[VEPA therapy in B-lymphoma].

48 previously untreated adult patients with B-lymphoma were treated with VEPA therapy. The complete remission rate was 79.2%. 48% of the patients were still alive after 4 years of the therapy. About 90% of the patients of stage II was expected to be cured by the therapy. VEPA therapy is greatly effective to B-lymphoma.

[Induced hypertension chemotherapy combined with radiotherapy in 44 advanced cancer patients].

This report presents the results of induced hypertension chemotherapy combined with radiotherapy for 44 advanced cancer patients (involving locoregional and/or remote recurrences). The lesions were 16 breast cancers, five soft tissue sarcomas, four lung cancers, four malignant lymphomas, three uterine cancers, two tongue cancers, and one parotid gland cancer, soft palate cancer, malignant thymoma, pancreatic cancer, vaginal cancer, urinary bladder cancer and rectal cancer each. The results, ive., eight complete remissions, 20 partial remissions and 16 cases of no change, were almost satisfactory.

Structural characterization of the cytoplasmic pole of band 3 from bovine erythrocyte membranes.

In an earlier study, we found that chymotryptic digestion of band 3 isolated from bovine erythrocyte membranes produces a 38,000-Da fragment in nonaethyleneglycol-n-dodecylether solution or a 50,000-Da fragment in deoxycholate solution as a primary fragment [Makino et al. (1984) J. Biochem. 95, 1019]. In the present study, these fragments were purified in an aqueous medium without detergent and their structural properties were examined. Several lines of evidence showed that the 50,000-Da fragment constitutes the entire cytoplasmic pole of bovine band 3 and that the 38,000-Da fragment is a subfragment of the 50,000-Da fragment. The large fragment was suggested to be divided into two distinct regions, the 12,000- and 38,000-Da portions, differing in their conformational thermal stability. However, attempts to identify the 12,000-Da portion as an isolable segment were without success. The cytoplasmic pole was characterized as a dimer which adopts an elongated gross conformation with helix of approximately 35%. Treatment of the fragments with dimethylmaleic anhydride dissociated the dimers into the monomers, accompanied by a significant conformational change of the 38,000-Da portion. Comparative studies suggested that the cytoplasmic domain of bovine band 3 has structurally different region(s) from that of human band 3, though their gross conformation shows extensive similarity.

Purification and some properties of a low molecular weight trypsin inhibitor from acute pancreatitis urine.

Urinary trypsin inhibitors (UTIs) from the urine of a patient with acute pancreatitis consisted of three forms with different molecular weights. These were highly purified by ammonium sulfate precipitation, Sephadex G-75, SP-Sephadex C-25 and trypsin-Sepharose 4B column chromatography. The lowest molecular weight of UTIs was estimated to be 6,200 daltons. Moreover, five residues of N-terminal amino acids and a C-terminal amino acid were the same as those of pancreatic secretory trypsin inhibitor.

Second primary malignancies in lymphoma patients.

We evaluated the occurrence and the type of second malignancies among 74 patients with Hodgkin's disease (HD) and 407 patients with non-Hodgkin's lymphoma (NHL) who were treated at the National Cancer Center Hospital for more than one year. Fifteen patients developed a second malignancy. In 10 of these patients the second cancer was gastric cancer, but no cases of acute nonlymphocytic leukemia were encountered. The observed number of second cancers in females among the HD patients was significantly (p less than 0.005) greater than the expected incidence based upon the number of age-adjusted person-years both for all cancers and for stomach cancer. However, no significant differences between males and females in the NHL patients were found. Furthermore, no significant differences were seen in any of the groups between the observed and expected numbers of second malignancies according to the treatment.

Clinicopathologic investigation of ovarian endometriosis: a comparative study of cystic and non-cystic types.

This clinicopathologic study involved 129 patients with ovarian endometriosis. All the lesions from the patients were classified macroscopically into two types being either cystic or non-cystic. The results are as follows. Endometriotic lesions were bilateral in 58 cases and unilateral in 71 cases. The average age of the patients was 38.8 and gravidity 1.8, with 43.1% of them being infertilite. In 12.2, 45.4 and 84.6% of them, irregular menstruation, hypermenorrhea, and dysmenorrhea were recorded, respectively. Regarding complicated lesions, 49 patients had fibroid, 33 uterine adenomyosis, and 13 rectocervical endometriosis. Of the 129 cases, 61.3% were regarded as of the cystic type, 30.2% as of the non-cystic type, and 8.5% as of the cystic/non-cystic type. The average ages were 35.5 and 41.6 years for cystic and non-cystic types, respectively. The incidence of each type of complicated lesion mentioned above was greater in the non-cystic type than that in the cystic type. The pathogenetic investigation suggested that cystic endometriosis in the ovary might mainly be ascribed to the endometrioid metaplasia of the celomic epithelium and that non-cystic endometriosis might occasionally originate from metastasis or implantation of endometrial tissues. In addition to this, the lesions which were histologically diagnosed as endometriosis might sometimes include obsolete follicle cysts undergoing luteinization.

[The significance of reinduction chemotherapy with adriamycin for relapsed non-Hodgkin's lymphoma of Waldeyer's ring].

Of those patients with localized (CS I-II) non-Hodgkin's lymphoma of Waldeyer's ring who were treated with radiotherapy or radiotherapy plus chemotherapy and who suffered relapses at the National Cancer Center Hospital over the past 22 years, 24 cases were analyzed on the basis of their response rate and duration of remission with reinduction chemotherapy, and survival time after recurrence. The group [ADM (+)], treated with regimens including adriamycin, was compared with the group [ADM (-)] treated with regimens excluding adriamycin. Complete response was seen in 9 out of 11 cases (81.8%) and in 8 out of 13 cases (61.5%) for the ADM (+) group and the ADM (-) group, respectively (p greater than 0.1). The period of complete response of the ADM (+) and ADM (-) groups was compared using the logrank method. The prognosis of the former was significantly (p less than 0.01) better than that of the latter and the median duration of remission was 30 months and 7 months, respectively. When the survival after recurrence of the ADM (+) and ADM (-) groups was compared, the median survival time was 38 months for the ADM (+) group and 12 months for the ADM (-) group, but no significant difference was observed between the two groups (0.05 less than p less than 0.1).

Prognostic factors and therapeutic results in multiple myeloma.

Prognostic factors in 68 consecutive patients with myeloma treated at the National Cancer Center Hospital from 1962 to 1984 were analyzed. Median survival time from onset was 100 months for stage I, 72 months for stage II, and 26 months for stage III of the Durie and Salmon's clinical staging system. It was 55 months in patients with normal renal function and 18 months in those with abnormal renal function. All early deaths occurred in patients with stage III disease. Hemoglobin level, bone lytic lesions and presence of Bence Jones protein were also significant prognostic factors. On the other hand, heavy chain as well as light chain subtypes of monoclonal immunoglobulin (M-component) and M-component production rate did not influence the survival of myeloma patients. The analysis of chemotherapeutic responses and survival curves according to the chemotherapy used in this study (alkylating agent vs Vinca-alkaloid plus alkylating agent) did not disclose any significant difference between the two groups. The overall response rate was 67%. The survival time from the initial chemotherapy of responding patients was significantly longer than that of nonresponders.

Computed tomography in congenital ocular motor apraxia.

Unusual computed tomographic findings were observed in four patients with congenital ocular motor apraxia (COMA). These (CT) findings were characteristic in the posterior cranial fossa, with a dilated or deformed shape and size of the fourth ventricle, particularly its upper portion. One case revealed partial agenesis of the cerebellar vermis. It was suggested that the four patients subjected to CT showed abnormal topography of the cerebellar vermis or the brain stem. We postulate that these CT findings in COMA may have an important role in its pathogenesis.

Proteolytic digestion of band 3 from bovine erythrocyte membranes in membrane-bound and solubilized states.

Bovine band 3 in membrane-bound and solubilized states was digested with chymotrypsin, trypsin, and papain. Bovine band 3 in red blood cells was fragmented by the proteases in a 5 mM NaH2PO4-Na2HPO4 buffer containing 0.3 M glucose, pH 8.0, but not in a 5 mM NaH2PO4-Na2HPO4 buffer containing 0.15 M NaCl, pH 8.0, in which human band 3 is cleaved by chymotrypsin and papain. When compared with the known data for human band 3, however, major fragments of bovine band 3 derived from intact cells, inside-out vesicles and unsealed ghosts were similar to those of human band 3, except that tryptic fragments were formed on the extracellular attack. The results suggest that bovine band 3 adopts a quite similar molecular arrangement in the membrane to in the human case. However, it was strongly suggested by molecular weight evaluation of fragments that the only detectable water-soluble 38,000-39,000 dalton fragment does not account for the entire hydrophilic pole of the band 3 molecule exposed in the cytoplasmic region of the membrane. When isolated band 3 was treated with the enzymes in a 2% solution of nonaethyleneglycol n-dodecyl ether, the major product was indistinguishable on sodium dodecyl sulfate-gel from the water-soluble fragment of the cytoplasmic domain origin of band 3. This fragment lost its resistance to further enzymatic degradation when treated with dimethylmaleic anhydride, thus band 3 oligomers were converted into their monomers. The chymotryptic 38,000 dalton water-soluble fragment obtained in nonaethyleneglycol n-dodecyl ether solution was a subfragment of a 50,000 dalton piece which was produced in a 2% solution of deoxycholate after chymotrypsin treatment of band 3.

Hepatic gamma-glutamyl transferase and hepatic fibrosis in patients with alcoholic liver disease.

The activity of gamma-glutamyl transferase (GGT) in the liver of 22 patients with alcoholic liver disease, 24 with nonalcoholic liver disease and 12 normal controls was measured. The enhanced hepatic GGT activity was higher in alcoholic patients and nonalcoholic patients than in the normal controls. No statistical significance of difference could be found in hepatic GGT activity between alcoholic and nonalcoholic patients, while serum GGT activity was markedly elevated in alcoholic patients compared to nonalcoholic patients. There was better correlation between hepatic and serum GGT activity in alcoholic patients. Furthermore the enhanced hepatic GGT activity could contribute to the pathogenesis of liver fibrosis as a result of the investigation for hepatic GGT activity and histological liver fibrosis.
