Malignant histiocytosis and encephalomyeloradiculopathy complicating coeliac disease.

A 62 year old Irish woman with an eight year history of probable coeliac disease developed brain stem signs, unilateral facial numbness and weakness, wasting and anaesthesia in both lower limbs. Over the next two years, a progressive deterioration in neurological function and in intestinal absorption, and the development of anaemia led to a suspicion of malignancy. Bone marrow biopsy revealed malignant histiocytosis. Treatment with cytotoxic drugs led to a transient, marked improvement in intestinal structure and function, and in power of the lower limbs. Relapse was associated with bone marrow failure, resulting in overwhelming infection. Post mortem examination confirmed the presence of an unusual demyelinating encephalomyelopathy affecting the brain stem and the posterior columns of the spinal cord.

Testicular venography in impalpable testis.

77 venographic studies of impalpable testes have been reviewed. All patients subsequently underwent surgical exploration. In 28 studies a pampiniform plexus was identified and in 27 a testis was present at the site of the pampiniform plexus. In 20 studies the testicular vein was blind-ending and in only 1 was testicular tissue present at the site of the blind-ending vein. Testicular tissue was absent in the remaining 19. There were 29 technical failures or an inability to identify the full length of the testicular vein because of patent valves. Testicular venography is most useful in localising the position of an impalpable testis but does not determine in all cases whether testicular tissue is present.

[Renocerebral oxalosis following glycerin infusion].

At the autopsy of a 55 years old male patient renocerebral oxalosis was found, the most frequent cause of which--the ethylenglycol poisoning could be excluded by toxicological examination of the organs. Authors suppose that intravenous administration of 500 ml of 10 per cent solution of glycerol several hours before the death for the treatment of cerebral oedema might have caused the renocerebral oxalosis. The fact that in the catabolism of glycerol, ethylenglycol and xylit causing most frequently renocerebral oxalosis glycoxylic acid, precursor of the oxalic acid in the organism is formed, seems to evidence this supposition.

Thyroid-bound Y-chromatin-like fluorescence in the nuclei of thyroid adenomas.

Brilliantly fluorescent supernumerary chromocenters indistinguishable from the Y-chromatin have been often found electively in the thyroid nuclei. The authors compared the occurrence of this Y-chromatin-like fluorescence in 31 thyroid adenomas obtained from 30 autopsy cases (10 males, 20 females) with non-adenomatous thyroid tissues of the same cases. All adenomas had follicular structure but one with papillary structure. The frequency class of Y-chromatin-like fluorescence of adenomas was lower in 15 cases out of 31 than that of the same non-adenomatous thyroid tissue. The Y-chromatin-like fluorescence had a negative count in 13 cases of 31 adenomas but only in 3 cases of normal thyroid tissues from 30 persons. The disappearance of the nuclei with Y-chromatin-like chromocenters is supposedly connected with cellular dedifferentiation of thyroid adenomas.

Non-random appearance of Y-chromatin-like fluorescence in the nuclei of thyroid and brain and its chromosomal background.

Non-random distribution of fluorescing chromatin bodies resembling Y chromatin was observed among the cell populations of various organs obtained by autopsy of 52 adults. This type of Y-chromatin-like fluorescence is more frequent in thyroid and brain than in other organs. The Y-chromatin-like bodies of thyroid nuclei are not closely correlated with the brilliantly fluorescing autosomal blocks of lymphocyte mitoses of the same persons, as it was demonstrated in surgical cases.

MR-guided biopsy of the breast after lumpectomy and radiation therapy using two methods of immobilization in the lateral decubitus position.

We describe two methods of breast immobilization using the lateral decubitus position to increase patient comfort and access to the axillary tail for MR-guided biopsy in the postsurgical or irradiated breast. The first method uses a compression device with good immobilization but poor patient tolerance. The second approach uses a thermoplastic mesh material to form a rigid exoskeleton around the breast: immobilization is adequate and patient acceptability is good. The latter method is preferred and requires formal evaluation in larger scale trials.

Primary jejunoileal tumors: a review of 45 cases.

We reviewed 45 patients who underwent surgery for primary jejunoileal tumors over a 15-year period. There were 16 benign and 29 malignant tumors, which included 13 lymphomas, 7 adenocarcinomas, 7 carcinoid tumors, and 2 leiomyosarcomas. Eighteen patients, 13 of whom had benign tumors, presented with intestinal bleeding and 5 tumors were found incidentally at laparotomy. Benign lesions were more frequently sited in the jejunum while malignant lesions were more common in the ileum (p less than 0.001). Lesions presenting with hemorrhage were more likely to be benign than malignant (p less than 0.001) and were more commonly sited in the jejunum than in the ileum (p less than 0.05). Visceral perforation (31%), intestinal obstruction (21%), and an abdominal mass (17%) were other presenting features in patients with malignant tumors. In spite of a wide variety of investigations, the correct diagnosis was reached preoperatively in only 31% of patients. Surgical management included either limited bowel resection or segmental resection with regional lymphadenectomy. Operative mortality was 13% and morbidity was 36%. Actuarial 5-year survival for all malignant tumors was 24%, being 64% at 5 years for carcinoid tumors, 20% at 30 months for adenocarcinomas, and 10% at 42 months for lymphomas. These results reemphasize the need for a high index of suspicion and early laparotomy in patients with obscure intestinal symptoms if the prognosis of small bowel tumors is to improve.

Accessibility of antigenic sites recognized by AUA1, HMFG1 and HMFG2 monoclonal antibodies: its influence on antibody binding of live cells.

We assessed the immunoreactivity of live and alcohol-fixed monolayers of HRA-19, a rectal adenocarcinoma cell line, to the monoclonal antibodies AUA1, HMFG1 and HMFG2. Differences in staining patterns between live and alcohol-fixed colonies were found. The well-polarized cells forming the centers of the monolayer colonies showed strong membrane staining when the cells were alcohol-fixed prior to AUA1 incubation, but showed no staining when the cells were alive during the incubation. When AUA1 incubation was done both before and after alcohol fixation, membrane staining was again seen, ruling out the possibility of antigenic modulation. Incubation of live cells with AUA1 together with EDTA showed strong staining of dissociating cells. It is concluded that AUA1 antigenic sites, which on polarized cells are basolateral in location, are inaccessible to the antibody-containing culture fluid, which bathes the apical aspects of the cells, but they become accessible after alcohol fixation, or treatment with EDTA. HMFG1 antigenic sites are located on the apical cell membrane, and accordingly, no differences were seen between incubation of live and alcohol-fixed cells when incubated with HMFG1. The antigenic sites of HMFG2 are partly intracellular, and in our monolayer model, the staining of live cells was weaker and more scarce than on alcohol-fixed cells. It is concluded that immunostaining of cytological and histological material of tumours may not adequately predict antibody binding on live cells, and thus, these findings are of importance in the context of selection of monoclonal antibodies for clinical radio-immunotargeting.

Fine needle aspiration cytology of a recurrent juvenile granulosa cell tumor.

Fine needle aspiration biopsy was performed on a recurrent juvenile granulosa cell tumor of the ovary in a 24-year-old woman. The cytologic appearances and the results of histochemical, immunocytochemical and ultrastructural studies are described, and the differential diagnosis with other ovarian tumors is discussed.

[Localization of protease inhibitors in the chorionic villi of the human placenta, by means of the immunoperoxidase technic].

Alpha-1-antitrypsin and alpha-1-antichymotrypsin have been studied in human placentas, from the first trimester to term pregnancy, by immunohistochemical method. Both proteinase inhibitors were localized to the cytoplasm of the villous syncytiotrophoblast, with a decreasing intensity of staining towards term pregnancy. The Hofbauer cells were intensely positive for intracellular alpha-1-antitrypsin and alpha-1-antichymotrypsin at all gestational ages. According to data of the literature we suggest that the existence of the proteinase inhibitors in the placental barrier possibly represents an important part in the defence mechanism of the placenta and may play a particular role in modifying the maternal immune response towards the foetus.

Hypocomplementaemic urticarial vasculitis, angio-oedema and 'lupus-like' disease: association with C4B null allele.

A patient who presented with urticarial vasculitis, recurrent angio-oedema and gastro-intestinal symptoms in association with a 'lupus-like' disorder is documented. Other 'auto-immune' phenomena such as Sjogren's-like syndrome and polyarthritis were present in association with antibodies to RNP, low C4 and a null allele was demonstrated at the C4B locus. Response to varied therapies was poor and unsustained.

Cytologic diagnosis in the management of gallbladder carcinoma.

Cytologic investigations of 16 proven cases of gallbladder carcinomas were analyzed in order to establish the usefulness of cytological methods, including exfoliative bile cytology, fine needle aspiration and endoscopic brushing, in the preoperative and intraoperative diagnosis and management of gallbladder carcinoma. Fine needle aspiration cytology had a sensitivity of 88% and exfoliative cytology of bile a sensitivity of 50%, but both brushings performed yielded false negatives. A preoperative diagnosis was made in 69% (9/13) of cases, and cytologic diagnosis was helpful in planning surgical management. We recommend its wider application in the preoperative diagnosis of gallbladder carcinoma.
