Fine structures of utricle of developing chick embryo exposed to 2G gravity.

Utricular otoconia of the chick embryos incubated under continuous 2G load were examined under a scanning electron microscope. Morphological changes were confined to the process of otoconial formation and the otoconial size. Otoconial formation was remarkably delayed in the utricle of 2G centrifuged chick embryo. Giant otoconia, which sometimes amounted to over 100 microM in length, existed along the marginal zone of the utricular otolithic membrane. These results indicate that the 2G state influences the otoconical formation.

[Pharmacokinetic and clinical studies on SY5555 dry syrup in children].

SY5555 is a new oral penem antibiotic. Pharmacokinetic and clinical studies using SY5555 dry syrup (powder which is dissolved before use) were performed in pediatric patients. 1. Pharmacokinetic investigation Peak plasma concentrations of SY5555 after dose of 5 mg/kg, 10 mg/kg and 15 mg/kg were, respectively, 1.58 +/- 0.37 micrograms/ml, 2.78 +/- 0.54 micrograms/ml and 5.28 micrograms/ml at 1 hour. The average half-life with 5 mg/kg administration was 0.94 +/- 0.05 hours, that with 10 mg/kg was 1.46 +/- 0.31 hours and that with 15 mg/kg was 0.88 hours. 2. Clinical investigation Enrolled in the study were 15 patients including 5 with acute otitis media, 5 with urinary tract infections and 1 each with pharyngitis, tonsillitis, bronchitis, pneumonia and subcutaneous abscess. Responses were excellent in 4 patients, good in 8 patients, fair in 2 patients and poor in 1 patient. In the assessment of the bacteriological efficacy, 8 out of 10 strains of organism identified previous to treatment were eradicated and 2 strains were unchanged, hence the eradication rate was 80.0%. 3. No adverse reactions attributable to the drug were observed and good drug compliance were obtained. From the above results, it has been concluded that SY5555 is a highly effective and safe agent for mild to moderate respiratory and urinary tract infections in children.

[Amnestic syndrome after right temporo-occipital subcortical hemorrhage].

A case of right temporo-occipital subcortical hemorrhage with amnestic syndrome was reported. A 65-year-old woman presented with visual disturbance and headache. CT and MRI demonstrated a hemorrhagic lesion in the right occipital lobe involving parahippocampal gyrus. Neurological examination on admission revealed left homonymous hemianopsia and anterograde and retrograde amnesia. Neuropsychological examination revealed marked recent memory disturbance, but she had visual retention and verbal retention, and her value in WAIS (Wechsler Adult Intelligence Scale) was normal. After 40 days, she was discharged without improvement of amnesia. These findings suggest that the cortico-medial temporal lobe pathway was disturbed by subcortical hemorrhage, and we think there are connective fibers between visual and verbal memory systems. This is the first report of hippocampal amnesia following temporo-occipital subcortical hemorrhage.

cDNA cloning and expression of bovine endothelin converting enzyme.

We have cloned cDNA encoding bovine endothelin converting enzyme (ECE). The predicted amino acid sequence of bovine ECE consisted of 758 amino acid residues. Northern blot analysis revealed that ECE mRNA was abundantly expressed in lung. Co-expression of the cloned cDNA of bovine ECE with human preproET-1 cDNA in CHO-K1 cells resulted in the production of mature ET-1 and this production was inhibited by phosphoramidon.

High-resolution scanning electron microscopic study of cell organelles of the chick's vestibular ganglion.

The fine intracellular structures of the chick's vestibular ganglion were studied three-dimensionally using the scanning electron microscope (SEM) after freeze-fracturing with 50% dimethyl sulfoxide solution and placement in 0.1% osmium oxide solution. Three patterns of perikaryal myelination could be distinguished in the vestibular ganglion cells: heavily myelinated, lightly myelinated and unmyelinated. Intracellular organelles such as the Golgi apparatus, endoplasmic reticulum and mitochondria could be demonstrated clearly. The stereoscopic structures of the Schwann cells and nerve fibers were also clearly demonstrated.

Right atrial to left atrial shunt through foramen ovale during pneumoperitoneum for laparoscopic cholecystectomy.

A 62-year old female patient in whom paradoxical right atrial to left atrial shunt flow through the foramen ovale was detected by transesophageal echocardiography during pneumoperitoneum for laparoscopic cholecystectomy was reported. This patient had a history of mitral valve replacement with residual tricuspid valve regurgitation. It was considered that the onset of paradoxical interatrial shunting through the foramen ovale might be caused not only by mechanical ventilation with positive end-expiratory pressure but also by pneumoperitoneum, especially in patients with heart disease, even if the intra-abdominal pressure was maintained at 12 mmHg.

Laparoscopic deroofing for symptomatic liver cysts: report of a case.

We present herein the case of a 73-year-old woman with symptomatic liver cysts and cholelithiasis for whom successful deroofing of the liver cysts and cholecystectomy were performed via laparoscopy. Histopathological diagnosis of the resected cyst wall and direct observation of the inner surface of the remaining cyst wall confirmed that they were benign biliary cysts. The laparoscopic deroofing of liver cysts is therefore not only an effective means of treatment, but it also facilitates the establishment of an exact differential diagnosis.

A case of cerebellar infarction occurred with the 8th cranial nerve symptoms.

A rare case, 32-year-old man, of cerebellar infarction with the occurrence of the 8th cranial nerve symptoms was reported. On the neuro-otological examination, hearing test and caloric test showed a severe hearing loss and no response on the right side, respectively, and the spontaneous horizontal nystagmus fixed to the left direction was observed. Magnetic resonance (MR) imaging showed the infarctions in the areas of anterior inferior cerebellar artery (AICA) and posterior inferior cerebellar artery (PICA) on the right side. On the 14th illness day, the 8th cranial nerve symptoms disappeared, and on the 12th illness day, right hearing level and caloric response were significantly improved. We suggested that such an early recovery of the subjective symptoms and neuro-otological findings may be attributable to the recanalized circulation disturbance or the development of collateral circulation.

In vitro evaluation of platelet/biomaterial interactions in an epifluorescent video microscopy combined with a parallel plate flow cell.

Suitable evaluation systems are critical for ranking various biomaterials in order to develop a method to design and synthesize nonthrombogenic biomaterials. We have recently developed an in vitro test system to evaluate platelet/biomaterial interactions in whole blood. The system consists of a parallel plate flow cell and epifluorescent video microscopy (EVM). A glass coverslip coated with a polymer was incorporated into the flow cell, and blood was perfused using a syringe pump via a polymer-coated PVC tubing connected to the flow cell. Whole human blood was anticoagulated with heparin (2 U/ml), and the platelets were labeled with the fluorescent dye mepacrine (5 microM). This system permitted real-time and dynamic observations of platelet/biomaterial interactions in whole blood under a defined flow condition. In order to evaluate the feasibility of this system, two different segmented polyether-polyurethanes (SPEUs), PU-PTMG(650) and PU-PTMG(2000), were chosen as test polymers. Surface characteristics verified with electron spectroscopy for chemical analysis (ESCA) and contact angle measurements showed similar results in both SPEUs. Blood was perfused at a wall shear rate of 200 s-1 for 20 min. Excitation light was applied for 2 s at 1 min intervals. The real-time image was then analyzed at each time point for the percentage of surface area of platelet coverage. Plasma beta-thromboglobulin (beta-TG) levels were also measured before and after each run. PU-PTMG(650) showed a significantly higher number of adhered platelets than PU-PTMG(2000) at each time point.(ABSTRACT TRUNCATED AT 250 WORDS)

Impaired stereoacuity due to a lesion in the left pulvinar.

A right-handed patient with a haematoma in the left pulvinar showed impaired stereoacuity. In contrast with previous reports, he did not show peripheral visual extinction or prolonged reaction times for targets on the side contralateral to the lesion.

Intra-abdominal venous pressure during laparoscopic cholecystectomy.

Superior vena cava (SVC) and inferior vena cava (IVC) pressures were measured serially during laparoscopic cholecystectomy in which the intra-abdominal pressure was maintained at 12 mmHg. The influences of alteration of position from 15 degrees head-down to 15 degrees head-up and of the operative procedure of holding the gallbladder up to the right subphrenic space on SVC and IVC pressures were mild. IVC pressure was maintained almost equal to the intra-abdominal pressure during prolonged continuous pneumoperitoneum lasting longer than 60 min, while SVC pressure did not change significantly during operation. The discrepancy between SVC and IVC pressures underwent no change during continuous pneumoperitoneum.

Potassium/energy and phosphorus/calcium ratios reflect the quality of nutrition.

A nutritional survey and questionnaire regarding dietary habits were administered to 649 men ranging in age from 35 to 64 years receiving a one-day health screening examination. First, comparisons of nutrient intake based on the nutrient rate of the Recommended Dietary Allowances for Japanese (RDA), and the total score on the questionnaire regarding dietary habits according to the RDA rates of total energy and calcium (Ca) were investigated. Then, the comparisons of intake of food groups, intake of nutrients and the total score on the questionnaire regarding dietary habits according to potassium/energy (K/E) ratio and phosphorus/calcium (P/Ca) ratio were determined. Last, differences between comparisons with the nutrient rates of RDA and comparisons with K/E ratio and P/Ca ratio were investigated. The following results were obtained: 1. Even in the group with RDA rates of total energy or Ca of > or = 90% and < or = 110% the RDA rates of some other nutrients were out-side the appropriate range. 2. In the group with high K/E ratios, as compared to the group with low ratios, the intake of potatoes, pulses, fruits, green/yellow vegetables, other vegetables, soy sauce, other beverages, fish/shellfish, and milk products showed high values, while cereals and fats/oils showed low values. For this reason, in the former group as compared to the latter, the intake and RDA total energy rate showed low values whereas the intake and RDA rates of nutrients such as Ca, Fe, and vitamin A were low. 3. In the group with high P/Ca ratios, as compared to the group with low ratios, the intake of beer and meats showed high values while that of pulses, green/yellow vegetables, and milk products showed low values. For this reason, in the former group as compared to the other groups, despite the fact that the intake and RDA total energy rate tended to be higher, the intake of nutrients such as Ca, P, K, and vitamin A showed lower values. 4. The RDA rates of all of the nutrients of group with a high K/E ratio and the group with a low P/Ca ratio generally showed largely appropriate values. 5. These results suggest that indices reflecting the balance of minerals such as the K/E and P/Ca ratios more objectively reflect the quality of nutrition than comparisons based on the RDA rates of total energy and specific-nutrient.

[A study of thyroid hormone levels of inhabitants of the cadmium-polluted Kakehashi River basin].

We compared thyroid hormone levels of inhabitants (19 men and 16 women) of the cadmium (Cd)-polluted Kakehashi River basin in Ishikawa Prefecture, with those of subjects (23 men and 47 women) living in a non-polluted area. In addition, we investigated the relationships between the thyroid hormone levels and indices of renal dysfunction induced by Cd exposure. The following results were obtained: 1) The free T4 level of females was significantly lower than that of controls. 2) The T3 level of inhabitants of both sexes was significantly higher than that of controls. 3) The level of free T4 among females became lower with the increases of urinary beta 2-microglobulin (beta 2-MG), urinary protein, urinary sugar, urinary amino acids and serum creatinine (Cr) levels, and with decreases of creatinine-clearance (CCr) and %TRP. 4) We could not find any relationship between the increase of T3 and the indices of renal dysfunction induced by Cd exposure in either sex.

[Equilibrium of small animals in "drop shaft" experimentation--preliminary study on the frog with vestibular neurectomy].

We have newly designed instruments for equilibrium experimentation, using a drop shaft, and studied the behavior and EMG Patterns of frogs with or without unilateral vestibular neurectomy, as a preliminary study at JAMIC (Japan Microgravity Center). The results are reported herein and the efficacy of the drop shaft in equilibrium experimentation is also documented. 1) In unilateral vestibular neurectomized frogs, posture--head tilt, leg flexion on the affected side, and leg extension on the opposite side--is maintained and persists under microgravity. Rolling to the affected side was also observed. 2) The latencies of the EMG responses from M. tibialis anticus and M. longissimus dorsi were prolonged on the affected side. 3) The frog is suitable for use in equilibrium experiments utilizing a drop shaft. 4) The JAMIC drop shaft can be used to obtain constant and stable microgravity, 10(-4) G for 10 sec, which is similar to that in space. Drop shafts are suitable for equilibrium experimentation.

Dose-response relationship between total cadmium intake and metallothioneinuria using logistic regression analysis.

The dose-response relationship for environmental cadmium exposure was assessed using logistic regression analysis. The prevalence of metallothioneinuria was employed as a response variable, while age and total cadmium intake, calculated from the average cadmium concentration in rice and duration of residence in the cadmium-polluted area, were used as explanatory variables. The target population comprised of 1843 cadmium-exposed and 240 non-exposed inhabitants of Ishikawa, Japan. The individuals were divided into 96 subgroups by sex, age (4 categories), cadmium concentrations in rice (3 categories) and length of residence in the polluted area (4 categories). Only total cadmium intake had a significant association with the prevalence of metallothioneinuria. In the non-exposed subjects total cadmium intakes corresponding to 2.5% prevalence of metallothioneinuria were calculated. Based on metallothionein levels expressed as either microgram/l urine or microgram/g creatinine, the total intakes were: 2.221 or 2.207 g in men and 2.365 or 0.319 g in women, respectively. Most of these values were similar to those reported by us previously, employing simple regression analysis. It is concluded, therefore, that a maximum allowable intake of about 2 g cadmium is a reasonable estimate for preventing the cadmium-induced renal dysfunction.

Biological monitoring of cadmium exposure in itai-itai disease epidemiology.

Cadmium (Cd) concentrations in blood and urine were significantly higher in itai-itai disease patients, suspected patients and inhabitants in Cd-polluted areas in Japan. Urinary Cd concentrations were mainly related to the body burden of Cd and did not decrease for several years following cessation of Cd exposure. There was a close association between urinary Cd excretion and the occurrence of beta 2-microglobulinuria (beta 2-mg-uria). Probit regression analysis between urinary Cd excretion and, beta 2-mg-uria indicated that the threshold value may be set around 4 micrograms/g creatinine (cr.). Studies on inhabitants in Cd-polluted areas showed that urinary, beta 2-mg levels of more than 1000 micrograms/l or 1000 micrograms/g cr. were irreversible, while increased urinary excretion of beta 2-mg was associated with increased mortality. At present, beta 2-mg in urine is not a suitable indicator for monitoring renal tubular dysfunction in the general population since the significance of slightly increased excretion of urinary beta 2-mg has yet to be clarified.

Dose-response relationship between total cadmium intake and beta 2-microglobulinuria using logistic regression analysis.

The dose-response relationship between total cadmium intake and beta 2-microglobulinuria was investigated using logistic regression analysis in order to consider the effect of age on this association. The target population consisted of 1850 inhabitants of the cadmium-polluted Kakehashi River basin in Ishikawa prefecture, Japan. They were divided into 58 subgroups (27 in the men and 31 in the women) by four factors of sex, age, rice cadmium concentration and length of residence in cadmium-polluted areas. Logistic regression analysis was performed for this dose-response relationship, and both age and total cadmium intake were significantly associated with beta 2-microglobulinuria. It was confirmed that total cadmium intake had a significant association with beta 2-microglobulinuria, independent of the aging effect.
