Microvascular decompression of the eighth cranial nerve for disabling tinnitus without vertigo: a case report.

We report a patient with disabling tinnitus that responded dramatically to microvascular decompression of the 8th cranial nerve. The most annoying, episodic, high pitched tinnitus disappeared immediately after the operation. Two types of constant tinnitus either persisted or recurred after the procedure. Nevertheless, the remaining tinnitus is much reduced compared to the preoperative level. The patient was able to return to his previous work 2 months after operation.

Ipsilateral hemiparesis caused by subarachnoid hemorrhage in a patient with a ruptured middle cerebral artery aneurysm: a case report.

The authors report a patient with a ruptured middle cerebral artery (MCA) aneurysm who presented with ipsilateral hemiparesis. A computed tomographic (CT) scan and cerebral angiograms suggested that the ipsilateral hemiparesis was most likely due to compression by hematoma of the secondary motor area in the island of Reil. Two other cases that demonstrated ipsilateral motor weakness after rupture of MCA aneurysms have been reported. We emphasize not only the importance of considering dysfunction of the secondary motor area in patients with hemiparesis, but also the difficulty in differentiating ipsilateral secondary motor area dysfunction from contralateral primary motor area dysfunction in certain cases without CT scanning and angiography.

[Advantages and limits on the surgical therapy of esophageal cancer].

During a 11-year period, 315 patients of squamous cell carcinoma of the thoracic esophagus were treated. Notwithstanding the great majority of the patients had advanced cancer and operative contra-indications were very limited, the percentage of the operation of total patients was 94% (297/315) and that of operative mortality was 7.0% (20/297). As a standard surgical procedure, subtotal esophagectomies were performed through a right thoracotomy, followed by a laparotomy with simultaneous reconstruction via the posterior mediastinal route. On the 181 cases of the squamous cell carcinoma of the thoracic esophagus resected and reconstructed by this method before 1977, the survival rate of 1-year was 50%, that of 3-years was 19% and that of 5-years was 10%. Post operative T-shaped irradiation was performed in about a half of the total 181 patients. The efficacy of the postoperative irradiation was discernible in patients of each stage. It is expected that a precise schedule of chemotherapy and immunotherapy combined with post-operative irradiation might improve the prognosis of the esophageal cancer.

[One-way valve extension tube for continuous cerebrospinal fluid drainage system].

We designed an extension tube with built-in one-way valve to be used in the external CSF drainage. Placement, replacement or removal of the value can be performed easily and rapidly in patients who require continuous external CSF drainage. Advantages of the system using this one-way value extension tube were discussed. History of ventricular drainage was also mentioned.

[Grading of tricuspid regurgitation by pulsed Doppler echocardiography and liver function tests].

Recently, pulsed Doppler echocardiography has been used for the quantitative evaluation of tricuspid regurgitation (TR). However, there are some problems about the clinical significance of TR diagnosed by Doppler examination probably because of the excellent sensitivity in detecting TR. In the present study, we used liver function tests as an indicator of the visceral damage caused by TR and compared them to the severity of TR estimated by Doppler examination. Forty Doppler examinations were performed in 33 cardiac patients including seven, in which the examinations were repeated because the severity of TR changed during the clinical course. The severity of TR was classified into four grades according to the maximal extent of the regurgitant signal. Abnormal liver function was defined by the abnormalities of at least one of the following six tests: cholinesterase (CHE), gamma-GTP, leucine aminopeptidase (LAP), total bilirubin, alkaline phosphatase and GPT. Abnormal liver function tests were present in one of eight patients without TR, two of 10 with 1 + TR, four of seven with 2 + TR, seven of eight with 3 + TR, and six of seven with 4 + TR. The incidence of abnormal liver function tests increased parallel to the grade of TR and was very frequent in patients with 3 + or 4 + TR. TR diagnosed as greater than 3 + by Doppler examination is considered to have an important clinical significance and an indication of the intensive therapy.

[Atrial septal defect associated with an abnormal chamber of the right ventricle: report of a case].

A case of atrial septal defect associated with an abnormal chamber behind the right ventricle, which was heretofore undescribed, was reported. A 41-year-old woman was admitted to our hospital for the evaluation of a heart murmur. A Levine 3/6 ejection systolic murmur and a fixed splitting of the second heart sound were audible. Two-dimensional echocardiography disclosed an atrial septal defect and an abnormal chamber behind the right ventricle. This accessory chamber communicating with the right atrium was separated from the right ventricle by a membranous structure. The motion of this membrane showed a wide excursion just like that of the atrio-ventricular valve. On the M-mode echocardiogram, the anterior tricuspid leaflet and this membrane were recorded simultaneously without intervening septal echoes and this finding was very similar to that obtained in the single ventricle. Cardiac catheterization revealed that the pressure pattern in the abnormal chamber was that of the right atrium but its intracardiac electrocardiogram showed a right ventricular pattern. The complex morphological aspect of this case was confirmed by angiography and surgery. The developmental process of this very unusual anomaly is not clear, but it may be a kind of congenital anomaly related to Ebstein's anomaly.
