IFN-gamma seemed to upregulate the production of nitric oxide by macrophages and subsequently mediated the killing of schistosomulae in the lung. On the other hand, spleen cells of immunized mice showed only faint interleukin-4 production in response to r-calpain in vitro, suggesting that immunization with r-calpain alters the Th1-Th2 balance in murine hosts even during a Th2-promoting S. japonicum infection. Furthermore, histopathological study of the livers of immunized mice showed that granulomas formed around eggs were diminished in both size and number. Egg production by female worms was clearly decreased in immunized mice, suggesting that r-calpain also has antifecundity effects. Taken together, these results point to S. japonicum calpain as a potential vaccine candidate for both worm killing and disease prevention, possibly through the induction of a strong Th1-dominant environment in immunized mice.

Serum immunoglobulin G immune response to Helicobacter pylori antigens in Mongolian gerbils.

The Mongolian gerbil model for Helicobacter pylori infection is an animal model that mimics human disease. We examined the serum immune response to H. pylori infection in gerbils by enzyme-linked immunosorbent assay (ELISA) and Western blotting, both with whole-cell (H. pylori) extracts. A total of 66 7-week-old specific-pathogen-free male gerbils were inoculated orogastrically with H. pylori strain ATCC 43504. Sera were collected 1, 2, 4, 8, 12, 26, 38, and 52 weeks after H. pylori inoculation. Sixty-nine noninfected gerbils and their sera were used as controls. The specificity of the ELISA was 95.7%. The frequency of seropositivity increased over time: 2 of 10 (20%), 7 of 10 (70%), and 7 of 7 (100%) samples of sera from inoculated gerbils were positive for H. pylori at 2, 4, and 8 weeks postinoculation, respectively. Western blot assays showed that the primary immunoglobulin G (IgG) response against low-molecular-mass (25-, 30-, and 20-kDa) proteins appeared after a lag period of 2 to 8 weeks after inoculation. Antibodies against 160-, 150-, 110-, 120-, 80-, 66-, and 63-kDa proteins were observed 12 weeks after inoculation. The early reactive 30-kDa protein was identified as a urease alpha subunit by N-terminal amino acid sequencing. After 26 weeks, two groups of animals could be distinguished: one group developed ulcers (n = 5), and the other developed hyperplastic polyps without ulcers (n = 19). Gerbils in the gastric ulcer group showed significantly higher serum anti-H. pylori IgG levels than did gerbils in the hyperplastic group (P = 0.001) as measured by ELISA. Furthermore, a higher proportion of animals developed antibodies to H. pylori proteins of 26, 25, and 20 kDa in the ulcer group than those animals with hyperplastic polyps (75 to 100% versus 17 to 50%) in Western blot assays. These results highlight the importance of the immune response of the host in the development of H. pylori-related gastric lesions.

Bayesian analysis of mixtures of factor analyzers.

For Bayesian inference on the mixture of factor analyzers, natural conjugate priors on the parameters are introduced, and then a Gibbs sampler that generates parameter samples following the posterior is constructed. In addition, a deterministic estimation algorithm is derived by taking modes instead of samples from the conditional posteriors used in the Gibbs sampler. This is regarded as a maximum a posteriori estimation algorithm with hyperparameter search. The behaviors of the Gibbs sampler and the deterministic algorithm are compared on a simulation experiment.

[Cytotoxic activity and cytokine gene induction of Asp-hemolysin to vascular endothelial cells].

We examined the effects of Asp-hemolysin from Aspergillus fumigatus Fresenius-Muramatsu strain on the viability and cytokine gene expression of human umbilical vein endothelial cells (HUVEC). The cell viability of HUVEC was reduced to 50% by 100 micrograms/ml of Asp-hemolysin. However, lower concentration of Asp-hemolysin (< 30 micrograms/ml) had no effect on the cell viability. The mRNA expression of such cytokines as tumor necrosis factor-alpha (TNF-alpha), interleukin (IL)-1 beta, IL-6, IL-8 and granulocyte-macrophage colony stimulating factor (GM-CSF) were also observed in HUVEC cultured with 30 micrograms/ml of Asp-hemolysin.

[Effects of low density lipoprotein and oxidized low density lipoprotein on the cytotoxic activity of Asp-hemolysin to murine macrophages].

We examined the effects of human low density lipoprotein (LDL) and oxidized LDL (Ox-LDL) on the cytotoxic activity of Asp-hemolysin from Aspergillus fumigatus Fresenius-Muramatsu strain to mouse peritoneal macrophages (M phi). The inhibitory effects of LDL and Ox-LDL on the cytotoxic activity of Asp-hemolysin to M phi increased in a dose-dependent manner, and the effect of Ox-LDL was greater than the inhibitory effect of LDL. Furthermore, the binding of Asp-hemolysin to LDL or Ox-LDL was observed by western blot analysis of the culture medium. These results suggest that the inhibition by LDL or Ox-LDL on the cytotoxic activity of Asp-hemolysin to M phi was due to the binding of LDL or Ox-LDL to Asp-hemolysin in the culture medium.

Relationship between ultraviolet-B sensitivity and cyclobutane pyrimidine dimer photorepair in rice.

Among Indica rice cultivars (Oryza sativa L. cvs.) that belong to the aus ecotype from the tropical Bengal region, where the amount of ultraviolet-B (UV-B) radiation in the solar radiation is relatively great, Marich-bati cultivar has exhibited resistance to UV-B radiation, while Surjamkhi cultivar appeared to be less resistant. We have examined the susceptibility to cyclobutane pyrimidine dimer (CPD) induction by UV-B radiation and the ability to photorepair CPDs using these two cultivars. UV-B radiation produced similar dimer levels in the leaves of the two cultivars. In contrast, the ability to photorepair CPDs in the UV-sensitive Surjamkhi cultivar was lower than that in the UV-resistant Marich-bati cultivar. These results were similar to our previous data, namely, that a UV-sensitive Japanese rice cultivar (Oryza sativa L. cv. Norin 1) cultivated in the moderate climate of Japan is deficient in its ability to photorepair CPDs. Thus, these results suggest that a strong correlation exists between the sensitivity to UV-B and the photorepair deficiency, and that a low ability in CPD photorepair may be a principal factor in determining the UV-B sensitivity in rice plants.

Evaluation of the automatic fluorescent image analyzer, Image Titer, for quantitative analysis of antinuclear antibodies.

By making comparisons with the usual manual method, we evaluated an automatic fluorescent image analyzer (Image Titer, Tripath Imaging, Burlington NC), the software for which was developed to simplify measuring indirect immunofluorescent antinuclear antibodies (FANAs). In this new system, images of the stained sample are displayed, and it measures the FANA titer and staining pattern using only 1 slide per subject and does not required the staining of a series of diluted samples as does the manual method. This system showed good reproducibility and linearity for 4 types of control serum samples (with homogeneous, speckled, discrete speckled, and nucleolar staining patterns). In 132 serum samples, consistency between the methods was 100% for the FANA staining pattern and 93.9% for the FANA titer. The Image Titer system detected each pattern in samples with 2 mixed patterns. This system should partly reduce labor and lead to results with minimum differences among individuals, including newly trained persons.

Short-term hyperthyroidism followed by transient pituitary hypothyroidism in a very low birth weight infant born to a mother with uncontrolled Graves' disease.

Transient hypothyroxinemia in infants born to mothers with poorly controlled Graves' disease was first reported in 1988. We report that short-term hyperthyroidism followed by hypothyroidism with low basal thyroid-stimulating hormone (TSH) levels developed in a very low birth weight infant born at 27 weeks of gestation to a noncompliant mother with thyrotoxicosis attributable to Graves' disease. We performed serial thyrotropin-releasing hormone (TRH) tests in this infant and demonstrated that TSH unresponsiveness to TRH disappeared at 6.5 months of age. The maternal thyroid function was free triiodothyronine (FT(3)), 21.1 pg/mL; free thyroxine (FT(4)), 8.1 ng/dL; TSH, <0.03 microU/mL; thyroid-stimulating hormone receptor antibody, 52% (normal: <15%); thyroid-stimulating antibody, 294% (normal: <180%); and thyroid-stimulation blocking antibody, 9% (normal: <25%) on the day of delivery. A nonstress test revealed fetal tachycardia >200 beats per minute, and a male infant weighing 1152 g was born by emergency cesarean section. Thyroid-stimulating hormone receptor antibody was 16% and thyroid-stimulating antibody was 370% in the cord blood. We administered 10 mg/kg per day of oral propylthiouracil from day 1. Tachycardia along with elevated FT(4) and FT(3) levels in the infant decreased from 200/minute to 170/minute, 4.7 ng/dL to 2.9 ng/dL, 7.0 pg/mL to 4.8 pg/mL, respectively, in the first 33 hours. At 5 days, FT(4) and FT(3) were 1.1 ng/dL and 2.9 pg/mL, respectively, and we stopped propylthiouracil administration. Although FT(4) decreased to 0.4 ng/dL, TSH was quite low and did not respond to intravenous TRH by 14 days of age. We began daily levothyroxine 5-micro/kg supplementation. The responsiveness of TSH to TRH did not become significant until 4 months old and normalized at 6.5 months old. At this time, levothyroxine was stopped. We conclude that placental transfer of thyroid hormones may cause hyperthyroidism in the fetal and early neonatal periods and lead to transient pituitary hypothyroidism in an infant born to a mother with uncontrolled Graves' disease.

Incidence of genetic subgroups of hereditary spinocerebellar ataxia in Fukushima Prefecture.

The prevalence of each type of hereditary spinocerebellar ataxias (SCAs) was genetically determined in Fukushima Prefecture, and the results were compared to those in other areas of Japan. The genetic analyses were done in 29 patients with dominant SCA and 5 patients with SCA with negative family history. Machado-Joseph disease was identified in 41.3% of the cases, SCA6 17.2%, dentatorubral-pallidoluysian atrophy (DRPLA) 6.9% and unknown 34.5%. The incidence is clearly different from those of Miyagi and Yamagata Prefectures as SCA1 has not been identified in our region, and is in fact similar to that of Hokuriku or Kanto Provinces. An apparent difference in the incidence of each SCA may be attributed to the historical and geographic regional difference in the distribution of inhabitants and also to the small size of the SCA population we have so far investigated. In addition, 2 of the 3 genetically identified DRPLA in this study were not clinically diagnosed, and one of them was thought to be sporadic. Late onset DRPLA may thus be misdiagnosed to other disease categories, when dementia was not apparent at the time of onset.

Chemical characterization of organic carbon dissolved in natural waters using inorganic adsorbents.

Dissolved organic carbon (DOC) in water samples from Lake Biwa was chemically characterized by two inorganic adsorbents with completely different surface characteristics. The two adsorbents were HIO (hydrous iron oxide) and SG (silica gel). Solutions of reference standard materials were analyzed concerning their adsorption behavior to HIO and SG for bovine serum albumin (BSA), fulvic acid extracted from the bottom sediments of Lake Biwa, phthalic acid, and starch. The adsorption of DOC to HIO was mainly controlled by ligand exchange and electrostatic interaction; that of SG was by electrostatic interaction. It was found that in a weak acid solution of around pH 5, BSA adsorbs to both HIO and SG, but that fulvic acid, phthalic acid and starch only show adsorption to HIO. Using these characteristics, DOC samples in natural water samples were characterized into pro-DOC, which adsorbs to both HIO and SG at pH 5, and car-DOC, which only adsorbs to HIO at pH 5. The DOC samples in Lake Biwa on October 7, 1997, at sampling sites Nb-2 and Nb-5 (south basin of Lake Biwa, the depths were about 2 and 4 m), and Ie-1 (north basin of Lake Biwa, the depth was about 75 m) were characterized. The pro-DOC has different values, depending on their sampling sites and depths, and had the maximum value of 0.42 mg C l(-1) at the surface water of Ie-1, and had the lowest values at middle to deeper water depths (0.18-0.27 mg C l(-1)). The car-DOC showed a relatively stable value at Ie-1 regardless of the depth (0.63-0.83 mg C l(-1)), and the maximum value was observed in Nb-2 and Nb-5 (1.2 and 1.3 mg C l(-1)). The ratios between car-DOC and pro-DOC concentrations were 0.2-0.5, and had different values for different sampling sites and depths. The ratios were significantly different for surface water samples where the biological activities are high and for bottom water samples where decomposition predominates.

[Central nervous system involvements in Duchenne/Becker muscular dystrophy].

Duchenne/Becker muscular dystrophy (DMD/BMD) are the most common inherited muscular diseases caused by mutations in the dystrophin gene. The identification of novel dystrophins in the brain has recently implicated its absence or malfunction etiologically in mental retardation (MR). We therefore examined the relationship between molecular abnormalities and clinical phenotypes. Deletions of the dystrophin gene were analyzed in a total of 137 DMD/BMD patients (DMD 94, BMD 43) to determine central nervous system (CNS) symptoms. The mental capacity was assessed and patients with IQs below 70 were defined as mentally retarded. Thirty-nine percent of DMD boys and 12% of BMD patients were classified as mentally retarded. Eight DMD and 2 BMD patients were diagnosed as having autism. Forty-four percent of DMD and 79% of BMD patients had deletions in the dystrophin gene. All the DMD/BMD patients with deletions upstream of the 5' end of the gene were mentally normal. All of DMD/BMD patients with MR and/or autism had deletions containing the 3' end, although some patients with similar deletions were mentally normal. Our data suggest that Dp140, Dp71 and/or Dp116, the C-terminal translational products of dystrophin, may be related to MR and/or autism in DMD/BMD. However, there was an exception in our series. Three of eight sibling pairs in our cases had different phenotypes, although they had the same mutations in the dystrophin gene. Thus the CNS phenotypes were not determined by the mutations of dystrophin gene alone, and the interaction of dystrophin with other nuclear genes may play important roles.

[Problems inherent to the care of patients with ALS].

A questionnaire survey was conducted in September 2000 to clarify the actual state and problems inherent to the care of patients with amyotrophic lateral sclerosis (ALS). The subjects were members of a patients' association, and 176 members answered the questionnaire (response rate. 64.9%). Patients being taken care of at home accounted for 76.7% of those who answered the questionnaire and those under mechanical ventilation (MV) at home for 40.3% of respondents. Compared with the results of the same survey administered by the patients' association in 1992 and 1996, the figures showed an increase of the patients being taken care of at home. Of the subjects who were receiving domiciliary services under the long-term care insurance system introduced in April 2000, those who considered the services had improved under the new system accounted for 44.2% of the respondents, and those who thought the services had worsened for 25.0%. The latter percentage included a greater number of patients under MV or who were using an aspirator and those with overworked caregivers. Moreover, the burden for the caregiver did not necessarily decrease when the patient was admitted to hospital, because attendance by the caregiver or additional costs for services were required. The new insurance system is thus considered to be inadequate and unsatisfactory for seriously disabled people, such as patients with ALS.
