Nucleolus-associated localization of J chains in IgG 1(lambda)-producing myeloma cells.

By using both direct and indirect immunofluorescence technics and the PAP method, the authors observed an unusual immunocytochemical staining pattern in myeloma cells producing IgG 1(lambda). J chains were localized mainly as intranuclear spots, compared with gamma and lambda chains that are diffusely distributed in the cytoplasm with dense perinuclear accumulation. The present findings on the location, size, and number of the intranuclear spots suggest that J chains are nucleolus associated. The authors hypothesize that J chains are synthesized in the cytoplasm and transported into the nucleolus, where they associate with nucleolar proteins, or that they are synthesized within the nucleolus, which participates in biogenesis of ribosomes.

Phagocytic myeloma cells. Report of a case and review of the literature.

The authors have reviewed 17 cases of the phagocytic multiple myeloma, including seven cases reported in Japan but not cited in the English literature and an additional new case investigated in the authors' laboratory. They would like to add these Japanese cases to the English literature. The salient features of these 17 cases were summarized to have a better understanding of the clinical aspects as well as of the pathogenesis of the phagocytic myeloma cells. The following two clinical points should be noted: The erythrophagocytosis by myeloma cells may be responsible for the hemolytic anemia in multiple myeloma. Myeloma cells were found in the peripheral blood in more cases of this series than expected. The pathogenesis of this phenomenon, related to the cellular origin of plasma cells, may be interpreted differently, as discussed in this report.

Autoantibodies, including antibasement membrane antibody, in patients with selective IgA deficiency.

Laboratory data are presented on 48 patients with selective IgA deficiency. Sixty percent of the patients had autoimmune diseases or related disorders. We found antibasement membrane antibody (ABMA) in the sera of four IgA deficient patients and the incidence of ABMA seemed to be related to clinical symptoms. The incidence of various other antibodies was found to be increased in selective IgA deficiency. This phenomenon might be derived from the lack of local immune system. Inadequate IgA barrier might permit the exessive gastrointestinal absorption of food antigens or autoimmunogens in metabolic products which are excreted into the gut lumen.

T-Lymphocyte subpopulations in hemophiliac patients.

Laboratory data are presented on 15 patients with hemophilia A. Mean OKT4/OKT8 ratio in hemophiliac patients was significantly lower than in the controls. Although no patients with acquired immunodeficiency syndrome (AIDS) have been found in Japan, hemophiliac patients are supposed to be at increased risk of AIDS by our studies.

Extramedullary plasmacytoma of the parotid gland: report of a case and review of the literature.

A case of primary extramedullary plasmacytoma (PEMP) with local amyloid deposition arising in the left parotid gland of a 78-year-old woman is described. The present case was studied with immunologic techniques with results confirming that the neoplastic cells were of monoclonal nature, producing lambda-light chains alone. PEMPs of the salivary glands are rare, with only 8 cases reported in the literature. The salient features of these 9 cases are summarized. Problems connected with the clinical, pathological and therapeutic aspects of PEMP with regard to the rare parotid location are discussed.
