[Fundamental study of piperacillin sodium in term and premature neonates].

Piperacillin sodium (PIPC) is a semisynthetic penicillin displaying high antibacterial activities against Gram-positive and Gram-negative bacteria including Pseudomonas sp., Proteus sp., etc. It acts bactericidally and is stable against beta-lactamases. The usefulness of PIPC in the treatment of infections in mature and premature neonates was investigated and the following results were obtained. The pharmacokinetics (half-life, distribution volume, total body clearance) of PIPC after 50 mg/kg intravenous drip infusion in 10 cases of neonates were examined. Relationship between T1/2 and hours after birth was clearly determined. Adverse effects and abnormality in laboratory test values were not observed. It is considered from the above results that PIPC may be an useful antibacterial agent for the treatment of infections in neonates.

[Changes of dehydroepiandrosterone (DHA), 16 alpha OH-DHA and estriol levels in maternal peripheral blood during late pregnancy and labor--measured by gas chromatography-mass spectrometry with application of deuterated steroids as internal standards (author's transl)].

Serum levels of DHA, 16 alpha OH-DHA and estriol (E3) were determined in maternal peripheral vein by gas chromatography-mass spectrometry (GC-MS) with application of deuterated steroids as internal standards during late pregnancy and labor. Deuterium labeled 3,4,4-d3-DHA, 3,4,4-d3-16 alpha OH-DHA and 2,4-d2-E3 were newly synthesized and highly specific method for determination of serum DHA, 16 alpha OH-DHA and E3 was developed by GC-MS. The mean total steroid concentrations in the third trimester (III), in the stage when patients had irregular contraction (P) and during the first stage of labor (1st) were as follows; DHA, 612.9 +/- 265.3, 877.5 +/- 365.6 and 1522.5 +/- 580.6, 16 alpha OH-DHA, 30.2 +/- 20.2, 49.4 +/- 21.9 and 161.3 +/- 43.6, E3, 50.9 +/0 25.7, 70.4 +/- 19.8 and 108.6 +/- 44.5 ng/ml (mean +/- S.D.). The values of these three steroids increased significantly from the III to the 1st. Correlation between the levels of total DHA and total E2 measured in ten patients individually in the III was 0.8665 (p less than 0.001) which changed to -0.6650 (p less than 0.01) and -0.5362 (p less than 0.05) in the stage of P and 1st. ACTH was given to eight pregnant women in the third trimester, and response of these steroids to ACTH was studied. In addition to these steroids, cortisol (F) levels were also measured by RIA. Following the administration, F increased significantly whereas no changes of DHA, 16 alpha OH-DHA and E3 levels were noticed suggesting that these steroids may not be influenced by maternal adrenal secretion. From the results obtained above, it is suggested that the endocrinological environment in maternal blood as indicated by correlations between DHA and E3 levels, changes prior to the onset of labor.

[Experimental and clinical studies on the Jikei total knee prosthesis (author's transl)].

Total knee prostheses for a dog were developed and were inserted into knee joints of seven adult dogs to study the mobility of the artificial joint, the degree of fixation to bone and the durability of the prosthesis, to examine the reactions in the surrounding tissues and the internal organs in the JIKEI total knee prosthesis in clinical use. As a result of surgery, it was proved that the length in the hinged part of the prosthesis needed to be seventy percent of the length of the excised area of the joint to gain good mobility and supportability of the knee joint after surgery. As a result of findings based on necropsy, wear at the hinged part of the prosthesis was mild and debris caused in wear was not harmful for the living body if breakage does not occur there. Histopathological findings revealed that collagenous fibrous tissue with new bone formation was observed on the inner wall of the bone around the stem of the prosthesis four months after surgery. Then it was proved that the fixation of the prosthesis to the bone was sufficient. Therefore the author would like to mention that bone cement is not necessary for the fixation between bone and prostheses. The JIKEI total knee prosthesis (universal type) based on the results of these experimental studies has been developed and was used to the clinical cases. The clinical results until four years after surgery have been most satisfactory with loss of pain, good supportability and movement of joints in almost all of the cases.

Case report. Fetal cardiac failure resulting from endocardial fibroelastosis.

An autopsy was performed on a female stillborn infant with generalized edema associated with ascites. On gross examination of the heart the pulmonary trunk was atretic from its origin of the heart to the bifurcation. The ductus arteriosus was abnormally wide. The right ventricle was hypoplastic and its endocardium was diffusely thickened. The inner surface of right ventricle appeared pale, obscuring the identification of trabeculae corneae and papillary muscles. At the posterior wall there was an extremely thin lesion where no apparnet cardiac muscle was contained. The tricuspid orifice was stenotic and the valve was hypoplactic. The right atrium was markedly enlarged and its wall was thickened. The foramen avale had patency of the valvular competent type. Atrial septal defect (ASD) was not present. The left ventricle was remarkably dilated with markedly thick wall. Ventricular septal defect (VSD) was present immediately beneath the right coronary cusp of the aortic valve. Light microscopic examination of the right ventricle revealed a marked increase of collagen and elastic fibers with extensive deposits of calcium not only in the endocardium but also in the deep portion of myocardium. Fibroelastosis appeared particularly prominent in the subendocardial layer. The muscle fibers surrounded by these penetrating fibers and deposits of calcium showed degeneration. The entire myocardial layer of the specific area in the right ventricle where the wall was paper-thin was fully replaced by fibers and calcium. Inflammatory cell infiltration was found only at some places of the pericardium. The left ventricular wall also showed fibroelastosis, although the degree of involvement was much less as compared to that seen in the right ventricle. There were no significant pathological changes in both atria.
