Relation of electrocardiographic left ventricular hypertrophy with and without T-wave changes to systemic blood pressure, body mass, and serum lipids and blood glucose levels in Japanese men.

Left ventricular (LV) hypertrophy, especially combined with an abnormal ST-T, is considered 1 of many coronary risk factors. Seven hundred forty-nine Japanese men were selected according to their electrocardiographic findings, i.e., normal electrocardiogram, LV hypertrophy without an abnormal ST-T segment, LV hypertrophy with a flat T wave, and LV hypertrophy with a negative T wave. Coronary risk factors were compared among these 4 age-matched groups. Groups with LV hypertrophy with negative or flat T waves had larger body mass index (24.9 vs 22.9 kg/m2), higher mean systemic blood pressure (111 vs 95 mm Hg), larger LV mass (265 vs 157 g), higher blood glucose (110 vs 100 mg/dl), higher serum triglyceride (148 vs 122 mg/dl), higher total cholesterol (206 vs 198 mg/dl), and lower high-density lipoprotein cholesterol (47 vs 54 mg/dl) than the normal group or the group with LV hypertrophy without T-wave change. Among these risk factors, blood pressure and glucose remained higher even after the adjustment by body mass index or by body mass index and blood pressure. Electrocardiographic LV hypertrophy with a changed T wave signified higher risk of coronary artery disease in Japanese men.

Mutations in the gyrA and parC genes associated with fluoroquinolone resistance in clinical isolates of Citrobacter freundii.

We determined partial sequences of the gyrA and parC genes of Citrobacter freundii type strain, and then examined 38 C. freundii clinical strains isolated from patients with urinary tract infections for the association of alterations in GyrA and ParC with susceptibility to fluoroquinolones. Our results suggest that in C. freundii DNA gyrase may be a primary target of quinolones, that an amino acid change at Thr-83 or Asp-87 in GyrA is sufficient to decrease susceptibility to fluoroquinolones, and that accumulation of changes in GyrA with the simultaneous presence of an alteration at Ser-80 or Glu-84 in ParC may be associated with the development of high-level fluoroquinolone resistance in C. freundii clinical isolates.

Intermediate TCR cells in mouse lung: their effector function to induce pneumonitis in mice with autoimmune-like graft-versus-host disease.

The lung comprises lymphocytes even under noninflammatory conditions. We investigated the presence of NK cells, extrathymic T cells, and thymus-derived T cells in this organ. As shown previously in mouse liver, two-color staining for CD3 and IL-2R beta-chain (IL-2Rbeta) identifies CD3- IL-2Rbeta+ NK cells, CD3int IL-2Rbeta+ cells (int indicates intermediate; of extrathymic origin), and CD3high IL-2Rbeta- cells (high indicates high; of thymic origin). These populations were present in the lungs of normal mice in a unique manner (i.e., NK cells > CD3high cells > CD3int cells). The proportion of CD3int cells increased in the lung with age. In athymic mice, only CD3int cells were detected in the lung. In contrast to CD3int cells in the liver, the majority of these cells in the lung did not express NK1.1 Ags. Other properties of CD3int cells in the lung were the same as those in the liver, e.g., double-negative CD4- 8- cells and gammadelta T cells. CD3int cells in the lung contained forbidden clones similar to those in other organs. When (B6 x bm12) F1 mice (Ly5.2) were injected with CD3high cells of B6-Ly5.1 origin, F1 mice fell victim to chronic graft-vs-host disease and pneumonitis, showing the expansion of CD3int cells. Almost all of them were of recipient origin (Ly5.2+). More importantly, such CD3int cells induced autoimmune-like pneumonitis when injected into irradiated F1 mice. CD3int cells isolated from the lung in mice with graft-vs-host disease exerted autologous cytotoxicity against thymocytes in vitro. These results suggest that extrathymic T cells exist in the lung and that their expansion may be responsible for inflammatory lung diseases.

Expansion of an unusual population of Gr-1+CD3int cells in the lymph nodes and other peripheral organs of mice carrying the lpr gene.

Granulocytes and extrathymic T cells are often activated simultaneously, but they are absolutely separate populations in normal mice. However, some abnormal extrathymic T cells (i.e., CD3int cells) seen in mice carrying the lpr gene were found to express a granulocyte marker, Gr-1. Such mice include MRL-lpr/lpr mice and SCG mice. In parallel with an age-associated increase of IL-2Rbeta(low)CD3int cells which contained double-negative CD4-8- and B220+CD2- cells, Gr-1+CD3int cells increased in number in the lymph nodes and other peripheral organs. In addition to a major population of IL-2Rbeta(low)CD3int cells, there is a small population of IL-2Rbeta(high)CD3int cells which produce normal Fas mRNA and Fas molecule from the lpr gene. It was found that both IL-2Rbeta(low)CD3int and IL-2Rbeta(high)CD3int cell populations contained Gr-1+ cells. IL-2Rbeta(high)CD3int cells tended to contain a higher proportion of Gr-1+ cells than did IL-2Rbeta(low)CD3int cells. More interestingly, Gr-1+CD3int cells expressed a considerable level of mRNA of the mG-CSF receptor, similar to granulocytes. The present study thus yielded further information on an unusual property of abnormally expanding CD3int cells in mice carrying the lpr gene.

Vascular dysfunction induced by elevated glucose levels in rats is mediated by vascular endothelial growth factor.

The purpose of these experiments was to investigate a potential role for vascular endothelial growth factor (VEGF) in mediating vascular dysfunction induced by increased glucose flux via the sorbitol pathway. Skin chambers were mounted on the backs of Sprague-Dawley rats and 1 wk later, granulation tissue in the chamber was exposed twice daily for 7 d to 5 mM glucose, 30 mM glucose, or 1 mM sorbitol in the presence and absence of neutralizing VEGF antibodies. Albumin permeation and blood flow were increased two- to three-fold by 30 mM glucose and 1 mM sorbitol; these increases were prevented by coadministration of neutralizing VEGF antibodies. Blood flow and albumin permeation were increased approximately 2.5-fold 1 h after topical application of recombinant human VEGF and these effects were prevented by nitric oxide synthase (NOS) inhibitors (aminoguanidine and N(G)-monomethyl L-arginine). Topical application of a superoxide generating system increased albumin permeation and blood flow and these changes were markedly attenuated by VEGF antibody and NOS inhibitors. Application of sodium nitroprusside for 7 d or the single application of a calcium ionophore, A23187, mimicked effects of glucose, sorbitol, and VEGF on vascular dysfunction and the ionophore effect was prevented by coadministration of aminoguanidine. These observations suggest a potentially important role for VEGF in mediating vascular dysfunction induced by "hypoxia-like" cytosolic metabolic imbalances (reductive stress, increased superoxide, and nitric oxide production) linked to increased flux of glucose via the sorbitol pathway.

Integration of HTLV-1 provirus into mouse transforming growth factor-alpha gene.

Human T cell leukemia virus type 1 (HTLV-1) provirus integration was investigated in mice inoculated with MT-2 cells, a HTLV-1 producing human T-cell line. From spleen DNA derived from a MT-2 cell-injected mouse, we cloned a HTLV-1 integration site by ligation-mediated PCR. The nucleotide sequence showed that HTLV-1 provirus was integrated into an intron of the mouse transforming growth factor-alpha gene in the reverse orientation. This result provides for the first time molecular evidence that mice can be infected with HTLV-1.

Chymase is activated in the hamster heart following ventricular fibrosis during the chronic stage of hypertension.

Chronic pressure overload induces cardiac tissue remodeling. Chymase is known to regulate matrix metabolism and angiotensin II formation. In the present study, we investigated the pathophysiological functions of chymase in the pressure-overloaded hamster heart induced by a two-kidney, one-clip (2K1C) hypertension procedure. Fibrosis and apoptosis were observed in the pressure-overloaded hearts of 2K1C hamsters 32 weeks after clipping, but these histological changes were not detected at 16 weeks. Heart chymase-like activity of 2K1C hamsters at 32 weeks increased 5.2-fold compared with that at 16 weeks, while angiotensin-converting enzyme was not activated. Chymase might be involved in cardiac tissue remodeling during the chronic stage of hypertension.

An electron paramagnetic resonance study on the mechanism-based inactivation of adenosylcobalamin-dependent diol dehydrase by glycerol and other substrates.

Adenosylcobalamin-dependent diol dehydrase undergoes mechanism-based inactivation by glycerol or other substrates during catalysis. X-band electron paramagnetic resonance spectra of holoenzyme were measured at -130 degrees C after reaction with such substrates. After short time of incubation, broad signals assigned to low-spin Co(II) of cob(II)alamin and doublet signals assigned to an organic radical intermediate derived from each substrate were observed with 1,2-propanediol, 1,2-ethanediol, glycerol and meso-2,3-butanediol with the magnitude of their exchange interaction (J-value) decreasing in this order. A substrate with the smaller magnitude of exchange interaction between low-spin Co(II) and an organic radical intermediate seems to be an efficient mechanism-based inactivator. Since the magnitude of exchange interaction decreases with the distance between radical species in a radical pair, these results suggest that a stabilizing effect of holoenzyme on radical intermediates during reactions decreases with the distance between Co(II) and a radical.

Transgenic expression of hepatitis C virus structural proteins in the mouse.

Although hepatitis C virus (HCV) is a leading cause of morbidity and mortality worldwide, the role of viral cytopathic effects remains unclear. To study the biosynthesis of HCV structural proteins and their pathogenic role, we constructed transgenic mice, expressing type 1b HCV structural proteins (core, E1, and E2) in liver tissues. Two liver-specific promoters were used. The mouse major urinary protein (MUP) promoter has been shown to be developmentally regulated with little or no expression in utero but high-level expression after birth. The albumin (Alb) promoter provides constitutive, high levels of transgenes in live. Expression of both HCV transgenes was detected in several lines by Northern blots, HCV-specific reverse transcriptase-polymerase chain reactions (RT-PCR), and Western immunoblotting. Alb HCV lines showed higher levels of HCV expression than the MUP HCV lines. Immunohistochemical analysis revealed a predominantly cytoplasmic presence of core protein with occasional nuclear staining, and both cytoplasmic and membrane expression of the E2 protein in the transgenic livers. In both transgenes, the highest levels of both antigens were seen in perivenular hepatocytes, suggesting potential processing specificity in those cells. At six months of age, the livers of all transgenic lineages remained histologically normal. We concluded that HCV structural proteins are not directly cytopathic in this animal model.

Interleukin-10 and interleukin-1 receptor antagonists increase during cardiac surgery.

BACKGROUND: It has been reported that inflammatory cytokines such as interleukin-8 and 6 (IL-8, IL-6) increase during cardiac surgery and caused postoperative cardiac dysfunction. Therefore, it is important to investigate changes of suppressive cytokines such as IL-10, interleukin-4 (IL-4) and interleukin-1 receptor antagonist (IL-1ra) during cardiac surgery. METHOD: Serum levels of cytokines and IL-1ra were measured in 10 patients during cardiac surgery with cardiopulmonary bypass. Six blood samples were drawn after inducing anaesthesia. In each sample, serum IL-10, IL-4, IL-8, IL-6 and IL-1ra were measured by enzyme linked immunosorbent assay. RESULTS: Serum IL-6 and IL-8 concentration (19.1 +/- 8.8 pg.ml-1, and 13.4 +/- 5.2 pg.ml-1, preoperatively) increased to 227.5 +/- 191 pg.ml-1 and 81.0 +/- 56 pg.ml-1 at 60 min after declamping aorta (P < 0.01, respectively). Serum IL-10 concentration increased at 60 min after declamping the aorta compared with the preoperative value (from 1.0 +/- 0 pg.ml-1 to 552.0 +/- 158 pg.ml-1 P < 0.001]). Similarly, serum IL-1ra concentration increased from the preoperative value of 1331 +/- 896 pg.ml-1 to 43353 +/- 12812 pg.ml-1 at 60 min after declamping the aorta (P < 0.001). Positive correlations were obtained between IL-10 and IL-8, and between IL-10 and IL-6 (gamma = 0.7, gamma = 0.8, P < 0.001, respectively). CONCLUSION: These findings demonstrate that pro- and anti-inflammatory cytokines increase to maintain their balance during cardiac surgery.

Mizoribine improves renal tubulointerstitial fibrosis in unilateral ureteral obstruction (UUO)-treated rat by inhibiting the infiltration of macrophages and the expression of alpha-smooth muscle actin.

PURPOSE: Several lines of evidence suggest that the infiltration of macrophages and the expression of alpha-smooth muscle actin in myofibroblasts play important roles in the pathogenesis of tubulointerstitial fibrosis. However, the temporal sequence of these pathological changes or the dynamics in the expression of this cytoskeletal molecule in the process of tubulointerstitial fibrosis have not been precisely documented. MATERIALS AND METHODS: We investigated the infiltration of macrophages and the expression of alpha-smooth muscle actin in interstitial fibrosis caused by a unilateral ureteral obstruction (UUO) experimental model. RESULTS: The result showed that the macrophages were immobilized at the interstitium and alpha-smooth muscle actin was up-regulated in myofibroblasts of both cortex and medulla at day 3 when interstitial volume start to increase significantly. The highest expression of alpha-smooth muscle actin was detected at day 5 and the most intense infiltration of macrophages was noted at day 14 while the interstitial volume in renal cortex and medulla continued to increase until day 28. Furthermore, we investigated the effect of mizoribine, an immunosuppressive agent, on the interstitial fibrosis induced by UUO, demonstrating that mizoribine, but not prednisolone, significantly improves the tubulointerstitial fibrosis by suppressing the macrophage infiltration and the expression of alpha-smooth muscle actin. CONCLUSIONS: We discuss the pathological roles of macrophages and alpha-smooth muscle actin in tubulointerstitial fibrosis induced by UUO treatment. We also emphasize the pharmacological basis and clinical relevance of mizoribine in the treatment of interstitial fibrosis caused by obstructive nephropathy.

Expression of prostaglandin EP2 receptor mRNA in the rat spinal cord.

RT-PCR and an in situ hybridization analyses demonstrated expression of prostaglandin EP2 receptor mRNA in the subfield of the dorsal horn of the rat spinal cord. The intensive signals from the cells labeled with digoxigenin-11-dUTP were scattered through this region and they corresponded to neurons. EP2 receptor-mediated signal transduction is expected to play roles in modulating some neuronal functions such as transmission of nociceptive reaction or motor reflex.

Prostaglandin E1 prevents apoptotic cell death in superficial dorsal horn of rat spinal cord.

Neuronal degeneration, distinguished morphologically and biochemically from necrosis, was induced in the dorsal horn of the lumbar spinal cord by chronic constriction of the sciatic nerve in rats. Neuronal apoptosis in the territory of the spinal cord which receives afferent excitatory inputs from the sciatic nerve was confirmed by TUNEL-staining and electrophoresis of genomic DNA. The morphological changes including the appearance of dark neurones, as identified by toluidine-blue staining, were almost completely blocked by 10 microg/kg of the prostaglandin E (EP) receptor agonist lipo-PGE1, incorporating PGE1 in lipid microspheres for chemical stability and targeted delivery, but not by 10 microg/kg of carbacyclin a prostacyclin (IP) receptor agonist. Lipo-PGE1 also blocked the "ladder type" fragmentation of genomic DNA extracted from tissue in the affected area of the spinal cord. Since the regional blood flow in the subfield of the spinal cord was neither influenced by the chronic injury nor by application of the vasodilative prostaglandin, the effect of lipo-PGE1 must have been achieved via another mechanism. These results demonstrate that the neuronal apoptosis in spinal cord induced by sciatic constriction injury is more effectively inhibited by the EP receptor agonist PGE1 than by the IP receptor agonist carbacyclin.

Epidemiological features of moyamoya disease in Japan: findings from a nationwide survey.

To estimate an annual number of patients treated for Moyamoya disease in Japan and to describe the clinico-epidemiological features, a nationwide epidemiological survey was conducted in 1995. The study consisted of two questionnaires, which were distributed to departments randomly selected, of neurosurgery, neurology and pediatrics in hospitals throughout Japan. The first questionnaire inquired the number of the patients treated in 1994 and the second one detailed clinico-epidemiological information of each patient reported. Following major epidemiological findings emerged from the study: (a) The total annual number of patients treated for Moyamoya disease was estimated as 3900 (95% confidence interval (CI) 3500-4400) in Japan 1994, with the prevalence and incidence rates of 3.16 and 0.35 per 100,000 population, respectively; (b) the sex ratio (females to males) of the patients was 1.8; (c) the peak of age distribution of the patients was observed in 10-14 years old and a smaller peak in their forties; (d) the age at onset was under 10 years old in 47.8% of the patients, but some had developed the disease at the age of 25-49 years; (e) family history of Moyamoya disease was found in 10.0% of the patients; and (f) about 75% of the patients had normal activity of daily life or working ability even before treatment. The present findings were quite comparable with those obtained in the previous nationwide epidemiological survey in 1990.

Report on the nationwide epidemiological survey of exudative age-related macular degeneration in Japan.

The research Committee on Chorioretinal Degeneration, in cooperation with the Research Committee on the Epidemiology of Intractable Diseases, performed the second nationwide epidemiological survey of exudative age-related macular degeneration (AMD) in 1993. The number of exudative AMD patients in all ophthalmology departments throughout Japan was estimated to be 14,400 in 1993. This number was estimated to have almost doubled over the six year period since the initial survey in 1987. The number of people visiting ophthalmology departments due to exudative AMD was 53 males and 20 females per 100,000 population aged 50 years or over. The percentage of patients with bilateral disease increased with age and accounted for nearly 50% of those aged 80 years or over.

Numerical and functional characteristics of lymphocyte subsets in centenarians.

The immune system in the aged is a very interesting subject for study. In this study, analysis was extended to extrathymic T cells as well as NK cells and "conventional" T cells (i.e., thymus-derived T cells) in terms of their constitution and function in both healthy and unhealthy centenarians. Middle-aged persons were used as controls. Healthy and unhealthy centenarians showed lower levels in the proportion and absolute number of lymphocytes. The major change in the constitution of lymphocyte subsets was increased levels in the proportion of NK cells (CD56+/CD57+) and extrathymic T cells (CD3+CD57+). Inversely, conventional T cells decreased in proportion and function (i.e., proliferative response to mitogen). Although NK cells increased in centenarians, NK activity by whole lymphocytes and the purified NK fraction decreased. The difference between healthy and unhealthy centenarians was small in all parameters, the only difference being a lower level of expression of CD56 antigens on CD57+ T cells in unhealthy centenarians. These results indicate that there is a major shift in lymphocyte population from conventional T cells to NK cells and extrathymic T cells with aging. Concerning the age-associated increases in CD56+ T and CD57+ T cells, these cells correspond to NK1+ T cells in mice.

Localization of insulin receptor-related receptor in the rat kidney.

Insulin receptor-related receptor (IRR), a member of the insulin receptor family, is most abundantly expressed in the kidney. However, its endogenous ligand and physiological roles are still unknown. To elucidate the physiological role of IRR, an orphan receptor, in the kidney, we examined the localization of IRR mRNA and its immunoreactivity in the rat kidney by in situ hybridization and immunohistochemistry, respectively. IRR mRNA was found to be exclusively localized in the cortical collecting duct. The localization of IRR immunoreactivity was consistent with that of IRR mRNA. Furthermore, IRR immunoreactivity was found to be localized on the basolateral plasma membrane of the epithelial cells that were a minor cell subpopulation (20 to 30%) of the duct. The present findings indicated that IRR in the kidney was exclusively localized on the basolateral plasma membrane of type B intercalated cells of the cortical collecting duct.
