[A case report of extragonadal germ cell tumor with special reference to VAB-6 therapy].

A 24-year-old man with a large abdominal mass in addition to the left lymph node swelling was referred to our clinic. Neither testicular nodulus nor overt evidence of tumor development in the scrotal cavity was confirmed by careful palpation and testicular biopsy. Values of lactate dehydrogenase, alpha-fetoprotein, human chorionic gonadotropin (HCG) and beta-HCG in the serum were markedly elevated. The specimen removed from the left cervical lymph node demonstrated mixed type of choriocarcinoma and embryonal carcinoma. A partial response near complete remission was achieved following the VAB-6 combination therapy (cyclophosphamide, vinblastine, actinomycin D, bleomycin, cis-platinum). The patient has been in good health with normal tumor markers and abdominal CT findings.

Novel immunochemical characteristics of glutathione S-transferase isozymes of rat liver cytosol.

Reevaluation of the immunochemical relationships among the individual glutathione S-transferase (GST) isozymes (GSTs 1-1, 1-2, 2-2, 3-3, 3-4, 4-4, and GSTs with isoelectric points of 7.5 and 6.8) of rat liver cytosol was performed utilizing the immunoblot technique. As a result, we found that the respective isozymes of two isozyme classes of rat liver cytosol might possess a common epitope(s) which has been undetected by the Ouchterlony double-diffusion method. The assumption was further supported by the results of the effects of Fab' prepared from some anti-GST antibodies on the enzymatic activity of GSTs.

[A case report of crossed renal ectopia with renal stone].

Crossed renal ectopia is a relatively rare renal anomaly. A 49-year-old man with macrohematuria and right flank pain was admitted to our hospital. Various urological examinations were carried out, and a diagnosis of left crossed renal ectopia with renal stone was made. Pyelolithotomy was performed and stone analysis revealed a calcium oxalate. In Japan, 166 cases of crossed renal ectopia have been reported and 15 of them including our case were associated with urinary tract stones.

Hyperplasia of juxtaglomerular cells and renomedullary interstitial cells after renal arterial embolization in patients with renal cell carcinoma.

Renal tissue was obtained from 36 patients with renal cell carcinoma, some of whom received renal arterial embolization. The removed specimens was examined histopathologically and the concentration of some vasoactive substances in these patients was measured. Nephrectomy alone produced no discernible changes in blood pressure, vasoactive substances determined or histopathological findings of the kidney. Renal arterial embolization raised the blood pressure in association with the elevation of plasma renin activity (PRA) and urinary prostaglandin (PG) E2 excretion. A linear relationship was found to exist between PRA and mean blood pressure (r = 0.70, p less than 0.001). Hyperplasia of the juxtaglomerular (JG) apparatus, and high granularity of sudan black B granules in renomedullary interstitial cells were confirmed in removed kidneys of patients who had received embolization alone. Subsequently high renin production would be anticipated to influence overproduction of renal PG E2 in acute ischemic kidney in patients with renal cell carcinoma, and hypertension following renal arterial embolization appears to be caused by the hyperplasia of the JG apparatus.

A case of large intestinal malignant lymphoma.

Primary malignant lymphoma of gastrointestinal tract is relatively rare and the most of it are seen in stomach or small intestine, and in Japan only 130 cases of primary large intestinal malignant lymphoma were reported from the accumulating results of the postoperative cases in the 11th Congress of the Japanese Research Society for Cancer of the Colon and Rectum. This paper describes the case report of the primary malignant lymphoma originated from the cecum, and the review of the literature. The patient was 63 year-old female, who came to this hospital for slight fever and right lower abdominal pain that was gradually increasing. After the investigation by using barium enema and the intrapelvic CT, cecum tumor was detected. The ileocecal excision was performed, and revealed the 4 X 4.5 cm tumorous type lesion of which surface was slightly irregular. Histopathologically the tumor was follicular lymphoma (partial type), medium sized cell type by the Lymphoma-leukemia Study Group (LSG) classification. After discharge, cyclophosphamide was administered by 100 mg/day for six weeks, and the sign of the recurrence has not been observed.

Hyperbaric oxygenation for experimental bladder tumor. II. Hyperbaric oxygenation in combination with chemotherapy in N-butyl-N-(4-hydroxybutyl)nitrosamine-induced bladder tumors.

Two series of experiments were conducted to ascertain whether hyperbaric oxygenation (HBO) treatment with or without antineoplastic chemotherapy can really suppress tumor development in N-butyl-N-(4-hydroxybutyl)nitrosamine (BBN)-induced bladder tumor in rats. HBO treatment of 2 ATA of air saturating with 30-35% of oxygen for 90 min daily plus 0.5 mg/100 g body weight of 1-(4-amino-2-methyl-5-pyrimidinyl)methyl-3-(2-chloroethyl)-3-nitrosourea hydrochloride (ACNU) administration for 5 times suppressed the neoplastic spread of the bladder for 9 weeks and reduced the bladder weight in BBN-treated rats. Similar effects were also obtained in ACNU-treated rats, but to a lesser extent. HBO treatment associated with or without doxorubicin failed to suppress tumor growth in BBN-treated animals. Based on these findings combined with the previous study (part I), HBO treatment associated with ACNU might be available for the treatment of some cases of bladder tumors.

[Ectopic ureteral opening with common urogenital sinus. Review of the literature of 648 cases in Japan].

A 3-year-old girl was admitted with continuous enuresis. Urological examination and operation disclosed the left upper ureter ectopically opened into the vagina, and the vagina opened into the urethra (common urogenital sinus). Vesicoureteroneostomy was performed, and after the operation, incontinence disappeared. The 648 cases reported in Japan were discussed.

A novel role of fatty acid-binding protein as a vehicle of retinoids.

Intracellular transport and storage of retinoids were shown to be conducted by fatty acid-binding protein (FABP). When rat liver cytosol was gel filtrated, retinyl palmitate-binding activity was mainly eluted in the fraction with a Mr. of around 14,000, in which both FABP and cellular retinol-binding protein (CRBP) co-existed. From the binding analysis of purified FABP and CRBP to retinyl palmitate, FABP was found to have a relatively high affinity (Kd = 1.4 X 10(-6) M) to retinyl palmitate, while binding of retinyl palmitate to CRBP was scarcely detectable. By using anti-FABP serum, it was shown that FABP was distributed in organs relating to absorption and storage of retinoids, such as jejunum, ileum, and liver. In liver, the protein was localized in the parenchymal cells and with particularly high concentration in the perisinusoidal cells, probably fat-storing cells.

Clinical analysis of ureteral tumours with or without renal pelvic neoplasms.

Twelve patients with ureteral tumours with or without ipsilateral renal pelvic neoplasms were retrospectively analysed. Haematuria was the most common symptom. Location of all tumours was preoperatively detected by conventional diagnostic methods, such as intravenous or retrograde pyelography, washing cytology of the upper urinary tract, computed tomography, echogram and abdominal aortography. Cumulative proportion survivals of 1, 3 and 5 years were 81.9%, 68.2% and 45.5%, respectively. Patients with high Karnovsky rating survived longer (p less than 0.05) than those with low rating. Patients with low-stage tumours (T1, T2) showed longer survival rate (p less than 0.001) than those with high-stage tumours (T3, T4). Vascular invasion of tumour cells was present in removed specimens in 4 out of 7 patients who died or are alive with cancer, but none in those who survived without disease. Good therapeutic response could not be achieved unless surgery was performed.

Cortically induced masticatory rhythm in masseter motoneurons after blocking inhibition by strychnine and tetanus toxin.

In the first series of experiments, we studied whether or not strychnine (STR)-resistant inhibition of masseter motoneurons (MASS . MNS) was involved in their rhythmical inhibition that occurs during masticatory activity, induced by repetitive stimulation of the cortical masticatory area (CMA) in the cat. After systemic STR injection, repetitive CMA stimulation induced rhythmically alternating activity in the masseteric and anterior digastric nerves with a shorter cycle time than before STR-administration. The short-latency IPSPS in the MASS . MNS evoked by single shocks applied to the CMA were abolished. In contrast, repetitive CMA stimulation still induced a rhythmical alternation of EPSPS and IPSPS in the MASS . MNS, although the IPSPS were significantly reduced in amplitude. In the second series, we attempted to abolish the STR-resistant component of the rhythmical IPSP with tetanus toxin (TT). This was injected into one superficial masseter muscle of the guinea pig. In the majority of animals, repetitive CMA stimulation induced a tonic EMG superimposed by rhythmical bursts in the TT-intoxicated masseter muscle. Repetitive CMA stimulation induced a rhythmical sequence of EPSPS and superimposed spikes in the MASS . MNS innervating the TT-intoxicated masseter muscle in paralyzed guinea pigs. It was concluded that: (1) the cortically-evoked short-latency inhibition of MASS . MNS is STR-sensitive, as is part of the rhythmical inhibition during CMA-induced mastication; and (2) rhythmical inhibition is not essential for the central generation of the rhythmical activity in the MASS . MNS.

Low level of superoxide dismutase activity in pheochromocytoma.

A copper-zinc superoxide dismutase found in the cytosol and intermembrane space of mitochondria, and a manganese superoxide dismutase detected in the mitochondria were determined in pheochromocytomas and normal adrenal tissues. Manganese superoxide dismutase activity in pheochromocytomas was lower than that in the normal adrenal tissues but copper-zinc superoxide dismutase activity was almost identical. The total catecholamine content in pheochromocytomas was greater than that in the normal adrenal tissues, and negative relationships were noted between superoxide dismutase activities and total catecholamine content in pheochromocytomas alone. The low level of manganese superoxide dismutase activity might be a characteristic of pheochromocytomas and the decrease in manganese superoxide dismutase activity may not be attributed solely to a decrease in the amount of mitochondria but to a nonspecific abnormality in mitochondrial enzymes.

Liddle's syndrome, an uncommon form of hyporeninemic hypoaldosteronism: functional and histopathological studies.

Liddle's syndrome was diagnosed in a 72-year-old man who presented clinically with hypertension and muscle weakness. This disorder has been characterized by hyporeninemic hypoaldosteronism, hypertension, hypokalemia and enhanced erythrocyte sodium influx. Administration of spironolactone failed to correct the hypertension and electrolyte abnormality, which subsequently improved with triamterene therapy and a low salt diet. However, suppression of the renin-angiotensin-aldosterone system remained unchanged after this treatment. In addition, an atrophic juxtaglomerular apparatus and hypertensive lesions in the arterioles were confirmed by kidney biopsy after triamterene therapy. Therefore, a process of intrinsic hyperactive distal sodium reabsorption, probably affected by aldosterone-independent sodium transport into erythrocytes, appears to be important in the pathogenesis of this syndrome. Triamterene therapy, which usually is performed in patients with this disease, might not be the ultimate therapy in the future even if electrolyte abnormalities were to be improved temporarily.

Effects of a new antibiotic, isohematinic acid, on the resistance of mice to experimental infections.

Isohematinic acid, an antibiotic newly isolated from the culture broth of Actinoplanes philippinensis SANK 61681, was assessed for its ability to enhance nonspecific resistance to bacterial infections against Escherichia coli and Pseudomonas aeruginosa in mice. This agent, as well as BM 12,531 (Azimexon), was found to prolong the survival of normal mice infected with E. coli and also of compromised mice infected with either E. coli or P. aeruginosa, whose defense system had been deteriorated by treatment with carboquone, an alkylating agent. Like BM 12,531, isohematinic acid administered to normal mice significantly increased the nitroblue tetrazolium reducing potency of polymorphonuclear leucocytes (PMN), indicating that the microbicidal activity of PMN was enhanced by these agents. In addition, in the compromised mice these agents were able to restore the number of peripheral blood leucocytes, which had been reduced to about 30% of the normal level by carboquone. These results suggest that isohematinic acid, like BM 12,531, enhances nonspecific resistance to these bacterial infections by stimulating the microbicidal activity of PMN and inducing leucocytosis.
