[Two pulmonary tuberculosis cases with HIV infection].

UNLABELLED: It was reported that HIV-infected persons were at much higher risk to develop active tuberculosis than HIV-none-infected persons, about 10% of whom might develop active tuberculosis through their lives, almost identical percentage of HIV-infected persons had developed active tuberculosis annually. In Japan, 2838 HIV-infected persons including 621 AIDS-cases were reported by the end of June 1993. Oct. Ten HIV-infected cases of active mycobacteriosis have been reported in literatures or on scientific meeting. We experienced two tuberculosis cases with HIV-infection recently and will report herein. FIRST CASE: A 23 years-old male student of Japanese-language school from Myammer. He was admitted to our hospital because of high fever and cough. His chest X-ray film taken on admission showed left hilar and mediastinal lymph nodes swelling, calcification of left hilar lymph node and infiltration in middle lung field. Sputum smear for acid fast bacilli was strongly positive. The cultured isolates were identified as Mycobacterium tuberculosis by DNA probe methods and were susceptibility tests were sensitive to all antituberculous drugs. Tuberculin skin reaction was negative. Laboratory data on admission: serum albumin level was 2.7 g/dl, A/G ratio was 0.75, CRP was 26.4 dg/ml, HBe antigen and antibody were positive, HIV antibody was positive by PA method and Western blot method, total lymphocyte count was 410/microliters, total T lymphocyte count was 303/microliters, total B lymphocyte count was 29/microliters, CD4+ T lymphocyte count was 37/microliters, CD8+ T lymphocyte count was 279/microliters, CD4+/CD8+ ratio was 0.1.(ABSTRACT TRUNCATED AT 250 WORDS)

Malignant exophthalmos associated with multiple myeloma.

We report a patient with malignant exophthalmos associated with multiple myeloma which showed no evidence of direct orbital involvement of plasma cells. This exophthalmos had similarities with Graves' ophthalmopathy, but the patient had no detectable autoimmune thyroid diseases. Plasmapheresis was effective not only for the treatment of heart and renal failure due to the myeloma kidney but also for the malignant exophthalmos. As the serum monoclonal IgG level was decreased by plasmapheresis, the improvement of proptosis, visual acuity, and hypertrophy of the extraocular muscle as measured by magnetic resonance imaging were observed. It is suggested that humoral factors removed by plasmapheresis might be involved in the pathogenesis of this nonendocrine exophthalmos.

[Allergic bronchopulmonary aspergillosis (allergic bronchopulmonary fungal diseases)].

Allergic bronchopulmonary aspergillosis (ABPA) is an allergic disease caused by viable Aspergillus in a relatively large bronchus and by the type I and type III allergies against the fungus. The clinical findings are characterized by recurrent pyrexial attacks (fever, cough and mucopurulent sputum containing mucous plugs, numerous eosinophils and the fungus), radiological evidence of recurrent collapse and consolidation in different areas of the lung, a blood eosinophilia and elevated serum IgE levels. Fungi other than Aspergillus may cause similar allergological diseases. Therefore, they should be call allergic bronchopulmonary fungal diseases (ABPF). Many diagnostic criteria of the disease have been proposed by many different authors, but, a return should be made to the original report by Hinson et al, 1952.

[A basic study on the plasma and serum levels of eosinophil cationic protein (ECP) in bronchial asthma].

A basic study on the blood eosinophil cationic protein (ECP) as an index for eosinophil activation in bronchial asthma was performed. 1) ECP concentration in the serum collected 2 hours after blood sampling and the peripheral eosinophil count were significantly higher in asthmatic patients than in normal individuals. A correlation between these two parameters was observed in normal controls but not in asthmatic patients. These findings suggest that the degree of eosinophil activation, which is reflected by the blood ECP concentration, varies with the clinical condition in asthmatic patients. 2) The plasma and serum ECP levels were compared. The serum ECP was higher than the plasma ECP in both normal controls and asthmatic patients. In asthmatic patients, however, plasma ECP levels poorly correlated with serum levels: the serum levels were markedly elevated in some cases, and only the plasma levels were significantly elevated during symptomatic periods compared with stable periods. Therefore, attention must be paid not only to the serum ECP concentration, but also to the plasma ECP concentration and the plasma-serum difference. 3) This plasma-serum difference in ECP concentration may be due to the in vitro release of ECP during coagulation after blood sampling rather than the effect of alpha 1-macroglobulin concentration, because supernatants from stimulated platelets elicited ECP release from eosinophils.

Sterol carrier protein2 (SCP2)-like protein in rat aorta.

Immunoblot analysis using affinity-purified antibody against sterol carrier protein2 (SCP2) showed that SCP2-like protein exists in 105,000 x g supernatant of rat aorta. Analysis of subcellular distribution of SCP2-like protein in rat aorta was determined with enzyme immunoassay (EIA). The highest level of SCP2-like protein was observed in cytosolic fraction, while the lowest level was in nuclear fraction. Analysis of marker enzymes in subcellular fractions showed that catalase, a marker enzyme of peroxisomes, leaked to cytosolic fraction to a significant extent during subcellular preparation, suggesting that SCP2-like protein in cytosolic fraction of rat aorta might be partially originated from peroxisomes. In vitro addition of homogenous SCP2 purified from rat liver dose-dependently stimulated the formation of [14C]cholesteryl esters from exogenously added [14C]cholesterol by acyl-CoA:cholesterol acyltransferase (ACAT) in microsomal preparation of rat aorta. However the addition of cytosolic fraction did not enhance cholesterol esterification by ACAT, most likely due to a markedly low level of SCP2-like protein in this fraction. The role of SCP2 in the formation of cholesteryl esters by ACAT in rat aorta was discussed.

Effect of ozagrel (OKY-046), a thromboxane synthetase inhibitor, on theophylline pharmacokinetics in asthmatic patients.

The effect of ozagrel (OKY-046), a selective thromboxane A2 synthetase inhibitor, on theophylline disposition was studied in 12 asthmatic patients. Ozagrel was administered at a dose of 200 mg twice daily for 24 weeks to 4 outpatients receiving oral theophylline medication. Blood samples were drawn from each patient visiting the hospital at one to four-week intervals. There were no clinical significant changes between the time course profiles of serum theophylline concentration during the treatment and those after the cessation of ozagrel dosing. In addition, another 8 hospitalized patients received a single intravenous infusion of aminophylline (200 mg as theophylline) before and after ozagrel treatment of 200 mg twice daily for 7 days. No statistical significant alteration in elimination half-life, total body clearance or volume of distribution of theophylline was observed by the administration of ozagrel. These results suggest that ozagrel does not inhibit the metabolism of theophylline despite having an imidazole ring in its chemical structure.

Immuno-affinity purification of specific antibodies against human gastrin releasing peptide (h-GRP) by the h-GRP(1-8)-linked polydimethylacrylamide resin.

A new method for immuno-affinity purification of specific antibodies against human gastrin releasing peptide(h-GRP) was developed. The antiserum GP(No. 6201) elicited by h-GRP-BSA conjugate was heterogeneous and reacted not only with h-GRP and its fragments but also partially with other structurally related peptides, such as other GRPs (porcine, canine, and chicken), bombesin, and neuromedin-C. To obtain specific antibodies against human GRP, antiserum GP was purified by column chromatography on the amino-terminal octapeptide h-GRP(1-8)-linked polydimethylacrylamide resin. The antibody thus obtained was highly specific to amino-terminal sequence of h-GRP and hardly reacted with other GRPs (porcine, canine and chicken), bombesin, and even carboxy-terminal h-GRP fragments in ELISA.

[An epidemiologic study on patients with lung disease, using a data base management system and the histopathologic diagnosis].

A new pathologic data base management system was introduced in the Section of Pathology, Clinical Laboratory, of this hospital for an efficient sorting and searching past data and for the rapid epidemiological study of them. By the use of this system, the incidence and the age and sex distribution of various diseases were studied in a total of 1,264 patients received pathological examination for surgically operated specimens or biopsied materials at the Chest Disease Research Institute Hospital, Kyoto University, from January, 1986 to December, 1988. A gradual increase in the number of frozen section samples of surgically removed organs and of open lung biopsies was noted during this period. Adenocarcinoma, squamous cell carcinoma, small cell carcinoma and large cell carcinoma amounted to 42.5%, 32.9%, 12.2%, and 7.5% of primary lung cancers, respectively. The male-to-female ratio of patients with primary lung cancers was 2.9:1 but there was no significant difference in the age distribution among four types of the primary cancers in both sexes. Two hundred and ninety nine (32.6%) patients were diagnosed as non-neoplastic lung diseases: 121 infectious (13.2%), 69 granulomatous (7.5%), 62 pulmonary fibrotic (6.8%) and 47 miscellaneous non-neoplastic (5.1%) diseases.

[Luminol-dependent chemiluminescence of peripheral neutrophils in asthmatic patients].

To evaluate the role of the neutrophil in the pathophysiology of bronchial asthma, we examined the luminol-dependent chemiluminescence of peripheral neutrophils (CL) in steroid-dependent asthmatic patients during an acute attack (n = 18), during stable period (n = 10), and in normal controls (n = 21). When stimulated with formyl-methionyl-leucyl-phenylalanine (fMLP 0.1 microM), the CL of patients in acute attack was lower than that of normal controls. (5.29 +/- 1.25 vs. 9.29 +/- 0.84 relative light units p less than 0.01) Among 18 patients, we could examine 10 patients during the stable period after receiving steroids. The CL of those 10 patients during the stable period was significantly higher than their CL during acute attacks, when evaluated by the paired t-test (p less than 0.05), and was not very different than in normal controls. When stimulated with opsonized zymosan (5 mg/ml), CL was similar in the three groups. We also examined the CL of peripheral neutrophils of normal persons after the preincubation with serum of patients during acute attack and stable period. When stimulated with fMLP, CL after preincubation with serum of patients during acute attack was lower than CL after preincubation with the serum of patients during the stable period. It was suggested that the respiratory-burst activity of peripheral neutrophils in acute asthmatic attack was reduced to the chemotactic stimulus of fMLP.

Prolongation of skin allograft survival in rats by a novel immunosuppressive agent, FK506.

FK506, an immunosuppresant, was isolated from Streptomyces tsukubaensis. Intramuscular administration of FK506 (0.32 mg/kg or more) 5 days a week for two weeks after grafting prolonged the acceptance time of F344 skin allograft to WKA rats. Similar results were obtained with cyclosporine at 32 mg/kg or more, but other immunosuppressives (i.e., prednisolone, azathioprine, and bredinin) gave only a marginal prolongation. The prolonging effect of FK506 was obtained in various donor-recipient combinations across a major or minor histocompatibility barrier. The agent also prolonged the acceptance time of mouse skin xenografts to rats. Furthermore, maintenance doses of 3.2 or 0.32 mg/kg twice a week after an initial 14-day treatment with the agent at 3.2 mg/kg gave graft survival as long as the treatment was continued for more than 120 days. Our findings show that FK506 has a potent immunosuppressive effect in rats and suggest that the agent merits further investigation.

FR-900520 and FR-900523, novel immunosuppressants isolated from a Streptomyces. I. Taxonomy of the producing strain.

A new subspecies of the genus Streptomyces, the proposed name of which is Streptomyces hygroscopicus subsp. yakushimaensis subsp. nov., is described. Soil isolate, strain No. 7238, produces the novel immunosuppressants, FR-900520 and FR-900523. The organism is characterized by its gray aerial mycelium color, hygroscopic spore mass and spiral spore chains with warty or spiny spore surfaces. It is nonchromogenic. Strain No. 7238 shows characteristics most closely related to Streptomyces antimycoticus and S. hygroscopicus, although there are differences in physiological characteristics and carbohydrate utilization. In terms of morphological characteristics, strain No. 7238 is different from S. antimycoticus, but resembles S. hygroscopicus. The differences are not sufficient to establish a new species. It would be most suitable to designate strain No. 7238 as a new subspecies within the species of S. hygroscopicus.

FR-900520 and FR-900523, novel immunosuppressants isolated from a Streptomyces. II. Fermentation, isolation and physico-chemical and biological characteristics.

FR-900520 and FR-900523, novel neutral macrolide immunosuppressants, were isolated from the cultured broth of Streptomyces hygroscopicus subsp. yakushimaensis No. 7238. Their molecular formulae were determined as C43H69NO12 and C42H67NO12, respectively. The compounds suppressed immune response in vitro. IC50 values of FR-900520 and FR-900523 for mouse mixed lymphocyte reaction were 0.55 nM and 1.6 nM, respectively. FR-900520, the major component, clearly prolonged skin allograft survival in rats.
