Genetic polymorphisms of tobacco- and alcohol-related metabolizing enzymes and human esophageal squamous cell carcinoma susceptibility.

Many human cancers are caused by synthetic or natural chemical compounds in the environment. Esophageal squamous cell carcinoma has been reported to be epidemiologically associated with tobacco and alcohol consumption. We studied the association between genetic polymorphisms of tobacco- and alcohol-related metabolizing enzymes and esophageal squamous cell carcinoma susceptibility. We examined genetic polymorphisms of the CYPIA1, GSTM1, CYPIIE1, ADH2, and ALDH2 genes in 94 Japanese patients with esophageal squamous cell carcinoma and 70 unrelated healthy Japanese persons. There were no significant differences between healthy controls and patients with esophageal cancer in the polymorphisms of the CYPIA1, GSTM1, and CYPIIE1 genes. On the other hand, there were significant differences in the ADH2 and ALDH2 polymorphisms between healthy controls and esophageal cancer patients. The ADH2(1)/ADH2(1) and ALDH2(1)/ALDH2(2) genotypes were independently and significantly higher in esophageal cancer patients than in healthy controls. Furthermore, persons with the combined genotypes ADH2(1)/ADH2(1) and ALDH2(1)/ALDH2(2) were at extraordinarily high risk for esophageal squamous cell carcinoma, with an odds ratio of 17.9 (p < 0.001). Thus polymorphisms of alcohol-metabolizing enzymes, that is, ADH2 and ALDH2, may be useful for screening patients at high risk for esophageal cancer, which might facilitate clarification of esophageal tumorigenesis and prevention of esophageal cancer.

Staging of esophageal carcinoma in vitro with 4.7-T MR imaging.

PURPOSE: To determine the accuracy of 4.7-T magnetic resonance (MR) imaging for evaluating the mural invasion of esophageal carcinoma. MATERIALS AND METHODS: Sixty-seven esophageal specimens containing 70 squamous cell carcinomas were studied with a 4.7-T MR system with a 4-cm-diameter surface coil. Spin-echo MR images were obtained with a field of view of 4-6 cm, matrix size of 256 x 256, and section thickness of 3 mm, which resulted in a voxel size of 0.08-0.16 mm3. MR findings were compared with histopathologic findings, which served as the standard of reference. Tumor invasion on MR images was determined by consensus of two radiologists. RESULTS: T2-weighted images depicted six to eight layers of the normal esophageal wall; these correlated well with layers identified with histologic examination. In 66 (94%) of 70 cases of esophageal carcinoma, the depth of mural invasion visualized with MR imaging correlated well with that determined with histopathologic examination. The stage determined with MR imaging, however, was higher in two (3%) and lower in two (3%) cases than that determined with histopathologic examination. MR imaging enabled visualization of the gross features of tumor growth pattern, tumor ulceration, and adjacent lymph node involvement. CONCLUSION: MR imaging in vitro allows accurate assessment of the mural invasion of esophageal carcinoma and thus potentially enables preoperative histopathologic staging.

A clinical comparison of definite moyamoya disease between South Korea and Japan.

BACKGROUND AND PURPOSE: The goal of the present study was to clarify whether South Korean patients with moyamoya disease have clinical features similar to those of Japanese patients. METHODS: From 26 South Korean neurosurgical institutes, 296 definite cases were collected and analyzed statistically. These cases were then compared with 731 Japanese definite cases registered to the Research Committee on Moyamoya Disease of the Ministry of Health and Welfare, Japan. RESULTS: The Korean age distribution patterns showed two peaks that were similar to those seen in Japanese patients. The incidence of adult moyamoya disease in South Korea, however, was 20% higher than that in Japanese patients. The family occurrence rate was 1.8% in Koreans. The incidence of cerebral infarction and bleeding in Koreans was statistically higher, whereas transient ischemic attack and seizure were less than those in Japanese subjects. The incidence of infarction in children and that of hemorrhage in both children and adults were also statistically higher in Koreans. The incidence of hemorrhage was higher in females than in males. Both the age at onset and sex affected the disease type. Although most Japanese patients underwent direct bypass surgery and/or combined indirect bypass procedures, single encephaloduroar-teriosynangiosis was performed on 87.6% of all surgical cases in Koreans. Despite the higher incidence of hemorrhagic type in South Korea, the outcomes of the patients were similar to those of the Japanese patients. CONCLUSIONS: This study suggests that the clinical background of moyamoya disease in South Korea is essentially similar to that in Japan.

Inactivation of Bacillus spores by the supercritical carbon dioxide micro-bubble method.

Bacillus spores were effectively inactivated by the supercritical (SC) CO2 micro-bubble method. The micro-bubble SC CO2 treatment of B. cereus, B. subtilis, B. megaterium, B. polymyxa, and B. coagulans at 40 degrees C and 30 MPa for 30 min produced greater reduction (about 3 log cycles of reduction) than a similar treatment without a filter. The SC CO2 treatment of B. polymyxa, B. cereus, and B. subtilis spores at 45 degrees C, 50 degrees C, respectively, and 30 MPa for 60 min resulted in a 6-log cycle reduction of survival. The SC CO2 treatment under the foregoing conditions should offer higher efficiency than that of heat treatment at 100 degrees C for 60 min. In addition, the SC CO2 treatment (30 MPa, 60 degrees C, 30 min) of B. polymyxa and B. cereus spores also produced a 6-log cycle reduction.

Molecular cloning of a new type of cDNA for pheromone biosynthesis activating neuropeptide in the silkworm, Bombyx mori.

A neuropeptide named pheromone biosynthesis activating neuropeptide (PBAN) stimulates the pheromone production of lepidopteran insects. We have identified a different type of cDNA for PBAN, which shows two amino acid replacements in the region of the PBAN sequence previously reported. Single strand conformation polymorphism (SSCP) analysis revealed that the two types of cDNA originated from two allelic variants of the gene for PBAN.

Structure and activity of a new form of the glutamate transporter of the nematode Caenorhabditis elegans.

A Caenorhabditis elegans cDNA for a glutamate transporter was cloned and examined in this study. The predicted protein is 11 residues shorter at the N-terminus than Ceglut-1, which we previously reported. The protein, when expressed in Xenopus laevis oocytes, showed much higher glutamate transport activity than Ceglut-1, suggesting that the N-terminal sequence is critical in glutamate transport.

[Relapsing polychondritis: a case with respiratory failure].

A 48-year-old woman was admitted to our hospital with respiratory failure (Hugh-Jones IV-V). She was diagnosed as relapsing polychondritis 6 years ago. Her respiratory failure was due to pharyngial stenosis, deformity and inflammation of a trachea and lobar bronchus, and bronchial collapse. Her tracheobronchochondritis was managed by 500-700 mg/day of hydrocortisone and 50 mg/day of cyclophosphamide. Laboratory examination revealed only slight elevation of CRP and no elevation of anti-type II collagen antibody, although these parameters were very high on her first admission when she had severe polyarthritis, polychondritis of nose and auricles. Bronchoscopic findings were compatible with tracheobronchomalacia since pharyngial stenosis due to inflammatory pharyngitis and bronchial collapse due to tracheobronchochondritis were shown without lung parenchymal damage. We referred to the literature on tracheobronchomalacia which was associated by the varieties of respiratory and rheumatic diseases.

[Transarterial embolization of a vein of Galen aneurysm: a case report].

This report describes a case of a vein of Galen aneurysm. A 6-month-old baby with a slight head injury was referred to our hospital. CT scans and MR images demonstrated a vein of Galen aneurysm. Left carotid angiogram showed a vein of Galen aneurysm with a large feeding artery from the posterior cerebral artery. Transarterial embolization was performed using Galen coils and Interlocking Detachable Coils. Angiograms after the second embolization revealed complete occlusion of the aneurysm and postoperative course was uneventful.

[Operative procedures of T1 cancer of the lower thoracic esophagus].

Pathological results for 70 cases of T1 EiEa esophageal cancer resected in our department for the past 12 years, have shown 32 cases of mucosal cancer and 38 cases of submucosal cancer. Lymph node metastasis was recognized in 21 cases (30%). Moreover, positive nodes were observed only in submucosal cancer cases. Lymph node metastasis was mainly observed in the lower mediastinum and in the upper abdomen. However, it was frequently observed in the upper mediastinum (106) and sometimes in the supraclavicular area (104). The lymph node dissection should be performed in the mediastinum through the upper abdomen and neck. The subtotal esophagectomy with thoracotomy and the systematic dissection is common operative procedures, but the reduced surgery, i.e. transhiatal esophagectomy or the lower esophagectomy and proximal gastrectomy, is also indicated in some cancer patients.

[Histological findings in aneurysm treated with IDC: scanning electron microscopical study].

We reported the scanning electron microscopic findings of an aneurysm embolized with interlocking detachable coils (IDC). A 78-year-old man was referred to our hospital because of left hemiparesis and dysphagia. CT scan showed multiple lacunar infarction in the bilateral corona radiata. Angiography demonstrated a small incidental aneurysm of the right internal carotid artery (IC-PC junction). The left hemiparesis gradually improved and 2 weeks after admission the aneurysm was treated with IDC. Three coils were placed in the aneurysm and post-operative angiography demonstrated complete embolization. Dysphagia caused by the pseudobulbar pulse did not improve and, 4 weeks after endovascular surgery, the patient died of aspiration pneumonia. An autopsy was performed and the scanning electron microscopical findings of the aneurysm showed development of endothelial cells on the coil surface. Many clinical reports supported the usefulness of Guglielmi detachable coils (GDC) and several experimental reports demonstrated the intimal proliferation in aneurysms embolized with coils. However, pathological changes of the aneurysms treated with coils have been reported in only four clinical cases. There was no evidence of endothelialization in any of these cases. To our knowledge, our case is the first report describing the endothelial proliferation over the coils. It is necessary to accumulate histopathological data and long-term follow-up in humans treated with coils.

[Dissecting aneurysm of the middle cerebral artery with subarachnoid hemorrhage and brain infarction: a case report].

This report describes a surgical case of a dissecting aneurysm of the left middle cerebral artery with subarachnoid hemorrhage and ischemic insult. A 48-year-old man with pure motor aphasia was referred to our hospital. CT scans demonstrated subarachnoid hemorrhage in the left frontal region. CT scans on the next day showed a small brain infarction in the left precentral artery and central artery territories. The first left carotid angiograms showed stenosis at the M2 portion of the middle cerebral artery. On the 16th day, the second left carotid angiograms revealed a pearl and string sign. We diagnosed a dissecting aneurysm of the middle cerebral artery. The dissecting aneurysm was wrapped using the pterional approach and STA-MCA anastomosis was also performed. The postoperative course was uneventful and the motor aphasia improved.

Interleukin-8 in bronchoalveolar lavage fluid of patients with diffuse panbronchiolitis or idiopathic pulmonary fibrosis.

This study was designed to clarify the contribution of IL-8 as a specific neutrophil chemotactic factor in the human respiratory tract in various pulmonary diseases. The neutrophil chemotactic activity (NCA), neutrophil counts and IL-8 concentration in the bronchoalveolar lavage fluid (BALF) obtained from normal volunteers (NV), control patients (CP), patients with diffuse panbronchiolitis (DPB) and patients with idiopathic pulmonary fibrosis (IPF) were examined. Neutrophil counts, NCA and IL-8 concentration in BALF obtained from patients with DPB or IPF was significantly higher than that from NV or CP. The IL-8 concentration correlated with neutrophil count and also correlated with NCA in BALF from patients with IPF, whereas there was no correlation between these factors in BALF from DPB. These results suggest that the contribution of IL-8 to neutrophil accumulation of the lower respiratory tract is different between IPF and DPB.

Thrombin stimulates platelet-derived growth factor release by alveolar macrophages in rats--significance in bleomycin-induced pulmonary fibrosis.

Thrombin is a multifunctional enzyme generated at sites of vascular injury, and is known to be increased in the lungs in some types of fibrotic lung disease. In this study, to determine whether thrombin is associated with fibroblast growth and pulmonary fibrosis in these disorders, we examined whether a growth factor for fibroblasts (platelet-derived growth factor, PDGF) was released by thrombin-stimulated alveolar macrophages (AM). The culture supernatants of rat AM stimulated with 1 or 10 U/ml of thrombin showed a significant increase in fibroblast growth-stimulating activity (FGA). Pretreatment of the AM supernatant with anti-PDGF-AA antibody significantly decreased the FGA, but pretreatment with anti-PDGF-BB antibody did not. The supernatants of AM stimulated with thrombin also increased the growth of fibroblasts from the lungs of rats with bleomycin-induced lung injury. These results indicate that thrombin stimulates AM to release PDGF-AA, which is responsible, at least in part, for fibroblast growth and the development of pulmonary fibrosis in some types of fibrotic lung disease.

[Sexual dysfunction].

According to the definition of the ICD-10 or DSM-III, sexual dysfunction is not caused by organic disorder or disease, and psychogenic. But, the patient who is suffered from sexual dysfunction caused by organic disorder or disease, has many psycho-social problems. So, in this article, all types of sexual dysfunction are subjected. Especially sexual hypofunction is mentioned. And the new type of sexual dysfunctions such as computer sex are also explained. Sex therapy of the Masters and Johnson method and the new sex therapy of Kaplan, H. are explained.

[A case of rheumatoid lung exacerbated by acetaminophen-induced pneumonitis].

A 57-year-old man was admitted with a high fever, dry cough, severe dyspnea and an interstitial shadow bilaterally on chest roentogenogram. Although his illness was not diagnosed, he was treated with a high dose of methylprednisolone (1 g/day for 3 days) for acute interstitial lung disease. As the 3-day treatment was not effective, high-dose methylprednisolone therapy was repeated. Subsequently, he was treated with prednisolone (60 mg/day), after which his condition improved. After 8 months, the patient caught cold for which he was treated. Subsequently his previous lung disease appeared again. His illness, improved after steroid therapy. The patient had been treated with Shin-Ruru-A tablets during his first admission. A lymphocyte stimulation test for Shin-Ruru-A-Tablet, PL granule, and acetaminophen (which is the common constituent of the former two drugs), was positive. Polyarthralgia, bone lesions joint swelling, and a positive rheumatoid factor test were present on first admission. Therefore, his illness was diagnosed as rheumatoid arthritis (RA). As the interstitial shadow remained after treatment of the lung disease, a thoracoscopic lung biopsy was performed. The specimen revealed an intensive lymphocytic infiltration, perivasculatitis, and thickening of the alveolar septa. These findings corresponded with those of lung disease associated with RA. The results suggest that lung disease associated with collagen vascular diseases may be exacerbated by drug-induced pneumonitis.

[A novel immune system].

We found a novel lymphoid cell lineage, V alpha 14 NKT cell, which is characterized by 1) the expression of both NK1.1 (NK receptor) and an invariant TCR encoded by V alpha 14 and J alpha 281 gene segments; 2) the expression of unusual phenotypes, such as NK1.1+, B220+, Mac-1+, HSA+, CD44+, CD45Rlow and MEL-14low; and 3) the extrathymic development: V alpha 14 NKT cells appear at d9.5 of gestation before thymus development. Moreover, the deletion of the invariant V alpha 14 TCR gene expression caused the lack of NKT cells in vivo, while transgene of the invariant V alpha 14 V beta 8 TCR in the RAG-deficient background resulted in the generation of only V alpha 14 NKT cells without other lymphoid cells. These results indicate the essential requirement of invariant V alpha 14 TCR for the development of NKT cells. Recent studies clearly show that V alpha 14 NKT cells, but not NK cells or T cells are the primary target of IL-12 in the IL-12-mediated tumor rejection.

Model of malignant pleural effusion of human lung adenocarcinoma in SCID mice.

Malignant pleural effusion (PE) is a frequent problem in lung cancer. In this study, we established a model of malignant PE of human adenocarcinoma cells, PC-14, in SCID mice. Intravenously injected PC-14 cells formed colonies in the lungs as early as week 4 after tumor inoculation, and produced bloody PE in all recipient SCID mice by week 8. Pretreatment of SCID mice with anti-mouse IL-2 receptor beta chain antibody (TM-beta 1) to deplete natural killer (NK) cells markedly promoted the production of bloody PE and metastases to multiple organs, such as the lungs, liver, kidneys, and lymph nodes 4 weeks after tumor inoculation. Histological studies indicated that PC-14 cells formed colonies in the lungs, and then invaded the pleura and spread to the pleural cavity. To establish cell lines with a high potential to produce PE, we harvested PE, expanded the tumor cells in vitro, and injected them into SCID mice again. By four in vivo selection cycles in this way we obtained PC-14-PM4 cells, which produce lung metastases and PE earlier than PC-14 cells. The survival of SCID mice inoculated with PC-14-PM4 cells was significantly shorter than that of mice inoculated with PC-14 cells. The expressions of adhesion molecules, such as CD44, CD49d, ICAM-1, and MHC class I, on PC-14-PM4 cells tended to increase compared with PC-14 cells. These changes of adhesion molecules seem to be one of possible mechanisms involved in higher metastatic potential of PC-14-PM4 cells. PE models with PC-14 and PC-14-PM4 cells should be useful for biological and preclinical studies on malignant PE produced by human lung cancer.
