[Comparison between continuous arterial infusion chemotherapy and mitomycin C microcapsule administration in primary and metastatic liver cancer].

Continuous hepatic arterial infusion chemotherapy (HAI) and chemoembolization by microcapsulated Mitomycin C (MMC-m. c) were performed in patients with unresectable hepatoma or metastatic tumors of the liver. MMC-m. c showed significant antitumor effect and improvement of survival rate in unresectable hepatoma and liver metastasis of the breast cancer. Especially in liver metastasis of the breast cancer, MMC-m. c gained 80% of partial response rate and 11 months in 50% survival time. HAI was effective in multiple liver metastasis of colon and stomach cancer, showing 40% of one-year survival rate. A combination chemotherapy of HAI and MMC-m. c was performed in two cases of liver metastasis of the colon cancer. Tumor response was 100% in partial response rate. One of them died at 8 months and the other is alive at 8 months now.

[Characterization of two choliocarcinoma cell lines and their sensitivity to MTX].

Both the sensitivity to MTX and hCG secretion were compared in two choriocarcinoma cell lines (BeWo and HCCM-5) and study on mechanisms about MTX-resistance was described. The uptake of nucleotides and protein precursors was decreased in dose-dependent manner in both cell lines when exposed to MTX, but HCCM-5 cells showed more sensitivity to MTX. In HCCM-5 cells, both hCG and hCG-beta levels in the medium were decreased with exposure to 10(-9)M MTX, but they tended to increase when exposed to higher concentration of MTX in contrast to the inhibition of 3H-thymidine uptake. However, in BeWo cells, an inverse relationship between the incorporation of 3H-thymidine and secretion of hCG was not clearly observed. The content of dihydrofolate reductase (DHFR) in HCCM-5 cells was about one-half of that in BeWo cells. Although the peak of 3H-MTX incorporation was observed at 120 minutes in both cell lines, HCCM-5 cells incorporated about 4 times higher 3H-MTX than BeWo cells. The difference of sensitivity to MTX between those two cell lines seems to be concerned with both the decrease of MTX transport and increase of intracellular DHFR levels.

[Case of basal cell adenomas arising from the bilateral parotid glands].

A 64-year-old woman with basal cell adenomas of the bilateral parotid glands is presented. The tumor mass on the right side measured 5.5 X 5.8 cm; on the left it was 6.0 X 7.5 cm. Histologically, the right side tumor was of trabecular type, that of the left, tubular type of basal cell adenoma. The latter lesion had been present for the past 30 years, the former for six years. This may be the first reported case of bilateral basal cell adenomas of the parotid gland.

Postnatal development of circadian rhythm in serum cortisol levels in children.

High-pressure liquid chromatography was used to study the development of blood adrenocortical circadian rhythm in a total of 64 children, ranging in age from 1 month to 15 years. Patients with endocrine diseases, congenital anomalies, and diseases of the central nervous system were excluded from this study. Determination of corticosteroid concentration was possible with 20 to 100 microL of serum. Twenty-four hour patterns were determined at six-hour intervals. A distinct circadian rhythm with an amplitude comparable to that of an adult emerged at approximately 6 months of age.

Histopathologic studies of undifferentiated carcinoma of the parotid gland.

Five-hundred-fifty-five primary epithelial tumors of parotid gland origin were examined. Eighteen cases could be defined undifferentiated carcinomas, constituting 3.2% of the total number or 10.2% of 176 malignant tumors. The undifferentiated carcinomas could be divided on the basis of the cell size into two types: (1) Small-cell type (12/18; 66.7%); the tumor cells were as large as, or slightly larger than lymphocytes. The cytoplasm was scanty with dark chromatin. Mitotic figures were numerous. The tumor cells in the periphery of the cell clusters exhibited a palisading arrangement. Electron microscopy revealed that these cells were rich in tonofilaments and connected by desmosomes. (2) Large-cell type (6/18; 33.3%); the carcinoma cells were roughly twice as large as those of the small-cell carcinoma. The tumor cells were spheroidal or spindle in shape with rich and clear cytoplasm. Many of the tumor-cell clusters showed macroalveolar structures, suggesting the possibility that this type occurred as a variant of poorly differentiated adenocarcinoma or epidermoid carcinoma. Clinicopathologically, there was no sex difference in the total number of the undifferentiated carcinomas, but the large-cell carcinomas were frequently encountered in females (83.3%). As to the age distribution, the carcinomas were found frequently in middle-aged patients from 30-49 years (66.7%). Many of the tumors were 2-4.9 cm at their greatest diameter (55.6%). Tumor mass less than 1.9 cm could not be found.

Histopathologic studies of basal cell adenoma of the parotid gland.

Five-hundred-thirty-one cases of primary epithelial tumors of the parotid gland were examined. The incidence of basal cell adenomas was 7.5% (40 cases). The tumors could be classified histologically into these subtypes: basal cell (21/40; 42.5%); tubular, (13/40; 32.5%); trabecular (4/40; 10.0%); and papillary (2/40; 5.0%). The pattern of classic basal cell adenoma predominated in the basal-cell type but was also seen in varying degrees in the latter three types with a reciprocal transition. Accordingly, it could be suggested that tubular, trabecular, and papillary types are variants of basal cell adenoma. The higher incidence of basal cell adenomas in our survey could be accounted for by this categorization. Histologically, basal cell adenomas presented various features; (1) cystic formation (26/40; 65.0%), being most frequently seen in tubular and trabecular types; (2) adenoid cystic pattern (4/40; 10.0%); (3) elastosis in the stromal tissues (2/40; 5.0%). Basal cell adenomas were clinicopathologically compared with 291 cases of pleomorphic adenomas. Basal cell adenomas were seen more often in female patients, (60.0%), as were pleomorphic adenomas (68.4%). They were observed more frequently in patients over 50 years of age and the average was ten years higher than for pleomorphic adenoma. The tumor size tended to be smaller (below 2 cm at the greatest diameter) than pleomorphic adenomas.

D-lactic acidosis in two patients with short bowel syndrome: bacteriological analyses of the fecal flora.

Two cases of D-lactic acidosis associated with short bowel syndrome are described. The administration of kanamycin to the patients showed a decrease in D-lactate in blood and urine in parallel with disappearance of metabolic acidosis. Bacteriological analyses of the fecal flora showed an increase in Lactobacillus buchneri in the first patient and Lactobacillus fermenti IVa in the second; both bacteria were sensitive to kanamycin. Quantification of in vitro production of D-lactate by each species of bacteria isolated from the feces revealed that Lactobacillus produced more D-lactate than other species of bacteria. These observations indicate that Lactobacillus may play an important role in the induction of D-lactic acidosis in patients with short bowel syndrome.

The cellular electrophysiologic mechanism of the dual actions of disopyramide on rabbit sinus node function.

To determine the contribution of disopyramide's suggested opposing direct depressant and indirect acceleratory actions on sinus node function, we studied the effects of disopyramide, 1 x 10(-7) to 1 x 10(-4) M, on isolated rabbit sinus node preparations using standard microelectrode techniques. Transmembrane potentials were recorded simultaneously from the sinus node and adjoining crista terminalis area. Disopyramide, as much as 1 x 10(-5) M, had no effects on the sinus cycle length. At a concentration of 1 x 10(-4) M, sinus cycle length was significantly prolonged due to prolongation of the sinus nodal action potential duration. During cholinergic blockade with atropine, 1 x 10(-6) M, disopyramide, 1 x 10(-7) to 1 x 10(-4) M, significantly prolonged sinus cycle length as a result of a prolongation of the sinus nodal action potential duration and a decrease of the slope of phase 4 depolarization. During cholinergic stimulation with carbamyl choline, 1 x 10(-9) M, disopyramide, 1 x 10(-7) to 1 x 10(-6) M, tended to reverse carbamyl choline-induced prolongation of the sinus cycle length (NS). This acceleratory action of disopyramide was caused by a significant increase of the slope of phase 4 depolarization. Disopyramide, 1 x 10(-7) to 1 x 10(-4) M, had no significant effects on corrected sinus node recovery time or on sinoatrial conduction time under any conditions studied. We conclude that disopyramide has a direct depressant action on normal sinus node cells at the upper therapeutic and toxic levels, which is enhanced during cholinergic blockade, and that disopyramide's acceleratory action appears only at much lower concentrations and only during cholinergic stimulation.
