[Determinants of nursing home admission of elderly patients and chances for prevention. A longitudinal study in Germany].

The aim of the following longitudinal study of institutionalization was 1) to ascertain the risk of institutionalization of an elderly person, and 2) to examine the factors related to institutionalization in old age. The probability of an old person to spend some portion of his life in any home for the aged is much higher than the 5% (Western Germany) or the 8% (Eastern Germany) which can be observed at any point in time. In the individual life-course up to 43% of men and up to 70% of women become institutionalized. The factors related to institutionalization are age, sickness, housing conditions, and marital status, whereas the gender difference is explained entirely by other factors. Furthermore, the probability to become institutionalized in old age is also a function of mortality.

[Social position and life expectancy. A cohort-specific analysis with data of socioeconomic factors].

The impact of social position on mortality has been explained by a variety of underlying causes, among which different living conditions and life-styles of social strata, different working conditions, and a selection process are the most important. This study relies on an indirect method of evaluating cohort mortality and life expectancy. The results confirm considerable impact of social status on life expectancy. Furthermore, living conditions as well as working conditions seem of relevance, whereas a simple selection process is falsified.

[Regional disparities in accessibility of inpatient treatment for elderly patients].

This article analyzes the supply of residential care facilities for old people in Germany with the aim to discover regional disparities. The analysis is based on the directory of residential institutions for old people (Altenheimadressbuch). On a national level the supply of residential care facilities in this data base is comparable with the results of the national statistic (Heimplatzstatistik). The results show considerable disparities that only to a minor extent coincide with the regional distribution of older people. Although there is no definite coherence between the supply of residential care facilities and the family care potential, there is a significant negative correlation between both.

[Residents of homes for the aged in Germany: characteristics of social structure and choice of nursing home].

In this paper some important aspects of institutionalization in old age are studied. Compared with old people living in private households, people living in residential care facilities show some sociostructural particularities which are hints at possible factors related to institutionalization. The following analyses of the factors related to institutionalization in old age result in a conformity of objective and subjective reasons: poor health, lack of social network and bad housing conditions in combination with needs for care are essential factors related to institutionalization. In addition, the question of denominational choice of the residential care facility and the question about migration in old age is examined more closely. With regard to the religious community it is amazing that, in spite of secularization, there are major interdenominational differences. A further analysis of regional mobility suggests some sociostructural differences: women and widowed persons for example do not move over far distances (from the private household to the residential care facility), compared to men and married persons.

Initial TPS serum level as an indicator of relapse and survival in colorectal cancer.

BACKGROUND: Tissue polypeptide specific antigen (TPS) measures a soluble fragment of cytokeratine 18 and may be regarded as a proliferative marker. MATERIALS AND METHODS: TPS was measured in 173 consecutive patients with colorectal cancer. Median follow up time was 36 months. Of 137 evaluable patients 39 developed metastases (P.D.) and 98 remained with no evidence of disease (N.E.D.). RESULTS: Initial TPS levels were elevated in 75% of P.D. patients compared to 32% of N.E.D. patients (p < 0.001), CEA levels were elevated in 26% of P.D. patients had elevated initial TPS compared to 35.5% of N.E.D. patients (p < 0.001), CEA was elevated in 33.3% of the P.D. patients compared to 1.3% of N.E.D. patients (p < 0.001). Survival and disease free survival were significantly shorter for patients with initial high TPS level. TPS was more sensitive than CEA in predicting relapse. CONCLUSIONS: These preliminary data suggest that TPS may be a prognostic factor for relapse and may help to allocate Dukes'B2 patients for adjuvant chemotherapy.

[Anticancerous chemotherapy trial with duborimycin. Analysis of 151 cases].

Duborimycin is a new antimitotic agent approaching danorubicin and adriamycin in activity which has been tried on 151 patients suffering from cancer of different types, is an advanced local/regional stage and/or metastatic disease. It was administered intravenously every fortnight in a mean unit dose of 400 mg, and the duration of the treatment ranged from 2 to 52 weeks. Objective improvement was registered in 56 patients of the 135 cases in whcih the results were assessed (around 41.4% of cases). In 4 cases the regression of tumour volume was greater than 50% (one of these cases was in melanoma, the other a sarcoma) and in 2 cases regression was complete (a squamous cell carcinoma and an embryonal testicular tumour). The subjective effects were appreciable in 53 of the 115 cases which could be studied (46%) and above all in the refractory pain of bony secondaries from breast cancer (a favourable response in 78% of cases). Manifestations of intolerance/toxicity were of a minor nature on the haematologic side, that cardiologic ones relatively frequent (18% of treated cases) and occasionally serious (2 cases of asystole). Great care is therefore necessary in supervision of the treatment. However, the first results obtained by this line of approach, notably in chemo-resistant forms of tumour such as melanoma and sarcomas, utilizing the very strict criteria in one analysis encourage further study of duborimycin in cases of this sort (preferably in association and in accordance with protocols of comparative trials) so that its place in cancer chemotherapy may be more precisely defined.

[Chemotherapy of soft tissues sarcomas. Summary of a decade and current trends].

The results of palliative chemotherapy applied to 123 patients with disseminated soft tissue sarcomas are reviewed with the recent literature data. With the exception of embryonal rhabdomyosarcomas of children, which are considered separately, the response of these tumors to chemotherapy is poor but not negligible. The association of several active drugs should permit substantial progress, especially in the initial stage of local treatment, when the number of disseminated cells is still reduced. A follow-up of controlled clinical studies must specify the modalities of multidisciplinary treatment in an effort to reduce regional recurrence or metastasis.
