BACKGROUND: The availability of nasal mask bi-level positive airway pressure (BiPAP) support in managing respiratory failure following cardiovascular surgery was studied. MATERIALS AND METHODS: BiPAP support was used for eight patients requiring postoperative prolonged respiratory support of 72 hours or longer. Their mean age was 65 years of age and the mean periods of postoperative endotracheal intubation was 12+/-5 days. BiPAP support was removed within 48 hours in six out of eight patients. Reintubation of an endotracheal tube was not necessary in all eight patients after the BiPAP treatment. RESULTS: The respiratory rates during the BiPAP management remained unchanged. The values of the respiratory index significantly (p<0.01) improved after BiPAP management (1.5+/-0.2 --> 0.9+/-0.2). A-aDO2 and Qs/Qt decreased (p<0.1) after the BiPAP management. There were no significant differences in central venous pressure and circulatory states during BiPAP support. CONCLUSIONS: In conclusions, BiPAP support is a noninvasive management technique for postoperative respiratory failure and may also prevent prolonged endotracheal intubation.
In the course of screening for Ras function inhibitors from plant extracts, we isolated lycorine from a chloroform extract of Eucharis grandiflora leaves. Lycorine induced flat morphology in K-ras-NRK cells after treatment for 2-3 days, whereas its morphological effect on NRK cells was weaker. Lycorine was found to inhibit protein synthesis specifically in cultured K-ras-NRK cells. It also lowered the cellular amount of Ras in 2-3 days.

In the course of screening for Ras function inhibitors, rocaglaol (1) and the related compounds, the known pyrimidinone (2) and the novel aglaiastatin (3), were isolated from a CHCI3 extract of the leaves of Aglaia odorata. The structure of 3 was elucidated as a novel cyclopentabenzofuran on the basis of its NMR spectroscopic data and by X-ray crystallographic analysis. These compounds (1-3) were potent inhibitors of the growth of K-ras-NRK cells, with IC50 values of 1-10 ng/mL, and induced normal morphology in K-ras-NRK cells at 10-30 ng/mL. They also specifically inhibited protein synthesis. Aglaiastatin (3) was slightly more potent than 1 and 2 in inhibiting cell growth. Aglaiastatin (3) reduced the amount of Ras, possibly by inhibiting its de novo synthesis.
A 79-year-old man with atrial septal defect (ASD) and tricuspid regurgitation was successfully operated upon. Preoperative examinations showed atrial fibrillation, moderate pulmonary hypertension and lung dysfunction. Direct closure of the ASD accompanying with tricuspid annuloplasty was performed. Postoperative administration of catecholamines continued for twenty-six days to maintain hemodynamics. An intratracheal tube was extubated two days after the operation. However, an additional respiratory support using a nasal continuous positive airway pressure method was required for six days. His postoperative physical activity has been improved with class I of the New York Heart Association classification. Surgical intervention may be the treatment of choice for aged patients over 70 years with an ASD.
Three patients over 70 years of age underwent surgical repair of secundum atrial septal defect (ASD). All patients had preoperative symptoms such as heart failure, atrial fibrillation and cardiomegaly. According to the New York Heart Association (NYHA) functional classification, two patients belonged to class II and one class III. Cardiac catheterization data revealed mild pulmonary hypertension and mild to moderate tricuspid regurgitation in all the patients. An ASD was directly closed in all the patients, and two of them required tricuspid anuloplasty. The use of catecholamine continued for two or three weeks in two patients because of postoperative cardiac dysfunction. Postoperative physical activity has improved in all the patients. Surgical repair of an ASD is recommended even for patients over 70 years of age unless they have major risks and severe pulmonary hypertension.
Intraoperative left ventricular rupture (type I) occurred following aortic and mitral valve replacement in 62-year-old man with osteogenesis imperfecta. Re-replacement of the mitral valve and the repair of left ventricular rupture were successfully performed. However, the patient died suddenly 3 days after the operation. Retroperitoneal bleeding due to the insertion of a continuous hemofiltration catheter via the femoral vein was detected at autopsy. Seven patients who underwent double valve replacements for aortic and mitral disease with osteogenesis imperfecta were reported with poor surgical results. We report our patient and discuss the relationship between osteogenesis imperfecta and left ventricular rupture.
Conophylline, a new vinca alkaloid isolated from the plant Ervatamia microphylla induced normal flat morphology in K-ras-NRK and K-ras-NIH cell lines, and lowered the increased uptake of 2-deoxyglucose in K-ras-NRK cells. Conophylline inhibited the growth of K-ras-NRK cells, but this inhibition was reversible. The alkaloid also inhibited the growth of K-ras-NRK and K-ras-NIH3T3 tumours transplanted into nude mice. On the other hand, it showed no effect on survival of the mice loaded with L1210 leukaemia. Thus, conophylline is a new antitumour vinca alkaloid that induced normal phenotypes in ras-expressing cells.
A 65-year-old man with left ventricular ejection fraction (LVEF) of 16% successfully underwent coronary artery bypass grafting (CABG). CABG used to be contraindicative for patients with LVEF below 20%. However, such patients are recently involved to indication of CABG if viability of the myocardium is proved. We consider CABG is indicated for a patient who keeps a sufficient cardiac output despite a low level of LVEF. Intraaortic balloon pumping which was started immediately after the induction of anesthesia was effective. Only eight patients with LVEF below 20% have undergone CABG in Japan. Thus, we reported our patient with some discussion on operative indication and perioperative management.
Hemodynamic effects of amrinone and the changes in pulmonary shunt were studied in patients for open heart surgery. Eighteen patients were allocated into two groups (group L, H) on the first day of surgery. Group L consisted of 10 patients who received moderate-dose catecholamine (DOA + DOB < 12 micrograms.kg-1.min-1) and group H consisted of 8 patients who needed high-dose catecholamine (DOA + DOB > or = 12 micrograms.kg-1.min-1) to maintain normal hemodynamics. After baseline measurement, amrinone infusion was started and the dose was increased by every 2 hours (5, 10, 20 micrograms.kg-1.min-1). At dose of 20 micrograms.kg-1.min-1, cardiac index increased significantly in group L but not in group H. SVRI and PVRI decreased by dose related fashion in both groups. Systemic arterial pressure was unaltered in group L while it decreased significantly in group H. PaO2 decreased and pulmonary shunt increased in both groups. These results suggest that inotropic response of amrinone depends on the basal level of myocardial contractility although vasodilative property and the effect on the pulmonary shunt are almost similar in both groups.
We present three patients who were complained of a high fever without infective signs clinically and bacteriologically after the implantation of a woven Dacron prosthesis impregnated with collagen for thoracic aortic aneurysm. High fever appeared on the third on fourth day after surgery, and continued for three to seven days. The maximum fever ranged 39.1 to 39.9 degrees C. There was a close relationship between the value of C-reactive protein and the duration of a high fever. There were no differences between patients with and without a high fever in white blood cell counts, and other hematological and biochemical examinations. Non-infective high fever may be encountered in the postoperative acute phase following the implantation of a Hemashield prosthesis.
We found a novel method to select anthocyanin-producing cells by a new means of cell staining. After staining with 0.2 ppm fluorescein isothiocyanate (FITC), the protoplasts from Aralia cordata cultured cells, which were composed of high anthocyanin-producing cells and non-producing cells, were observed to be distinguishable under a fluorescence microscope. The green fluorescence of FITC from the non-producing cells was clearly ascertained by the naked eye. On the other hand, the fluorescence of the anthocyanin-producing cells was completely missing. This phenomenon resulted from compensation of the green fluorescence (lambda max 525 nm) of FITC and the green light absorption (lambda max 530 nm) of anthocyanin. Although investigations of the flow cytometric method were developed on the basis of this phenomenon, the histograms of these protoplasts expressed the distinction between the anthocyanin-producing cells and the non-producing cells. In order to prove the technique, a highly productive cell line was selected from the heterogeneous populations of anthocyanin-producing or non-producing protoplasts from A. cordata cultured cells. The number of the sorted protoplasts containing anthocyanin was counted by the naked eye, the anthocyanin-producing protoplasts were approximately 90% in the whole protoplasts and the viability was 60-70%. Anthocyanin concentration the sorted protoplasts was 3-fold higher as compared with the protoplasts before sorting. The anthocyanin content of the cell line selected by this method was 7.7 +/- 0.6%/g dw.(ABSTRACT TRUNCATED AT 250 WORDS)
We investigated whether the left ventricular filling profile, defined as the early to late diastolic left ventricular filling volume ratio, during the preceding control beats actually affects the pulse pressure during a ventricular premature contraction (PVC). Twenty patients underwent invasive electrophysiological study for sinus bradycardia. VPCs with various coupling intervals were induced by right ventricular electrical stimulation, and the mitral filling flow velocity by pulsed Doppler echocardiography, the femoral arterial pressure curve and the electrocardiogram were simultaneously recorded. The early to late diastolic velocity-time integral ratio (Ei/Ai ratio) of the mitral filling flow velocity during the control beats which preceded the VPC was measured as an index characterizing left ventricular filling profile. The coupling interval of each VPC and the extrasystolic beat pulse pressure were measured. The ratio of the extrasystolic beat pulse pressure to the control beat pulse pressure was expressed in % (% extrasystolic beat pulse pressure). The correlation between the coupling interval and the % extrasystolic beat pulse pressure was investigated. Coupling intervals of 0.80, 0.70, 0.60, 0.50, and 0.45 s were used. At a coupling interval of 0.80 or 0.45 s, the % extrasystolic beat pulse pressure showed no significant correlation with the Ei/Ai ratio. In contrast, the % extrasystolic beat pulse pressure with coupling intervals of 0.70, 0.60, and 0.50 s showed a significant positive correlation with the Ei/Ai ratio (r = 0.67, 0.74, and 0.66, P < 0.01, respectively). In addition to the prematurity and the site of origin of the VPCs, the left ventricular filling profile during the preceding control beats may significantly affect the height of the pulse pressure during extrasystoles with medium length coupling intervals.
BACKGROUND: The eyelid dermatitis seen in elderly patients is a relatively heterogenous and troublesome disease. Most cases seem to be idiopathic and show resistance to standard dermatologic therapy. METHODS: Fifty-two patients with primary definite Sjögren's syndrome (4 men, 48 women, mean age 54 years) were enrolled in this study, and the prevalence of eyelid dermatitis was investigated. Diagnostic criteria for Sjögren's syndrome were based on the criteria proposed by the Japanese Ministry of Health and Welfare. Patch test was performed using ICDRG-European standard allergens and/or eye drops, hair dye, and cosmetics. RESULTS: Of the 52 patients, 22 showed eyelid dermatitis. These changes were much more frequent in elderly patients and showed a good correlation with the presence of ocular dry sensation. No significant difference was observed in clinical and other laboratory findings between patients with or without eyelid dermatitis. Although 8 of the 13 patients showed positive patch test reaction to various allergens, no close relationship existed between the use of a suspected substance and the onset or severity of eyelid dermatitis. CONCLUSIONS: These results suggest that the presence of rubbing dermatitis of the eyelid may be one of the cutaneous manifestations of Sjögren's syndrome.
A 80-year-old woman with right ventricular wall perforation and extraction into the left thoracic cavity by a permanent endocardial pacemaker (PM) lead was presented. The patient underwent an insertion of a permanent endocardial lead into the apex of the right ventricle for the treatment of sick sinus syndrome. The right ventricular perforation of the PM lead was incidentally diagnosed by recognizing it into the left thoracic cavity three months after the operation. Another PM lead could be inserted without removal of the former one.
Between 1980 and 1992, 84 patients over 30 years were underwent operative closure of atrial septal defect (ASD). Preoperative complaints and lower cardiac function were more frequent in the older than in the younger, but the clinical symptom of both generation improved significantly after surgery. Only a 67-year-old patient died after surgery due to low cardiac output syndrome. No long-term death occurred. The incidence of atrial fibrillation was increased postoperatively compared with preoperatively. Therefore, we recommend early surgical repair for aged ASD before thirties, if possible.
We have screened tropical plant extracts for substances that induce normal morphology in K-rasts-NRK cells. As a result we isolated an anthraquinone compound, damnacanthal, from the chloroform extract of the root of Morinda citrifolia. Damnacanthal induced normal morphology and cytoskeletal structure in K-rasts-NRK cells at the permissive temperature, without changing the amount and localization of Ras. The effect of damnacanthal was reversible, and the compound had no effect on the morphology of RSVts-NRK cells expressing the src oncogene. Thus, damnacanthal is a new inhibitor of ras function.
