Serum acute phase reactants in pediatric patients; especially in neonates.

We studied serum acute phase reactant (APR) levels in 45 pediatric surgical patients. Alpha 1-acid glycoprotein (alpha 1-AG) showed a peak value on day 3 postoperatively (P.O.) and alpha 1-antitrypsin (alpha 1-AT) showed a high value on days 3-5 P.O. These glycoproteins returned to normal levels by day 21 P.O., but the level of haptoglobin (Hp) remained high until day 21 P.O. The postoperative changes of alpha 1-AG and alpha 1-AT correlated with the process of recovery from inflammatory conditions, but C-reactive protein (CRP) reached a peak on days 1-2 P.O. and returned to normal limits by day 14 P.O. In patients with infection, CRP returned to the normal level rapidly before recovery from infection. Of the 3 glycoproteins, alpha 1-AG seemed to be a valuable indicator of the pathological conditions. Postoperative changes in APR levels should be useful for early detection of postoperative complications. Persistent ileus led to an increase in APR levels, as a consequence of an inflammatory reaction due to breakdown of the intestinal mucosal barrier.

Do we need ventriculography in the era of computed tomography?

The question is posed whether in the investigation of intracranial tumors, particularly in the posterior cranial fossa, CT and/or ventriculography should be practiced. We investigated 134 patients (93 were children up to the age of 14 years), all of whom had had both computed tomography and ventriculography. The results clearly demonstrate the superiority of CT compared with ventriculography. Ventriculography is a surgical intervention with risk for the patient; side effects may occur and serious complications can sometimes arise. Modern CT is producing pictures of high quality which are entirely sufficient for neurosurgical intervention. Very rarely does additional angiography have to be performed. The diagnosis of intracranial tumor can be fully established by CT and ventriculography is no longer necessary.

Intracranial venous angiomas.

Publications in the scientific literature are controversial in regard to the clinical significance of intracerebral venous angiomas. The present study of 11 patients with venous angiomas underscores the clinical importance of these lesions as potential causes of cerebral hemorrhage and obstructive hydrocephalus. The clinical and radiographic manifestations in 9 of these 11 patients correlated well. In cases of venous angiomas, the venous phase of angiography must be carefully scrutinized, particularly in patients with subarachnoid or intracerebral hemorrhage. Some diagnostic problems related to the angiography of venous angiomas are discussed.

Effects of ritodrine, a beta 2-adrenoceptor agonist, on smooth muscle cells of the myometrium of pregnant rats.

1 Effects of ritodrine on the electrical and mechanical activities of the myometrium of pregnant rats were investigated in relation to cyclic adenosine 3',5'-monophosphate (cyclic AMP) production and the actions of isoprenaline. 2 In the longitudinal muscle cells of the 22nd day of gestation, ritodrine (greater than 10(-8) M) hyperpolarized the membrane, reduced the membrane resistance and suppressed the amplitude of contraction. Increased concentrations of ritodrine (greater than 10(-7) M) suppressed the generation of spikes and increased the amount of cyclic AMP produced. These actions of ritodrine were much the same as those of isoprenaline but the dose-response curves of all parameters were shifted to the right. 3 The effects of ritodrine on the circular muscle cells at the 22nd day of gestation were similar to those observed in the longitudinal muscle cells. 4 In the circular muscle cells at the 17-19th days of gestation, ritodrine produced neither a hyperpolarization nor an increase in cyclic AMP production (up to 10(-5) M) but did reduce the plateau potential and did relax the tissue (greater than 10(-7) M). 5 In the longitudinal muscle cells, during the 17-19th days of gestation, the responses of the tissues were similar to those observed in circular muscles on the 22nd day of gestation. These actions of ritodrine were suppressed by propranolol (10(-6) M). 6 These results indicate that ritodrine is a beta-adrenoceptor agonist and its mechanism of action similar to that of isoprenaline. However, the potency of ritodrine was approximately 100 times less than that of isoprenaline.

Involvement of calcium in calcitonin induced stimulation of glycolysis in rat kidney in situ.

The effects of intravenous salmon calcitonin on tissue carbohydrate and redox metabolism were studied in rat kidney in situ and were compared to the effects of EGTA infusion. Calcitonin (0.8 MRC U, in bous) caused, after 10-20 min, a reductive shift of the cytosolic NAD redox pair, and an oxidative shift of the mitochondrial NAD redox pair. It also caused a stimulation of glycolysis with the cross-over between phosphoenolpyruvate and pyruvate. These results were reproduced by treatments of hypocalcemia with EGTA or with thyroparathyroidectomy. Most of the effects were opposite in direction to the reported effects of CaCl2 and parathyroid hormone. The above results suggest that in kidney calcitonin causes a stimulation of pyruvate kinase, and/or inhibition of phosphoenolpyruvate carboxykinase, and that the effects are related to changes in the intracellular calcium.

[Computed tomography and/or ventriculography? (author's transl)].

It is discussed if in intracranial tumors, especially in tumors of the posterior cranial fossa, a CT and/or a ventriculography should be practiced. We have made investigations of 134 patients, 93 of whom were children up to 14 years of age. Each case was undertaken computed tomography as well as ventriculography. The results are clearly demonstrating the superiority of computed tomography compared with ventriculography. Ventriculography is a surgical intervention stressing the patients, side-effects may occur, and sometimes serious complications are caused. Modern computed tomography is producing pictures of high quality, which are highly sufficient for neurosurgical intervention. Very rarely additional angiography has to be performed. The diagnosis of intracranial tumors can fully be established by computed tomography, whereas ventriculography is no longer necessary.

Neuroradiological manifestations of suprasellar pituitary adenomas, meningiomas and craniopharyngiomas.

The radiological manifestations of 16 pituitary adenomas, six meningiomas and 14 craniopharyngiomas were reviewed. The differential diagnosis of these tumors is possible with a few exceptions when skull radiography, CT, angiography and metrizamide cisternography are carefully evaluated. Prolonged injection angiography (PIA) is particularly important for demonstrating the contours of pituitary adenomas and meningiomas. Metrizamide cisternography is indicated when the tumor contours are obscure using PIA, or when the differential diagnosis is uncertain.

Primary intracranial sarcomas: radiological diagnosis with emphasis on arteriography.

The radiological manifestations of primary intracranial sarcomas may be non-specific and they vary widely according to their histological variations. Reticulum cell sarcomas, however, can be included in the differential diagnosis with a high degree of reliability when one observes a hypovascular mass with arterial encasement and deep medullary veins. Tumor vessels and tumor stains supplied by dilated feeding arteries suggest the possibility of a sarcoma of the brain or of the meninges in infants. Nine reticulum cell sarcomas, four undifferentiated sarcomas, and one fibrosarcoma are presented here with their radiological manifestations, especially on angiography.

Bilateral congenital multicystic dysplasia of the kidney.

The clinical and pathological features of congenital multicystic dysplasia of the kidney are reviewed and we report of a patient with bilateral dysplasia who lived longer than do most such patients. When multicystic disease of the kidney is bilateral, it is commonly confused with polycystic disease. Despite the fatal prognosis, differentiation is essential for appropriate genetic counselling since multicystic disease is not a familial occurrence.

Anomalous unilateral single pulmonary vein mimicking pulmonary varices.

Anomalous single pulmonary veins (AUSPV) is a rare anomaly of the pulmonary venous system. Six cases have been documented in the literature, and two additional are reported here. Since AUSPV mimics pulmonary varices on pulmonary angiography, careful interpretation of the pulmonary venous anatomy by bidirectional and stereoscopic angiography is mandatory for diagnosing this anomaly.

Electrical and mechanical activities recorded from smooth muscle cells of the human fallopian tube.

The membrane activity recorded from the circular muscle layer of the human Fallopian tube showed a unique pattern (slow potential changes), yet the hormonal and adrenergic influences appeared to be much the same as observed in he myometrium. Differences in electrical and mechanical activities, and also the sensitivity to catecholamines during the secretory or proliferative period were elucidated.

Alterations in the properties of the rat myometrium during gestation and post partum.

The relationships among progesterone and oestradiol contents in the sera, membrane activities, catecholamine actions and contents of cyclic nucleotides were investigated in the rat myometrium during gestation and post partum, and under conditions of hormone treatment. Progesterone contents declined and oestradiol contents increased in the sera on the 21st day of gestation. During the late stage of gestation, oestradiol related to regularity, and progesterone to an irregularity, in the spike generation of myometrial cells. In the longitudinal muscle, catecholamines dominantly activated the beta-adrenoceptors during gestation, but activation of the alpha-adrenoceptors was seen during and after delivery, while in the circular muscle, alpha-adrenoceptor dominancy altered to beta-adrenoceptors by application of catecholamines appeared simultaneously. At the late stage of gestation, oestradiol treatment rather than progesterone increased the cyclic AMP contents, in both muscle layers. In the light of the results, alteration in the myometrial properties during the last stage of gestation and delivery cannot be explained solely by progesterone withdrawal phenomena.

[Three cases of cystic neurinoma arising from the upper cervical nerve roots and extending into the posterior cranial fossa (author's transl)].

Three cases of cystic neurinoma arising from the upper cervical spinal nerve roots and extending to the posterior cranial fossa through the foramen magnum were reported. Case 1 was a 52-year-old female presenting marked bilateral papilledema, hyperactive right knee jerk, left positive Gordon reflex and trunkal ataxia. The mass was mostly cystic and arose from the left 1st cervical nerve root. It extended to the posterior fossa, occupied the cerebello-medullary cistern and displaced the left cerebellar hemisphere posteriorly and superiorly. Case 2 was a 39-year-old male presenting marked bilateral papilledema, neck stiffness, dysfunction of the right 7, 9, 10 and 11th cranial nerves, gaze nystagmus, Horner's syndrome, right limb ataxia, and ataxic gait. The mass was totally cystic containing yellowish fluid and was growing from the right 2nd cervical spinal nerve root. The mass extended to the posterior cranial fossa up to the right cerebello-pontine angle and compressed the medulla oblongata, upper cervical cord and 7th through 11th cranial nerves on the right side. Case 3 was a 66-year-old male presenting marked bilateral papilledema, gaze nystagmus, left hemiparesis, bilateral hyperactive deep tendon reflexes, numbness of the left fingers and ataxic gait. CT scanning revealed in the midline of the posterior cranial fossa a low density mass which was enhanced in a ring-like fashion. Vertebral angiograms showed an avascular mass displacing the PICAs upwards and elongating its cranial loops antero-posteriorly. The mass was totally cystic, arose from the left 2nd cervical spinal nerve root, extended to the posterior cranial fossa and occupied the cisterna magna and vallecula. Foramen magnum syndrome was discussed and the symptoms and signs presented in these three cases were compared with those presented by foramen magnum meningioma in other reports.
