Preoperative diagnosis of fallopian tube cancer by imaging.

Primary cancer of the fallopian tube (FTC) is among the most unusual gynecologic malignancies and rarely is diagnosed correctly before surgery. The imaging results of eight patients with FTC and four with benign tubal disease were analyzed. FTCs were small cystic or solid masses that typically were shaped like a sausage, a snail, or a gourd, regardless of clinical stage.

Bleeding patterns in ruptured posterior fossa aneurysms: a CT study.

Computed tomography of the brains in 20 patients with acute rupture of posterior fossa aneurysms was reviewed and analyzed retrospectively. Findings were compared with those from 44 cases described in the literature and with the findings in ruptured supratentorial aneurysms. Extravasated blood was observed in 19 of 20 patients (95%); intraventricular hemorrhage (IVH) in 17 of 20 (85%); and subarachnoid hemorrhage (SAH) in 13 of 20 (65%). These values were significantly higher than those previously reported and suggest that, during the acute phase of rupture, extravasated blood may be detected with the same frequency in either infratentorial or supratentorial ruptured aneurysms. Subarachnoid hemorrhage was accompanied by IVH, prominent in the fourth ventricle and without intraparenchymal hematoma, in 11 patients (55%). This pattern is highly suggestive of ruptured posterior fossa aneurysms. Intraventricular hemorrhage without SAH was noted in five patients (25%) and specifically represented ruptured posterior inferior cerebellar artery aneurysms. Subarachnoid hemorrhage without IVH was noted in only two patients (10%).

Dysembryoplastic neuroepithelial tumors: MR findings.

OBJECTIVE: We report MR findings in three patients with dysembryoplastic neuroepithelial tumor (DNT), which is a recently described benign tumor in childhood or young adolescents. MATERIALS AND METHODS: The MR studies of three patients with surgically proven DNT were reviewed. All cases presented with partial complex seizures. Axial and coronal T1- and T2-weighted MR imaging of the head was performed using a 1.5 T MR unit. T1-weighted imaging after Gd-DTPA was also performed. RESULTS: The tumors were located mainly in the cortical to subcortical area with very little perifocal mass effects on MR imaging. They were essentially hypointense on T1-weighted imaging and hyperintense well-demarcated mass on T2-weighted imaging and retained a thick gyrus-like configuration within the lesions. None of them exhibited contrast enhancement after administration of Gd-DTPA. CONCLUSION: Differentiation of DNT from gangliogliomas or other low grade gliomas is possible using these MR features and is important because DNT does not recur after epilepsy surgery, therefore postoperative radiation and chemotherapy are not needed.

Digital subtraction angiography for leg venography.

Venography was performed in 25 legs of 17 patients using digital subtraction angiography (DSA). Reliable diagnoses of deep vein thrombosis or patency were made in the majority of these cases. There were no complications. Multiple injections of contrast material were required to image the entire leg; however, the total quantity of iodine necessary for each leg averaged 9 g, significantly less than with conventional techniques. The preferred method consisted of injecting 15 ml of 100 mg iodine/ml contrast material followed by 15 ml of heparinized saline. DSA proved a safe and useful means of evaluating veins of the leg.

Pyelovenous backflow in left renal vein hypertension: case report.

Retrograde pyelography for a patient with left renal vein hypertension due to "nutcracker phenomenon" revealed pyelovenous backflow. This is considered to be the first actual demonstration of renal bleeding during the nutcracker phenomenon.

Cranial computed tomography in disorders of complex carbohydrate metabolism and related storage diseases.

Computed tomography (CT) was performed on 34 children with different disorders of complex carbohydrate metabolism and related storage diseases to obtain data on the degree of cerebral involvement. The main findings on CT were cerebral atrophy and hypodensity of the white matter. There was a great variability in these CT findings, even in siblings. Among the patients there were several in whom CT was normal, so a negative study does not exclude one of these disorders. These findings show that CT features such as cerebral atrophy or hypodensity are helpful in the evaluation of these disorders, though a diagnosis cannot be made on the basis of CT alone.
