[An alternative technique for coronary reattachment in aortic root replacement--the "shell" button technique].

A modified technique to the reattachment of the coronary ostia in the aortic root replacement is described. The left coronary ostium is excised semicircumferentially in an aortic button with its subostial quarter remained as a joint. A collagen impregnated 10-mm Dacron interposition tube graft is sutured rectangularly to the face of the button with the interrupted pledgeted horizontal mattress sutures all running through the button wall in outside-in fashion. The other end of the graft is beveled and sutured end-to-side cephalad to the aortic conduit in order to join each other with straightness assuring the distance between the right coronary button and the aortic conduit to be anastomosed directly. The left coronary button can incline passively with the long axis of the coronary graft, like a "shell", and this allows equivalent distribution of tension on the anastomosis and a straightness of the graft to create the laminar flow in it. The right coronary button is anastomosed to the aortic conduit opposite the ostium with a running over-and-over suture. This procedure has been used in a 35-year-old male with annuloaortic ectasia caused by Marfan's syndrome complicated with type A acute aortic dissection. The patient discharged on the 26th postoperative day without any complications nor angiographic problems. We found this procedure useful in terms of the security of the coronary anastomosis and geometrical arrangement of the conduits.

[Clinical evaluation of sulbactam/cefoperazone for lower respiratory tract infections. Correlation between the efficacy of sulbactam/cefoperazone and beta-lactamase].

The efficacy and safety of sulbactam/cefoperazone (SBT/CPZ) were evaluated in 42 patients with respiratory tract infections, including pneumonia (29 patients) and lower respiratory tract infections (5 patients). Overall clinical efficacy rates (excellent + good) were 79% in pneumonia and 80% in respiratory tract infections in 34 patients evaluated for clinical efficacy. It was excellent that the clinical efficacy rate was 92% in mild and moderate pneumonia. Pathogens isolated from sputa were 31 strains, including 8 strains of Pseudomonas aeruginosa, 7 of Streptococcus pneumoniae, 3 of Staphylococcus aureus and 3 of Haemophilus influenzae. Since the isolates were eradicated in 18 strains, replaced in 3, unchanged in 2 and unknown in 8, the overall eradication rate was 91%. The eradication rates were 89% in beta-lactamase producing strains and 100% in beta-lactamase positive sputum, and excellent or good in 19 (83%) of 23 patients with beta-lactamase negative sputum. The eradication rate was 88% in 5 patients with beta-lactamase positive sputum. One patient experienced a moderate rash. Abnormal laboratory test values were observed in 10 patients (26.3%), but these abnormalities were mild and transient. These results suggested that SBT/CPZ was effective and safe for the treatment of respiratory tract infections caused by beta-lactamase producing as well as beta-lactamase non-producing bacteria.

Expression of sialyl Lewis(a) as a new prognostic factor for patients with advanced colorectal carcinoma.

BACKGROUND: Sialyl Lewis(a) antigen (SLA) is considered to be a cancer-associated carbohydrate antigen. METHODS: A total of 309 surgically resected primary colorectal cancer specimens and 501 associated metastases to regional lymph nodes were analyzed immunohistochemically using a monoclonal antibody (NS19-9) recognizing SLA. The specimens were obtained from 1981 to 1988 at the Department of Surgery, Keio University School of Medicine (Tokyo, Japan). RESULTS: Sialyl Lewis(a) antigen expression was detected in 75.4% (233/309) of the primary tumors and in 78.5% (99/126) of the metastases to regional lymph nodes. Significantly stronger expression was noted in the regional lymph node metastases than in the primary lesions (P = 4.5E-7). Sialyl Lewis(a) antigen expression in the primary tumors correlated significantly with regional lymph node metastasis (P < 0.005), recurrence (P < 0.005), and postoperative survival (P < 0.001) as determined by univariate analysis. Sialyl Lewis(a) antigen expression in the regional lymph node metastases also correlated strongly with recurrence (P < 0.005) and survival (P < 0.01). As determined by multivariate analysis, SLA expression in the primary tumor correlated strongly with recurrence (P = 3.0E-6) and survival (P = 6.9E-3). CONCLUSIONS: Sialyl Lewis(a) antigen expression in primary tumors and metastases to regional lymph nodes is a useful marker for evaluating tumor aggressiveness and prognosis in patients with advanced colorectal cancer.

Transcriptional activity of a fluorinated vitamin D analog on VDR-RXR-mediated gene expression.

The transcriptional activity of the hexafluorinated derivative of 1,25-dihydroxyvitamin D3 [1,25-(OH)2D3], 26,26,26,27,27,27-hexafluoro-1,25-dihydroxyvitamin D3 [F6-1,25-(OH)2D3], was examined in cultured cells by a transient expression assay (CAT assay) using expression vectors for the rat nuclear vitamin D3 receptor (VDR) and the rat 9-cis-retinoic acid receptor (RXR beta), and a reporter plasmid containing a consensus vitamin D3 response element (VDRE) consisting of two directly repeated AGGTCA motifs spaced by 3 bp (DR3). At physiological concentrations, the transcriptional activity of F6-1,25-(OH)2D3 was 2-4 times more potent than that of 1,25-(OH)2D3 in both nontarget (HeLa) and target (UMR106) cells for 1,25-(OH)2D3. The transcriptional activity of F6-1,25-(OH)2D3 was also higher when the endogenous target gene (osteopontin), which has a VDRE related to the DR3 in its promoter, was induced. A gel-shift assay using DR3 as a probe and in vitro synthesized receptors showed that the ligand-induced DNA binding of VDR required RXR to form a heterodimer. Moreover, in this assay we found that F6-1,25-(OH)2D3 induced the receptor-DNA complex at a 10-fold lower concentration than 1,25-(OH)2D3 without influencing the dissociation kinetics. However, the binding affinity of F6-1,25-(OH)2D3 for VDR was slightly lower than that of 1,25-(OH)2D3. The increased DNA binding of ligand-bound VDR by introducing hexafluorines into 1,25-(OH)2D3 may potentiate the transcriptional activity. Thus, the higher biological activity of F6-1,25-(OH)2D3 may be exerted at least in part by enhanced transcriptional activity.

Intermittent cyclical etidronate treatment maintains the mass, structure and the mechanical property of bone in ovariectomized rats.

We examined the mechanical properties of rat bones treated with etidronate intermittent cyclic treatment (etidronate-ICT). Fifty Fisher rats, 7 months of age, underwent ovariectomy (OVX; n = 40) or sham operation (n = 10). The OVX rats were assigned to 4 groups injected with etidronate at the respective dose of 0, 2, 4, or 8 mg/kg of body weight (bw) 5 days each week for 2 weeks, followed by a 10-week period of no treatment. This regimen was repeated for 48 weeks. At the end of the treatment period, the ultimate bending strength and structural stiffness of the femoral midshaft in the OVX-alone group were not reduced compared with the values of the sham group, while those in the etidronate-treated groups were significantly larger than the values of the OVX group. The compressive stiffness and strength of the vertebral bodies specimens prepared from L3 and L5 vertebrae were markedly decreased in the OVX group compared with those in the sham group, but the values in the etidronate-treated groups were maintained at the same levels as those in the sham group. Mechanical properties at the tissue level, such as elastic modulus and material strength of the femoral cortex, were increased in etidronate-treated groups compared with the OVX group. The flexural modulus was also increased. The compressive elastic modulus of vertebral body in treated groups was preserved at the same level as that in the sham group. These results clearly demonstrate that in ovariectomized rats long-term etidronate intermittent cyclic treatment preserves the mechanical properties both at the whole bone and the tissue level.

Post-stenting enlarging false aneurysm of a saphenous vein graft.

Stenting seems to be a definitive procedure after failed balloon coronary angioplasty. This report describes a case of redo coronary bypass grafting and concomitant resection of enlarging false aneurysm of a saphenous vein graft that developed secondary to stenting for recurrent stenosis after serial percutaneous transluminal coronary angioplasty. It warns us of a pitfall in catheter intervention in an aged saphenous vein graft.

Development of an autoflow cruise control system for a centrifugal pump.

To improve the ease of driving a centrifugal pump that is afterload dependent, we have developed an automatic flow control system for the Terumo Capiox centrifugal pump system. This system consists of an autoflow cruise control system with a safety cutoff. The Capiox Pump Console 3000 was controlled by a personal computer through a serial communication line. In the usual manual mode, the motor speed knob works as a pump speed control, and in the autoflow mode, the same knob works as a blood flow rate control. After selecting and obtaining the desired flow rate, the mode was changed from manual to autoflow mode. In the autoflow mode, the computer compares the desired flow rate with the actual flow measured by an ultrasonic Doppler flowmeter and adjusts the motor rotational speed accordingly. During both in vivo and in vitro testing, this autoflow mode was able to return the changed flow that was disrupted by either clamping and declamping of the tubing or by the bolus injection of a vasomotor drug to the selected flow rate within 10 s without any significant fluctuation. In conclusion, the newly developed computer controlled autoflow system was able to produce a reliable and effective flow regulation for a centrifugal pump.

Temporal thresholds of reperfusion in the middle cerebral artery occlusion model in rats.

In this study, the middle cerebral artery (MCA) of adult male Sprague-Dawley rats was occluded by the modified Koizumi method to determine the temporal thresholds of reperfusion for the treatment of cerebral embolism. Regional cerebral blood flow (rCBF) and pathological findings were measured at 1 and 2 h of ischemia and after 24 h and 7 days of reperfusion following 1 or 2 h of ischemia. rCBF was decreased the most (less than 10% of control CBF) in the parietal cortex (Pcor) and the lateral caudoputamen (Lcp) at both 1 and 2 h of ischemia. There was no significant difference in rCBF in these areas between the 2 ischemic groups. The 2 h ischemia group clearly showed infarction in the area perfused by the middle cerebral artery (including the Pcor and Lcp) after 24 h and 7 days of reperfusion, while the 1 h ischemia group showed only slight infarction. These findings suggest that temporal thresholds of reperfusion in this model exist between 1 and 2 h of ischemia, and that rCBF levels during ischemia and the duration of ischemia are the most important factors in producing brain infarction.

Current strategy of circulatory support for profound heart failure.

The purpose of this study is to assess the current strategy of mechanical circulatory support for profound heart failure. In the last 10 years, 37 patients with profound heart failure underwent mechanical circulatory support after open heart surgery and 9 patients with non-cardiotomy cardiogenic shock received emergency circulatory support. All patients showed severe cardiac failure and/or fatal ventricular arrhythmia and required circulatory support as a life-saving measure. After cardiovascular surgery, 12 of those patients underwent venoarterial bypass (VAB), 13 had biventricular bypass (BVB), 8 had left ventricular bypass (LVB) and the remaining 4 patients received left ventricular assist device (LVAD). And 9 patients with non-cardiotomy cardiogenic shock received percutaneous cardiopulmonary support (or PCPS) as an emergency assist system. Weaning and discharge rates of the patients by the type of circulatory supports were 41.7% and 25.0% with VAB, 69.3% and 46.2% with BVB, 87.5% and 37.5% with LVB, 75.0% and 50.0% with LVAD, and 44.4% and 11.1% with PCPS, respectively. Clinical results of post-cardiotomy circulatory support (64.9% of weaning and 37.8% of discharge) were acceptable, but the patients with non-cardiotomy cardiac failure needed early application of more advanced circulatory support.

[Geometrical study on intra-arterial aortoinfundibuloplasty].

Newly-developed procedure for aortic annular enlargement was geometrically examined on its optimal annular augmentation. Valve prosthesis is accommodated by anterior annular split extending into aortopulmonary and infundibular septum and, eventually, protrudes into pulmonary valvular orifice in this procedure. Relationship between residual pulmonary valvular area (PVA) and aortic annular incremental radius (x) is given by: PVA = (r+x)2 [pi - 2 pi x/(r+x) + sin (2 pi x/(r+x))], where r is original aortic annular radius. Optimal aortic annular augmentation maximizing PVA and saving original PVA is 1.33 times and 1.70 times of original diameter respectively. Animal experiments employing six mongrel dogs demonstrated no significant increase of systolic peak pressure gradient in right ventricular outflow tract after the procedure standardized by this geometrical idea. Our procedure would bear radical enlargement of small aortic annulus.

[Experimental study on organ perfusion by intraaortic balloon counterpulsation with double balloon catheter].

Newly-developed catheter for the intraaortic balloon counterpulsation with cardio-renal assist was examined on animal model. The catheter consists of two balloon segments between which the celiac trunk, the superior mesenteric artery and the renal artery are to be placed. Statistical comparison on each hemodynamic element was carried out between the catheter (DBC), conventional single balloon catheter (SBC) of the same balloon volume as DBC and no assist (None) groups. There were no significant changes between SBC and DBC in cardiac output, femoral arterial flow and central venous pressure. Renal arterial flow and urinary output were significantly increased in DBC (None 14.4 +/- 3.9, SBC 22.6 +/- 5.6, DBC 31.0 +/- 6.9 ml/min in renal arterial flow, None 33 +/- 9, SBC 64 +/- 14, DBC 78 +/- 20 ml/h in urinary output, p < or = 0.05). After the remodeling adjust to the human stature, a clinical application would be considered.

Prolonged alkylcatechol-induced expression of c-jun proto-oncogene followed by elevation of NGF mRNA in cultured astroglial cells.

We have already shown that alkylcatechol markedly enhances synthesis/secretion of nerve growth factor (NGF) in cultured mouse fibroblasts and astroglial cells through immediate accumulation of NGF mRNA and that the stimulatory effect of alkylcatechol on NGF synthesis/secretion is synergistically enhanced by the coadministration of phorbol 12-myristate 13-acetate (PMA). The stimulatory effect on NGF mRNA expression of astroglial cells in culture by 4-methylcatechol (MC), an alkylcatechol, and/or PMA was blocked by treatment of the cells with cycloheximide, suggesting de novo synthesis of some cellular protein(s) is essential for the observed increase in the NGF mRNA level. The exposure to MC and/or PMA caused a rapid increase in c-fos mRNA content, which was immediately followed by an increase in c-jun mRNA, prior to NGF mRNA elevation. The expression of c-fos mRNA was transiently enhanced in all cases of the treatment with MC and/or PMA. The c-jun mRNA expression was also observed transiently when the cells were treated with PMA alone, while the expression of c-jun mRNA was pronounced and long-lasting after the treatment with MC, which was much further enhanced by the coadministration of PMA. The result that the profile of the change in c-jun mRNA expression resembled that in NGF mRNA expression suggests that the increase in c-jun mRNA is responsible for the subsequent increase in NGF mRNA after MC treatment. The contransfection of mouse astroglial cells with expression plasmids of c-fos and/or c-jun and NGF promoter gene showed that simultaneous expression of both c-fos and c-jun genes was necessary to enhance NGF promoter activity.(ABSTRACT TRUNCATED AT 250 WORDS)

Characterization of the zona pellucida glycoproteins from bovine ovarian and fertilized eggs.

Bovine zona pellucida (ZP) glycoproteins from ovarian egg emerged as three bands with molecular mass of 78 kDa, 64 kDa and 21 kDa in SDS-PAGE under reducing conditions. Endo-beta-galactosidase (E beta G) digestion of the glycoproteins yielded five products with molecular mass of 76 kDa (E beta G-76), 68 kDa (E beta G-68), 63 kDa (E beta G-63), 47 kDa (E beta G-47) and 21 kDa (E beta G-21) under the same conditions. The N-terminal amino acid sequences of E beta G-76 and E beta G-21 were identical. This fact together with the results of diagonal SDS-PAGE indicated that E beta G-21 (N-terminal region) is linked to E beta G-63 (C-terminal region) through disulfide bond to form E beta G-76. Immunoblot analysis using anti-pig ZP protein antibodies revealed that bovine E beta G-76, E beta G-68 and E beta G-47 correspond to pig PZP2, PZP3 alpha and PZP3 beta glycoproteins, respectively. The E beta G-76 and E beta G-68 components were shown to be specifically cleaved during fertilization.

Interactions among four subunits of elongation factor 1 from rice embryo.

To establish the subunit construction of elongation factor EF-1, interactions among four non-identical subunits of rice embryo EF-1 (alpha, beta, beta', and gamma) were analyzed with polyacrylamide gel electrophoresis. Complexes beta beta', alpha beta, alpha beta', and beta gamma were formed by mixing the two respective subunits. However, no complex was formed between EF-1 beta' and EF-1 gamma. Complexes containing three subunits like alpha beta beta', alpha beta gamma, and beta beta' gamma, were formed by mixing the three respective subunits. EF-1 was reconstructed when each subunit was added in the following order, beta, beta', gamma, and alpha. The affinity of EF-1 alpha for other subunits was as follows, beta beta' gamma > beta beta' > beta not equal to beta'. Likewise, the affinity of EF-1 gamma for other subunits was: beta beta' gamma > beta >> beta'. Phe-tRNA binding activity of the reconstructed EF-1 was about 90% of that of the native EF-1. From these results, we concluded that rice embryo EF-1 is constructed of equimolar amount of four subunits, alpha, beta, beta' and gamma.

Comparative study of five types of IABP balloons in terms of incidence of balloon rupture and other complications: a multi-institutional study.

The Multi-Institutional IABP Balloon Study Group in Japan (14 institutions) conducted a retrospective nonrandomized study to elucidate the incidence and type of IABP balloon-related complications relative to design and durability of five different clinically available balloons: TMP (n = 960), Kontron (n = 943). Datascope (n = 485), Mansfield (n = 226), and Aries (n = 189) balloons. A total of 2,803 patients (1,973 males, 830 females, mean age 62.1 years) spent a total of 243,856 h on the pump. Forty-nine balloons (1.7%) ruptured as recognized by the appearance of blood in the catheter (39 cases) or console alarm (4 cases). Ten patients required surgical removal of the balloon due to entrapment. Other IABP balloon-related complications requiring surgical intervention or with a lethal outcome occurred in 89 patients (3.2%). They included lower limb ischemia (61 cases), hematoma (11 cases), extensive dissection (6 cases), perforation (5 cases), entrapment without balloon rupture (3 cases), and mesenteric infarction (3 cases). The incidence of rupture, other major complications, and total complications, respectively, for each balloon was 0, 2.7, and 2.7 for TMP, 1.6, 4.3, and 5.9% for Kontron, 4.1, 1.9, and 6.0% for Datascope, 1.3, 2.7, and 4.0% for Mansfield, and 5.8, 3.7, and 9.5% for Aries. In conclusion, the TMP balloon demonstrates a significantly lower rate of rupture while the incidence of other complications for the 5 balloons is not significantly different.

[A case report: reopacified thrombosed dissection of Stanford type A].

A 60-year-old male, who had been hospitalized with a Stanford type B acute aortic dissection 3 years ago, was admitted with severe disquiet caused by discomfort of the chest and the neck. Enhanced CT scans demonstrated the ascending aorta compressed by non-opacified false lumen, so-called "thrombosed dissection" or "closing aortic dissection", and normal descending aorta. Aortography showed no intimal tear in the ascending aorta and aortic arch. "Hibernation therapy" with pentobarbital was performed to be sedated over during 13 days. 29 days later follow-up CT scans showed reopacified false lumen and enlargement of the ascending aorta, although he had no complaints during the medical therapy. Large intimal tear just under the brachiocephalic artery and giant false lumen without reentry occupying the ascending aorta was found by the repeat aortography. Graft replacement of the ascending aorta was successfully performed. This case suggested that thrombosed dissection might have the risk of recanalization of clotted false lumen asymptomatically, and it would be important to take care of radiographic changes during the medical therapy even if the patient had no symptoms.
