Down's syndrome associated with seminoma.

A case of unilateral seminoma in Down's syndrome is reported. The relation of testicular cancer with Down's syndrome and the necessity of the follow-up biopsy of the contralateral testis are discussed.

[Combination chemotherapy of vincristine, ifosfamide and peplomycin in patients with reactivated prostatic cancer].

Ten patients with reactivated stage D prostatic cancer were treated with the combination therapy of vincristine, ifosfamide and peplomycin (VIP therapy). This treatment gave a good relief of bone pain in 4 out of 8 patients (50%) and improvement of dysuria in 2 out of 6 patients (33%), but objective responses of primary and metastatic lesions were poor. The over-all response rate was 20% by Karnofsky's category of response, 10% by the response criteria of Shida et al., 50% by National Prostatic Cancer Project criteria in the United States and 10% by the response criteria of Koyama and Saito's group. The toxicity of VIP therapy was mild to moderate except for hepatic insufficiency shown in one case.

[Hormonal environment and antiandrogenic treatment in benign prostatic hypertrophy].

The plasma luteinizing hormone (LH), follicle stimulating hormone (FSH), prolactin, testosterone and human growth hormone (HGH) response to insulin-induced hypoglycemia in patients with benign prostatic hypertrophy and age-matched control patients were not different. Although all 6 drugs used were effective for treating these benign prostatic hypertrophy patients the 3 drugs, chlormadinone acetate, oxendrone and allylestrenol, were especially recommended.

In vitro characteristics of rat mesangial cells in comparison with aortic smooth muscle cells and dermal fibroblasts.

Rat glomerular mesangial cells were cultured and their antigens were compared with those of aortic vascular smooth muscle cells and dermal fibroblasts. Glomeruli, aortic, and dermal explants were cultured for 3 weeks and subcultured in the same conditions. These cultured cells were evaluated by indirect immunofluorescence studies using antibodies against Thy-1 antigen, desmin, and chicken gizzard actin. Most of mesangial cells were positive for Thy-1, desmin, and actin. On the other hand, fibroblasts were negative for desmin, and smooth muscle cells stained Thy-1 scarcely, and were negative for desmin. In the latter two cells, actin-positive fibrils were thinner and fainter than mesangial cells. These results indicated that mesangial cells could be distinguished in vitro from vascular smooth cells and fibroblasts by immunofluorescence microscopy.

Primary aldosteronism treated by trilostane (3 beta-hydroxysteroid dehydrogenase inhibitor).

The practicability and tolerability of trilostane, a competitive inhibitor of 3 beta-hydroxysteroid-delta 5-dehydrogenase, for the therapy of primary aldosteronism was assessed in 1 patient with aldosterone-producing adenoma (APA) and 3 subjects with idiopathic adrenal hyperplasia (IHA). Trilostane afforded reduction of plasma levels of aldosterone, progesterone, deoxycorticosterone, 17-OH progesterone, cortisol, delta 4-androstenedione, and urinary excretion of 17-hydroxycorticosteroid. Conversely, circulating levels of dehydroepiandrosterone, dehydroepiandrosterone sulfate, and urinary excretion of 17-ketosteroids were increased following this drug therapy. Suppression of mineralo- or glucocorticoid biosynthesis was accompanied by an increase in plasma renin activity. One patient with APA or 3 subjects with IHA showed slight or remarkable improvement of hypertension and hypokalemia. Based on these findings, efficacy and tolerability of trilostane appear to aid in the treatment of IHA.

[Serum and urinary tissue polypeptide antigen (TPA) in patients with urological cancer].

We measured the serum and urinary TPA in 64 patients with urological cancer, 55 patients with benign urological disease and 37 healthy volunteers. The serum TPA levels in patients with pre-treated cancer were significantly higher than those of healthy volunteers, but showed no significant difference compared to those in patients with a benign urological disease. The urinary TPA levels in patients with pre-treated cancer were significantly higher than those with a benign urological disease, but in some patients with urinary tract infection, very high urinary TPA concentrations were observed. Therefore, the determination of serum and urinary TPA may be useful in detecting urological cancer, but they were not specific tests. On the other hand, as lower serum and urinary TPA levels were observed after treatment of bladder and prostatic cancer, their determinations were considered to be useful in following up patients with these cancers.

A nephropathy induced by immunization of rats with EHS tumour.

Immunization of rats with Engelbreth-Holm-Swarm tumour induced a nephropathy immunohistopathologically similar to Heymann nephritis except that IgG was deposited along the tubular basement membranes. Autoantibodies against both brush border and basement membranes of proximal tubules were found in the sera. Immunoperoxidase staining of the tumour cells and absorption studies suggested that the tumour cells produced the FX1A-related antigens. Immunodiffusion analysis suggested that the antigens which induced this nephropathy differed from gp 330 and gp 600 which have been reported to be responsible for Heymann nephritis.

[Cis-platinum used for the prevention of the recurrence of adrenal cortical carcinoma: report of a case].

The right suprarenal mass was found in a 21-year-old housewife. Her major clinical features were amenorrhea, polydipsia and buffalo hump obesity. Endocrinological and roentgenological studies suggested the presence of Cushing's syndrome due to adrenocortical carcinoma in addition to ipsilateral renal stone. The huge adrenal tumor and renal calculus were successfully removed. The histological diagnosis was adrenocortical carcinoma. Seventeen days after the operation, cis-platinum was administered to prevent the recurrence of tumor development. No recurrence has been observed for approximately 2 years after the surgery. Long follow-up must be pursued to clarify the real efficacy of cis-platinum treatment.

[XX-male in a case of male infertility].

A 26-year-old man with the chief complaint of male infertility for 3 years was referred to our clinic. Physical examination of the patient revealed normal male habitus except for a small testis. His semen analysis showed no sperm. Levels of plasma LH and FSH were high compared with normal ones but that of plasma testosterone was within the normal range. Histological examination of his testis showed no spermatogenesis. Chromosome analysis with peripheral blood revealed 46, XX. H-Y antigen could be detected by in vitro cytotoxicity tests. By the above findings, the diagnosis of XX-male was made. Literature dealing with this disease is scanty. Only 18 cases of XX-male including our case were collected in Japan. The etiology and clinical features of this disease are discussed in the present report.
