[Multiple malignant tumors combined with oral cancer from annual of the pathological autopsy cases in Japan, 1984-1988].

During the 5-year period from 1984 to 1988, a total of 944 cases had been presented multiple (3 or more) primary malignant tumors in annual of the pathological autopsy cases in Japan. Forty-one cases (4.3%) had lip and oral cancer. Of this group, triple were described 36 cases, quadruple 3 cases and more discrete multiple 2 cases, respectively. It is more frequent in men than in women, the ratio being 12.7:1. The age range is from 34 to 86 years with a mean of 66.9 years. In these cases with oral cancer, the another primary site is almost always found in digestive system (90.2%), e.g. esophagus (36.6%), stomach (34.1%) and colon and rectum (34.1%).

[Coronary revascularization with reduction of serum cholesterol level using LDL apheresis in patient with homozygous familial hypercholesterolemia].

We reported a case of a 48-year-old male with homozygous familial hypercholesterolemia who underwent coronary revascularization successfully. Coronary artery bypass grafting (CABG) utilizing internal mammary artery graft to LAD, and percutaneous transluminal angioplasty for residual stenosis after CABG was able to relieve symptomatic myocardial ischemia. LDL apheresis every two weeks in addition to combined drug treatment had maintained total cholesterol at an acceptable level (120-280 mg/dl) before and after CABG. It was confirmed by repetition of coronary angiography at one year after CABG that all grafts were widely patent, and the native coronary artery did not accelerate the atherosclerotic lesion. It was important in patients with homozygous familial hypercholesterolemia, to carry out active coronary revascularization with reduction of serum cholesterol level by using LDL apheresis.

[Antagonism of diazepam-induced sedative effects by flumazenil in patients after surgery under lumbar spinal or epidural anesthesia].

The efficacy and general safety of flumazenil (YM684), a specific benzodiazepine antagonist, have been evaluated. Forty-seven patients scheduled for surgery under lumbar spinal or epidural anesthesia with diazepam sedation received flumazenil at the end of the procedure. Criteria of efficacy were the degree of sedation and antegrade amnesia. Before injection all patients were heavily sedated with the mean dose of 27 mg of diazepam (range 10-50 mg). After the mean dose of 0.21 mg of flumazenil (range 0.2-0.4 mg), all patients were awake or drowsy within 1-6 min but sedation recurred in one patient 1 hour later. The amnesia was eliminated by flumazenil in 34 patients (72%). No serious side-effects or hemodynamic changes were observed after flumazenil. It is concluded that flumazenil is an effective antagonist of sedation induced by diazepam.

Signal transduction in the phosphate regulon of Escherichia coli involves phosphotransfer between PhoR and PhoB proteins.

PhoB protein is the transcriptional activator for genes in the phosphate regulon of Escherichia coli, such as phoA and pstS, that are induced by phosphate deprivation. PhoR protein activates PhoB when phosphate is limiting and inactivates it when phosphate is in excess. We constructed a plasmid with a mutant phoR gene (phoR1084), which encoded a PhoR protein (PhoR1084) lacking the amino-terminal hydrophobic region of the intact protein. The cells carrying the plasmid overproduced PhoR1084, which was recovered in the soluble fraction of the cell lysate. We purified the Phor1084 protein and showed that it was autophosphorylated in the presence of ATP, and the phosphate group on the protein was rapidly transferred to PhoB. The phosphorylation of PhoB protein occurred concurrently with the acquisition of the ability to activate transcription from the pstS promoter. On the basis of these findings, we propose that phosphorylated PhoB protein activates transcription from the promoters of the phosphate regulon, and that the role of PhoR is to catalyze the formation and breakdown of phosphorylated PhoB in response to phosphate concentrations in the medium.

Identification of a site that mediates transcriptional response of the human beta-actin gene to serum factors.

In quiescent fibroblasts, low-level transcription of the beta-actin gene occurs, and it is rapidly induced by growth factors in serum. Deletion analysis of the 5' upstream region of the human beta-actin gene using a CAT assay showed that the region between +765 and +783 downstream from the cap site, which had enhancer activity in growing NIH3T3 cells, also acted as a serum-response element in quiescent cells. The data suggest that the 19-bp element acts positively as a serum response element. This sequence contains a CC(A/T)6GG sequence which is found in the promoter region of many cytoskeletal and muscle-specific actin genes.

DNA bending and binding factors of the human beta-actin promoter.

Transcription of the beta-actin gene is rapidly inducible in response to serum stimulation. To determine the regions responsible for serum inducible and basal level expression, the human beta-actin promoter was subjected to mutational analysis. Two distinct elements, the CCAAT homology and the beta-actin specific conserved sequences, were found by a chloramphenicol acetyltransferase expression assay and sequence comparisons, and then analyzed for possible functions. Using a DNA bend assay, it was shown that the conserved sequences included the core of a sequence-directed bend of DNA. Gel mobility shift and DNase I protection assays revealed that the conserved sequences and the CCAAT homology were recognized by binding factors in HeLa cell extracts.

Effects of TCDD on the EGF receptor of XB mouse keratinizing epithelial cells.

TCDD was found to cause a marked inhibition of 125I-epidermal growth factor (EGF) binding to its receptor on the cell surface of XB mouse keratinizing epithelial cells (XB cells) cultured in vitro. The EC50 concentration was estimated to be on the order of 3 x 10(-11) M 24 hours after TCDD administration. As early as 12 hours after the addition of 10(-9) M of TCDD, XB cells showed signs of a decline in 125I-EGF binding levels. The level of such EGF receptor downregulation reached a maximum at 24 hours, continued until day 2, but completely recovered by day 3. This was accompanied by a rise in protein kinase activities, particularly those of the protein tyrosine kinases during the initial period of 6-24 hours. To test the hypothesis that the EGF receptors of the cells, by showing TCDD-induced symptoms of downregulation, actually are being activated and triggering EGF-like signals, we examined the effects of both TCDD and exogenously added EGF on cell morphology, colony formation degree of keratinization, the pattern of activation of protein kinases and de novo protein synthesis, and EGF receptor phosphorylation. Based on the similarity of cell responses to these between TCDD- and EGF-treated cells, we concluded that TCDD, directly or indirectly, causes activation of the EGF receptor. In contrast, 12-O-tetradencanoylphorbol-13-acetate (TPA), which is known to downregulate EGF receptors by blocking their protein tyrosine kinase, produced dissimilar end results. The balance of evidence support the notion that the action of TCDD in this cell line is tightly coupled to the activation of the EGF receptor and that one of the key consequences of such a biochemical change is that it signals these cells to commit to terminal differentiation.

Stimulation by transforming growth factor-beta of epidermal growth factor-dependent growth of aged human fibro-blasts: recovery of high affinity EGE receptors and growth stimulation by EGF.

The stimulatory effects of transforming growth factor beta (TGF-beta) on epidermal growth factor (EGF)-dependent growth of adult and newborn human fibroblasts were investigated. EGF-stimulated growth in low serum of dermal fibroblasts from a 41 year-old adult (HSF-41) was less than half that of newborn foreskin fibroblasts (HFF). The EGF-stimulated growth of HFF after 55 population doublings (HFF-55) was similarly reduced. The decreased growth response to EGF of fibroblasts aged in vivo and in vitro appeared to result principally from a decreased sensitivity to EGF due to a decreased number and affinity of high affinity EGF receptors (H-EGFR). Pre-incubation of HSF-41 and HFF-55 with 25 pM TGF-beta enhanced the growth responses of these cells to EGF and increased the levels of high affinity EGF-binding by these cells. Thus, the stimulation by TGF-beta of EGF-dependent growth of human fibroblasts aged in vivo or in vitro is mediated by increased levels of high affinity EGF binding.

Solution syntheses of two enkephalin-containing peptides, peptide E and dynorphin(1-24), using Nin-(2,4,6-triisopropylphenylsulfonyl)tryptophan.

Two enkephalin-containing peptides, peptide E and dynorphin(1-24), were synthesized by conventional solution methods employing a new Trp derivative, Nin-(2,4,6-triisopropylphenylsulfonyl)tryptophan [Trp(Tps)]. All protecting groups employed including the Tps group were removed by treatment with 1 M trifluoromethanesulfonic acid (TFMSA)-thioanisole in trifluoroacetic acid (TFA) at the final steps of these syntheses. Subsequent purifications by Sephadex G-25 chromatography, CM-Biogel A ion exchange chromatography, and reversed-phase high-performance liquid chromatography afforded highly purified samples. Both synthetic peptide E and dynorphin(1-24) exhibited high in vitro opioid activity. The usefulness of this new tryptophan derivative for practical peptide synthesis was established through these syntheses of complex Trp-containing peptides.

The effects of a maternal preparation program on mother-infant pairs: a pilot study.

This quasi-experimental pilot study examined the association of a maternal preparation program with womens' competence in maternal care behaviors, self-perceived adaptation to the maternal role, and satisfaction with the maternal preparation received in conjunction with obstetric and delivery care. Sixteen subjects participated in the program. A cost-benefit questionnaire was completed by the program participants to examine whether the availability of such a maternal preparation program would influence future selections of a hospital for delivery. Our Occupational Therapy Maternal Role Preparation Program was provided to the subjects in four sessions. The program included material on physiological changes in the new mother, orchestration of activities of daily living, infant development and individual differences, and the mother-infant relationship. Results were statistically significant only for the factor of the mothers' satisfaction with their obstetric care and preparation for the maternal role, in favor of the treatment group. In addition, all 8 members of the treatment group reported that they thought the program was helpful and would recommend it to other mothers.

Application of video surgery to orthodontic diagnosis.

The purpose of this project was to develop a practical methodology for predicting the visual facial changes resulting from the movement of anterior teeth. The armamentarium includes an 8-bit audiovisual computer, a video camera, a digitizer, and a cephalogram of the patient. The system outputs the patient's profile on a personal computer video display. The image is superimposed on the soft tissue line of a digitized cephalogram plot on the same display. The video profile image is transformed from analog to digital signals. The profile image digitized by the computer is modified according to average ratios of soft tissue to hard tissue changes induced by modifications to the hard tissues. The program incorporates the spline function to smooth the profile image.

[Lymphoepithelial cyst of the upper neck: report of a case].

A case of lymphoepithelial cyst of the upper neck is presented. The patient was a 77-year-old woman. After her lesion was extirpated under clinical diagnosis of salivary gland tumor, identified as lymphoepithelial cyst (so-called branchial cyst). Histopathologically salivary gland was observed in the cyst wall composed of lymphoid tissue and was demonstrated highly active amylase-enzyme (S type) in the cyst fluid. These findings are convicted that our case originates ductal epithelium entrapped in cervical lymph node (inclusion theory of Bhaskar).

[Case of TMJ arthrosis occurred during orthodontic treatment of patient with unilateral cleft lip and alveolus].

A female patient of fifteen years old with anterior crossbite was treated orthodontically with multi-bracket system. During the orthodontic treatment clicking and trismus were appeared. According to the consultation, it was found that the TMJ disorder was in the stage of the closed lock. The patient was treated with the manipulation technique and the anterior repositioning appliance was applied. After three months, the pain and trismus were relieved. The maxillary arch was expanded laterally to eliminate the premature contact caused by the maxillary and mandibular canines. Consequently, intercanine width of the maxillary arch was increased and the cuspal interference was disappeared. After three month observation, it seemed that the symptoms of TMJ were recovered. Then, orthodontic appliances were removed and the retainers were set. It was concluded that the examination of TMJ condition may be necessary for orthodontic patients with anterior crossbites.

[An eighteen-year evaluation of the Bjork-Shiley valve prosthesis].

We evaluated the long-term (18 years) results of 356 patients undergoing valve replacement with Björk-Shiley valve prosthesis (aortic, 212; mitral 120; double valve, 24) between 1970 and 1988. Actuarial survival rates were 90% (18 years) for AVR, 80% 'years) for MVR and 90% (8 years) for DVR. Actuarial rates of thromboembolism were 99% (18 years) for AVR, 98% (8 years) for MVR and 94% (8 years) for DVR. Actuarial rates of freedom from events (including valve failure, thromboembolism, reoperation and prosthetic valve endocarditis) were 82% (18 years) for AVR, 95% (8 years) and 94% (8 years) for DVR. There were no differences in these results among spherical disc, convexo-concave disc and monostrut valve. In conclusion, this study demonstrated that Björk-Shiley valve showed a low incidence of postoperative events. These results endorse our choice of the Björk-Shiley.

[Extended aortic arch anastomosis for aortic coarctation in neonate and early infancy--experience in 5 patients].

The aortic coarctation (CoA) was repaired with extended aortic arch (AA) anastomosis in 5 consecutive patients, 2 in neonate and 3 in infancy, ranging in age from 3 to 106 days. The associated lesions were ventricular septal defect in 2, atrial septal defect in 2 and complete atrioventricular canal in 1. Patent ductus was found in 4 and prostaglandin E1 was required in 2 with response in 1 patient. The AA was hypoplastic in 2 patients with arch to descending aorta diameter ratio below 0.5. The CoA repair was performed as the first procedure in all. The primary anastomosis was possible in all between AA and the descending aorta without undue tension after removal of the all ductus tissue and extensive mobilization of the aorta. Aortic clamp time was 28 minutes in average. The intraoperative pressure gradient after repair across AA was 0 to 10 mmHg (average 4.6 mmHg). All except 1 with preceding hepatic dysfunction tolerated the procedure without complication or operative death. Postoperative evaluation at 1 to 11 months showed no significant pressure gradient between upper and lower extremities. This method appears to be useful for CoA in early infancy particularly when AA is hypoplastic.
