Microsurgical anatomy of the lower cervical spine and cord.

The authors dissected the cervical spine and its surrounding structures from 40 adult cadavers under a surgical microscope. The anterior part of the spine and spinal cord was examined after vertebrectomy. The posterior longitudinal ligament (PLL) consists of two layers; the anterior one is termed the deep layer, and the posterior one is termed the superficial layer. These two layers adhered together loosely. In the lateral portion of the spinal canal, the superficial layer joined the periradicular sheath at the level of the intervertebral disc spaces and joined the dura mater at the level of the vertebral bodies. After the removal of the deep layer, the anterior internal vertebral venous plexus was seen on top of the lateral part of the superficial layer. The venous plexus was embedded between the double-layered PLLs, was not located in the epidural space, and was not seen in the medial part of the PLL. The PLL without venous channels on top of it was about 10 mm in width at the levels of the intervertebral disc and about 5 mm at the levels of the vertebral body. The anterior root exit zone (AREZ) was an elliptical shape; the transverse length of the AREZ was about 2 mm, and the longitudinal length was 10 to 15 mm. The average number of anterior rootlets on the AREZ was 17 to 25 and tended to decrease in the lower cervical spinal cord. The posterior structures were examined after en bloc laminectomy.(ABSTRACT TRUNCATED AT 250 WORDS)

Rapid appearance of connexin 26-positive gap junctions in centrilobular hepatocytes without induction of mRNA and protein synthesis in isolated perfused liver of female rat.

In the adult rat liver, the gap junction protein connexin 32 (Cx32) is evenly distributed in hepatocytes within the liver lobules, while connexin 26 (Cx26) is preferentially localized in hepatocytes in periportal zones. We report here that Cx26-positive gap junctions rapidly appear in the centrilobular hepatocytes of adult female rat livers during a 30 minute perfusion of the liver through the hepatic portal vein with a 1:1 mixture of Dulbecco's modified Eagle's medium (DMEM) and oxygen transport FC-43 fluid at a physiological flow rate without any changes in the distribution of Cx32. The change in the localization of Cx26 was closely related to that of E-cadherin, and there was no significant increase in the amounts of Cx26 protein and mRNA. The appearance of Cx26 in the centrilobular hepatocytes was inhibited by treatment with cytoskeleton disruptors such as colchicine and cytochalasin B, and intracytoplasmic transport inhibitors such as brefeldin A. The liver perfusion induced the appearance of Cx26 in the centrilobular hepatocytes only in female rats. Estrogen treatment of ovariectomized rats caused the appearance of both Cx26 and E-cadherin in centrilobular hepatocytes not only in the perfused liver but also in the non-perfused liver. Our results indicate that in the rat liver: (a) the localization of Cx26 can be modulated by a post-translational mechanism; (b) E-cadherin may play an important role in the formation of gap junctions composed of Cx26; and (c) the formation of gap junctions is regulated by female steroid hormones.

Hormonal regulation of connexin 43 expression and gap junctional communication in human osteoblastic cells.

We have recently shown that connexin 43 (Cx43), a major gap junction protein in osteoblasts, is expressed with an increase in cell density (CHIBA, H. et al. (1993). Cell Struc. Funct., 18: 419-426). In the present study, we examined what kinds of hormones and cytokines regulate the gap junction protein in osteoblastic cells, using a human osteoblastic cell line (SV-HFO) after reaching a confluent density to avoid influence of cell proliferation. Either retinoic acid (RA) or transforming growth factor-beta 1 (TGF-beta 1) induced the Cx43 expression of SV-HFO cells, as revealed by Northern blot analysis and immunocytochemistry. These modulators also increased gap junctional intercellular communication, in terms of the extent of dye transfer. On the other hand, 1 alpha, 25-dihydroxyvitamin D3 did not influence the Cx43 expression and gap junctional intercellular communication of the cells. These results suggest that RA and TGF-beta might maintain bone tissue as an organized tissue in vivo by increasing intercellular communication of osteoblastic cells.

Studies of diabetic nephropathy. I. Effects of storage time and temperature on microalbuminuria.

The effect of storage time and temperature on the immunological turbidimetric measurement of a low concentration of albumin in urine was investigated. In storage at -20 degrees C, the albumin level decreased, but the rate of this decrease differed considerably among specimens. However, under storage at room temperature for 2 weeks, or at 4 degrees C for 5 weeks albumin levels did not show significant changes. At -40 degrees C and -80 degrees C there were only slight decreases. At -40 degrees C decreases were slightly greater than at -80 degrees C. Therefore, -80 degrees C was found to be the optimal temperature for long-term storage of urinary albumin. Some of the specimens showed a 50% decrease in albumin level after storage for 9 weeks at -20 degrees C, but remained unchanged after storage for the same period at -80 degrees C. A pair of specimens preserved at -20 degrees C and -80 degrees C were isolated by SDS-PAGE (sodium dodecyl sulfate-polyacrylamide gel electrophoresis). After electrophoresis, urinary proteins were stained by silver staining to observe bands, and albumin content was determined by immunoblotting. A decrease in albumin concentration was also observed by densitometric detection.

Microwave ablation of the atrioventricular junction in vivo and ventricular myocardium in vitro and in vivo. Effects of varying power and duration on lesion volume.

Catheter ablation in ventricular tachycardia has achieved only limited success using direct current (DC) and radiofrequency (RF) energy, due to either high complication rates or a limited lesion size. Microwave energy represents a possible alternative source of energy for percutaneous ablation of the ventricular myocardium. However, an optimal method for titration of the dose of microwave energy to achieve the desired lesion volume has not yet been established. The safety and efficacy of microwave ablation of the atrioventricular (AV) junction were studied in 11 dogs in vivo. The relationship between lesion size following microwave ablation and the power output of or exposure duration to microwave energy was also examined at disparate sites in each superfused left ventricular epicardium in vitro. To observe the pathologic changes in the myocardium after microwave ablation, microwave ablation of the endocardium of the left ventricle was carried out in 9 dogs in vivo. Complete AV block was achieved in 10 of the above 11 dogs with a mean of 5 applications of microwave energy. The lesion volume in vitro demonstrated a parallel increase with power (r = 0.76) and duration (r = 0.81). The mean lesion volume at 30 sec was: at 10W, 0.8 +/- 1.6; 20 W, 34.7 +/- 10.3; 30 W, 34.7 +/- 22.4: 40 W, 64.7 +/- 64.4; 50 W, 87.2 +/- 42.3; 60 W, 85.8 +/- 38.1; 70 W, 124.7 +/- 36.5; 80 W, 134.2 +/- 49.0 mm3. The mean lesion volume at 80 W was: at 15 sec, 32.6 +/- 37.8; 30 sec, 101.2 +/- 46.4; 60 sec, 180.6 +/- 80.1; 120 sec, 291.8 +/- 122.7; and 180 sec, 459.3 +/- 204.6 mm3. The ablated lesions showed discrete, homogeneous coagulation necrosis with sharp margins from the adjacent normal myocardium. Microwave energy may thus be more effective than RF energy, and have a lower risk of complications and arrhythmogenesis than DC energy when used for ablation in ventricular tachycardia.

A patient with chronic hepatitis C who simultaneously developed interstitial pneumonia, hemolytic anemia and cholestatic liver dysfunction after alpha-interferon administration.

Following a three-week administration of alpha-interferon (IFN-alpha), a 62-year-old woman with chronic hepatitis C manifested fever and dyspnea and showed diffuse infiltrative opacities on chest roentgenograms. Her laboratory data included results of anemia with reticulocytosis, a decreased complement level and hepatitis with elevated ALP, LDH and gamma-GTP. Because laboratory data also revealed a positive lymphocyte stimulation test for IFN-alpha, this cytokine was considered to be responsible for the development of interstitial pneumonia, hemolytic anemia and cholestatic liver dysfunction due to its immunomodulatory effects. Although these three disorders have been reported to develop singly after IFN-alpha therapy, this is the first report of a patient in whom these disorders occurred simultaneously.

[A study on distribution of Porphyromonas gingivalis in oral cavity by using a non-radioactive DNA probe method].

The purpose of this study was to evaluate the distribution of Porphyromonas gingivalis (P. gingivalis) in the dentition using a non-radioactive DNA probe and to compare the presence of P. gingivalis with the clinical parameters. Fifteen adult periodontitis patients and 6 clinically healthy volunteers were examined. Subgingival plaque samples were taken from 4 sites of all the remaining teeth. Probing depth and bleeding on probing (BOP) were also recorded. Plaque samples were investigated using a whole genomic DNA probe from P. gingivalis modified with bisulfite. In the patient group, the detection percentage and the amounts of P. gingivalis present were statistically compared with probing depth and BOP. As a result, P. gingivalis was detected in all 15 patients and 3 out of 6 healthy individuals. The detection average was 31% in the patient group. When the probing depth was over 4mm, 6mm or the BOP was positive, the detection percentage of P. gingivalis significantly increased in the patient group. As more P. gingivalis was identified, the percentage of the sites with deep probing depth or that were BOP-positive increased. However, P. gingivalis was also detected in the clinically healthy sites, and also it was not detected in some sites with deep probing depth or that were BOP-positive. These results indicate that P. gingivalis plays an important role, but is not the only microorganism responsible for adult periodontitis.

Parabolic flight experiments on physiological data acquisition and processing technologies using small jet aircraft (MU300).

The parabolic aircraft flight provides a short low gravity environment for approximately 20 seconds, which may not be sufficient for a research on the physiological phenomenon induced by actual weightlessness in space. However, the method is still useful to reveal essential and characteristic feature of physiological signs, and is available for testing hardware and also training of crew member during altered gravity. This paper reports the summary of parabolic flight experiments recently conducted as a NASDA program (1990-1992). The program is providing opportunities in low gravity research with small jet aircraft for researchers and agencies. The flight experiments in the life science area have been conducted mostly focused on a physiological changes and basic methodology which may be effective under the altered gravity condition. In this study, the following research team, NASDA, Research Institute of Environmental Medicine, Nagoya University, Toyohashi University of Technology, Tokyo Metropolitan Hospital, Torey Research Center and JSUP were involved and coordinated for the research.

[Anti-inflammatory effect of a traditional Chinese medicine, ren-shen-yang-rong-tang (Japanese name: ninjin-youei-to), on alveolar macrophages stimulated by RANTES or TNF-alpha].

This study was performed to clarify the effect of RANTES or TNF-alpha on rabbit alveolar macrophages and the anti-inflammatory effect of NINJIN-YOUEI-TO on alveolar macrophages stimulated by RANTES or TNF-alpha. Phagocytosis of alveolar macrophages was dose-dependently stimulated by RANTES or TNF-alpha. However, phagocytosis of alveolar macrophages was completely suppressed to the basal level when alveolar macrophages were preincubated with NINJIN-YOUEI-TO at 37 degrees C for 20 min. These results suggest that NINJIN-YOUEI-TO has an anti-inflammatory effect on alveolar macrophages through a process still to be determined.

[Transmission routes of hepatitis C virus: analysis of anti-HCV-positive pregnant women and their family members].

OBJECTIVE: Our purpose was to study the intrafamilial transmission of hepatitis C virus (HCV) and the mother-to-child transmission rate of HCV in babies born to HCV carrier mothers. STUDY DESIGN: Anti-HCV antibody (anti-HCV) was tested for 2,528 consecutive pregnant women using an EIA first or second generation kit. Babies born to anti-HCV-positive mothers and their family members had their anti-HCV and HCV RNA checked. These babies were prospectively studied every 3 month till 1 year and therafter every 6 month. RESULTS: 1.19% of this population were positive for HCV. Eighteen of the 32 anti-HCV-positive pregnant women (56.3%) had HCV RNA. Six of 26 of their husbands (23.1%), 4 of 21 of their mothers (19.0%) and 6 of 11 of their fathers (54.5%) had anti-HCV, which were much higher prevalence rates than in the general population (p < 0.001). Two among 30 samples of cord blood from babies born to these pregnant women (6.7%) had HCV RNA. Among 29 children born to 18 HCV RNA-positive pregnant women, 4(13.8%) had HCV RNA, but none of 27 children born to 14 HCV RNA-negative pregnant women. CONCLUSION: It is suggested that mother-to-child and intrafamilial transmission of HCV exists. Mother-to-child transmission, including by the transplacental route, occurs and the rate may be from 5 to 15%.

Efficient amplification of the HLA-DQA1 gene in single genomes using a semi-nested polymerase chain reaction.

A semi-nested polymerase chain reaction (PCR) was introduced to amplification of the HLA-DQA1 gene, which is a single-copy gene, in a single genome. The limitation of template DNA for genotyping is 1 ng of genomic DNA, or more in the case of ordinary PCR. When reamplification with the same primers was performed, primer dimer was generated and the sensitivity was not improved. We designed a semi-nested primer for the second round of PCR, using the semi-nested PCR, more than 3 pg of template DNA could be amplified and typed. Furthermore, this method was applied to amplify DQA1 gene in single human sperm having haploid DNA, and followed by typing with sequence-specific oligonucleotide (SSO) probes. The semi-nested PCR technique was found to enhance the sensitivity of the amplification reaction and allowed the successful typing of the HLA-DQA1 gene. This is helpful for genotyping from samples with extremely small amounts of DNA, such as forensic or ancient DNA samples.

Circulating ICAM-1 levels in children with atopic dermatitis.

Circulating intercellular adhesion molecule-1 (ICAM-1) levels were measured in 48 patients with atopic dermatitis (mild in 29, moderate in 13, and severe in 6) and in 32 controls. Circulating ICAM-1 levels gradually increased with the severity of atopic dermatitis (216 +/- 68 in control, 323 +/- 66 in mild, 376 +/- 92 in moderate, and 555 +/- 93 ng/mL in severe atopic dermatitis, respectively). A significant correlation was observed between serum ICAM-1 and eosinophilic cationic protein levels in patients with atopic dermatitis (r = .71, P < .01). Although a significant positive correlation was observed between serum eosinophilic cationic protein levels and peripheral eosinophil counts (r = .63, P < .01), only a weak correlation was observed between serum ICAM-1 levels and peripheral eosinophil counts (r = .35, P < .05). Changes in serum ICAM-1 levels in ten patients treated with ketotifen for 3 months were also demonstrated. A close correlation was observed between serum ICAM-1 levels and the disease severity (r = .76, P < .01). From these results, we speculate that serum ICAM-1 level may reflect the extent of inflamed tissue in atopic dermatitis.

Circulating endothelin-1 levels in patients with bronchial asthma.

We attempted to clarify the relationship between circulating endothelin-1 levels and bronchial hyperreactivity in asthmatic patients. Sixty-two asthmatic patients were studied for plasma endothelin-1 levels during acute asthma. The relationship between plasma endothelin-1 levels and pulmonary function (FEV1/FVC) was examined in 17 patients. Twelve patients were studied for changes in plasma endothelin-1 levels during methacholine inhalation tests. Plasma endothelin-1 levels during acute asthma correlated with the severity of acute asthma defined with the Clinical Asthma Evaluation Score, and those during the methacholine challenge test correlated with the grade of bronchoconstriction (P < .01). A significant negative correlation was observed between plasma endothelin-1 levels and FEV1/FVC ratio (P < .01). These results suggest that circulating endothelin-1 levels may reflect the severity of acute asthma.

[Antitumor effect of UFT against differentiated thyroid cancer].

We have determined the levels of 5-FU, tegafur and uracil in the thyroid cancer and normal thyroid tissue in the patients with differentiated carcinoma who were administered UFT 600 mg/day p.o. preoperatively for six days. 5-FU and uracil levels in the thyroid cancer tissue were significantly higher than in normal thyroid tissue. However, tegafur level did not show significant differences in any tissues. Two cases with differentiated carcinoma, which resulted in PR after prolonged administration of UFT were presented. These findings suggest that oral administration of UFT for a long term is a useful treatment for advanced differentiated thyroid cancer.

Potential forensic applications of minisatellite variant repeat (MVR) mapping using the polymerase chain reaction (PCR) at D1S8.

Minisatellite variant repeat (MVR) mapping using the polymerase chain reaction (PCR) at D1S8 (MS32) was applied to samples from various human tissues. All DNA samples obtained from an individual's organs at autopsy consistently gave the same digital diploid codes. Even 1 ng of genomic DNA was sufficient to obtain authentic diploid MVR coding ladders. MVR-PCR could be reliably applied to DNA isolated from bloodstains, saliva stains, seminal stains and plucked hair roots, and should become a powerful tool for individual identification in forensic investigations.

[Comparative trial of granisetron versus granisetron plus methylprednisolone for the prevention of nausea and vomiting induced by cancer chemotherapy].

Preclinical and clinical studies have demonstrated the effectiveness of granisetron in preventing emesis induced by antineoplastic chemotherapy. This comparative study was undertaken to investigate the efficacy and safety of granisetron (40 micrograms/kg) and granisetron plus methylprednisolone (MPL; 10 mg/kg). Sixty-eight patients were given granisetron 170 times and thirty-nine patients were given a combination of granisetron and MPL 81 times. Sixty-one patients received the treatment in crossover fashion during the same chemotherapy regimens. The emetic and nausea episodes were counted during the 24 hours following each chemotherapy treatment. Complete response, no emesis or less than two episodes, were obtained in 75.3% (128/170 times) of patients receiving granisetron alone compared to 85.2% (69/81 times) of patients receiving the combination of granisetron plus MPL. There were no significant differences in complete responses between the two groups, male and female, and each age group. However, 11 of eighteen patients receiving granisetron alone who responded mildly, if at all, with respect to nausea and vomiting, showed a complete or major response when MPL was added to granisetron. There were two patients who developed temporal paresthesia of the both hands as an adverse effect, but there was spontaneous recovery after 3 hours. Our data suggested that granisetron plus MPL was slightly more effective than granisetron alone and a safe antiemetic drug.

Force-generating domain of myosin motor.

To understand the underlying mechanism of force generation by myosin motor, it is crucial to know which part of the molecule is essential for the process. Recent structure determination of myosin motor domain at atomic resolution has revealed that the domain comprises two smaller domains, the "ATPase domain" consisting of only an N-terminal segment of the heavy chain and the "neck domain" consisting of a long alpha-helix of the heavy chain and two light chains. This atomic structure begs the question of whether both domains are required for force generation. To answer it, we genetically truncated the head to generate a recombinant fragment composed of the "ATPase domain" alone. The truncated head drove sliding movement of actin filaments and generated force in a novel in vitro assay system, which allows us to hold a specific site of the head on a glass surface. These results indicate that the compact ATPase domain functions as a force-generating machinery of the myosin motor.
