[The effect of dopamine and prostaglandin E1 on urine oxygen tension (PuO2)].

We reported that PuO2 reflects renal medullary perfusion and decreases during N2O-O2 isoflurane (GOI) or sevoflurane (GOS) anesthesia. In the present study, we examined the effect of dopamine (DOA) or prostaglandin E1 (PGE1) on PuO2 during GOI anesthesia. Twenty four patients from elective surgery with GOI anesthesia were randomly divided into three groups, i.e. control group (n = 8), DOA group (n = 8) and PGE1 group (n = 8). PuO2 decreased significantly in the control group, while in the DOA and PGE1 groups, PuO2 was unchanged. These results suggest that DOA and PGE1 maintain renal medullary perfusion under GOI anesthesia.

[An experimental study of chemoembolization for VX2 bone tumors].

An experimental study of preoperative chemoembolization using cis-platinum crystals was performed in rabbit to test a drug delivery system used for malignant musculoskeletal tumors. The cis-platinum was crystallized from solution, and injected into the animals. The average crystal size was 393 x 222 microns, and their properties were the same as the properties of the solution. A VX2 bone tumors was transplanted into the distal femoral metaphysis of a rabbit in the form of a tumor cell suspension. A total of 20 rabbits were divided into four groups. In one group (Group A), chemotherapy was not used to treat the tumor. In the second group (Group B), a cis-platinum solution was injected into a subcutaneous vein at one week after the VX2 transplantation. In the third group (Group C), a cis-platinum solution was injected into the proximal femoral artery. In the fourth group (Group D), cis-platinum crystals were injected into the proximal femoral artery and a tourniquet was applied to prevent blood flow to the knee (below the tumor) during the injection. X-rays and arteriograms were performed periodically. At 3 weeks after the transplantation, the rabbits were sacrificed and the lesions were examined histopathologically. Serial body weights and tumor growth were also assessed. The X-rays and arteriograms in Groups C and D showed less destruction of the adjacent bone, and less vascularity in the tumor than those in Groups A and B. Wide degenerative or necrotic areas inside the tumor were observed macroscopically in Groups C and D. Focal necrosis was most frequently observed microscopically in Group D. It was concluded from the results of this experimental study that chemoembolization was an effective preoperative procedure that could be used as a drug delivery system to treat a malignant musculoskeletal tumor.

[Systemic-pulmonary artery shunt using Golaski graft: trial for measurement of the shunt flow].

For the systemic-pulmonary artery shunt operation, the modified Blalock-Taussig shunt was the first choice for procedure in our institution. Since 1990, Golaski knitted Dacron graft (4 or 5 mm in diameter) was used for the prosthesis. Ex-vivo flow calibration of the electromagnetic flow meter (Nihon Koden, MFV-3100) to Golaski graft showed good correlation between the real flow and value measured by the electromagnetic flow meter. Shunt flow was measured in the consecutive clinical fifteen cases. The shunt flow per body surface area of the patient who required additional shunt operation was 721 ml/min/m2 and one patient in whom the congestive heart failure developed after the shunt operation, had the shunt flow of 3,022 ml/min/m2. The adequate shunt flow in these cases was ranged from 745 to 2,820 ml/min/m2 (mean +/- 1 SD, 1,490 +/- 587.8). Therefore we performed the systemic-pulmonary artery shunt operation using Golaski graft to get the shunt flow of 1,000 ml/min/m2 (approximately a third of cardiac index) for the guide of good results.

The "turn-over" technique for selective cerebral angiography.

In older patients with severe aortic atherosclerosis or pronounced tortuosity of the brachiocephalic vessels, selective catheterisation may be impossible. In order to overcome this difficulty we introduced a new "turn-over" technique. This involves introduction of a long guidewire through the catheter, advancing it and turning it over just above the aortic valves and finally introducing it into each major vessel, followed by the catheter. Selective catheterisation was completely achieved with the turn-over technique in 13 patients. Turning over the catheter just above the aortic valves and introducing it into the vessels was very easy.

Primary malignant fibrous histiocytoma of the diaphragm: report of a case.

We present herein the case of a 55-year-old man with primary malignant fibrous histiocytoma (MFH) of the diaphragm. He was admitted to our hospital with right anterior chest pain, where diagnostic imaging showed a huge tumor in the right diaphragm with a tumor thrombus in the vena cava, spreading to the left lobe of the liver and the pericardium. Although invasion into the right lung was identified at the time of surgery, the entire tumor was removed. Histopathological examination of the resected specimen revealed the characteristics of MFH. We discuss the clinical features of this patient in comparison with the few previous reports on MFH of the diaphragm.

Successful intra-arterial fibrinolysis of the anterior choroidal artery in the acute stage of internal carotid artery occlusion: case report.

A 27-year-old man was admitted to our institution with the sudden development of right hemiparesis and dysarthria beginning an hour after the onset of symptoms on August 9, 1992. The patient was found on admission to have right hemiparesis (2/5), hemihypesthesia, hemianopia, dysarthria; he had transient atrial fibrillation. No abnormalities were detected on computed tomography (CT) scans, and cerebral blood flow studies undertaken following conventional CT scans revealed no low flow regions in the left cerebral hemisphere. But cerebral angiography disclosed an occlusion of the left internal carotid artery with well-developed cross-circulation via the anterior communicating artery and embolus lodged at the level of the anterior choroidal artery. Superselective fibrinolysis using Tracker-18 and 420,000 units of urokinase resulting in complete recanalization of the left anterior choroidal artery without distal migration of the embolus. Immediately after the procedure, his neurologic disturbance underwent complete resolution. In summary, fibrinolysis could be performed but limited to anterior choroidal artery in a case with an occlusion of the internal carotid artery with well-developed cross-flow via the anterior communicating artery; the patient's neurologic condition may deteriorate suddenly if fibrinolysis is incomplete and the embolus migrates to the internal carotid artery.

Percutaneous transluminal angioplasty for acute stroke due to stenosis of major cerebral vessels: report of two cases.

The Stealth dilation catheter was introduced for the intravascular treatment of two patients with acute and unstable ischemic stroke due to hemodynamically significant stenosis in the one case of the middle cerebral artery and in the other of the vertebral artery. Cerebral perfusion study on the two cases showed a moderately low flow area in the right cerebral hemisphere and in the left cerebellar hemisphere, respectively. Percutaneous transluminal angioplasty (PTA) using the Stealth catheter was conducted. The two cases showed marked improvement in their neurologic state just after PTA with marked increase in cerebral perfusion in the relevant regions.

Persistent infection with SIVmac chimeric virus having tat, rev, vpu, env and nef of HIV type 1 in macaque monkeys.

A chimeric human and simian immunodeficiency virus carrying the tat, rev, vpu, env, and nef genes of human immunodeficiency virus type 1 was generated. The chimeric virus, NM-3n, grew competently in peripheral blood mononuclear cells from cynomolgus monkeys like the parental SIVmac. Two cynomolgus monkeys and one rhesus monkey inoculated with NM-3n raised antibodies to SIVmac Gag and HIV-1 Env. The antibodies raised in the cynomolgus monkeys persisted for at least 1.7 years. The antibodies contained virus neutralizing activity not only to the original chimeric virus but also to the parental HIV-1. Infectious viruses were isolated from one of the cynomolgus monkeys 37 and 63 weeks after inoculation and from the rhesus monkey continuously from 6 weeks after infection onward. The recovered virus maintained its chimeric structure but included several clones with mutations in the env V3 region. When the recovered virus was inoculated to another rhesus monkey, no difference in the frequency of virus recovery was seen from the originally infected monkeys. These carrier monkeys have so far shown no sign of the disease.

Genetic analysis of Escherichia coli O9 rfb: identification and DNA sequence of phosphomannomutase and GDP-mannose pyrophosphorylase genes.

Subcloning, transposon insertion, and deletion analysis revealed that the Escherichia coli O9 rfb region is about 12 kb in size. The region encodes at least seven polypeptides of 89, 74, 55, 50, 44, 41 and 39.5 kDa. Southern hybridization analysis of rfb regions of E. coli O8 and O9, and Klebsiella O3 and O5 serotypes (all of these O polysaccharides are mannose homopolymers and the structures of the repeating unit of E. coli O9 and Klebsiella O3 are identical) showed that a central region specific for E. coli O9 and Klebsiella O3 is flanked by two regions common to all four. Complementation experiments using strains with known defects and specific tests for the enzymic activity showed that the 50 and 55 kDa polypeptides, encoded by the common region, are phosphomannomutase (PMM) and GDP-mannose pyrophosphorylase (GMP), respectively. Nucleotide sequencing of the region revealed the presence of two genes, rfbK and rfbM, analogous to the corresponding genes of Salmonella typhimurium. In E. coli O9, rfbK and rfbM encode proteins of 460 amino acids (50,809 Da) and 471 amino acids (52,789 Da). The amino acid sequence of GMP was conserved in RfbMs of E. coli O7 and Salmonella groups B, C1 and C2, CpsB of S. typhimurium, AlgA of Pseudomonas aeruginosa, and XanB of Xanthomonas campestris. The phylogenetic trees of PMM and GMP were different in topology and in the evolutionary distances from ancestors.

Expression of transforming growth factor-alpha in the normal cervix and in benign and malignant lesions of the uterine cervix.

OBJECTIVE: To investigate the expression and distribution of transforming growth factor-alpha (TGF-alpha) in the normal cervix and in benign and malignant lesions of the uterine cervix. SUBJECTS AND METHODS: Immuno-histochemical reactivity with a monoclonal antibody against TGF-alpha was examined in tissue specimens from 15 normal cervices, six cervical polyps, four cervical condylomata acuminata, 34 cervical intra-epithelial neoplasias, 35 invasive squamous cell carcinomas, five adenocarcinomas, and three mixed adenosquamous carcinomas. RESULTS: Normal squamous cells of the exocervix were found to be negative for TGF-alpha immunoreactivity, whereas reserve cells and metaplastic squamous cells in the transformation zone were positive for TGF-alpha. Although TGF-alpha immuno-reactivity was variable in the cervical condylomas, most cases of cervical intra-epithelial neoplasia with or without koilocytotic atypia were negative for TGF-alpha. In the invasive carcinomas, however, TGF-alpha immuno-reactivity was observed in 17 out of the 35 cases of squamous carcinoma, and in all cases of adeno- and adenosquamous carcinomas. In addition, intense TGF-alpha immuno-reactivity was found in clinically advanced tumours. CONCLUSION: These results suggest that the expression of TGF-alpha is associated with squamous metaplasia in the normal cervix, and that TGF-alpha may play an important role in cervical carcinogenesis, especially in its progression.

Preparation of organic solvent-tolerant mutants from Pseudomonas aeruginosa strain PAO1161.

Most organic solvents (OSs) are toxic and inhibit growth of microorganisms even at low concentrations. Therefore, they are used to sterilize microbial cultures and to maintain solutions in a sterile condition. However, the physiological basis of such phenomenon is poorly understood. Although there are some microorganisms that can utilize a number of OSs as their sole carbon and energy sources, OSs must be provided as a vapor or at a very low concentration to avoid growth inhibition.

Survey of fluctuating fetal body weights in Sprague-Dawley rats.

During the past 9 years of reproductive and developmental toxicity studies in Sprague-Dawley rat, we noticed that the average fetal body weight on day 20 of gestation in 81 control groups fluctuated significantly from study to study. The other parameters such as number of implants, resorption rats and number of live fetuses were relatively stable. With information cooperatively given by the animal supplier, we could pin down the possibility of the fluctuation. The animal supplier introduced Sprague-Dawley rats into Japan from the U. S. twice in the past. The documents we analyzed indicated that the fluctuation in fetal body weight was caused by different two colonies which were introduced into Japan at the different times.

Percutaneous transluminal angioplasty of stenotic primitive hypoglossal artery--case report.

A 76-year-old female presented with vertebrobasilar insufficiency due to a severe stenosis of the right primitive hypoglossal artery (an unusual carotid-basilar anastomosis) manifesting as recurrent transient ischemic attacks (TIA) associated with quadriparesis and cerebellar ataxia with vertigo, nausea, and vomiting. She had been treated with 100 mg of aspirin per day, but TIA associated with the same symptoms persisted. Cerebral blood flow (CBF) studies disclosed a region of moderately low flow in the posterior fossa. Cerebral angiography demonstrated that the posterior fossa was supplied via the right primitive hypoglossal artery, which was severely stenotic at its origin. Percutaneous transluminal angioplasty using a Stealth catheter, 3.0-mm diameter and 10-mm long, successfully dilated the stenosis. No TIA occurred postoperatively, and a marked increase in CBF was demonstrated in the posterior fossa.

Dural arteriovenous fistula caused by sinus thrombosis--case report.

A 58-year-old male with a history of idiopathic sinus thrombosis presented with gradual onset of gait disturbance, dementia, and involuntary movement in the upper extremities. Cerebral angiography demonstrated a dural arteriovenous fistula fed by a falx cerebelli branch originating from the left vertebral artery and draining into the inferior vermian vein, the straight sinus, a cortical vein lying on the inferolateral surface of the left cerebellar hemisphere, the ipsilateral superior petrosal sinus, the sigmoid sinus, and the internal jugular vein. Endovascular embolization under fluoroscopic control resulted in complete disappearance of the involuntary movement. Sinus thrombosis may result in development of dural arteriovenous fistula which can cause life-threatening complications, so aggressive therapy should be considered.
