Dependency of O2-affinity effects on O2 consumption in the isolated rat heart.

We have studied the effect of a decrease in hemoglobin-O2 affinity (increased P50) on O2 delivery in the non-paced, isolated, blood-perfused rat heart before and after coronary vasodilatation with and without an increase in myocardial O2 consumption (MVO2) produced with isoproterenol. Changes in perfusate P50 were produced with orthoiodosodium benzoate (OISB). Low concentrations of isoproterenol (0.74 micrograms/liter) caused no significant changes in coronary blood flow (QCOR) or MVO2 per beat. Perfusion with OISB-treated (8 mM) blood increased P50 from 29 to 33 mmHg at constant pH. MVO2 per beat increased significantly, QCOR did not change, and the ratio QCOR/MVO2, a reflection of the flow/metabolism distribution, decreased to values obtained in the absence of isoproterenol. With high doses of isoproterenol (5.0 micrograms/liter), MVO2 per beat and QCOR/MVO2 increased. Addition of OISB (13 mM) increased P50 from 29 to 39 mmHg, with no significant reduction in either QCOR or the QCOR/MVO2 ratio. Our data suggest that a decrease in blood-O2 affinity affects myocardial O2 delivery depending on the initial metabolic requirement: at basal MVO2 changes in the distribution of myocardial blood flow are probably secondary to the effects of PO2 on vessels that supply metabolically less active regions; at high MVO2 and following a maximum increase in capillary density, changes in vascular PO2 appear less effective than locally generated metabolic vasodilators, and distribution of blood flow is unaffected.

[Clinical factors relating to the incidence of post-transfusion hepatitis following open heart surgery].

A retrospective study of post-transfusion hepatitis (PTH) has been made of 168 patients who had open-heart surgery between 1978 and 1981 at the Nagasaki University Hospital. The criteria of PTH was defined as an elevation of SGPT over 50 units with more than 2 weeks duration that occurred later than 3 weeks after the transfusion. PTH developed in 27 of the 168 patients (16%) and 26 of them were of the non-A, non-B type. The statistical analysis of these patients demonstrated that the following clinical factors significantly increased the incidence of PTH subsequent to open-heart surgery: (1) Use of plasma fractionating products made from pooled plasma, such as PPSB (p less than 0.001). (2) A large amount of blood and blood component transfusion (over 21 units, p less than 0.01). (3) Prolongation of the operation time (more than 330 minutes, p less than 0.05), cardiopulmonary bypass time (more than 90 minutes, p less than 0.05) and aortic cross-clamping time (more than 45 minutes, p less than 0.01), and (4) Surgery for cardiac lesions with left heart overloads (p less than 0.01).

Effects of ortho-iodo-sodium benzoate (OISB) on oxygen-hemoglobin affinity and its relation to plasma concentrations in dogs.

Effects of ortho-iodo-sodium benzoate (OISB) on oxygen-hemoglobin affinity were studied in unanesthetized beagles with simultaneous measurement of its plasma concentrations. Oxygen-hemoglobin affinity changes correlated well with the plasma concentrations of the drug. Intravenous administration of 100 mg/kg of OISB increased P50 significantly from the control value of 28.6 +/- 0.8 mmHg to 29.5 +/- 0.7 mmHg and 30.0 +/- 0.5 mmHg, 5 min and 30 min after the infusion, respectively. P50 gradually decreased thereafter, and after 1 hr period, elevation was no longer significant. Administration of 200 mg/kg of OISB caused a significant increase in P50 from the control of 29.3 +/- 0.3 mmHg to 30.5 +/- 0.4 mmHg, 31.1 +/- 0.3 mmHg, 31.1 +/- 0.3 mmHg and 30.5 +/- 0.5 mmHg, at 5 min, 30 min, 1 hr and 3 hr, respectively. P50 elevation declined thereafter to control levels. Plasma concentrations of the drug reached its highest level 5 min after the administration and declined gradually thereafter. All significant elevations of P50 were accompanied by the plasma concentrations of the drug more than 200 micrograms/ml. Biological half-life of the drug was found to be about 5.5 hr.

Doublet-outlet right ventricle [S,D,L] with subaortic ventricular septal defect and pulmonary stenosis: successful repair using an intraventricular conduit.

An 18-year-old Japanese girl with double-outlet right ventricle (DORV) [S,D,L], subaortic ventricular septal defect and pulmonary stenosis was successfully treated under cardiopulmonary bypass by resection of the obstructing subpulmonic infundibulum, pulmonary valvulotomy and construction of an intraventricular conduit between the ventricular septal defect and the aorta. The postoperative course was uneventful, except for mild low-output syndrome, and she was discharged 7 weeks after surgery in good condition. She is at present free from all untoward cardiac symptoms.
